Employee stand next to co-worker who was using a jackhammer to unplug feeder in 10B Slag Cement Silo when a lump of slag came loose in the silo causing jackhammer to slip out of the co-worker's hand and hit ee in the l. lower torso. Accident occurred on 9/4/07, He did not report incident or seek medical treatment until 9/7/07.
Employee was discarding a bag of garbage that contained a peace of metal siding. When he attempted to throw bag into the dumpster it reflected off the side of the dumpster and bounced back. The piece of metal that was in the bag stuck through, cutting the employee on left bicep.
In the process of washing rock off of Grizzly Bars, the Bars became slippery. Individual had finished cleaning and was proceding to climb out of unit when he slipped before exiting. The individual landed on his back and right elbow. 5 ribs were hurt and a lacerated elbow.
While using chain wrench, breaking loose backhead from hammer, he felt slight pinch and pushed wrench in wrong direction to release pressure.
Employee was climbing down the ladder of the water truck. His finger got caught on the bracing of the ladder, lacerating his pinky finger on the outside of his right hand. Employee received stitches from local emergency room.
While driving a golf cart west on the east/west road near the finish mill, golf cart proceeded through north/south road intersection near the 2-story office building and was struck by a cement tanker traveling south on the north/south main road.Cement tankers waiting to loaded and parked along the main road may have obstructed employee's view.
The plant maint crew reported to complete repairs on screen. As 1 EE was cutting loose a guard with a torch, another EE was wetting down the urethane screen media with a water hose, which is proper procedure. A piece of slag fell down the sand flume, starting a fire from below. (30 min to extinguish fire). MSHA immediate notification was made.
Employee was stripping forms. He slipped and fell backwards and the form fell on his leg. He did not report injury till 3 days later when he was crawling on rebar. He complained to Supt. about some pain 6/14/07.
Employee was helping with changing impact crusher hammers. He got his left little finger smashed between the hammer wedges. His injury required stitches.
EE was walking beside rail road track, checking in rail cars that had come in the previous night. While walking on rail road gravel, EE twisted ankle, falling to the ground.
Employee was handling a rubber hose which places concrete. Rubber hose was being cleaned out after use. The rubber hose "flopped" around when being cleaned, striking employee in left inner arm (bicep area). Causing a contusion.
While socketing cable clive pin it suddenly went free into place causing partial severing of fingertip (right ring fingertip).
EE was lifting a 100,000 lb A-Frame with the 100 ton Link Belt crane using two 7,300 lb nylon chokers. one of the chokers slipped shifting the load to one choker which broke allowing top of the A-Frame to fall across the cab with the injured employee inside.
Employee was cutting a bore made of steel from a Euclid truck using the air arc. Bore came out of the tube and crushed his left pinky finger
Employee was assisting in removing bearings from a screen. He took a 4 lb. hammer and struck the bearing race so the bearings would break loose, when a piece of metal entered his right eye. The employee was not wearing safety glasses.
While performing maintenace on Hazemag crusher; unexpectly, a hydralic hose bust very near employee's left hand. Even thought employee was wearig a protected leather glove the force of the hydraulic hose pressure penetrated the glove and injected hydraulic fluid into his left index finger.
Employee's thumb was caught between forklift cage and handrail as he was attempting to hook cable choker around forklift to remove welding machine from upper deck of rail loadout.
Employee manually handling a 8 foot long piece of channel iron which "rolled" up onto his right ring finger causing a fracture to finger.
While removing the guard off the oversize belt tail pulley, employee caught his ring finger (right hand) between guard and the guard mount.
Employee was removing dust buildup from around lower door of calciner. EE said it was a very windy day and when he was removing the material it blew back into his face and eyes. The dust high of lime caused redness to both eyes.
While cleaning under the feeder motor with a chipping hammer the tip, of the chipping hammer broke off and hit his left thumb a piece of metal embedded in his thumb and it had to be medically removed.
While moving blowbars next to south Hazemag crusher EE used a 3 ft prybar to lift when prybar slipped falling on top of his right foot. This caused a laceration that needed 13 stitches to close. He was wearing steel toe boots.
Employee was supervising calciner wall fabrication on 3rd floor of preheater tower, wind was blowing at high gust causing dust to blow into face.
