Accident type, without injuries

A BOLT OF LIGHTNING STRUCK A POWER POLE, STARTING A BRUSH FIRE. THE FIRE AFFECTED AN AREA 120 FT X 300 FT. NO STRUCTURES ARE IN THIS AREA AND THERE WERE NO INJURIES. THE FIRE WAS EXTINGUISHED AT 11:48 PM.

Fall to the walkway or working surface

While cleaning up around a ditch pump, EE turned on a water hose and while attempted to unravel it she slipped and fell on both knees. Did not seek treatment until 3/29/2006. The claim was protested based on time of medical treatment but later was determined on May 16, 2006 to be work related.

Struck by falling object

While the worker was pulling the pump case door, the work leader was pushing on the pump case door. The door slipped off the hinge and fell on the worker's right foot above the steel toe of his boot. The door hangs on a J-hook hinge pin. The hook either slipped out or was not engaged in the hole correctly. Worker employed by CES America, Tucson, AZ

EE was using a steam cleaner/power washer. He was cleaning off some equipment for repair. He had his hand and finger on the wand and trugger. As he pulled on the hose, the wand pointed downward and hot water spray struck him on the top of the foot.

Struck by falling object

Removing center cutting edge on RT 556 dozer, using torch to cut bolts. Employee was struck by a falling edge when last bolt was cut. Lacerations to head and shoulder.

WHILE STEPPING OVER A 12"" PIPE, EE CAUGHT RIGHT HEEL ON A CONCRETE SADDLE, SLIPPED AND FELL INJURING LOWER BACK. EE WAS RELEASED WITH LIFTING RESTRICTIONS.

WHILE SHEARING 1 1/2"" FLAT BAR AT THE SHEARING POINT, EE PLACED RIGHT HAND ON THE NOTHCING POINT. WHEN THE IRON WORKER CYCLED THE SHEAR CLOSED AND THE NOTCHING POINT CLOSED WHICH LACERATED THE 2ND AND 3RD FINGERS OF RIGHT HAND

EE reported he was walking on a wooden floor when an improperly secured trap door allowed EE to fall through to the next level. Note: EE lost one day of work, because of additional required x-rays by doctor.

While dismounting #465 water truck, employee injured his right foot when he stepped on a rock that was under water. The resulting injury was a fracture to a bone just below the little toe on the right foot.

EE slipped off rear bumper of service truck in the rain and injured wrist. Went to hospital for xray and was given first aid treatment. Was not diagnosed with a fracture until 03/02/05. Put on loss days status on 03/02/05.

While in the process of jacking a haultruck, the truck rolled forward and tipped the superjack up in the air. EE exited the superjack and landed on his wrist, fracturing bone and dislocating his left shoulder.

While cleaning drill deck. employee lifted a bit basket and felt pain in his right groin area. Restricted work began 9-2-05.

EE WAS DISMOUNTING A PENSKE SERVICE TRUCK WHEN LOST FOOTING & FELL APPROX 9 FT AND INJURED. EE IS AN EMPLOYEE OF THE ASARCO MISSION SITE. HE WAS WORKING APPROX 3 DAYS WHEN THE INCIDENT OCCURED.

Employee was blading when the grader hit boulder causing jarring motion. Employee did not seek medical treatment until May 5, 2005.

Injured was installing a shovel gantry cable. A choker cable was wrapped around the gantry cable & lifted into position. A crane was used to swing & pull the cable into the gantry equalizer. Pulling caused the choker to slip from around the gantry cable & crane sheave block struck injured in back.

Employee observed a co-worker standing with his back to a radiant heater. The co-worker's coveralls had caught on fire. Employee along with another employee assisted the co-worker in extinguishing the fire and removing his coveralls.

Employee reported he was trying to unchoke cone of the lime tank. Dry lime sprayed and got inside the employee's boots on his feet. Note: Injury became Restricted Reportable on 06/01/05 after doctor placed work restrictions on employee.

