Electrical Contractor (ID K893) on our mine site was operating a F-350 pickup ascending a haul road when the vehicle suffered a brake failure and began rolling backwards. Individual removed their seat belt and jumped from the moving vehicle resulting in left lower extremity being "run over" by the L front tire of the pickup as it rolled backwards.

Mostrando 50 de 52Tail Pulley on conveyor was plugged and stopped the conveyor. Conveyor was locked out and gear box brake was set to prevent the belting from moving backwards. As material was removed relieving pressure, belting moved forward pulling miner into tail pulley.
Miners finger tip was mashed when caught between a drive belt and a pulley when trying to change the drive belts.
Employee received a cortisone shot to EE's left shoulder on 2/4/22 for an injury that occurred on 1/26/22.
Employee was operating a 988K loader feeding a stationary crushing and screening plant. The operator drove the loader over a large rock, adjacent to the muck pile, causing operator to lose control of the loader. This caused injury to the operator's back. The operator required off site medical treatment and time off per medical professional
Mixing materials with loader on 5/26/21, strained shoulder when driving into pile. Treated injury with stretching exercises. On 9/1/21, employee sought treatment with orthopedist, diagnosed with overextension of shoulder and prescribed physical therapy and given a cortisone shot.
Strain due to jarring motion from impact of material being loaded in dump bed of 50 ton haul truck. Injury initially reported as first-aid but worsened over time and advanced to seek medical treatment.
Loader operator was pulling material from a wall and part of the wall fell on the bucket lifting the back end of the loader to buck up and back down. Loader operator reported hitting their head on the headrest.
TM WAS DRIVING RENTAL WATER TRUCK AND BOUNCED THROUGH RUTTED AREA CAUSING PAIN IN BACK. PHYSICAL THERAPY PRESCRIBED TO STRENGTHEN BACK MUSCLES STARTING 10/14
The employee was trying to adjust a chain on the telescoping conveyor. The cable clamp holding the sliding conveyor in place came apart and the conveyor belt slid down crushing the employee.
Employee was adjusting an under guard on a cross conveyer. The belt caught the guard and pulled the employees hand in where EE received 12 stitches to the top of the right hand
A miner operating a CAT 980 loader developed back pain over the past several months resulting in a modified duty to take a 30 minute break for every 3 hours of mobile equipment operation.
The employee was on a conveyer belt. Harness was on. The employee failed to follow lockout tagout procedures. The belt was started and the employee jumped from the belt causing a hairline fracture to the right ankle. Employee is currently on light duty.
When returning to the excavator to be loaded the Articulating Truck ran through a wet area of the haul road under repair causing the driver to bounce upward and landing back into EE's seat abruptly resulting in injuring EE's lower back.
WHILE SEPARATING ELECTRICAL CONNECTION IN ENGINE COMPARTMENT, CONNECTOR CAME LOOSE, CAUSING EMPLOYEE'S RIGHT HAND TO STRIKE THE FAN ON THE ALTERNATOR, CUTTING A KNUCKLE.
Employee was stockpiling in same pile with another loader. As employee rolled into the pile to dump the bucket there was an uneven layer of material the employee did not notice which caused the loader to bounce resulting in an acute cervical myofascial strain to the neck.
Operating loader and hit a pot hole bruising employee's sciatic nerve.
Miner strained back and neck while operating skid steer.
Vehicle incident
While employee was operating a FEL he began to feel pain in his lower abdomen. He could not identify any event or specific time the pain began. The employee went to a local clinic where he was given prescription medication and placed on light duty for seven days.
Employee was driving a haul truck when he hit a dip in the road which caused his seat spring to compress and hit the floor in the cab causing a shock to his back.
Removing shaft from S-60 Derrick Stiff Leg crane draw works. Shaft slid inside and at the same time shifting weight upwards. Injury either tried to hold the shaft and it pulled him in or he did not have time to get out of the way, smashing his left forearm between the shaft flange and draw works.
Employee was operating a 988H loader to load a haul truck with aggregate materials. The ground the loader was operating on was unprepared and uneven. While driving loader on this terrain the employee reported his lower left back started to ache from the bouncing and jerking around inside his loader.
