Accident type, without injuries

Haul truck 450 drove up a berm causing the truck to flip on its right side. The operator was unhurt. Accident is still under investigation.

Contractor (ID#LUO) was performing work by changing out clogged pipe lines that fed the Raconite holding tank. A cutting torch was used that set off vapors within the pipe that let to overpressurizing holding tank.

On 12/31/12 Employee was stepping down off of ladder he missed the last step placing his foot onto the ground at which time he felt a pop in his right Knee. Employee was treated as first aid. On 4/30/2013 Employer received MRI results that show Employee has a tear of the meniscus in the right knee.

Employee climbed out from under a rubber tire dozer and stood up. The sun was in his eyes and he lost his balance. He fell and his right hand struck a lock ring from a big tire that was laying on the ground.

EE was walking down stairway to lower level in crusher building. When employee stepped off the step onto the landing, he rolled his right ankle. During the examination of the accident, there was an uneven surface of built up dried mud on the stair landing. The uneven surface was crowned in the middle about 1 1/2"" high & sloped down to both sides, the width of the stair tread.

Employee was changing out the #5 diesel pump. Employee was climbing in and out of the stemming machine to assist his partner. Approx 1 hour after completing the task, employee felt soreness in his left knee. Due to days off, employee did not begin losing time until 11/12/2012

Employee was climbing up and down ladders all night working on the 944 loader. When he descended a ladder he felt a pop in his knee. Employee went in for surgery on knee 12/20/2012, becoming a lost time injury.

Employee was helping lift a shovel cable connector (pothead) and felt pain in left shoulder. Became a lost time when he went in for surgery 11/08/2012

HT operator was injured when the truck he was operating skidded into and struck the rear of a haul truck stopped on a ramp.

Employee slipped while dismounting the rubber tire dozer twisting his left ankle.

Employee was working on a loader in a tight space for an extended period of time causing a pinched nerve in his neck.

Employee was being loaded at the 579 shovel when the shovel dipper clipped the bed of the haul truck. The truck jolted causing the seatbelt to tighten up aggravating abdominal problems of the operator.

As employee picked up wheel barrow and began to move forward his left leg slipped and went underneath the wheel barrow.

Employee complained about pain in her forearms, shoulders in back due to excessive keyboard entries. Employee was injured on 06/27/2012. Did Not begin losing time until 09/03/2012.

Employee was pulling a water hose, and slipped on wet concrete.

Struck by falling object

Employee was removing the heel band with air arcing on the ES18 dipper. The crew antisipated the heel band was one piece but was worn and the heel band gave way before they expected. The heel band fell and struck the ground bouncing onto the employees left foot. Resulting in a fracture.

Employee was working on the blast pattern when he slipped. As he fell he caught himself with the right arm causing pain to his right shoulder. Employee worked normal job. Went in for surgery 10/22/2012, became lost time.

Flash burns (welding)

Employee reported a slight irritation to his eyes and slight headache. Employee was wearing all required PPE for welding. Employee refused medical treatment.

Over-exertion in lifting objects

Trying to load an acetylene bottle into a pickup, employee sustained an abdominal strain. Incident was first aid, incident became reportable when employee started to lose time on 6/12 due to surgical procedure.

Fall from machine

Employee was standing on the right rear platform of the rubber tire dozer. When he turned around he lost his balance and fell approximately 4 feet landing on his shoulder and head. Employee has a broken collar bone.

Employee states she was hit by the shovel dipper while being loaded.

Over-exertion in pulling or pushing objects

Mine personal moved electrical parts storage cabinets out of PM Bay for event. When they moved them back in they did not position then correctly. Tech had to reposition them by pushing and pulling them. Mine personnel was assisting with fork lift. He didn't feel any pain that day but the next morning (Sat.)he could not straighten his back. Cabinets weighed approx. 500-600lbs.

Employee states he sprained his right knee while walking south on the east side of the railroad scale and lost his footing. **Employee did not want to seek medical care at the time*** Employee was jogging during 04/14/2012 weekend and his right knee began to hurt.

Employee was using the press at the tank house, pressing section B-10. Employee fell off the east side of the press and hit his head on the concrete causing a cut to the back of his head and right knee fracture.

Employee states he was injured due to a jarring motion while driving a Haul truck.

Employee was attempting to unload an acid car. He turned around to turn off the air valve, when an air hose fell from the support rail and struck him on the back side of his head. This knocked him down onto the catwalk injuring his head, neck and back. causing pain.

Employee was driving haul truck, and he began the dumping process. When the bed reached 4' raise, the hoist cylinder failed causing the bed to lower abruptly. This caused a jarring motion to the employee. He felt no pain at the time, but later began to have stiffness in the upper back and neck. Resulted in neck strain and thoracic strain.

Employee was hit in the head when the end of the roller platform shifted in the trunion causing the upper edge of the platform to hit the employee.

