While walking around a block and saw ee slipped on a small rock. While trying to catch self EE's wrist got caught on the block and EE's wrist was lacerated.

EE lost balance while splitting a block and fell. EE was wearing working at heights protection, however EE's retractable failed to lock up. Before use inspection was conducted, it worked fine. Retractable has been put out of service.

employee slipped on rocks and scraped their left arm and right hand. Initially it was a first aid, however few days later it got worse, and employee was diagnosed with cellulitis

Employee was walking with boot laces untied. Employee tripped on untied laces. Employee broke a bone in their right ankle as they fell.

Crane operator made a misstep while exiting the crane. This resulted in the operator landing on right arm injuring it

Employee was sitting in a chair working on a wire saw control box and the chair shifted and EE fell out of the chair to the ground from a seated position. Review of the incident inconclusive of chair malfunction or employee weight shift could have caused the fall. Chair was disposed of regardless.

Miner was walking to work location slipped and fell same level and landed on their left side.

Dimensional stone extraction crew miner needed a tool from another area of the ledge. Instead of using provided access to that area, miner tried to walk across a small (approx 10" wide) ledge of a column to get there that was roped off. Miner slipped on rocks/mud and fell down approx. 7 feet and landed on shoulder causing pain.

EE stepped on a plank that was acting as the floor of the splitter pit and the plank broke and EE fell and bruised EE's left calf

Conducting secondary breaking operations. When EE was pulling the wedges the stone broke and EE fell to a lower level fracturing and dislocating EE's ankle.

Walking in front of Tractor Trailer and stepped into a divot and hurt hip

Breaking Stone and fell between the stones and one stone rolled back onto EE's leg

Tried to climb on a low rise berm to secure a DTH Drill and slipped off landing on feet and tweaked back.

Walking in Quarry and twisted EE's Knee

Employee was walking near the entry driveway of the quarry when EE slipped and fell on a rock.

Trying to step onto slab to put timbers on and tripped and fell causing EE to sprain wrist.

EE was walking on a trailer, slipped and fell because of the frost.

Pushing stone on track and the stone caught on a section causing ee to fall down.

Moving Power cord and rolled ankle

Miner fell from highwall 14'6" to the lower level. Miner suffered 5 broken vertebrae and laceration to the back of head. Miner was ambulatory after the fall.

While handling the hand hammer drill and walking to EE's work area, EE slipped and fell on the ground. The hammer landed on EE's left hand, index finger.

Employee was removing drill steel (approx. 10 feet long) from a drill machine. He tripped on a rock with the steel in his hand. He tried to protect himself from the fall and upon impact the weight of the steel caused his left wrist to twist backwards.

Working on floor, tripped on something (wood or rock) fell and landed hitting his right hand on a sharp rock.

Reaching over edge of a block for a T-square, lost his balance falling approx 5 ft. Landed in some rubble on his right ankle and leg.

Attempting to lift a truck tire edge so he could get the skidsteer bucket under it to move it to another location.

Thawing mud and ice conditions at top of wall. Miner stepped off ladder to edge of wall. The frozen mud gave way and he slipped, falling 8-1/2 ft backward. Landed on right side/knee.

Employee was working on a Volvo L330D Loader from a temporary platform constructed for the purpose of gaining access to the area in need of repair. The employee attempted to re-position the equipment by moving it, while standing on the platform and steering. Platform collapsed and employee fell.

Miner was decending on ladder tied off ladder. Ladder slipped, miner lost balance and fell approx. 8 ft. Hit right shoulder and head on floor. Iced/heat applied to shoulder and neck immediately. Headache and stiffness in shoulder/neck area.
