EE was welding legs for skirt rubber on the tunnel belt he was tied off with a safety harness on the side where there was no catwalk to stand on, it had started to rain and he lost his balance, his safety harness prevented him from falling. He has continued to work since 08/18/2010 but had problems with his right shoulder and he had surgery on 1-27-11 to repair his rotator cuff.
EE took a hearing test which put him over the limit on exposure to noise I was made aware on 8/2/10 that this is a reportable accident to MSHA. I apologize for failing to report this incident within the 10 working days as required in Part 50.
EE was removing an old dust cover inside the chute under #3 dryer from the 5th floor. Another member was on the 6th floor,EE told him to reach him a torch down thru the chute when he did this he heard EE ignite the torch and a cloud of dust came up onto the 6th floor. EE received burns to his head.

Member stepped out between the hand rails after cleaning the head shaft on the coarse vessel. As he stepped out, he placed his foot unexpectly on a small piece of wood that was on the floor, which caused his ankle to pop. He has been taken off work until his injury has been properly diagnosed.

Member was dismounting the D-9 Dozer when he stepped down from track, his foot slipped & he caught himself with left arm, causing strain to his arm & neck. Went to ER Room and was told to see specialist. We are reporting because of 10 day policy This is information we have at this time.
She was cleaning the clean coal silo belt when she tripped over a rock causing her to fall to the ground. She was taken to the hospital and diagnosed with hairline fracture to her foot.
Employee was wiring and mounting control devices in an out of service motor starter. He was mounting control devices in an empty compartment above the starter when he apparently drilled through the floor of the compartment into an energized buss tap.

Member was going down the steps from the elevator control room when his heel caught a step causing him to lose his balance which caused undue strain on his left arm and shoulder. He did have corrective surgery on his shoulder on Feb 27, 2007 to correct his condition. He hasn't missed any scheduled work days because of this accident.
EMPLOYEE WAS INSTALLING A HYD. MANIFORD ONTO A D-9 DOZER WITHOUT ASSISTANCE. HE SLIPPED ON HOSE & FELL WITH THE LOAD, TWISTING HIS BACK MUSCLE. MANIFOLD WEIGHS APPROX. 50 LB.

Employee rested left hand on diverter. Middle finger was caught between frame and diverter causing laceration. He was taken to williamson Hospital for treatment, stitches used to close wound.
EMPLOYEE WAS CARRYING A 4" HAND-HELD GRINDER UP THE STAIRS. AS HE REACHED THE TOP, HIS TOE WENT UNDER THE LIP OF THE STEP CAUSING HIM TO STUMBLE & FALL. THE EMPLOYEES' LEFT ELBOW STRUCK THE FLOOR CAUSING A FRACTURE. THIS WAS EE'S 3RD DAY AT THIS PLANT.
Employee assisting removal of old beam at mechanism. Standing on beam operating a chain ratchet, beam shifted causing him to fall. He struck left elbow on grizzly.
WHILE CLOSING THE DOORS ON RAILROAD CAR, EMPLOYEE'S BAR SLIPPED CAUSING HER TO FALL. BAR STRUCK HER ON RIGHT LEG ABOVE KNEE. SHE ALSO LANDED ON LEFT HIP.

Employee was cleaning auto greaser with air on it. Greaser over turned onto air, turned on catching his thumb and cutting the tip. He was taken to the E.R. for stitches.
Employee was pulling used beltline with an it-28 loader, backed up onto the berm, and turned the unit over. he was taken to the e.r. for treatment. x rays were negative, however he had a bruise on this right hip and the dr told him to stay off of his feet for a few days.
Employee was trouble shooting a cut off switch for the R-5A refuse belt. As he was coming down from the structure, he slipped on loose refuse and fell. He struck a beam 24 inches below and injured his right side.
EMPLOYEE WAS WORKING IN THE REFUSE FEED CHUTE. AS HE CLIMBED OUT OF THE CHUTE HE CUT HIS RIGHT KNEE ON A PIECE OF METAL. HE WAS TAKEN TO THE E.R. FOR TREATMENT AND STITCHES TO CLOSE THE WOUND.
EE was woring inside the heavy media vessel replacing chain and flights. after completion of the job he complained of pain in his low back and hip.
EE WAS HELPING PLACE A TOP ROLLER ONTO THE BELT STRUCTURE. HE REPORTED THAT AS HE LIFTED THE ROLLER AND TRUNED TO PLACE IT, HE INJURED HIS BACK MUSCLES.
Ee reported that he was injured on 4-22-04 while working on a vessel in the prep plant. He alleges that he bent over to pick up a 4" valve weighing about 30 lbs & injured his back & right leg. This was reported late due to the ee being at work, taking vacation & personal time before reporting the alleged injury.
MEMBER WAS GETTING OLD PIPE OUT OF PLANT. PUSHING PIPE ACROSS FLOOR ON DOLLY WHEN DOLLY DROPPED IN DITCH MASHING HIS HAND CAUSING LACERATION.
MEMBER WAS LOADING SALT INTO THE BACK OF A TRUCKWHEN HE STARTED HAVING CHEST PAINS.

EE PLACED A CHUTE PORTION TO AN ESCAPE TUBE OPENING ON A 4 WHEELED PULL CART TO BE ROLLED TO THE RECLAIM TUNNEL. THE CHUTE SHIFTED AND KNOCKED THE HANDLE OF THE CART ONTO HIS FOOT. THE EE WAS WEARING STEEL TOPS, BUT WAS NOT WEARING METATARSALS WHICH ARE REQUIRED BY PCC AND WOULD HAVE PREVENTED THE INJURY.
