Employee was assisting in installing a discharge line from a pump when Employee did not respond to verbal calls, Supervisor went to check and found Employee face down and unresponsive.
326 FL Excavator operator tracked over berm into Settling pond
The sub-contractor was found unconscious in EE's vehicle by EE's co-worker. The car was locked, co-worker attempted to wake EE. Co-worker called 911, then got mine superintendent, who came to the scene, broke the window, removed EE from EE's car, and started CPR. EE was then pronounced deceased at the hospital.
Employee was operating front end loader in the coal shed. For unknown reasons employee left the loader and was engulfed in the coal pile near the grizzly bars for the feeding system.
A miner entered the air separator to clear a blockage. The miner became engulfed in the material.
Miner used a front-end loader to carve a path into stacked material, then exited the loader and entered the newly-carved path on foot. The newly-created unstable wall of the material collapsed and covered the miner, causing a fatality.
Dredge operator was missing and eventually found at bottom of lake.
Complicated seizure disorder (not work related)
A miner was servicing a floating pump. While working, the pump capsized, trapping the miner underwater.
EE entered feeder when gate would not close. While cutting out a piece of metal that was blocking the slide gate, material sloughed off and engulfed the EE.
Weld crew initiated an argon purge on a 30" pipe that had been fit up afternoon prior. When argon purge was established, two welders began to make their root pass welds. Upon inspection "sugaring" was observed. a welder entered the open end of the 30" pipe for unknown reasons where they were found by a coworker to be unresponsive. No witnesses observed them enter the pipe.
Was beginning to storm, miner was walking towards cover when EE was struck by lightning.
Rollover accident/fatality/ongoing investigation.
Contract employee entered rail car GATX 22394 at approximately 11:30 am, to wash out the rail car. At approximately 12:00, a Thiele employee went to check on him and observed him lying on the floor of the car. Emergency response was activated immediately. Employee was recovered from the rail car by EMS personnel and pronounced deceased.
Truck driver entered empty bulk trailer tank for unknown reason, and was later discovered unresponsive inside the trailer.
Security guard fell or slipped from the cab of his personal vehicle into a shallow diversion ditch at the base of a berm along a roadway. He was found with his face submerged in approx 12" of water. The death was drowning & that a contributory condition was marked alcohol intoxication. The case was not determined to be chargeable until December 22, 2014.
Victim was last observed accessing the stern if barge NBN-611. Shortly after, he was noticed to be missing. Snow was present on the barge. Damage or impairment is unknown at this time.
A miner fell into the Tennessee River while attempting to take draft measurements on the barges parameter. EE had to cross from the permanent floating work-barge to the 1st empty barge EE was walking up-river on the barge. EE fell from the barge into the river workers immediately called for the Marshall County Rescue Squad.
While performing a section repair in sidewall of a large earthmover tire the associate was discovered unconscious and unresponsive inside the tire by a mining company worker.
EE was loading boat onto trailer, another EE in pickup started to pull up, wheels started spinning. Other EE went to get loader and chain, when he got back EE told him that boat had floated off trailer, EE was standing beside pickup. The other EE went to shop to get hook to try to retrieve boat, when he returned, EE's boots were by the water and EE was missing.
Employee was traveling #2 Longwall Bleeders and when he did not check in, we sent someone in to look for him. He was found unresponsive with no vital signs. The cause of death is still being investigated.
Employee got out of cab and stepped toward window & knee gave out. Employee did not slip or trip, footing was good, maintained 3 point contact, from a flat surface, but as employee shifted his weight his knee gave out.
A death of an employee occurred on or in the Ohio River. The death is still under investigation & the autopsy report has not been released. The cause is unknown at this time. Report is being filed under protest due to jurisdictional issues yet resolved. The deceased was working on empty barge when last seen. Upon his disappearance, a search commenced & his body found approx 2:30am.
Owner of J&P equipment cleaning was working on a steam cleaning engine when one of his employees noticed that he was slumped over the engine. This is still under investgation by MSHA.
