While climbing up the ladder of a loader, employee slipped and caught themself with right arm. Complained of right shoulder pain. Was treated as a first aid, was determined later by MRI employee needed surgery to repair a rotator cuff tear in shoulder. Employee lost no time or had restrictions until 5/4/2017.

The driller had plugged up the pipe, then pulled back a joint to the table and broke it loose and raised the top head. The driller noticed the inner barrel was plugged and leaned forwards towards the table a little when trapped air pushed drill cuttings out of the hole and sprayed into face and eyes.
EE was unloading a super jack, when the jack handle swung abruptly striking EE in the left knee. EE sustained a stretched ligament and bruise and was put off work.
10/11/2016 employee dismounted the dozer and turned to take a step and felt left knee pop. Employee kept working, but knee continued to bother employee off and on. Employee opted for surgery and became a lost time 1/2/2017.
Employee received an electrical shock when EE grabbed the strongback and the crane block at the same time.
Employee tripped on raised concrete pad and fell to knees. Over time employee felt pain in right knee. A right medial meniscus tear was discovered, and surgery was performed on 3/21/2017. Lost time began 3/22/2017.
Employee was using an impact wrench when the bolt caught glove. This caused glove and middle finger to wrap around the socket causing a fracture to right middle finger.
Employee was called to the #4 drill to verify a low coolant light. While checking the coolant in the radiator, coolant sprayed onto the mechanic scalding hands, arms, nose and torso.
Employee was tightening flange bolts on discharge valve when EE felt pain in EE's lower back.
On 2/13/16 filterman was walking up stairs when EE tripped, and injured EE's right shoulder. The employee never reported the injury, and filed for disability and Worker's Comp. The WC claim was denied on 4/7/16. Employee requested a hearing, and after investigation it was determined to be a WC's claim. The carrier rescinded the denial on 8/12/16 and informed ASARCO on 8/16/16.
Employee alleges January 14, 2016 incident caused an injury to back that EE was not aware of. We feel this incident was not caused by an occurrence at work and is not work related. We are filing this report only to terminate a citation. We will contest this citation.
Employee was washing WT508 and turned the water valve off and it broke off and the water pressure knocked employee down causing a fracture to left arm.
Employee was climbing up ladder, when EE twisted to the left. Employee fell from ladder when sciatica nerve flared up. Employee struck right shoulder on a nearby heating unit. Employee returned back to work Regular Duty on 06/09/2016. Employee's follow up visit revealed a tear in right shoulder muscle which requires surgery. Employee began losing time on 06/24/2016.
Employee was using a bar to align cutting edges on a track dozer when EE heard/felt a pop in EE's elbow followed by pain, 3/11/2016. Referred to specialist, doctor took employee off of work 4/27.
While exiting the 416 haul truck the employee fell from the mounting ladder onto right arm. causing a hairline fracture to right forearm.
Employee placed hand on guard grate for stability while working at #6 acid tank, grating broke free and pinched left index and middle fingers causing lacerations and fractures.
While installing a sweep on PP02, employee was holding the pipe and EE's coworker was trying to hit the pipe into place with a hammer and struck employee's right middle finger. 8/18/2016 Dr. treated employee with a splint to EE's finger.
EE was changing drill steel on Drill 4. EE was guiding the steel over the hole in deck when steel swung over and smashed left hand between the steel and the carousel. Damaging left middle finger.
Employee was cutting a sticker off an electrical cord, when employee drew knife towards left hand and pierced left index finger knuckle. Employee received 1 stitch.
Employee was removing transmission from a truck, when the wrench slipped. This caused ee to strike a bracket and cut hand. EE received 4 stitches.
Employee was removing teeth from a shovel dipper when EE's finger got pinched between teeth as EE set it on a pallet.
EE tripped on an inclined grating, and caught fingers through grating bending hand forward. EE sustained a fracture to right hand.
Employee was shoveling, when the shovel slipped under employee's weight and employee struck hand on a hanging guard. This resulted in a laceration across left hand. The employee received 11 stitches.
The employee was loading a shovel tooth onto the bed of a pickup truck, and smashed left hand ring finger between the tooth and truck side rail. This resulted in a laceration to ring finger and required 6 stitches.
Repairman was trimming edge of a rubber panel for a double deck screen. The blade snapped and remainder of the blade went between left thumb and index finger cutting the left hand. The employee received 5 stitches.
The employee was working in the Mining Operations Dispatch office. The employee was getting up from the chair when right hand got caught between the chair arm and the bottom of the counter-top lip. This resulted in a laceration to right pinky finger and required 8 stitches and a tetanus shot.
Employee was striking a copper started in the stripping machine when a bent piece of copper cut the employee on the left wrist requiring two stiches.
Employee was walking around shovel when slipped in some loose rocks. Put out hand to catch fall and cut left palm. Had three stiches.
Employee cut right knee while cutting a conveyor belt. Employee was pulling the knife towards self while making the cut. No Lost Time, No Restricted duty resulted from this injury. Employee received 3 stitches.
Employee was sanding a piece of angle iron. The sanding disc grabbed the angle iron which moved back to the employee's hand striking the top left thumb. Injury did not result in a Lost Time or Restricted Work. Employee received 10 stitches.
Employee was in a hurry to unplug the crusher chute and cut right index finger. Required 5 stitches.
Employee was cutting a 1/2 inch air hose with pocket knife. The knife slipped and cut left forearm. Sutures were required.
While removing a bearing cap on a 4-15/16"" bearing that was about 4' above the #10 tripper, employee was working from floor level as the cap was lowered. It slipped from hands, striking employee in left leg causing laceration.
Employee sat down on a roller seat, the seat came out from under and ee reached back to catch self and got finger pinched between the seat and shop floor, causing cut.
Employee was Diagnosed with a Standard threshold shift in the left ear, per Doctor.
Employee was diagnosed with a standard threshold shift in hearing of the right ear per company doctor, M.D..
Employee was working on 10 tripper and started experiencing cramps in legs and vomiting. Was treated for heat exhaustion at emergency room.
Employee is diagnosed with Standard Threshold Shift in hearing, diagnosed by a doctor, in the left ear. The physician indicates the shift does appear to be related to workplace noise exposure. This STS is considered late-reporting due to the mishandling of paperwork.
