Employee was using hand drill when drill bit grabbed causing the drill to twist in the employee's right hand. This resulted in a fracture to right hand.
Sample tube capsule holder pinched left hand middle finger resulting in fracture.
Driller was adding addition to auger. When the auger stop rotation the helper reached for the pin as the driller reversed rotation and the head of the bolt pinched his thumb against the outer portion of the yoke. Helper had a fracture to right thumb.
Employee operating hand grinder with right hand while grinding a chain conveyor connector pin being held with left hand. Connector pin believed to have caught grinder's flap disc, then ricocheted around the grinder's guard, and struck his neck causing a puncture requiring stitches. Employee's body position allowed the connector pin to pass under the face shield.
Employee was going to change tip on torch. He walked through some welding fumes from another employee completing that task and became sick. Reaction to the fumes.
Employee was cutting metal on hydraulic shear when distal right index finger tip was amputated by shear.
Cutting bolts overhead, slag from cut dropped and blew into ear. Slight, burn to ear.
Employee was using a grinder to buff a shaft. While he was running the grinder one of the strings from his hooded sweatshirt fell out of his coat and got wound up in the shaft of the grinder behind buffing wheel. This caused the grinder to get sucked up to his face striking him in the mouth. When it hit him it chipped a corner off of his front tooth
Employee was jackhammering a vertical refractory wall in the calciner, working from his knees when he felt a shooting pain in his back.
Grinder cut to left fore arm requiring 8 stitches. NOTE this was just reported to me. The supervisor listed this as first aid treatment. The only medical bill sent listed exam. The employee is no longer employed with us. No one mentioned any stitches until today.
The point (tip) of the rock breaker hammer was jammed in-between two of the fingers at the primary feeder. The EE placed his hand on one of these fingers. The rock breaker hammer moved down and struck the back of the EE's hand. We brought the EE to get X-rays on the hand which came back negative - nothing broken. Hand was bruised.
EE was holding pressure on a cutting edge bolt, pushing bolt into place for removal with air wrench using the palm of his hand. The bolt started turning and cut a circular pattern in right palm
Employee was moving a barge with a winch cable when the barge shifted causing the cable to strike the employee in both shins causing abrasions. Employee did not seek treatment until 01/25/2012 when it was discovered that the abrasions had become infected making the incident reportable.
On 11.21.11 EE went to start up a Goodwin Submersible Pump when the hose which was attached jumped and struck employee in lip causing laceration. EE received stitches and was released to return to work. No medication was administered.
Employees were trying to dislodge an oversize rock that was stuck in the crusher hopper. They had opened an 18" work door to jackhammer the rock. After 4 hours of work, a smaller baseball size rock fell on to the crusher reel and the spinning motion sent the rock flying out the access door striking the employee in the face/nose. INJURY WAS TREATED AS FIRST AID UNTIL 10/25
Employee was drilling a hole with a hand drill when the bit grabbed, twisting the drill and the employee's right hand and fracturing the employee's right ring finger. The injury became reportable on 10/06/2011 upon discovery of the fracture.
Operating dozer for past several months. Employee complains of upper back strain B/T shoulders.
Employee was welding in a kiln, when a chain hit him on the side of the head. Employee was wearing a hard hat and safety shield.
Employee was cutting metal grating with a hand grinder when the grinder kicked and lacerated his right leg.
EE was assisting another EE cutting pipe on chop saw. While moving next section of pipe, EE right hand made contact with still spinning blade. Saw had been shut off, but blade continued to spin down from previous cut. Cut went through leather gloves and into EE right thumb which required stitches to close. EE on restricted duty.
EE's were pushing tail pulley of drag conveyor with hydraulic ram to remove shims. EE was hit on right thigh by hydraulic ram when it slipped out of position under pressure. EE sustained Medial Collateral Ligament Sprain to right knee. EE working with restrictions.
EE using portable grinder with cut-off wheel to remove section of dust collector duct when wheel got stuck in pipe. Grinder kicked back and hit EE in chest and left cheek under face shield. Treated at clinic, left cheek needed 9 stitches, return to work, no restrictions.
Employee was drilling when the hand drill got caught on material and twisted the drill, causing his right hand to smash into a metal flange fracturing the hand.
Employee was struck by a drill steel (12"x4-1/2") when it fell from a Gill Rock Beetle Drill. Employee suffered collapsed lung, fractured vertabrae, ribs and collar bone.
Employee using a battery operated concrete grinder when grinder struck a burr in the concrete causing the grinder to jump and lacerated employee's left index finger requiring 8 stitches.
Employee was performing maintenance duties including drilling holes and installing equipment when metal entered eye.
While in the process of welding, hot weld slag fell onto the boot if ICM employee. The weld slag burned through the boot, causing a burn to his foot.
