Employee was coming down the stairs from the maintenance shop lunch room when his left foot slipped from the step next to the bottom step causing him to fall to floor level injuring his left leg.
The employee was removing a set of wheel chocks and felt a ""pop"" in his right shoulder.
While removing a tramp release cylinder from the CR02 MP1000 crusher utilizing a come along, the cylinder tipped forward coming in contact with contract employees right foot causing a contusion to the metatarsal portion of the foot.
Employee missed last step while dismounting a company tractor. (MM 20244). Employee fell on left arm and sustained multiple fractures.
On 6/19/2013 employee had a first aid injury. Operator was coming down the ladder when his right foot slipped causing him to twist the right knee. On 1/9/2014 received notice that employee is having surgery on right knee with a causal relation to the first aid that occurred on 6/19/2013. Employee has been full duty up to surgery date of 1/16/2014.
Employee slipped while descending tailings embankment.
While hitting a shovel tooth with a double-jack, a piece of metal shot up/off and struck the employee on his left inner thigh.
Low support beams in a work area outside/near filter plant maintenance.
The employee was steam cleaning the drill. He was up on the mast of the drill. As he went to squeeze the handle to activate the cleaner, the nozzle jumped and he lost control of the wand. The high pressure water cut the tip of his boot and sprayed hot water onto his left foot causing 2nd degree burns to his foot.
Employee was assaying blast hole samples and she starting having pain in her arms.
Employee hit a rut while operating a scraper. The employee failed to adjust and set the air-ride seat. Employee injured his lower tailbone area.
Operator was pulling power cable for the shovel and hurt his right shoulder. On 11/4/2013 Employee saw company doctor and was placed on restricted duty. Right shoulder strain.
While assisting in the Auto Valve change on the #2 filter at the Tank House, employee's thumb got caught between the valve disc and body causing a laceration of the left thumb.
Employee was casting at the CR4 stockpile. When he curled the bucket back, it hit hard and caused the loader to shake, jarring his back.
Employee was moving cable when he lost his footing, and fell backwards on ground. Employee landed on right elbow felt pain in both elbows. Employee was treated as a first aid. On 1/9/2014 he sought medical treatment due to ongoing pain and was placed on restricted duty.
Electrician was pulling wiring through conduit when he felt a hot pain in his abdomen
Employee was dismounting road grader at the 2890 tie down and missed the last step causing him to fall backwards onto the ground.
Employee was guiding/signaling the crane operator that was hoisting a beam when the beam became lodged under another beam, when the tension was released the beam swung and hit the EE's hand breaking the first meta carpel bone in his right hand.
Employee went to lean on the concrete barrier and miss judged the distance. Employee fell to the ground and sustained a lumbar strain.
Employee strained back moving a 20lb hammer. Employee went to put it in the tool rack while getting ready to repair bed in the welding bay. Employee became a reportable injury on 12/20/2013 when company became aware that he was placed on restricted duty.
EE and another ee were loading a 4'X4'X1/4"" steel plate onto the bed of truck #185. EE was pushing the plate over the top of the tool box while another ee was on the other end guiding it to the bed of the truck when the plate came down pinching his middle index finger near the palm of his left hand. EE was wearing his gloves at the time of the incident.
Employee was walking out to replace the battery in unit 197 when he stepped in a small hole jarring his back. With the battery in hand and tweeting to the right, he stepped down off the step he was using, his back gave out. He went to the ground in pain and had trouble getting up. Employee sustained a lumbar strain.
Employee had a 9lb wrench fall from a elevated platform striking right ring finger and sustaining a fracture.
While employee was trying to break nose bars using another nose bar he smashed his finger causing a fracture, employee committee an unsafe act by not using the proper tool for the task.
EE was unscrewing a bale, the pipe turned in the clamp, the bale fell on his hand, crushing the hand between the clamp and bale.
Operator was disconnecting discharge hose at #4 acid station and hose blew off and sprayed him on side of face and back of neck causing employee to fall and sprain his wrist.
The truck shop boiler makers were assigned to clean up behind the bed repair area. Employee was directing using hand signals for EE on the forklift to separate two sheets of 600 plate. Employee signaled the operator to lift the plate to install a 4x4x4 piece of wood to place between the plates. While twisting and pushing Employee felt a pull in the lower center of his back.
Operator was walking to the porta johns when he twisted his ankle.
Employee along with 2 other MRT members were setting up a harness rescue with the training manikin. After they attached the lanyard to the hand rail they pushed the manikin over the edge. When the manikin fell employee right thumb got caught up in the lanyard. Employee was wearing gloves.
Employee was troubleshooting the PP01 cables and erroneously touched the high voltage pothead and contacted one phase of 4160 volts with his right hand.
Employee was riding as a passenger in VN975. The van was coming up the hercules ramp when it hit a pothole in the haul road and jarred the occupants of the van.
