WHILE EE WAS USING THE WATER HOSE TO WASH UNDER THE TRUCK SCALES HE BUMPED HIS HEAD AGAINST THE CROSSBAR UNDER THE SCALE (WAS WEARING HARDHAT).
EE WAS CLOSING RAILCAR DOOR. WHILE REACHING UNDER CAR HIS RIGHT FOOT SLIPPED THROUGH GRATE ON HOPPER, CAUSING HIM TO FALL FOWARD. HE HIT FACE ON DOOR OF RAILCAR. CUTTING UPPER LIP.
AFTER OPERATING THE FRONT END LOADER FOR THE DAY, AT THE END OF THE SHIFT, EE FELT A SEVERE PAIN IN HER LOWER BACK. SHE CALLED HER SUPERVISOR TO HELP HER GET OFF HER LOADER.

REACHED OVERHEAD FROM STANDING POSITION AND PULLED ON HAND BRAKE OF RAILCAR TO SET BRAKE OVEREXERTING AND PULLING ON BACK.
