Employee stated that EE was checking and repairing the screens in the shaker. EE was in a seated position and proceeded to stand up when EE slipped. EE reached for the belt with EE's right hand to catch self. The belt moved and EE's finger contacted the pulley causing a laceration to EE's right 5th digit. The plant was not in operation at the time of the incident.

Employee was using a crowbar to put tension on the new V belts on the HP-400 Crusher; the crowbar slipped out of place and struck them across the upper facial area.

The IE was operating a stand-up skid loader on a concrete pad removing spillage. Employee was dumping a bucket over the edge of the pad, machine traveled too far over the edge of pad. Machine tilted forward causing the right ankle to become wedged in a gap on the machine.

The employee entered the feeder/jaw area with fall protection to remove a large boulder that could not be crushed. After accessing the area by lifting a safety gate, the employee began clearing smaller rocks. While sitting down taking a break to drink water, they accidentally contacted the gate, causing it to fall onto their left knee and resulted in a laceration.

The employee was cutting a hose with a utility knife when the blade passed through the hose and lacerated the employee's right forearm

While performing overhead hydro-blasting operations in the hopper area of the scrubber, hardened material adhered to the scrubber wall became dislodged and fell from the scrubber wall. The falling material struck the employee on the left hip area.

EE was taking door off coal hopper when they removed top bolt the bolt on the bottom of the door was loose causing the door to swing down onto the thumb

Employee was lifting and turning their leg when it broke. This is the employee that had previously broke their leg at their house in August 2025 and it's the same exact leg. It fractured in the same location of a previously installed screw.

Employee was prying out some fender bolts in the process of removing the bolts the pry bar slipped causing the right index finger getting pinned between the pry bar and equipment frame.

Employee was removing a pulley wheel of the primary shaker when the chain hook broke off striking the employee in the face area. The employee needed eye surgery to repair the eye.

The injured employee and another employee were attempting to clear out a plugged jaw discharge chute. While hitting the chute with a sledge hammer in order to dislodge the plug, the injured employee hit the edge of the chute, pinching a finger between the chute and the handle of the hammer, fracturing the tip of their finger.

Employee had MSHA STANDARD THRESHOLD SHIFTS (STS) during hearing test.

Coworker noticed employee appeared to be napping while on the telehandler. Coworker instructed crew member to wake them up before they got in trouble. Crew member alerted other crew members when they found employee unresponsive. The crew was able to wake =employee while crew member went to get a car. The crew loaded employee into the car and transported them to the ER.

Using box knife to make hole in belting. laceration to right thigh, 2 staples

The employee was descending the ladder from the top deck of the cone when a hand slipped approximately midway down, causing them to fall to the deck below. Reported to management 10/30 when diagnosed with a hernia.

Screen plant 'middle hopper'. Employee using tooling to remove material blockage, as the material stopped flowing. As the material dislodged, momentum forced employee's forearm, through tooling', into frame of hopper, resulting in laceration.

Employee was struck in the left leg by a screen that was dropped from an elevation. The screen contacted the handrail of the stairway, slid down the handrail, and struck the employee in the left leg as they were attempting to access the stairway. This resulted in an avulsion fracture to their left fibular head.

Employee was removing a large piece of sandstone from the primary pump. In the process of removing the sandstone the stone shifted causing the pinning of employees left hand.

Employees thumb was pinched between stand and component while adjusting stand. Fracture and laceration to thumb.

Employee suffered minor hip and arm pain, due to rolling down from the top of a railcar while wearing fall protection. Employee came into contact with railcar.

Helping wash material off mill head and put chains on head for lifting when chain head rolled on top of right foot. Cause a broken big toe on right foot

Employee was working on replacing skirting on this particular conveyor. Employee was kneeling down when they stood up and turned, resulting in a knee strain. As of 8/15/25 employee has an MCL tear right knee.

The employee was working with another employee replacing a conveyor roller. The injured employee was holding the roller as another employee was taping it in place with a 3-pound hammer. The injured employee moved their hand, and it was struck by the hammer.

The employee had removed the pin on the tooth and was tapping it with a small sledgehammer on top to dislodge it from the shank. In the process of striking the tooth, a fragment of metal splintered off the tooth, striking the employee in the thigh.

Employee was trouble shooting a station on the sand classifier when the lid of the station fell on EE's right causing the employee to experience pain in EE's hand.

Employee was stuck by shop bay panels due the high winds experience on that day. Employee started to have shoulder and head pain due to the impact of the flying object.

