While driving a golf cart west on the east/west road near the finish mill, golf cart proceeded through north/south road intersection near the 2-story office building and was struck by a cement tanker traveling south on the north/south main road.Cement tankers waiting to loaded and parked along the main road may have obstructed employee's view.

EE was lifting a 100,000 lb A-Frame with the 100 ton Link Belt crane using two 7,300 lb nylon chokers. one of the chokers slipped shifting the load to one choker which broke allowing top of the A-Frame to fall across the cab with the injured employee inside.

Employee's thumb was caught between forklift cage and handrail as he was attempting to hook cable choker around forklift to remove welding machine from upper deck of rail loadout.

Note: Accident initially was not a MSHA Reportable. While using the vacuum truck to clean up a spill behind #l Mill, the vacuum hose suctioned itself to the employee's right leg consequently pulling his legs apart which caused him to slip and twist his left knee.

EE was operating his Komatsu truck and did not notice that someone else has inflated his lumbar support. After driving his truck 6.5 hours, he noticed his back was bothering him a day before.

Employee was reinstalling cover plate on roller pair. He was holding plate with one hand and installing bolt with other when it rotated and struck him on the arm.

The haul road on hill was washed away leaving a large pothole. Driver was going up hill and truck down shifted right as tire hit pothole causing seat to slam down on frame, resulting in a pulled muscle in his back.

Employee removed an inspection plug from a rotary feeder while it was still in motion. A short moment later he was distracted and turned his body. When he turned his left index finger entered the inspection plug making contact with the rotating vane assembly. He suffered an avulsion on the tip of his index finder.

Rock slid down face as shot was being dug striking the rear of the truck employee was operating. On investigating, there were no witnesses to actually verify vehicle being struck.

EE was loading out of a tight shot. Due to the way he was operating the loader, he was beating himself up inside the cab unnecessarily.

Driver had an empty truck, returning from primary, running over pot hole. He jarred his neck. Continued to haul when he began to have increased pain and stiffness.

Stemming a boulder from a loader, when the loader moved the boulder, the boulder fell onto the right leg, fell & twisted right leg/knee.

Employee drove Front end loader through drainage ditch, loader bounced and caused lower lumbar strain. Doctor released employee to light duty.

Ee was under truck removing drive shaft to repair fuel pump in gas tank. Parking brake was engaged but when drive shaft was removed the parking brake released. With the tires not being chocked and parked on a slight grade, the truck began to roll back. Ee is scheduled for consultation on 8/3/04 to have small bruise drained.

EE WAS RUNNING A VOLVO LOADER AT THE SURGE PILE. A LARGE ROCK SHIFTED IN THE BUCKET, CAUSING THE LOADER TO BE JERKED OR JARRED AROUND. THE EE FELT PAIN IN HIS LOWER BACK JUST AFTER THIS HAPPENED.

Employee fell in a ditch while avoiding an out of control customer dump truck. The incident aggrevated an existing back condition.

EMPLOYEE WAS UNLOADING A RAIL CAR OF MATERIAL. HE HAD CLIMBED ON TOP OF RAIL CAR TO CHECK IF 3RD POD WAS EMPTY, AT SAME TIME RAIL CAR WAS OFF BALANCE AND STARTED TO ROLL. EMPLOYEE JUMPED OFF OF CAR LANDING ON GROUND INJURING BOTH WRISTS.

EMPLOYEE DEVELOPED BACK PAIN WHILE OPERATING AN OFF-ROAD TRUCK. HE DID NOT REPORT UNTIL AFTER HIS TERMINATION ON 3/9/04. HE REPORTED ON 3/23/04 HE WAS UNABLE TO PROVIDE SPECIFIC DETAILS THAT LED TO HIS COMPLAINT.

WHILE LOADING SHOT ROCK ONTO HAUL TRUCK, A PIECE OF ROCK BROKE LOOSE FROM THE HIGHWALL AND FELL ONTO LOADER BUCKET. IT CAUSED HIM TO COME FORWARD IN THE CAB AND HE HIT THE BOOM CONTROL. HE BOUNCED VERY HARD AND BACK UP AGAIN JARRING HIS BACK.

WE BELIEVE THAT EE WAS WALKING FROM THE FUEL PUMPS TO THE SHOP.

WHILE ATTACHING THE CYLINDER TO THE BOOM ARM THE SUPPORTING CABLE SLIPPED. EE S RIGHT HAND WAS INT HE PINCH POINT. MIDDLE FINGER ON RIGHT HAND WAS CRUSHED PARTIALLY AMPUTATED.

WAS DRAGGING A FAN ASSEMBLY WITH BOOM TRUCK HOIST, BUMPED INTO A TABLE OR THE GRILL WHICH WAS LEANING AGAINST THE TABLE & THE GRILL FELL HITTINGHIS LT. KNEE.

WHILE ATTEMPTING TO MOUNT A MOVING RAILCAR, THE VICTIM FELL AND WAS STRUCK BY RAILCAR, CAUSING LACERATIONS AND CONTUSIONS TO LEFT HIP AND PARTIAL AMPUTATION TO LEFT FOOT.

EE WAS ORGANIZING A STOCK OF 4" PLATES. FORKLIFT OPERATOR WAS LIFTING PUNCH PLATE WITH FORKS WHILE EE WAS SLIDING THEM INTO PLACE. WHEN EE SLID THE PLAT, IT SLIPPED OFF THE FORS, CATCHING EE'S HAND BETWEEN 2 PLATES.

OPERATOR WAS DRIVING LOADED HAUL TRUCK TO CRUSHER WHEN HE FELL ASLEEP AND STRUCK BERM ON SIDE OFHAUL ROAD CAUSING TRUCK TO TURN OVER.

EE WAS OPENING THE TAILGATE FROM THE DUMP TRUCK. THE TRUCK ROLLED BACK AND EE WAS PINNED BETWEEN THE TRUCK AND A PILE OF DIRT.

EE WAS BACKING 100 TON TRUCK TO A BERM ON LEVEL #1 IN ORDER TO DUMP A LOAD OF RIP-RAP INTO THE BASEMENT. UPON CONTACT WITH REAR TIRES TO BERM, LEVEL BEGAN FALLING AWAY, STARTIG WITH RIGHT REAR SIDE. THEN ENTIRE BERM FAILED CAUSING TRUCK TO FALL BACKWARDS OVER HIGHWALL.

LACK OF UNDERSTANDING THE TASK BEING PERFORMED AND IMPROPER POSITIONING.

AFTER OPERATING THE FRONT END LOADER FOR THE DAY, AT THE END OF THE SHIFT, EE FELT A SEVERE PAIN IN HER LOWER BACK. SHE CALLED HER SUPERVISOR TO HELP HER GET OFF HER LOADER.
