Struck against stationary object

Employeee was exiting DCS out the rear door when the employee hit the cement curb at floor level with left foot and twisted lower ankle/leg area and was unable to walk on it afterwards

Over-exertion in lifting objects

Employee retrieved a 5 gallon bucket filled with oil from the back of pickup truck. After carrying the bucket from the vehicle, the employee using left arm, lifted the 5 gallon bucket, weighing ~40 lbs, up and over the discharge piping for the fresh water booster pump. Causing pain to EE's shoulder.

Over-exertion in lifting objects

Employee had mounted the 515 shovel after lunch. EE tried to pull the ladder with the left arm and foot but could not pull it up that way so EE used the right arm and as EE pulled up the ladder EE felt a pop in the shoulder and after a while started feeling a burning sensation.

Flash burns (electric)

Employee was checking measurements in #3 cyclone feed pump starter. Employee was in front of starter with door open when an arc flash incident occurred.

Struck against stationary object

EMPLOYEE WAS WORKING AT 517 SHOVEL AND IN ROUTE TO THE VEHICLE CARRYING TWO ROLLS OF WIRE WHEN SUDDENLY THE HIGH WALL NEARBY BEGAN TO FAIL. EMPLOYEE STARTED TO RUN AWAY FROM THE IMMEDIATE DANGER AREA AND DID NOT NOTICE THE TRUCK WAS BEHIND THEM. WHEN EXITING THE AREA THEY RAMMED THEIR SHOULDER INTO THE VEHICLE. EMPLOYEE WAS ON REGULAR DUTY UNTIL SURGERY ON 09/27/2018

Over-exertion in pulling or pushing objects

Employee was moving a oil barrel by rolling it when EE felt instant pain in EE's right shoulder.

Fall from piled material

Employee was observing Haul Truck Dump from on top of the dump berm. 316 While climbing down the berm, when EE's foot touched the ground, EE's leg buckled causing EE to fall to the ground. In EE's attempt to catch self with EE's hands EE received a laceration to EE's left palm.

Struck against a moving object

Employee was operating the 532 Loader, and attempting to bring down a section of overhanging material. When the overhanging material broke loose and fell into the bucket the rear-end of the loader lifted off the ground then came back down abruptly. When this happened employee was jolted up and down in the operator's seat.

Over-exertion in lifting objects

Employee went to tire shop to retrieve a mounted tire on rim. EE rolled wheel and tire over to back of service truck when trying to lift tire into bed of truck EE felt pain in EE's lower back.

Employee was exiting the 400 Track Dozer. When EE went to step around the opened cab door EE's right foot slipped off the platform and EE caught self with both hands on the cab door. At this time employee felt a slight strain in the lower back.

Clean up on B ball mill washing material from behind mill, employee stepped back into an open launder (grating removed) stepping 2.5' down spraining ankle.

Employee was lifting an 8x8 tie at CR4 when they lost their footing. Employee was able to catch themselves and in doing so strained their lower back.

Over-exertion in lifting objects

Employee bent over to pick up 1"" Drive air impact when lifting tool employee felt a pain on the left arm and left side of the neck. The air impact tool weighs between 30 to 50 pounds of weight. Employee was moving the tool out of the walk area.

Fall down stairs

Operator was going east down the stairs and missed the first step falling to the ground and landing on right shoulder. Employee was seen by site ERT and sent to a local clinic where it was determined employee had a fractured right clavicle.

Employee was hiking down a hill when stepped off a rock and pulled calf muscle. EMPLOYEE DID NOT RECEIVE MEDICAL TREATMENT UNTIL 6/20/2018

Employee was removing engine turbo, during removal process employee started to pull turbo off of mounting studs by wiggling back and forward. Once turbo came free of studs which caused the turbo to jump forward pinching employee's left ring finger causing a laceration.

Employee was working under the loader. As EE came out from under the machine EE felt a pop in the knee.

Employee was using a clay digger to scale down the rock box and grabbed hold of the bit and pinched the tip of index finger.

Over-exertion in pulling or pushing objects

Employee was installing some hoses on the mast of a drill, when they got stuck EE pried on the hoses with a small pry bar to move them and felt pain in EE's right shoulder.

Fall from machine

Employee had been doing bench work on Q-bench. EE went to dismount the track dozer and as EE stepped on the last step on the push arm there were rocks and caused EE to lose EE's balance and fell to the ground causing scrapes to EE's back, head, elbow and buttock area

Struck against stationary object

Employee was greasing the threads on the #1 STD bowl for install. Employee was using their hands to wipe the new grease onto the threads and hit a burr on the thread and cut the right middle finger about one inch on the inside of the finger towards the tip.

While cutting pipe with a chainsaw, some residual water came out causing the mechanic to recoil striking another nearby mechanic's hand causing laceration requiring 6 stitches.

EE was disconnecting electrical harness & connector would not release. EE used screwdriver to release locking pin on connector. Connector broke, allowing screwdriver to slip and puncture EE right hand between thumb and index finger. EE was take to clinic.

Struck by flying object

Employee was using a rivet buster to knock in bolts. One of the bolts gave which caused the liner to fall. From this the rivet buster shot up and hit the employee by right eye.

Operator was trying to remove large rock from tracks of TD516 when the rock shifted pinching the operators fingers. This caused laceration to right middle finger. Two sutures required.

Struck by falling object

Employee was removing the driveline from under a service truck, when it came apart and struck employee above the right eye requiring 3 stitches.

Employee had been welding earlier in the day and while in the process of wiping the sweat from EE's head EE used EE's sleeve and got foreign body in EE's eye. After it was reported EE was taken to an eye specialist who removed the material from EE's eyes.

EMPLOYEE WAS ASSISTING IN CHANGING DRIVE BELTS ON #3 SCRUBBER AT THE CRUSHER. EMPLOYEE HAND SLIPPED IN GREASE ON THE BELT AS THEY WERE MANUALLY TURNING. LEFT HAND MIDDLE FINGER GOT CAUGHT BETWEEN BELT AND PULLEY CAUSING LACERATION REQURING SUTURES.

Struck by flying object

Employee was operating the wet grind overhead crane for pulling the grinding rods out of #6 rod mill. The bundle was laying down on the grating on one end the other end was leaning up against the mill discharge box. A fan was blowing when employee was lifting the bundle of rods and that's when EE felt something enter eye.

Employee was installing a throat liner on the discharge end of rod mill. While holding the cable with right hand, the liner spun and pinched left ring finger causing laceration.

Struck by rolling or sliding object

Employee was checking to see if the water hose was in the 11 ball mill feed-line and it was not. The air lance was in the feed line instead, EE grabbed the water hose to put it inside the 11 ball mill feed line and the water and sand started moving around started pulling water hose out felt sharp pain and noticed the hand was bleeding.

Employee states that EE was cleaning the #5 scrubber before EE went on break. At break time, EE took glove off and noticed blood inside EE's glove from a laceration to right middle finger. EE is unsure how it occurred.

Employee was shoveling under the wear belt when they felt dust enter the right eye.

Employee was sliding the Knuckle boom extinction back in and pinched right ring finger and received 4 sutures.

Fall to the walkway or working surface

While un-spooling wire rope, employee was walking backwards and tripped over test weights.

Employee is diagnosed with a Standard Threshold Shift in hearing by Dr. in right ear. This has not been determined as work related at this time but is consistent with noise induced hearing loss.

Contact with heat

Employee was working on the 585 drill when ee started to feel light-headed and dizzy. Employee was starting to get heat exhaustion.
