Employee caused pinch point by reaching through hole in workbench table. Approx 1,025lb part was pushed for hole alignment while trying to bolt on rigging for mobile crane lift. Part and table caused a pinch point which amputated the distal Phalanx of the left index finger.

Employee got foreign body in her left eye.
Employee slipped while servicing the 518 shovel.
While stepping off trackmobile, employee slipped on loose dirt and twisted his right ankle.
Employee from the SX-EW was leaving the top of the cell 7 section. When he stepped down he lost his balance due to uneven ground, and fell onto left knee. 10 minuets later he felt pain in his left knee. He was treated as first aid, and was seen in the ER department 11/6/11. Treated for knee pain.
Employee struck his finger with the hammer handle in between a flange on the stand pipe.
Employee after fueling his truck, lost his balance with the fuel hose in his hand. Employee began having back spasms. Employee began losing time 11/03/2011
Employee stated that the shovel operator intentionally contacted the bed of her truck. The shaking motion injurred her neck and right shoulder area. The employee did not begin losing time until 11/02/2011
Employee stepped over a pipe and felt discomfort in right knee. Incident was first denied as not being work related, comp carrier accepted claim and company was notified on 10/10/2011. Claim is still being contested by company.
Employee was struck by cable which was being pulled. The cable landed on his foot causing the employee to pull his leg out, twisting his left knee.
While in the process of removing the locking pin from the Case 580M backhoe, an employee was pinned between the outrigger and boom causing injury to the employee.
While dismounting the Jacob's ladder that was located in the rock box of CR3 crusher, employee stepped on the rock box door and rolled his right ankle.
The Employee was climbing down the flex ladder on HT490. He stepped down off ladder and planted his left foot on the ground. He then placed his right foot on the ground; uneven ground which resulted in him spraining his right knee.
Employee alleges that while driving Haul Truck #380 through the Michelin Hair pin turn the driver's seat suspension bottomed out. This caused pain in his upper back area.
We were cutting a hopper, there was a small strip of rubber that had caught fire as employee was on fire watch. As he went to put the fire out, he leaned in and slid his leg on a piece of metal that cut him on his leg. He was then taken to first aid.
Employee was driving a haul truck being loaded by the ES17 shovel. When the material was placed in the bed of the haul truck it jarred the truck and employee, resulting in a back sprain.
While in the process of changing out filter sectors EE slipped on the grating covering the feed distributor box for the filter tub. EE experienced low back pain.
Whip hose on air greaser burst and hot grease cut operators chin and face. Also injected grease into left leg of operator.
Shovel operator was getting off the 16 shovel on the manlift and as she got close to the ground to step off she accidentally may have bumped the switch causing the lift to move in which she fell to the ground injuring her left ankle.
At the end of the shift the employee was coming down from the haul truck stairs when she lost her footing. This resulted in her sliding down the stairs injuring her right knee (laceration) also her low back and neck resulting in low back strain and neck strain.
Employee was walking down the stairs getting off the equipment, when she slipped. Employee landed on her feet and felt pain in right thigh. This resulted in a Thigh contusion
Stepping off platform, misjudged distance, fell forward landing hard on right foot. Sustained sprain of foot. Incident was reclassified to Recordable Lost time on 5/25/2011, when restrictions could not be accommodated.
The operator was jarred while being loaded. This injury occurred on 05/08/2011 but did not begin losing time until 05/24/2012. (1 year later)
Employee states that while reaching over to re-position a drill rod he felt a pop in his right groin. Employee states this happened on 04/12/2011, but did not report it until 04/20/2011.
Employee claims that at the end of his shift he was getting off the water truck when he fell. He got up and walked off to his van pool never reported any accident to the mine. hip fracture. Hospital contacted ASARCO today with a verbal phone call. There is no evidence supporting that the accident occurred on property.
Employee lost footing on soft berm material falling backwards. He landed on his left side with his left arm extended behind him.
The operator of water truck 498 lost control and struck a berm. The water truck flipped over. Accident still under investigation.
Employee was setting up for slime outs. She stepped onto loose section of grading, this resulted her falling through to the next level. Employee received multiple contusions.
Employee was working on a drill rig, trying to align the drill steel when the drill deck wrench which is powered hydraulically moved pinching his left foot between the deck wrench and the stop bar.
While adjusting a part that was put in the back of a truck, employee stepped back and into a track, causing him to fall onto his left hand. Required stitches.
Employee was driving haul truck, and placed his hand under the arm rest to open the window. At that same time he struck a pothole shifting him to the opposite side of the truck, jarring the left arm and shoulder resulting in left shoulder sprain.
Employee was operating a haul truck and was being loaded by the 02 shovel. The 02 shovel bucket made contact with the haul truck jarring the employee in the haul truck cab, resulting in a low back sprain.
While replacing handrail the rail shifted smashing his middle finger on his right hand. Injury resulted in stitches to his finger.
While employee was boarding the transportation van, the employee slipped on the door rocker panel causing the employee to do the ""splits"" and falling backwards.

Employee was transferring reagent from tote tank, while walking around the containment area she tripped on a rock. This caused her to fall hitting her left knee on the concrete. This caused a laceration to the left knee which required stitches.
Employee was knocked off his feet by tensioning cable while putting the pianta on the #516 shovel.
Tech was getting out of the service truck and was using the 3 points of contact and slipped and fell onto the ground on 2 separate occasions during the work shift. Nothing was reported because he felt fine. The next day tech was very sore and was sent to clinic.
Employee was driving a wedge into the idler cap on side of the frame of Dozer 521 with a 4lb sledge hammer. He was holding the wedge with his gloved hand when the wedge slipped and he struck his left ring finger with the hammer. He received stitches to the finger.