Working on pre-screen that was deenergized, removing mud buildup from the belt. The belt rolled and caught right hand (last two finger tips) between belt and roller. Worker had gloves on.
Cleaning sand from under conveyors w/ a telehandler. Employee turned to dump out the material in a pile. His right front dropped into a soft spot and the telehandler flipped over onto its side.
Climbed on loader to grease bearing while conveyor was moving left arm & shoulder got caught between return roller and belt.
Employee was operating a manlift. A loader backed into the manlift at the operator's basket. The employee received broken bones to his left foot.
EMPLOYEE WAS REPLACING CHAIN ON CHAIN DRIVE OF TRAVEL UNIT FOR STACKER. AS EMPLOYEE WAS FITTING CHAIN HE PLACED HIMSELF BETWEEN DRIVE & STACKER WHEELS. RIGHT FOOT WAS UNDER WHEEL AND TRAVEL UNIT WAS ENGAGED. CO-WORKER FOUND EE TRAPPED UNDER STACKER. FRACTURED TIBIA AND RIGHT FOOT.
MOVING CONVEYORS WHEN CHAIN ON LOADER BUCKET SWUNG OUT AND STRUCK EE ABOVE RIGHT EYE CAUSING LACERATION NEEDING 4 STITCHES.
Breaking loose rock bridge off the conveyor. He started conveyor to move the large rock. Rock rolled about 10 feet down conveyor striking his left lower leg.
AFTER SHOVELING OUT TAIL PULLEY, EE WAS WALKING UNDER AN OVERHEAD CONVEYOR WHEN A ROCK CAME OFF THE OVERHEAD CONVEYOR & STRUCK THE EE IN THE BACK OF THE HEAD AT THE BASE OF HIS HEAD UP TO 3" ABOVE THE NAPE OF HIS NECK.
WHILE EMPLOYEE WAS OPERATING A CAT SKID STEER LOADER CLEANING OUT UNDER A SCREEN DECK, THE LOADER LUNGED FORWARD AND IN AN ATTEMPT TO KEEP FROM GOING FORWARD, HE PULLED A GROIN MUSCLE. HE WAS INJURED BECAUSE HE FAILED TO WEAR A REQUIRED SEAT BELT WHILE OPERATING THE LOADER.
EE WAS CLEANING ROCKS FROM TAIL PULLEY USING HANDS INSTEAD OF A SHOVEL. THE BELT ROLLED BACK ONCE THE CLOG WAS CLEARED CAUSING EE TO PINCH HAND & ARM BETWEEN THE CONVEYOR AND THE HEAD PULLEY.
EE CAME TO WORK 10/7/03 COMPLAINING OF BACK PAIN. HE WORKED 10/8/03, 10/9/03 & 1/2 DAY 10/10/03. HE WENT TO THE DOCTOR 10/13/03. NOT KNOWING WHAT CAUSED HIS PAIN HE PLANNED ON USING HIS UNION INSURANCE. THE DOCTOR RELEASED HIM BACK TO WORK WITH NO RESTRCITIONS ON 10/15/03.
EE WAS OPERATING LOADER. CLAIMS "ROUGH RIDE" CAUSED BACK PAIN.
WORKER WAS TRAVELING TOO FAST DOWN AN INCLINE. HE ATTEMPTED TO MAKE AN "S" TURN AND ROLLED THE BOX OF HIS TRUCK 360' AND LAY THE CAB ON ITS SIDE. HIS THIGH HIT THE STEERING WHEEL BRUISE IT.
WHILE OPERATING A SKID STEER LOADER THE WORKER HIT A BUMP DOING TO FAST RESULTING IN A LOWER BACK INJURY THIS INCIDENT IS UNDER INVESTIGATION TO DETERMINE IF THIS HAPPENED AT WORK OR AT HOME.
THE TRUCK WHEEL IMPACTED A BERM CAUSING THE OPERATOR TO IMPACT THE INSIDE OF THE CAB RESULTING IN A SHOULDER BRUISE AND INJURY.
