Accident type, without injuries

A fire was observed behind the cab of the bulk emullsion truck. The truck was fully involved within minutes. The blast pattern was evacuated and approximately 70 minutes after the fire started an explosion occurred. The bulk emullsion truck was a total loss.

Employee sought medical treatment on 1/3/2007, at this time his injury became a reportable injury. Employee was walking through mud, slipped and twisted right knee, while trying to catch himself, twisted left knee.

While lifting a pipe high enough to get a come-a-long chain under it, EE pulled a muscle in his lower back.

While checking the coolant level in the radiator of a diesel pump, the coolant boiled over causing it to hit EE on the right side of the face and eye causing first and second degree burns to those areas.

While moving a 9 7/8"" drill bit on the drill deck the employee over-extended his body which resulted in back strain.

Employee was cleaning up between two belts and as he was shoveling onto one belt, fine material came up under his safety glasses and got in his eyes. He has irritation in his left eye.

While cutting a haul truck frame with liquid oxygen/propane torch, EE was instructed to cut the piece 75% through and then wait for the foreman to secure the piece before the final cut. EE cut 100% through, at which point the 6000lb started to fall. EE was pinned less than 5 min. as foreman placed bottle jack, to extricate EE. As soon as EE free, immediately called ambulance.

Struck by flying object

While working on a feeder, dust blew into employee's eyes. Worked two more days after incident and was seen at an Emergency Room on Friday the 24th of February. Started losing time on February 24th.

While preparing to hoist grease barrels to the E-21 Conveyor deck with the use of a hydrolift, EE's left foot was pinned between an outrigger and the ground causing a contusion to the affected area.

While pulling on a 3/4 inch water hose, injured sustained a shoulder right dislocation. Injury occurred when hose hung up on a rock as injured was pulling it.

While operating a 553 Bobcat, employee sustained a crushing injury to her right foot. Claims she was bounced around in the cab and when she tried to reposition her body, her foot slipped off the cab frame. When she lowered the bucket, it pinched her foot between the cab and the bucket arm assembly.

Employee was making a turbo repair and cut his thumb on heat shield.

While riding in passenger seat of a haul truck, injured sustained a bruised right knee when trainee stopped quickly and injured's knee contacted lower dash area. Injured did not seek medical attention until 5-1-06. Physical therapy perscribed.

While walking toward a pump in the vicinity of the Filter plant / Moly Plant, employee slipped and fell causing a fracture to his left upper arm.

While adjusting the packing of C-1 Tailings pump, employee pushed 10"" wrench to tighten the packing. Employee felt a ""pop"" and then experienced a lot of pain to his left arm and left shoulder.

Employee stepped on a piece of grating. Grating slipped causing him to fall through approx. 2 feet. He sustained a left shoulder sprain, contusions to left 4th and 5th fingers, and a right knee abrasion.

Employee was taking a piece of 1.5"" pipe out of the storage rack and a piece of flat bar .5"" x 2"" x 6' caught on the pipe and was pulled out from rack. The flat bar struck the employee on the top of the left foot.

Employee while cleaning track hopper feeder, was struck by a small piece of rock. Employee sustained a contussion to his right 5th finger.

While working on the primary crusher dump pocket grizzly platform, employee sprained his right ankle.

EE ws cleaning a lube pump, while standing on a wooden slat. He did not step off completely, catching his right foot on the edge of the wooden slat and the floor causing his right knee to buckle outwards.

EE was shoveling soft ground and hit cement, which reverberated through his left wrist and arm. It hurt at the time, but was not enough to stop him from working. When he woke up on 9/21/06, his left wrist was swollen and sore. Dr diagnosed as a sprain. D @ A negative. Light duty until follow up on 9/26/06. Only scheduled day missed 9/21/06 for appt. 9/25-9/26 @ Geotemps.

Employee went down the haul road above tire shop. When he got off his truck he stepped on a rock and twisted his left knee.

Employee was assisting pipefitters in the installation of a 4"" plug valve. The valve slipped pinching left 5th digit between plug valve and saddle. This resulted in a laceration from the tip of the finger to the first joint.

Employee was unplugging a cable when he felt a pop in this elbow. ***Employee did not seek medical attention and become a restricted case until after 8-16-2006.

Injured using a crowbar to advance a copper cathode into the load-out hopper, sustained a bruise to the top of the right hand. Injury occurred when the falling cathode caught the crow bar tip, causing injured's hand to strike a near-by metal brace. Light duty was prescribed.

While working on #8 Ball Mill, employee was using a pry bar to loosen a liner and felt pain in his right testicle. He worked until April 5th regular work, and went to a doctor on April 6th. At that time he was placed on restricted duty. Referred to a specialist at that time. Currently he is working with a lifting restriction.

Employee was using a air impact tool when tool slipped and blew dirt into eye.

Employee was stringing welding cable, he threw the cable over a seven foot high guard he experienced discomfort.

Fall from ladders

Employee was climbing the ladder to hold a flashlight for his partner. The ladder leaned to the left and employee fell landing on his right side.

Injured was struck on right side of hard hat and neck when a new hoist cable & becket unwound from a spool. Injury occurred when injured cut the rope holding cable end to spool. Physical therapy was prescribed on 1/10/07.

Employee was getting into her vehicle when she hit her head on the door jam. ***Employee has always been on regular work but started having physical therapy.

EE was cutting a piece of rubber skirting with a razor knife. The knife slipped and he sustained a one inch laceration to the posterior side of the middle joint on the right thumb.

Injured, evading a swarm of bees, tripped and fell as he ran down an embankment toward his pickup. The fall resulted in a sore and bruised right shoulder. X-rays were negative. An MRI has been recommended.

While employee was removing a shackle from a liner he rec'd a cut on his right forearm from the sharp edges of the liner. EE rec'd stitches and was released to full duty.

Flash burns (electric)

While taking amp reading, overloads failed and flashed across phase to phase causing an extreme arc resulting in flash to both eyes. Prescription drug was administered.

Contact with hot objects or substances

3 men were replacing mounting bolts on a HP1000 crusher. While installing a 2' I beam pusher, they attempted to strike an arc, instead they got a flash fire from the arc strike. The bottom hydraulic hose under pressure must have had a pinhole leak putting out a vapor cloud. No one saw any traces of the hydrualic leak before starting the job.

EE was cleaning the #1 regrind area in the mill and he struck his elbow on a beam. This caused a contusion on his right elbow. Doctors perscribed a splint to be worn on his elbow for a week.

While pulling on a come-a-long chain and come-a-long swung around and hit employee's wrist causing a contusion to the area. Also a splint was given to him to wear for a few days.

While exiting the operator's cab of #577 Drill, employee slipped and jammed his left thumb against the handrail. He went to the doctor the same day and was released for work with a splint on his left thumb.

While closing a 4"" plug valve on #3 filter, employee strained his left thumb and wrist.