Employee cut right middle finger, while cleaning the tail of the bucket in the lime mill. Right middle finger was partially amputated after being trapped between the chain and sprocket of bucket elevator.
At the end of shift the operator was exiting the Porta-john at the Guzman Project work area when she stepped on a rock, causing her ankle to twist; she then fell to the ground. Ankle strain. Was treated as first aid. Became reportable on 6/6/2013.
Employee was assigned to rebuild the bed from HT406. During assessment he was sitting on a rolling stool. After inspecting the frames he rolled out and thought he was clear of the bed and stood up. This knocked his hard hat off he was diagnosed with a neck strain.
Employee was climbing over motor taking measurements, stepped on lip of motor and rolled ankle.
Employee was sliding metal plates from a pallet. Employee felt a pain in lower left arm bicep. Employee returned to work (restricted) 05/08/2013
Right tibia was fractured/employee was moving manlift to work site. He hit a bump in the road causing basket to lift up quickly. employees feet came off a basket of manlift when he came down he stuck his feet to the basket causing a fracture to his right tib.
While employee was hooking up the coupler he felt a pain in his back. Patient did not become reportable until 5/13/2013.
Received ICA claim on 5/8/2013. Employee reports on April 25, 2013 he injured his left hand. He saw a specialist and has been placed on restricted duty.
Employee performing pre-op on equipment. When stepping down off bumper with dried mud he slipped and fell to the ground. Employee thought he was fine and continued working. The following the day (Saturday) his back started hurting at home and went to the doctor. Testing revealed employee had contusion of the back and one rib with a closed fracture.
Rigging strap shifted while moving load striking the employee in the right arm. Employee returned to work (Restricted Duty) on 04/12/2013.
Employee's left hand spun around the extension injuring the tip of the middle finger while working on HT426 installing the left rear tire. Employee was using a one inch impact. Employee became reportable on 4/26/2013 when he was diagnosed with a fracture of the middle left finger.
Employee was carrying a battery to put in HT with another employee. When he felt a pain in his low back. Material treatment on 2/27/2013 for low back strain.
While boarding the truck employee injured his shoulder.
Employee was carrying boxes down the stairs when he felt a pop in his left knee. Knee strain
Employee was lubricating the bead of a Michelin tire for mounting and received a laceration to the middle finger of his right hand from a loose piece of metal sitting somewhere in the bead area of the tire.
Employee was setting up on a job and there was a gust of wind causing some dust to blow up and under his safety glasses. Employee had some dust in his right eye. Employee reported the incident immediately after he was done flushing his eye. Employee was sent to the nurses station to be check out. On 11/5/2013, employee was sent out for foreign body removal from right eye.
Employee had crawled between PP16 & PP17 at the regrind cleaner sump to get to the spray bar and repair it. While crawling towards the spray bar, employee caught his right index finger on the grating next to pumps and cut it.
Employees were using an air nail gun to frame the walls in the new ass in the truck shop. Employee attempted to nail his side when the nail gun went off and struck his lower forearm. Employee was checked by the nurse transported to clinic treated and released back to work.
Individual was removing wire tire with a pocket knife, it slipped cutting left hand which required stitches. Individual was not waring gloves.
Stepping out of the training room, must of stepped wrong because felt pain in right knee.
Officer was leaning over to pick up something while sitting on a chair and the chair rolled out from under her causing her to fall on the floor and hurt her wrist.
Employee was getting on the bobcat at the load out tunnel, he grabbed the right side safety handle and stepped on to the bucket. His left foot fell between the bucket and loader. Employee began to fall. He pulled hard on the handle pulling a muscle in his right shoulder. Dr ordered physical Therapy 3/28/2013
One employee was cutting a chain with a grinder and the other employee was holding the chain. The grinder got out of control and went across the employee holding the chain left hand cutting his three fingers. 12 stitches were required.
Employee was confirmed to have a STS on October 24, 2013 by the company doctor. This STS has not been deemed an industrial at this time.
Unknown, employee over a period of time of working throughout the concentrator has sustained a standard threshold shift of the left ear in comparison to audiogram of 04/17/2001
Employee was audio tested on 8/19/2013. On 09/16/2013, Dr. notified the company that this employee has a standard threshold shift on hearing. The hearing loss has not been ruled as industrial at this time.
On 2/14/2013 Company Doctor diagnosed employee with a standard threshold shift in hearing of the left ear. This has not been determined as work related at this time.
Employee was audio tested on 12/10/2012. On 01/11/2013 Dr. notified the company that this employee has a standard threshold shift on hearing. The hearing loss has not been ruled as industrial at this time.
Unknown, employee was found unconscious on stairway due to medical issue.
Employee reported chest pains and was taken to the emergency room. He was diagnosed with bradycardia and chest pains that were not work related.
