Accident type, without injuries

A fire was reported at the landfill. Water trucks were dispatched. The fire brigade responded several minutes later. Because of the nature of the landfill the fire fighters were not allowed out on the landfill. The fire was extinguished in approximately one hour.

Caught in, under or between a moving and a stationary object

While attempting to move an adapter for the 994 Loader that fell of off a Service truck, the adapter slipped from the hands of the employee pinning his 2nd finger of left hand between the adapter and railroad track. The resultant injury was an amputation of the 1st joint of that finger.

Fall onto or against objects

While climbing over a rail to access a work platform, EE lost his footing and landed scissor style on the top rail, injuring his groin area.

While conducting normal clean-up duties @ the Primary Crusher, employee experienced low back pain.

Struck against a moving object

While being loaded at the #516 Shovel, a bucket of boulders were put into the truck bed and caused the truck to move around causing employee to experience back pain. She started physical therapy on March 18th and then went back to the doctor and they took her off work on March 27, 2008.

Fall onto or against objects

While climbing off of the dozer, employee slipped and fell on the track hurting his tailbone. Amendment: EE began missing work 4/4/08.

Employee was inspecting flotation cells. When finished, jumped down 30? to catwalk injuring left knee.

Struck against a moving object

While operating #380 haul truck towards the 2850 dump, the truck started to lose power. As the truck started to roll back, employee applied the brakes and attempted to turn into the highwall. At that point the truck rolled onto its side. He received fractures to the right leg, wrist and a few ribs.

Over-exertion in lifting objects

Employee was breaking down medium duty size tires by hand when he felt a strain in his lower back.

Employee was descending concrete at Admin building. Near bottom of stairs employee stepped on debris twisting her ankle and fell catching herself on handrail.

While pulling on copper cathode to reposition it, the cathode fell on inj's right hand, resulting in a lacerated & fractured right long finger.

While EE was driving #391 Haul Truck, loaded with waste, to the 2970 Dump, he stated that he hit a depression on the dump; that caused him to rise in the seat and come down jamming his back.

Over-exertion in pulling or pushing objects

While unreeling shovel power cable from a spool, injured ""yanked"" on the cable and felt a pop and pain in the right shoulder. Injured began losing time from work on 8-13-07.

Over-exertion in lifting objects

While attempting to lift a bit basket from the drill deck, employee felt pain in his stomach area. He went to an Emergency room on May 14th, and on May 15th was told he had an umbilical hernia and was taken off work on the same day.

While attempting to force a gear shifter forward in order to dump a haul truck, EE felt a pop in his right elbow.

While standing on the ground watching the pull down chain being placed on the mast end sprocket,EE felt something go into eye.

Struck by falling object

Employee was using a port a power to remove a hydraulic cylinder from a haul truck when another employee dropped a plate used for a spacer that struck the employee in the ear.

While in the process of dismounting the prill truck to clean out the auger, employee did not realize that he was parked over a depression. He tried to catch himself and strained his right shoulder.

While installing a tramp cylinder on the #2 shorthead crusher, employee sustained a fracture to the fifth metacarpal bone on the right hand, when his hand was pinned between the handrail and tramp cylinder.

Fall down stairs

Contact made with employee on 7-12-07, confirmed that he did have surgery. Still do not have any medical data to support claim nor does our workers' compensation carrier. On 1-18-07 employee was walking down stairs, carring a box of tape, slipped on bottom step, fell to ground on right knee and right elbow.

Employee was walking down stairs and on the last step to floor twisted right ankle.

While engaging hydraulic drive on rope puller, the reel moved pinning hand between reel and frame of unit.

Employee and an assistant were lifting Rake blades to put in place for installation when on one lift he felt a strain in his back.

Struck against stationary object

Employee reported that he was checking on mechanics that were working on the 18 drill; when he was leaving the drill he walked under the cable-realer and hit his head. It jarred his neck when he hit the main frame of the drill. The injury did not bother him at the time. It started hurting the next day. We did not receive notification of injury until Aug. 17, 2007.

Employee was dumping ore from a wheelbarrow into a chute. While lifting wheelbarrow, lost center of gravity, causing wheelbarrow to tip over to the right side, striking left knee with handle.

