EE was taking a track pad out of equipment, and believes they injured themselves when they were knocking the pins out of the track.

EE was assigned to drain and refill the jaw crusher oiler, as EE was holding the bucket in place, EE felt EE's lower back give out.

Employee was doing daily inspection on jaw crusher, when a haul truck dumped its load at the top of the ramp. The material surged from the primary into the grizzly feeder and then a rock came out and struck employee on the back of the hardhat.

EE was working on loader seat and got hand pinned between scissor frame.

Employee was holding a pipe with left hand and a 4 1/2 angle grinder with right hand when the grinder kicked back striking the employee's left thumb.

Employee removed a worn crusher bar from the crusher and when setting it down on the frame of the motor base employee felt the right side of back give away.

EE was operating a John Deere 310E articulating haul truck when they hit a pothole causing the seat to bottom out and cause back pain.

Employee was attempting to remove a wheel chock when the employee lost footing slipping in the mud falling at the same level.

Employee's pinky finger was caught between a bucket tooth and the fork of a telehandler.

Loader operator noticed an A/C vent fell out of housing. While replacing A/C vent while A/C was running, finger closest to thumb on (L) hand encountered fan cutting finger tip.

Employee was helping another employee set a part on the rear end of a maintenance truck. The employee set the part down on right ring finger. The employee was taken to the doctor and diagnosed with a closed fracture of tuft of distal phalanx of finger.

Employee was climbing a step ladder when it broke. The employee fell approximately 3 feet to the ground spraining ankle.

While lowering a large piece of steel during dragline repair activities the steel hung up, and the injured miner moved in to help free the load. When the load became free it shifted towards the miner, striking them on the right shoulder and upper chest area

Employee was changing blow bars on a HSI crusher. EE used a small sledge hammer to hit one of the blow bars causing a piece of the blow bar to chip. The piece hit EE just inside EE's right nostril. The employee was taken to the emergency department where the piece of metal was removed and EE was given one stitch.

On 8/11/2020 the employee hit right knee on a metal lip while climbing into screen deck to check screens. The employee reported the injury on 8/25/2020 when employee requested to see a doctor and was diagnosed with a knee sprain.

Employee was working on the drill and when EE was finished EE stepped off the counter weight onto the grate platform and slipped and twisted (L)ankle. Employee went home to rest it figured it would be ok and while at home it started to get worse that's when EE decided to see a doctor on 1/18/20.

Employee's left knee became sore after operating 2 pieces of equipment

The employee was pulling the rubber door back on the screen deck when employee felt a pain in right arm resulting in a strain.

The employee struck knee on a piece of metal while accessing the dragline resulting in a contusion. EE received medical treatment on 4/30/2019.

IW strained right elbow while pulling water hose up stairs. IW was given restrictions on 11/05/2018 which IW was unable to do job duties at full capacity.

Death due to Natural causes.

While preparing for end of shift, the employee slipped on a small rock on way from one MCC room to the next causing a left ankle sprain.

Actual date of accident is unknown. Injured employee was pulling and adjusting hydraulic hose and felt something in stomach area approximately 5 months ago. Employee did not think anything of it at that time. Employee said they did not think it was that serious. It was just recently reported to injured employee's supervisor.

The employee was stepping out of the truck when EE stepped on uneven ground and rolled left ankle causing a sprain.

The employee was changing screens, when dust/rust was blown into eye causing the eye to be irritated. EE was wearing protective eye wear.

Injured employee was assisting service personnel on a dewatering pump. Subject slipped and cut lower right leg on unknown object.

Employee was checking the oil on the 385 excavator. He missed the last step while descending and fell to the ground.

Employee was struck on top of his head by a 4 x 8 sheet of plywood. He was wearing a hard hat at the time and did not seek medical treatment until Dec 23, 2014. He was released to return to work and was able to conduct his normal job duties. He continued to have neck pains and eventually elected to have surgery on June 15, 2015 making this a reportable accident.

Employee was spraying out the bed of a haul truck when a hose clamp caused him to receive a hand laceration. He sought medical attention, received sutures and returned to full duty.

Tripped and fell on loose rock, straining left knee.