Shoveling catwalks, felt back pop.Treatable first aid. Pain persisted through weekend, went to Dr. 10/23/06 for eval. Eval. was back strain, Dr. recomendended phy. ther.& continue working. Increased back pain, returned to Dr. 12/12/06. Dr. rx MRI, time off for 2 weeks. Emp. was re-eval. 12/14/06, following reading of MRI,was released to return back to work 12/18/06.
Employee was swinging a sledgehammer to assist in the removal of a crusher bowl liner, when his elbow struck the edge of the spider rim causing a laceration.
Lower back strain when lifting a box of paper. Went to the doctor and was release for duty. Had follow up visit on 10-18-06 and was prescribed 12 session of physical therapy.
Employee was releasing water hose from water truck to wash out bed of customer's truck. Employee was unreeling hose when his left index finger got caught in the reel. Employee suffered laceration to index and had to have stitches. Employee was wearing PPE (gloves).
Note: Accident initially was not a MSHA Reportable. While using the vacuum truck to clean up a spill behind #l Mill, the vacuum hose suctioned itself to the employee's right leg consequently pulling his legs apart which caused him to slip and twist his left knee.
EE was helping put in mainframe bolt on crusher. Another ee dropped the bolt in place. When the bolt dropped a burr on the bolt caught EE's thumb and lacerated it. He received 11 stitches.
Employee was removing the old worn teeth on a 988H loader bucket, when he struck the tooth with a hammer and a piece of metal broke free and lodged in the employee's upper right thigh. Eight stitches were required to remove the metal fragment.
In process of replacing blown hydraulic hose, he fell off tower deck from approximately 4 feet striking a wrench laying on ground and injured his left arm.
Haul truck operator was driving a 70-ton haul truck when she noticed smoke coming from the engine compartment. She quickly left the operator cab and started climbing down the ladder. She jumped from the ladder approximately 6 to 8 feet from the ground and sprained her leg. The employee applied ice to her knee that evening and returned to work the next day.
Employee was chipping burn bolts when a piece of chipping hammer came off, pierced his glove and became imbedded in his left hand.
Picking up a box of orange boosters, felt something pop in left side of back.
While climbing over ramp berm employee lost footing & put arm down to break fall. Employee hurt his wrist but refused medical care. One week later employee went to personal physycian ( July 1st) and had X-Ray performed revealed a fracture of the left wrist. ( employee through out week refused medical treatment numerous times)
Excavator operator was lifting pipe section for splice when pipe swung and mashed EE right hand against his knee causing his fingers to jam and possibly a hairline fracture in his small finger knuckel.
Using heavy duty single speed drill bit to redrill bolt holes in 988F lift arm, drill bit grabbed and jerked drill motor out of the employee's hand, causing strains to both wrists.
While reaching overhead to remove 1 1/2 ton chain- come-along using one hand, After unhooking come- a-long slipped out of hand, landing on top of other hand.
EE was operating his Komatsu truck and did not notice that someone else has inflated his lumbar support. After driving his truck 6.5 hours, he noticed his back was bothering him a day before.

WHILE USING CUTTING TORCH SPARK FELL INTO LEFT EAR. EMPLOYEE SAW LOCAL DOCTOR AND NO TREATMENT WAS RENDERED IN MAY, JUNE, OR JULY. CONDITION PERSISTED, OBTAINED SECOND OPINION, MINOR OUTPATIENT SURGERY WAS PREFORMED ON 8/24/06.
EE was installing under table plates. The plate was bumped by another employee and fell in the mill through the beam opening. It struck EE knocking him down. EE got back to feet, came out of small door and was going down stairs. Another employee made him lay down to check injuries, rescue was called.
Employee was reinstalling cover plate on roller pair. He was holding plate with one hand and installing bolt with other when it rotated and struck him on the arm.
Employee removed an inspection plug from a rotary feeder while it was still in motion. A short moment later he was distracted and turned his body. When he turned his left index finger entered the inspection plug making contact with the rotating vane assembly. He suffered an avulsion on the tip of his index finder.
Employee was removing lower thrust plate from cone crusher when it slipped and caught his finger between the plate and the crusher mainframe. Swelling of right index finger.
The haul road on hill was washed away leaving a large pothole. Driver was going up hill and truck down shifted right as tire hit pothole causing seat to slam down on frame, resulting in a pulled muscle in his back.
Employee was climbing back into sweeper truck on 4-18-06. He felt a pain in his left shoulder. He notified his supervisor & did not want to fill out incident report & did not want medical treatment. On 4-24-06 @ 6 AM he went to his supervisor & reported pain. Then requested medical attention.
