Employees were repairing the latch bar on the shovel dipper. The employee was prying with a bar to loosen the piece of the bar. The dipper door moved and pinched off the fingers on the employee's left hand.

Employee was descending 10 foot ladder, upon reaching second to last rung pant leg got caught in table causing to lose balance and twist right knee.
Employee was climbing up the ladder of the acid trailer to unhook the airline in the crows nest after unloading. EE lost balance as EE was climbing up the ladder and slipped while trying to grab the bar/railing. EE did not fall all the way down the ladder but caught self on the ladder while falling causing strain on EE's body.
Employee was descending haul truck 418 stepping off the ladder to the ground. Stepped with right foot on small rock and rolled right ankle resulting in right ankle sprain.
Employee was walking down steps and lost footing, grabbed hand rail causing strain to left arm.
Employee was assisting crane man while lifting cathodes from the floor when employee felt a sharp pain in left elbow. 12/9/21 is date of injury restrictions were applied on 12/14/21 with no heavy lifting of left arm only.

While using a grinder, it caught the corner and the grinder kicked back toward EE resulting in a break in the skin of EE's upper leg
Employee was operating haul truck, haul truck rolled back while employee was on deck causing employee to grab hand rail and hurt right shoulder.
Employee was performing the monthly inspection on a fire extinguisher, while putting the extinguisher back into the bracket employee's hand caught part of the bracket causing a cut.

Employee Was Removing Driver Side Hoist Cylinder from haul truck. The hoist came down on the drift and employee caught right middle finger in pinch point causing a laceration to right middle finger 7 sutures.
Employee was being transported in a bus to end shift. As the bus was traveling, it encountered a series of bumps that caused the bus to bounce. Employee was propelled off EE's seat impacting the top of the seat in front of EE due to EE not wearing a seat belt.
Employee was trying to open product valve on trailer from EE's knees and then decided to sit down to get more leverage and opened product valve. Employee stated that EE applied full force to get valve to move. When employee bent down to open another valve 10 minutes later EE felt pain in EE's back.
Employee was using a lift to moving power cable up to the cross member when it came loose and pinched EE's finger against the handrail on the lift.
Employee was performing an inspection on the lime mill, as employees was performing inspection they noticed the floor was getting flooded with water mixed with lime. Employee then got the Submersible pump and tried to start it, they then noticed it wasn't working. Employee went to move the pump when the hose came off and lime splashed into left eye.
Employee was moving man basket with a forklift, the man basket base/weight was not secured with the rods that hold the basket to base. The basket tipped off of the base and fell forward toward the truck and hit a piece of pipe that was laid across the bed causing the pipe to flip up and hit EE on the right side of head & knee.
Employee was assisting another technician with crack repair on the roof of a P&H 4100 Rope Shovel. EE was communicating with other tech and stepped backwards falling through the danger tape and into an open hole.