Injured pulled up shovel access ladder & it stuck causing injured to feel pain in lower back. Injured was returned to work ""regular duty"" on 3/21/07. On 4/23/07 ""light duty"" was prescribed, making injury reportable.

Struck by falling object

Employee was cutting straps holding a bundle of timbers together. When the strap was cut all the timbers came falling toward the employee hitting his right knee and right ankle.

Struck against a moving object

While operating haul truck, haul truck canopy was struck by electric shovel dipper bucket causing employee to sustain whiplash-type movement in cab while wearing seatbelt.

While removing last bar in the cathode set, employee was using another bar to dislodge stuck bar and struck her finger with it. Causing contusion to middle finger of the left hand.

While using hand to strike plunger of onboard fire suppression system employee felt pain in hand and wrist.

While attempting to extinguish a small fire on #380 haul truck, employee twisted his right knee while running down the ramp to the fire.

Driver lost control of haul truck and the truck slid. The employee felt a popping noise to her left shoulder and felt pain.

Employee was helping unplug tail sample pump in the mill. He was up on the pump, had positioned a flex hose over the discharge pipe and was holding it in place. Pump was turned on, he was unable to keep flex hose in place and consequently hot mud sprayed on his right arm and right side of face resulting in first and second degree burns.

Employee was picking up a pallet and received a splinter in his left hand.

Injured stepped down from approx one foot high concrete foundation and twisted his right ankle.

While working on the cable crew pulling a loop of cable, EE felt a pop in his right calf muscle. Received treatment at an Emergency Room and the diagnosis is a possible muscle tear.

Employee reported that while trying to pull cable from under rebar tube, cable got stuck between ground and rebar structure. Employee tried to throw remaining 10 feet of cable over structure, in doing so sustained a right anterior thigh strain-acute.

Employee reported installing liners at ball mill; while handling a liner his right hand was caught between the outer and intermediate liners, smashing his right hand. This resulted in a contusion of the hand.

Employee was getting off bus at water stand and stepped on a rock and twisted her ankle.

Employee was stepping up from the number three crane man deck, up on to the trolley deck; he felt some tearing in his right knee.

Employee was climbing up the ladder to the 16 Drill carrying his backpack, water jug and hot gloves. When he reached up he felt a strain under his right rib cage.

Employee was changing points on the 16 shovel. When he bent over and picked up the point he felt a pull on the right side of groin area.

Employee picked up a four foot step ladder and turned, starting to walk when he tripped over a fork of the forklift. He fell and hit his head on the opposite fork.

While descending stairway in order to get to #13/#14 Ball Mill Cam Gate, employee stepped off of the last step onto the ground level and sprained his left ankle.

Employee had removed grating over drive to CV 02 -- when he blew out the hopper, went to pull air lance out when he stepped backwards he tripped over the grater and fell backward *** employee did not seek medical attention until 6-16-07

Absorption of radiations, caustics, toxic and noxious substances

Employee noticed acid hose was not connected to car properly and was leaking. He went to reconnect hose when it came loose spraying acid on the employee. Employee had the proper PPE.

While securing a load with a ratchet strap, injured felt a pull in his left wrist while pushing on the ratchet handle. Light duty was prescribed.

While dismounting the boom control area of a jig hydrolift, EE was facing away from machine and when his feet contacted the ground he jarred his lower back. The resultant injury was a strain to the lower back.

While stripping a thin copper cathode, the cathode fell striking & cutting injured's left thumb. Sutures were required.

employee was walking across the top of the ES 18 when he slipped and fell landing on his hands and knees

While walking up stairs in tankhouse injured stubbed his toe on a step. As he fell forward his left small finger struck a sharp piece of fiberglass on the mid rail of the handrail, resulting in a cut left small finger.

Employee was dismounting the cable reeler when he slipped and fell causing a laceration to his cheek.

While performing cleanup duties, employee was pulling a 2"" water hose and slipped, falling on his left knee. He was placed on restricted duty for approximately two weeks. Will report when brought back to regular duties.

Employee was doing his pre-op inspection. When he stepped from the cab to the deck he twisted his right foot.