Struck by flying object

The South Sag Mill was being raised using 4 ram jacks and contract employee was shimming the lifting cradle when one of the jacks failed and blew apart. The flying debris hit the employee.

Employee was walking across the deck of HT425. She tripped on some bolts that hold an inspection cover to the deck. She fell to the deck injuring her right knee.

Fall from piled material

Employee climbed the Berm to see how much room he had to dump. While climbing down berm gave way causing employee to twist his back, wrist, and cut to his thumb from using his hand to catch his fall.

Employee was climbing off 626 boom truck. He was using the hand rails and steps on the bed. When he stepped off of the bottom step his left foot was on a rock and when he let go of the handrails his ankle turned and he fell to the ground. He extended his left arm to break his fall and fractured his left wrist. The witness tried to catch him in time but was unsuccessful.

Employee was working on HT427 doing a tire rotation. They were in the process of repairing a tire. They lifted the lock ring into place and Employee hurt his lower back.

On 11/22/2012 employee was climbing stairs slipped 2 steps. In the process of trying to regain his balance his right arm was holding the handrail slipped and pushed his right shoulder upward. Patient was treated as first aid. On 1/10/2013 employee was seen by doctor for continued pain and referred for Physical therapy on shoulder for 2 weeks. Reportable as of 1/10/2013.

Employee's left ankle gave out from under her causing her to fall forward using her hands to brake the fall. Employee has a foot sprain.

Employee was turning around and started to walk forward. He missed the step and twisted his left ankle.

Operator was walking across the deck of a haul truck and tripped on a bolt falling on her right knee and hand.

Employee was running from angry bull while removing it from thickener.

While lowering cathodes into the press a sheet got stuck. Employee raised the cathodes and grabbed the stuck sheet with his left hand. The sheet fell and caught employees hand cutting him between the 1st finger and thumb. Employee received stitches on his left 1st finger.

Employee was trying to knock a piece of core out of the shoe with a rawhide mallet and sustained laceration on front side of L thumb requiring 4 sutures.

Employee was using a bostitch air nailer to put together a frame. He placed one nail in the frame and was using his left hand to pull frame into place in order to put in second nail. Nail grazed the top of frame and entered Barrys upper left wrist approximately 1/2"". Resulting in a puncture wound to the left wrist sent for evaluation.

Employee was at the 9D2420 when he came down off of dozer he placed his left foot onto the ground onto a 6""-8"" rock causing his left ankle to roll out. he felt pain and came in for evaluation. Left ankle sprain

Over-exertion in lifting objects

Employee was doing work consisting of moving 10' H rods from the rack to the pipe truck and moving mud buckets and bags to the mixing tank. On 2/21 he complained that left bicep was sore and swollen. Employee was assigned less strenuous tasks for the rest of the day. Employee chose to stay home on 2/21 and rest his arm. He went to his family doctor later that day, on the 22nd.

Employee was closing the door to the warehouse when a gust of wind blew the door shut on her finger.

Descending down ladder, slipped and struck her left side of rib cage on hand rail. Incident was first aid case, X-rays on 9/10/2012, revealed fracture of left rib.

Employee was working under HT458 wearing his safety glasses when dirt and debris fell onto his face. Employee rinsed face off and was fine. On 8/31/2012 employee reported to work that he had something in his eye. Employee was treated and released to full duty on 8/31/2012 for remove of foreign object.

Employee had a foreign object in his left eye that he thought was flash burn. object was unable to be removed.

Over-exertion in wielding or throwing objects

On 5/17/2012 employee was using a slide hammer when he repositioned at 40 degree angle to get a better position he felt a pop & burn in Right shoulder, treatment as first aid. 4 weeks later employee reported that while working on small tire shoulder began to hurt again. Medical treatment was ordered on 6/21/2012.

Employee was injured on 5/16/2012 did not become reportable until 5/31/2012. Employee was dismounting drill, she was on the second step and lost her footing and fell to the ground. Was evaluated for back pain and released. On 5/31/2012 employee was seen for ongoing pain. Dr. ordered physical therapy.

Struck by falling object

Employee was checking HT for power problems. Employee proceeded under truck to check engine while under truck he scraped some dirt off the underside causing dirt to fall onto his head and clothes. Employee brushed off dirt, continued to work. He did not notice any irritation at first, after about two hours of working he noticed irritation in his eyes,

Caught in, under or between a moving and a stationary object

Employee was changing out main crank pulley on haul truck when he pinched his finger between the impact drive and the frame of the truck.

Struck by falling object

While removing a 1/2 ton come-along from his locker, tool slipped and crushed his right ring finger. 11 stitches were required.

Mill repairman was cutting conveyor belt with razor knife. Knife slipped and the employee cut his left hand pinky finger. 8 stitches were required.

On 2/16/2012 Dr. diagnosed Employee with a standard threshold shift on audiogram performed on 01/10/2012. The hearing loss has not been ruled as industrial at this time.