Block moved unexpectedly while greasing. Caused bruises to upper right side of body and left knee

EMPLOYEE WAS REMOVING BOLTS FROM THE END OF TRAVEL FRAME WHEN END CAME LOOSE, FLEW UP, HITTING HIM IN LIP & CHIN.

EMPLOYEE WAS SLIDING A BOARD ACCROSS CRUSHER, BOARD SLIPPED OFF OPPOSITE SIDE CAUSING THUMB TO BE PINCHED BETWEEN HYD. CYLINDER AND BOARD.

EMPLOYEE ALLEGED INSECT BITE. LOCATION AND TIME OF INCIDENT UNKNOWN.

VICTIM WAS PERFORMING REGULAR MAINTENANCE ON DRAGLINE, WHEN HE TRIPPED AND FELL ON THE CATWALK, BRUISING HIS KNEE.

VITIM WAS WEARING PROTECTIVE GEAR WHEN RESOCKETING A DRAG CABLE FOR A BE 1250W DRAGLINE. HE HELD ON TO THE CABLE TOO LONG POSITIONONG HIS MIDDLE FINGER AGAINST THE DRAG CHAIN. THIS RESULTED IN A FRACTURE AT THE FIRST KNUCKLE. EMPLOYEE HAD PREVIOUSLY SERVED NOTICE AND THIS WAS HIS LAST DAY OF EMPLOYMENT FOR THE COMPANY.

EE WAS CUTTING SKIRT RUBBER WITH KNIFE AND IT SLIPPED AND CUT HIS KNEE.

Employee fell in a ditch while avoiding an out of control customer dump truck. The incident aggrevated an existing back condition.

EE WAS USING WELDING ROD TO WELD MACHINE AND HOT WELD ROD WENT INTO BOOT. THIS WAS INITIALLY CLASSIFIED AS DOCTOR FIRST AID 1/20/04. EFFECTIVE 2/11/04 IT BECAME REPORTABLE/DOCTOR TREATABLE D UE TO REQUIRED FOLLOW UP TREATMENTS PREVENTING INFECTION.

HE STEPPED IN AN OPEN HOLE, FELL AND TWISTED HIS KNEE.

EMPLOYEE TRIED TO LIFT A CAGE DOOR BY HIMSELF. HE JERKED AND TWISTED CAUSING INJURY TO HIS BICEP. THIS INITIALLY WAS CLASSIFIED AS FIRST AID, NON-REPORTABLE ON 12/6/03. CONDITION DID NOT IMPRO VE RESULTING IN BICEP SURGERY 2/16/04.

HELPER STUCK PIPE TO UNLOCK BAR BEFORE OPERATOR COMPLETELY STOPPED ROTATION OF BAR.

EMPLOYEE WAS CUTTING A PIECE OF ALUMINUM TUBING WITH A MITER SAW.

EE WAS ADJUSTING FORKS ON FORKLIFT TO FIT SIZE OF PALLET TO BE UNLOADED FROM TRUCK.

AGGRAVATION OF OLD BACK INJURY. BACK STARTED BOTHERING HIM AGAIN AND REQUESTED TO GO TO DR.

EE WAS SHEARING A PIECE OF FLAT BAR WHEN A SLIVER FRACTURED OFF THE KNIFE & WENT INTO RIGHT WRIST.

EE WAS TIGHTENING BOLT ON PLANT SCREEN. WRENCH SLIPPED OFF STRIKING HIM IN UPPER LIP.

EE WAS IN A SQUATTING POSITION WELDING HANDRAILS DOWN TO TOP DECK OF DIRT SCREEN. HAS STRAIN OF LOWER BACK, PAIN RUNS DOWN RIGHT LEG.

EE WAS ATTEMPTING TO LIFT A STEEL LID & FELT A PAIN IN HIS CHEST

EE WAS MOVING AROUND THE TOP OF THE DRAGLINE. HE GRABBED A PIECE OF ANGLE IRON LOST HIS BALANCE, & FELT HIS BACK GIVE & PAIN IN HIS LEG

EE WAS WORKING ON CRUSHER (IT WAS VERY WINDY) EE DIDN'T NOTICE ANYTHING UNTIL LATER IN THE DAY. IRRITATION TO LEFT EYE.

EE WAS TRYING TO LIFT A PUNCH PLATE SCREEN BY HIMSELF WHEN HE FELT A PAIN IN HIS LOWER BACK.