Upper Pier on K4 Kiln welding retention blocks. Stepping down from ladder and stepped on things underneath the ladder causing employee to twist and strain back. As of 6/24/25 Modifications to desk duty by Medical professional.

Employee was greasing an excavator, after finishing greasing employee started to retract the grease line. In the process of retracting the line employee lost control of the line causing the grease gun handle to whiplash striking the employee's left wrist area.

Employee was underneath the primary shaker area when the chute door was left open causing large rock to bounce out. In the process of this action employee was stuck back of the head causing a small laceration.

EE was working on a metal tube sleeve for protecting a sensor. Bending in awkward position felt tightness in lower back. Prescriptions prescribed.

Employee was making small adjustments to a v-belt drive system and pinched left index and middle finger between belt and sheave. Amputation of tip middle finger required.

The employee overreached a guard to spray a head-pulley with brake cleaner to prevent the conveyor belt from slipping. As employee was spraying the head-pulley the pulley became in contact with the spray can, causing the can to get pulled in. In the process employee fractured right thumb.

Using a shovel to clear blockage on belt. it slipped and caused laceration on left index- 1 stitch and glue

Employee was doing some maintenance on the primary transfer belt. As the employee was leaning over the conveyor structure, employee started to experience pain around stomach area.

Employee was in changing a return roller in the process of standing up employee started to experience pain in the lower back area.

EE was repairing a hinge on a metal storage box and felt discomfort in left shoulder area. Clinic was called and EE opted for self-care. On 07/15/2024 EE was seen by a medical professional and was returned to work with no restrictions and continued self-care. On 07/22/2024 EE returned for a follow up visit and was given restrictions and scheduled for an MRI.

On 7-3-24, employee woke up with a sore back. During the shift, they operated a 980m Front End loader. About halfway through the shift, they got off the loader to work on the wash plant. They got back in the loader and around 10:00 a.m., they informed the Plant Manager of a minor pain in their back.

The EE was desending a ladder on the side of the cooler above the cooler outlet as another EE was removing material from the cooler outlet below in a skid steer. as the injured EE was on the ladder the EE in the skid steer pushed hot material into water creating steam that rose to the level of the ladder causing burns to the injured employees arms.

An employee injured their right bicep while picking up a folding table that had fallen out of the back of a pickup truck while on the highway during transportation. The MRI from this past Thursday, 06/27, shows a lower right tendon tear on right arm. EE's surgery is being done today to repair the tendon.

May 9, 2024 at 0800 hours when a manlift operator ran EE's manlift, an 8008 JLG, serial number 0300310942, into the canopy covering the C35 conveyor belt coming out of C35 tunnel. The operator was pinned between the roof edge and the man basket hand rail.

The employee was airing up/checking the pressures of the tires of the water truck. After checking the pressure of one of the tires, the employee went to grab the air chuck to add air to the tire. As EE approached, the tire blew out. The employee was about 4 feet away from the tire, but the air pressure blew EE's safety glasses against EE's eyes and debris into EE's eyes.

Pinch point, right hand laceration (stitches) and chipped bone in right pinky finger.

At appromently 8:30 an employee was using a chop saw in the maintenance shop when the ee notice a fire on the wall. The employee extinguished the the fire and continued to work, at 8:47 the employee noticed the fire had reingnited. MSHA was called at 9:12

On 2-26-24 EE was using a MAG drill to drill holes on the inside frame of a stacker. In operation EE attempted to clean the metal that was stuck to the drill bit. The drill bit caught EE's glove entangling EE's hand into the drill bit. An employee that was assisting hit the emergency stop on the drill. The entanglement resulted in a laceration to right hand.

Employee suffered lacerations to right hand while training a conveyor belt.

On 2-7-24 between 5 and 6 am. employee was checking the 57-rock stacker during routine monitoring of plant. EE noticed that belt #1 feeding the 57-rock stacker had stopped. EE started making way to the control panel to shut off belt to prevent overflow. On EE's way to the control panel to shut it off, EE stepped wrong hurting their left foot around the achilleas area.

Employee was stepping from a paddle boat being used as a temporary access to get onto the dredge. The boat shifted in the water as they were stepping onto the dredge. EE pulled the boat back with right leg, felt their knee pop, lost balance, and fell onto their knee. The dredge was inoperable and staged close to the bank. The boat was located between the bank and the dredge.

Employee was walking down the primary shaker stairs when slipped and fell. Employee started to experience pain on right ankle.

Dislodging material with metal bar, left hand slipped coming in contact with frame. Lacerated left ring finger, stitches required.