Employee operating a belt grinder on the thrust roller of kiln. Some metal dust from grinding process blew into his face and into right eye.
The employee was using a plasma cutter to remove liners from the inside of the kiln. A piece of slag popped bouncing off the kiln shell and the employees welding hood landing in his right ear.
EE was repositioning the slings to release the pan feeder head shaft when one of the slings slipped slightly causing the shaft to move pinching the EE's left thumb between the shaft and the drive sprocket.
Power washing slip form paver, power washer contacted knee and forced water into and past the knee joint.
EE received flash burns to his eyes while working in proximity to welding operation. EE did not notice until after getting home. EE recieved treatment on 9/11/2009. EE returned to normal duty on next scheduled shift.
The initial incident that led to the aggravated injury was on the 7/14/2009 - no doctor visit - first aid only. The employee (mobile equipment operator), was executing the cleaning task for mothballing. Tasks aborted.On 08/05/09, the employee visited the doctor citing pain. Doctor imposed restrictions/prescribed medicine and an appointment to revisit in 10 days.
Employee was welding on a rock bin. Had his safety glasses hanging around his neck. Went to lift up his welding shield and a piece of slag popped and entered his left eye.
While Jack Hammering broken bolt from the cradle frame, the hammer slipped off and pointed to the floor. As it was going down, a piece of the shaft guard of the adjacent equipment was sticking out and pierced his left ring finger, causing a laceration.
Employee was cleaning buildup in the preheater tower feedshelf area with jackhammer. When the buildup fell off the jackhammer slid forward pinching finger between housing.
EE was air arcing and a piece of hot slag entered his left ear. EE received burn.
Individual was cutting grating in a very tight spot, when he lost control of the 4" grinder and cut his right hand. Individual was supporting his weight with the right hand and grinding with left.
While putting air pressure on air hose to clean pipe below rotary feeder, employee was unaware of uncapped Chicago fitting at eye level. When air was turned on, dust from fitting blew into his right eye. Rx eye drops prescribed.
EE was changing cutting edge to loader bucket of 922 Cat. While loosening stove bolt, it slipped out of square hole, spun and cut EE's L index finger.
Employee was pulling the starter rope on a small generator. The engine compression was greater that the pulling power resulting in a backlash on the rope. The backlash pulled his fingers apart from the ring finger, splitting the web at the top of his left palm. He was wearing cotton gloves at the time of the injury.
Employee was using a hand held grinder to cut off a 2" nut from a bolt when a piece of metal struck the employee's eye. He was wearing safety glasses and face shield. First aid eyewash was given at the plant. When eye did not get better, he was taken to local clinic to remove foreign body from under eyelid.
Employee was attempting to position himself to tack-weld bolts and nuts on a drive shaft of enclosed piece of equipment. Employee was not wearing welding eye protection and the welding rod touched the steel enclosure and caused arc flash which caused flash burns to the employees eyes.
Employee was drilling a hole for an ice melt spreader mounting bracket, the 1/2" drill bound up when the bit broke through the metal plate causing the drill to spin. Employees little finger was caught between drill and frame causing a fracture of his little finger.
Employee was welding and grinding on hopper section. Employee finished grinding and went to put on welding hood. When employee put on welding hood, employee felt a foreign body in his left eye. Employee did not report injury immediatley, which may have caused the infection to develop in his eye. The injury was reported approximately 4 hours after the incident.
#1 raw mll liner replacement project. EE was using "T" handle jack hammer to remove #1 raw mill liner bolts. EE's hand slipped causing his right finger to be smashed between bit latch and jack hammer barrel. X-rays confirmed fracture to right finger between hand and first knuckle. Placed in splint and returned to work.
Individual complained of having something in his eye. He went to the nurse. The nurse removed 2 pieces of debris from his eye and could not remove the third. Sent him to clinic and his eye had to have several layers scraped to remove piece.
Brace lowered into place with crane, choker at upper end, come-a-long at lower end. Bolt put in upper brace to beam connection. Crane took over brace's weight, come-a-long was removed, attached in the horizontal direction to pull brace in place. As brace was pulled, bolt in upper end of the beam failed. Brace swung down, hit a hand rail, then employee on left thigh.
Employee was welding on a kiln. The steel had to be heated up and the welder had to weld from one side of his body 10 hrs a day. A buch of loose skin devloped on his left elbow, he complained of the pain. Took him to Dr. and therepy was administered.
EE felt irritation in his right eye. The irritation increased while at lunch. EE irrigated his eye & felt better. Driving home the irritation increased & became painful.Safety Mgr took ee to clinic for treatment. RX was issued. Irritation continued, ee was sent to Optometrist. He had a piece of slag imbedded in his cornea. It has been removed & was given drops.
