The loader was stuck and shifted causing the window to close on employee's finger.
Employee was walking near the entry driveway of the quarry when EE slipped and fell on a rock.
Miner was applying epoxy resin w/ fiberglass to block. That night had allergic reaction but returned to work 8/22. Reaction worsened that night and went for medical attention 8/23/15. Never reported it to supervisor until bill came in 10/14/15.