Employee somehow fell into the water. There were no witnesses so no details available.
Cause of accident is unknown at this time. Employee was working on a winch on a dredge boat and was found drowned behind the boat some time afterwards.
A Chemicals I&E Technician was sprayed in the face with a liquid assumed to contain condensated hydrogen fluoride. The miner was in the process of performing routine preventative maintenance on an instrument tap. The system that the miner was working on is expected to be under a vacuum under normal conditions. The injuries suffered by the EE resulted in his death.
The individual was discovered in a structure designed to settle aggregate fines. His job duties involved keeping the area clean from spillage & debris. The Structure wall is over 4 feet high and there was no reason for the individual to have access to the top of the structure. He was discovered floating in the structure.
THE EMPLOYEE WAS ENGULFED BY HOT CALCINE FROM THE NORTH ROASTER, THEN FELL APPROXIMATELY 30 FEET TO CONCRETE FLOOR. HE WAS TRANSPORTED TO THE E.R. WHERE HE WAS PRONOUNCED DEAD.
EE WAS INSIDE MIXING TANK WHEN A STEAM SPARGER OPENED AND RELEASED STEAM RESULTING IN A FATALITY.
EMPLOYEE ALLEGEDLY SUSTAINED A SPONTANEOUS FRACTURE OF THE LEFT ANKLE WALKING ON A CONVERYOR BELT WALKWAY.

AS THE TEAM WAS WALKING UP A STEEP DECLINE ON THEIR RETURN TO THE SURFACE, TWO TEAM MEMBERS COLLAPSED. ONE VICTIM WAS PRONOUNCED DEAD AT THE SCENE. THIS TEAM MEMBER WAS TRANSPORTED TO THE HOSPITAL WHERE HE DIED ON 10/23/02.

EE ENTERED AN AUGER HOLE AND SUFFERED ASPHIXATION DUE TO LACK OF OXYGEN.
APPARENTLY EE ENTERED THE BAGHOUSE ROOM COMPARTMENT SOMETIME BETWEEN 8/5 @ 6 OR 7 PM AND 8/6 @ 4 AM. THE DOOR APPARENTLY SHUT AND LATCHED WHEN THE EE WAS IN THE ROOM. THE CONDITIONS IN THE ROO M WERE HOT AND DUSTY.
HE WAS OPERATING A DOZER TO MOVE SAND LINE WHEN BANK CAVED IN CAUSING IT TO GO IN PIT.
EMPLOYEE WAS USING A HIGH PRESSURE WATER HOSE TO WASH OUT ACCUMULATED MATERIAL BELOW CONVEYER #11 (FLOATING) AND TAILPULLEY CONVEYER #4 (LAND)APPARENTLY FELL IN WATER PIT WHILE NOT WEARING A LIFE JACKET.
DREDGE OPERATOR, HEART ATTACK FELL INTO THE WATER DROWNED LIFE JACKET PROVIDED AND AVAILABLE BUT NOT WORN.
WORKER WAS STARTING WATER PUMP. HE APPARENTLY SLIPPED OR FELL STRIKING HIS HEAD ON A ROCK. HE FELL INTO THE WATER CAUSING HIM TO DROWN.
TWO EES WERE ATTEMPTING TO FIX A BROKEN ANCHOR CABLE WHEN THE BOAT FLAT BOTTOM CAPSIZED AND THREW THEM INTO THE RIVER. NEITHER WERE WEAR LIFE JACKETS. OTHER WAS ABLE TO HOLD ON TO SOME TANKS T HAT WERE INT HE BOAT AND GET HIMSELF TO THE SHORE. EE WAS LAST SEEN TRYING TO SWIM TO SHORE. EE WAS FOUND AROUND 7 PM 6-27-00 ABOUT 1 1/2 MILES
WAS NOT WEARING LIFT JACKET WHEN CLEANING OUT DEBRIS FROM CUTTERHEAD, FELL INTO WATER AND DROWNED.
INDIVIDUAL WAS FOUND DEAD IN SUMP.
