Employee walked around mechanic truck to get bolts and stepped on unlevel ground causing them to twist an ankle.

Employee was operating equipment, was in the process of adjusting the width of the forks on a JLG forklift. The employee attempted to push the fork from atop the vertical divider by using his foot. The employee placed his right hand in the crossbar support area to steady himself. As force was applied, the crossbar seated, smashing the employees thumb.

It appears that he fell or slipped over embankment into the drainage / outlet ditch. Approximate fall of 100ft-125ft. He was found by security after he did not return @ the end of shift.
Employee was climbing into cab of an excavator when his right hand slipped off the grab rail causing him to catch all his weight with his left arm and shoulder. He then stepped back and twisted his right knee. He continued to work his scheduled shifts until 10/17/2011 when he had surgery on his shoulder.

Apparently the driver attempted to down shift to a lower gear while descending a grade between road marker 4 & 5 on the haul road. He lost control in a curve & traveled through a berm impacting the hillside resulting in no apparent injuries to the operator.
EE stated that he was grinding, felt something in eye. He worked through shift of 5/31 sought medical attention after working shift of 6/1. RTW on 6/2 no lost time.
Employee was driving down roadway and met a 4-wheeler. He then attempted to hold vehicle on right (creek) side of roadway; apparently driving too close to shoulder and drove over berm. The truck rolled over onto its right side and into the creek.

While starting a brush fire the operator attempted to egress the area. He was clearing the roadway in front of the excavator; his left track trammed upon a stump which caused the excavator to overturn. The heat and blowing embers caused the excavator to catch fire. The operator was wearing his seat belt and received no injuries. The excavator was a total loss.

EE WAS ATTEMPTING TO OPEN THE OPERATOR CAB DOOR THAT WAS FROZEN. A CLIP LOCATED INSIDE THE DOOR HANDLE BROKE, ALLOWING THE ENTIRE DOOR HANDLE ASSEMBLY TO COME OUT OF THE DOOR. EE LOST BALANCE AND FELL FROM THE DOZER.

HE HAD CLIMBED INTO THE CAB & STARTED THE ENGINE & BEGAN TO DISMOUNT THE TRUCK WHEN HIS FOOTING SLIPPED, RESULTING IN HIM FALLING FACE DOWN APPROX. 3'TO THE GROUND. HE WAS NOT USING 3 POINTS OF CONTACT & ALSO HAD CLEANING SUPPLIES IN HIS HAND. HE COMPLAINED OF PAIN TO HIS FOREHEAD & ELBOW. SPRAINED ELBOW.

EMPLOYEE WAS REMOVING A HYDRAULIC FILTER FROM A 992G LOADER WHEN THE FILTER WRENCH HE WAS USING BROKE CAUSING HIM TO FALL FORWARD AND STRIKE HIS FOREHEAD ON THE BOTTOM OF THE METAL COMPARTMENT DOOR OF THE LOADER. HE WAS TAKEN TO THE HOSPITAL WHERE HE RECEIVED FOUR STITCHES TO TREAT THE LACERATION.
EMPLOYEE WAS DISMOUNTING THE ROCK TRUCK WHEN HE BEGAN TO EXPERIENCE PAIN IN HIS BACK. HE FAILED TO REPORT THE ACCIDENT TO MANAGEMENT UNTIL 9/26.