EE REPORTED HE WAS REMOVING AN OLD BEARING. EE'S PARTNER STRUCK THE BEARING RACE WITH A HAMMER AND A PIECE OF STEEL FLEW OFF AND HIT EE IN THE LEFT UPPER SHOULDER AREA. A SMALL PIECE OF STEEL (APPROX. 3/8"" X 1/8"") WAS REMOVED BY DOCTOR. EE WAS PLACED ON RESTRICTED DUTIES AT THE TIME OF THE REMOVAL OF THE STEEL.

INJURED STUMBLED ON A ROCK WHILE WALKING. AS HE REGAINED HIS BALANCE, INJURED FELT PAIN IN THE LOWER BACK.

Employee reported he was loading the silo ball bin with 5,6,7 overhead crane when dust fell into his eyes. Note: Injury became reportable restricted on 4-29-05 when Dr gave ee work restrictions.

EE WAS CLIMBING DOWN VERTICAL LADDER, SLIPPED AND FELL APPROX 4 FEET. EE LANDED ON METAL GRATING, SUSTAINING A RIGHT ELBOW CONTUSION.

Employee was climbing the ladder on haul truck on the second rung she hit her left knee on the first platform

Employee reported he was walking in front of the Air Conditioning shop when he fell on his right side hurting his left side/back. Note: Injury became a Reportable Restricted Injury when doctor gave employee restrictions on 03/09/05.

Injured was using a wrench on a valve handle. The handle broke and pipe wrench struck injured's right lower jaw. Injured states he momentarily lost consciousness. Injury resulted in minor soft tissue swelling of lower jaw.

While lowering cathodes into the press machine a cathode became lodged on one side causing it to tilt forward. The cathode bar was forced out of the rack striking employee on her hand.

Employee was repairing crack in dipper when he used chipper gun to remove dirt/mud off the dipper.

While removing a rotation head slide plate from between the head mounting bracket and mast, he caught his thumb between the rotation head bracket and the mast. The result was a tuft fracture and 3 sutures to the affected area.

EE was working on D10R in the radiator compartment and changing around two small pilot lines. The wind blew the radiator guard door closed and the door struck EE on the side of his right hand and pinched his hand. The fleshy side of the hand below the small finger on the side of the hand. Ernie stitched up.

Employee fractured right ring finger with hammer, took to clinic. Employee's were replacing bucket, tilt lever and center link pins on 950 G loader. Co-worker was knocking in pin to center link with hammer. Employee was holding onto the center link and the hammer deflected off of the loader frame, striking EE on his right ring finger.

EE WAS ATTEMPTING TO OPERATE A KNIFE VALVE THAT WAS STUCK. THE LEVER BROKE FREE, CAUSING EE'S RIGHT HAND TO DROP AND STRIKE A BOLT. LACERATED INDEX FINGER KNUCKLE - RECEIVED 3 SUTURES TO CLOSE THE WOUND. (VALVE ON WATER PIPE LEADING TO TAILS DAM.)

Employee was climbing off a 830 haul truck when he slipped. His arm caught a railing, causing an elbow/forearm strain.

EMPLOYEE WAS WALKING ACROSS DECK OF HAUL TRUCK, TRIPPED AND INJURED RIB AREA

WHILE REMOVING A BUTTERFLY VALVE FROM 566 DRILL, EE MADE SEVERAL CUTS WITH AN ACETYLENE TORCH. WHILE REMOVING A RUBBER HOSE THAT WAS IN THE WAY, EE PASSED HIS RIGHT HAND NEAR THE FLAME, CAUSING A BURN TO THE WRIST AREA OF THAT HAND. THE BURN SITE DEVELOPED BLISTERS THE NEXT DAY.

EMPLOYEE WAS STANDING UP WHEN HE HIT HIS HEAD ON THE DIACHARGE PIPEOF THE DREDGE MACHINE. ** Employee did not seek medical treatment until 3-09-2005. Claim is under investiagation has not been determined if a compensible claim.