While pulling pipe, employee's hand got caught in between pipe wrench and mast.
Employee was in the change-house getting dressed to start shift. He bent over to pick up his cloths when he felt a pain in his left hip, and ran down his leg. He was treated as first aid. He became reportable on 8/4/11. sprain low back.
EE was lifting wear caps onto a pallet. A wear cap slipped and he caught it with his left hand hyperextending his thumb. Resulting in a sprain to left thumb. Was treated as first aid, and was sent for medical evaluation on 7/14/11 when it became reportable.
While walking up from the parking lot in route to the change house, employee lost her footing and fell to her knees. The resultant injury was a sprain to the right ankle and a contusion to the left knee. She was prescribed an ankle brace as well as a knee brace. She was also prescribed physical therapy.
When getting ready to operate haul truck 419 employee tripped on loose deck panel, fell to her knees. Most of the weight went to her right knee and right hand.
While removing the lock ring on a haul truck tire rim on #4 tire the pry bar slipped smashing his left hand ring finger against the wheel motor.
While removing a portable vinyl partition(curtain), lifting the unit he felt pain in left elbow. Accident was first aid, became reportable on 5/31/2011 due to restrictions.
Doing demolition work, trying to knock down a loose piece of rock/mud, a 6 foot wooden plank with a piece of pipe on the end came loose striking employee on right shoulder, upper back.
EE was picking up a spool of nylon rope to place in the back of the pick up truck and was getting ready for the final lift when he felt pain in his back and shoulder. Thoracic sprain.
Employee had been climbing stairs, working on his knees. Sustained left anterior knee overuse tendonitis. Incident became reportable on 5/3/2011.
Employee was lifting a pump onto the rack. When he set it down it slipped and smashed his left tip of his ring finger. Employee was wearing gloves. Required stitches.
Working at tailing pump house she was standing in 1 foot of mud. When she attempted to free herself, she twisted her back. Sought outside medical attention on 4/19/11. Incident turned reportable on 4/19/11.
Employee was removing flange bolts with an impact gun at the CR4. When he removed the last bolt the flange spun cutting the left pointer finger. laceration required stitches.
Employee was doing cleanup removing material from under conveyor belt. As he pulled on the rake,right to left, experienced low back discomfort.
Employee states that while removing the head off the core pipe to dump the core he began having right wrist pain.
While employee was descending steps, employee slipped and fell down steps.
Employee was moving cable when he felt a pop in his left wrist. Treated with first aid, employee came in on 3/12/11 reporting he still had problem with wrist. He was sent out for medical treatment on 3/12/11, treated for left wrist sprain.
Employee was removing metal doors. while removing the doors, a piece of metal fell, employee jumped back out of the way. reported that low back hurt, he was treated as first aid. there is no mechanism of injury. he was sent out for medical treatment on 2/8/2011 and is now reportable, for low back pain.
EMPLOYEE WAS GETTING OFF OF BUS. HE LOST HIS FOOTING AND FELL OUT ONTO GROUND ONTO RIGHT KNEE, RESULTING IN CONTUSION AND KNEE SPRAIN.
Employee tripped over the pavement lip in parking lot.
Employee was disconnecting a down line from a rougher. The line separated spraying him with concentrate. Employee cut his finger avoiding the spray. Employee received 5 stitches.
Employee used excessive force in trying to open an ore car. Felt pain in left shoulder area. Sought outside medical attention on 10/03/2011.
Employee was walking on patio before his shift started. He was stung by a bee on his back and had an allergic reaction. He was transported and treated for allergic reaction to the bee sting and released to full duty from the hospital the same day. 8/29/2011
The employee was rehabbing hair pin insulator and bent down to take off a broken insulator. The insulator broke splashing electrolyte under his safety glasses and into his eyes. Reulting in Chemical burn.
While employee was walking and conversing with another employee, he stepped in a dip in the floor causing his right ankle to twist. He was prescribed an ankle brace.
Employee stepped on blade end of hoe, handle came up and struck employee on the mouth. Knocked out two teeth and chipped one tooth.
While alerting others of an approaching vehicle, employee backed up and tripped over a piece of equipment which was in the walkway. To break his fall, he put his hand out behind him against a dumpster and cut his right thumb. This injury required 7 sutures.
Employee was hooking up a pipe fusing machine to a pick-up truck when the machine moved forward and pinched the employees pinky finger of his left hand between the hitch and the receiver.
Employee outside of guard house when heavy wind kicked up dust into both eyes.
Employee was removing a 12"" C-Clamp from the bottom of a flat bed which was being installed. The C-Clamp came loose and fell, hitting the employee above his left eye, 4 stitches were required.
EE was installing screen deck panels; the utility knife he was using became situated beneath him and cut him.
Employee reports that while bending over to pick up a wrench another wrench hit him in the face.
REMOVING A HYDRAULIC PUMP ON HT450. HE REACHED UP TO START REMOVING A HOSE CLAMP and GOT SOME DIRT IN HIS FACE, HE BRUSHED THE DIRT AWAY, CONTINUED TO WORK. EE WAS AT LUNCH, HIS EYES STARTED TO GET IRRITATED. I TOOK HIM DOWN TO FIRST AID. It was determined that there was a foreign body in Right eye, it was removed by eye doctor.
Employee was operating dozer approximately 10 hours. Right wrist is sore from operating control lever for such a long period of time.
employee was removing a worn out lip shroud from ES01 using a double jack when a sliver of metal struck him on his right leg just below the knee. Causing a laceration. sutures (stitches) were applied to close.