EMPLOYEE WAS GETTING OFF RUBBER TIRE RIG WHEN HE LOST HIS GRIP AND FELL BETWEEN THE FUEL TANK AND THE BLADE SUPPORT.

EMPLOYEE WAS CLIMBING OFF LADDER OF HAUL TRUCK WHEN THE STEP BROKE CAUSING HIM TO FALL AND LAND ON HIS KNEE.

WHEN ON THE DECK OF #15 DRILL, EMPLOYEE WAS IN THE PROCESS OF STEPPING BACKWARD. WHEN DOING SO, HE STEPPED THRU AN OPENING IN THE GRATING ACROSS THE HATCH WITH HIS LEFT FOOT.

While dismounting loader employee slipped on ladder with right foot. Stopped his fall but caught his left foot on ladder. After about 10 minutes started to feel pain in left groin area.

An alleged incident involving a haul truck and a track dozer occurred. Several hours later after the shift had ended, the track dozer operator called and requested medical treatment for back pain. He was seen by the company doctor and was placed on restrictive duty for one day.

While closing a 10"" water valve, injured's feet slipped and he lost his grip with the right hand and caught his weight with the left hand. This resulted in a left wrist strain.

Employee reportedly was helping other workers move grinding rods in the mill. The crane pulled on the cables causing the rods to shift and in doing so, the cable and hook pinched employee's right ring finger resulting in an open wound that required sutures to close.

While charging the rod mills, employee got her hand caught between two rods. The results were a contusion to the hand. She was also prescribed a wrist brace to support the hand.

The employee was removing a thrust plate in an awkward position, the plate slipped and smashed his hand between the plate and the frame. The employee's right index finger was smashed.

Employee was cutting a 2 inch hose, overhead. When the hose fell to the ground the employee tried to hold on to it, causing a left shoulder sprain.

While guiding a pump case between the pump and stairway, employee pinched her right little finger between the pump case and the stairway hand rail. The resulting injury required sutures.

Employee was using a come along to change belts on a scrubber. He was backing off come along when it released, causing him to hit his left elbow on a metal plate. Received a 1 cm x 1/2 cm laceration.

Employee was walking to foreman's office when she stepped on uneven concrete floor, twisting right ankle. Employee did not have her work boots laced up.

Injured was using a shovel to dig out a process water line, felt a ""pop"" in his left elbow. He was referred for a MRI.

While tipping the hood of a dump truck injured felt pain in the left sholder adn around left side. Physical therapy was prescribed.

Employee was pointing out something in the motor, when a gust of wind spun the fan blade, which caught and cut the middle finger on his left hand.

While adjusting the jaw on a vise, Employee's left hand slipped off, jamming his finger against jaw adjustment rod. His finger was splinted and that is why it was reported.

Pressurized diesel fuel line released causing diesel fuel to spray in his right eye.

While removing a 4 inch fusion machine from under a pipe, injured felt pain in his right wrist.

During installation of the right track of a dozer, injured used a 16-pound hammer to reposition the zipper link. When the link was struck a small airborne piece of steel struck & embedded beneath the skin of his right abdomen.

While installing a latch bar on the #516 Dipper door, employee's left thumb was caught between the latch bar and the latch bar tunnel. He received 7 sutures in the thumb.

While operating #471 water truck providing dust control to #3 tailings dam, employee rolled the truck. The truck landed upright with minimal damage. The employee was treated and released for regular work. Physical therapy was prescribed.

Over-exertion in lifting objects

While lifting a lube pump off of his right shoulder and placing it into the tank hole, EE felt a pain in his right shoulder. EE starting receiving physical therapy on May 8, 2007.

Caught in, under or between a moving and a stationary object

While installing main frame liners at the primary crusher, ee's right bicep was caught between the liner and the wall. The resulting injury was a small bicep tear and possible tendon tear. Sutures were given.

Contact with heat

While conducting clean-up around E-16 conveyor in our Secondary Crusher, employee experienced dehydration issues. He was taken to an Emergency Room and later released with the diagnosis of Heat Exhaustion.

Contact with heat

Employee was replacing crowd motor on #17 shovel, Employee got overheated and had to be sent to the medical center for dehydration.
