As the employee was ascending the ladder, felt a pop in left shoulder

Fall onto or against objects

An employee missed a step on the truck step ladder and fractured EE's right leg. EE was in the process of unloading a delivery to the Mission magazine when incident occurred.

Struck by falling object

Dump bed fell from cribbing causing multiple fractures to hand

Employee was performing EE's area work inspection when EE slipped and heard a pop in the right knee. Resulted in aggravation of old knee injury.

Fall from machine

Employee was dismounting from equipment. While activating the ladder to exit, they began descending before the ladder had fully reached the ground. As a result, the ladder accelerated rapidly. This sudden movement caused employee to lose balance and fall sustaining injuries.

When checking spigots at tailings walking, they lost footing and fell into a ravine/ditch onto their chest.

Operator was stepping off of pickup truck and right food stepped in a hole and rolled right ankle, resulting in a sprain. restricted duty began on 12/11/2025

Struck against stationary object

The driver was completing a pre-trip inspection on the trailer. Driver was crouched down and stood up, thinking they had enough clearance, and hit their head on the trailer. Driver was wearing a hard hat, which compressed their neck.

Over-exertion in lifting objects

Employee picked up a plywood bulkhead for a concrete form and when they turned to put it in place it caused a strain in their back.

Caught in, under or between a moving and a stationary object

When they placed a hand up to stop the door/panel from slamming, their left ring finger went into the hinge section. When the door closed it caused injury.

Over-exertion in pulling or pushing objects

Contractor employee was tightening bracket on the pulverizer when they felt their right thumb pull back

Caught in, under or between running or meshing objects

Employee was feeding a cathode through the roller when the end of the cathode grabbed the tip of their left middle finger and pulled their hand into the guard. Resulted in avulsion of left middle finger tip.

Fall to the walkway or working surface

Employee was walking in the tie down area around the water truck, when they slipped and landed on their right wrist, resulting in a fracture. Employee was placed on restricted duty.

Employee was walking in a dim area when EE tripped and fell over forks from a forklift, resulting in right shoulder pain. On 4/22/2025, employee was placed on Restrictive Duty

Caught in, under or between running or meshing objects

Injured employee was spinning a pulley sheave by hand to remove an impeller and caught two of EE's left fingers in the belt. EE's left middle finger was badly lacerated and EE's left ring finger sustained a fracture.

Fall from machine

While dismounting a trailer, Temp employee encountered a loose step causing EE to fall 3 feet onto ground scratch on left elbow and left foot pain resulted in left foot fracture.

Over-exertion in lifting objects

Assigned to change out a steering pump and in process used a come-along to hoist up pump into place, felt a pop in left bicep

Employee was washing under the 219 ball mill. Employee crossed over the 212 cyclone feed pump discharge line by stepping on the angle iron brace supporting the pipe. While stepping over, employee slipped and rolled an ankle. Employee was placed on restricted duty 02/18/2025.

Employee was using a hammer on a metal bolt when debris went into R eye

While installing a shock brushing in truck 143's lower A arm, EE went to knock it in with a hammer, EE missed striking EE's Right thumb. received 6 sutures

Employee was picking up a 5-gallon bucket of ore one handed when they felt a pop and pain in right shoulder

Employee was installing a new bearing using a 10lb sledgehammer when they hit their foot just past the steel toe. resulting in the fracture of the minimus pedis (Little toe)

Employee lost EE's footing and stumbled causing EE to lacerate EE's head on a steel window frame of a connex box. Employee received 6 Staples

Struck by flying object

On 05/08/25 employee struck in arm by foreign object did not report the incident. The following week the incident was reported. Foreign body removed. 4 stitches received. Notified 05/27/2025 that the injury was accepted as an industrial injury.

Employee was drilling a hole in a new disconnect box. The drill bit got caught and pulled the box into employee's hand. Caused a cut and tuft fracture to right index finger.

Employee was using 1 inch impact gun on bolt under truck. Bolt was not coming off causing the employee to adjust position. As employee was adjusting the impact gun it bounced back hitting employees face causing a small chip top of *R* tooth. 4/10/25 EE did elect to repair tooth, dental work completed

Employee was diagnosed with a standard threshold shift by a doctor on 09/18/2025. This hearing loss is consistent with noise induced in the left and right ear.