EE WAS OPERATING LOADER TRAVELING OVER ROADWAY AND HIT LARGE RUTS IN ROADWAY WHICH CAUSED PAIN IN HIS RIGHT SIDE OF NECK, LOWER AND UPPER BACK.
OPERATOR WAS IN A BOBCAT SKIDLOADER & GOT WHAT HE THOUGHT WAS DUST IN HIS EYES. 2 DAYS LATER HE WENT TO DR. AND HAD A PIECE OF ROCK IN HIS EYE
I WAS OPERATING THE LOADER. THE REAR WHEEL FELL IN A HOLE CAUSING THE LOADER TO SWAY. I BRACED MYSELF IN THE CAB BY EXTENDING MY ARMS TO THE INSIDE SIDES OF THE CAB, ABSORBING THE SHOCK. NO IN JURY WAS SUSTAINED AT THAT TIME. HOWEVER, MY SHOULDER STARTED HURTING 2 HOURS LATER.
EE WAS OPERATING A LOADER ON ROUGH GROUND; STRAINED HIS BACK.
LOADER RAN INTO THE FRONT OF WATERTRUCK CRUSHING IT IN. WATERTRUCK DRIVER WAS ABLE TO GET HIMSELF OUT OF WATERTRUCK. EE WAS TAKEN TO EMERGENCY CENTER AND CHECKED BY DR, DR RELEASED EE TO RETUR N TO WORK AS HE FOUND NO INJURIES. NO X RAYS OR PRESCRIPTIONS WERE ISSUED. 3 WEEK 6-18-01 AFTER THE INCIDENT THE EE STARTED EXPERIENCING PAIN AND RETURNED TO THE DR FOR A FOLLOW UP. DR ORDERE
CLEANING THROUGH ROLL WITH HAMMER. HAMMER SLIPPED AND WENT THROUGH ROLL. ARM WAS STUCK BETWEEN BELT TROUGHING ROLL.
WHILE DROPPING RAIL CARS THE INDIVIDUAL HAD TO DISMOUNT THE MOVING CARS TO GET TO ANOTHER CAR TO SET THE BRAKE. THE AREA WAS SNOWY AND ICY AND WHEN HE DISMOUNTED HE SLIPPED AND FELL ON HIS KNE E CAUSING A SPRAIN.
EE WAS SITTING IN DUMP TRK (CAT773B#3) WHEN THE LOADER OPERATOR WAS DUMPING 2ND SCOOP OF QUARRY RUN MATERIAL INTO TRK & A ROCK FELL OUT & HIT CANOPY ABOVE CAB, SHAKING EE
CLEANING ROCK FROM AROUND A BELT WHEN A 4" ROCK FELL FROM FEEDER AND BELT AND HIT HIM IN THE BACK.
SUBJECT STEPPED OUT OF THE BOBCAT WHILE IT WAS RUNNING. HE PLACED HIS FOOT ON BUCKET CONTROLS CAUSING THE BUCKET TO RAISE AND PIN HIM AGAINST THE MACHINE.
WHILE WORKING, WENT TO REMOVE A RIPPED PIECE OF CONVEYOR BELTING. WHILE PLANT WAS STOPPING, THE RIPPED BELTING FELL BETWEEN COUNTER WEIGHT AND GOT CAUGHT. THE RIPPED BELTING WAS WHIPPED AROU ND. IT TAGGED THE LOWER LEGS, CAUSING WELTING AND CUTS.
A PICK UP USED TO TRANSPORT THE INDIVIDUALS TO THE WORKSITE WAS DRIVING ON AN AREA WHERE TOPSOIL HAD BEEN RESPREAD AND ROCK PICKING WAS IN PROGRESS. THE DRIVER DID NOT SEE A HOLE LEFT BY A ROC K AND IT IT WITH THE PICK UP. THE PASSENGER, NOTEXPECTING THIS, WAS BOUNCED AROUND AND HIT HER HEAD ON THE ROOF OF THE PICKUP CAUSING THE INJURY.
