Accident type, without injuries

Uncontrolled fire occurred at the #1 standard crusher polyurethane / rubber screens in the secondary crusher building. 1 contractor and 1 Asarco employee were cutting retaining bolts with a oxygen/acetylene torch starting the fire with hot slag.

Struck against a moving object

Employee reports while driving haul truck backing into #2 shovel truck struck pothole at same time the bucket was loading the dump bed. Driver felt pain in lower back. On 3/12/2021 the company received notification that a lumbar strain is the diagnosis of injury that occurred on 12/26/2020 and is attributed to a work injury. Lumbar strain is fully resolved.

Fall from machine

The water was drained from the secondary containment vessel and was hoisted onto the boom truck bed. At approximately 10:50am the employee fell off the back/left side of the truck bed onto the wash pad. An EE checked on IE after the fall and called ""EE down, EE down, EE down at the wash pad,"" over channel 4.

Struck by flying object

Employee was doing blast hole sampling when allegedly a small piece of rock became embedded in left elbow causing irritation. Employee did not report injury until 5/26/2020. Employee did not become Lost Time until surgery 6/17/2020.

Over-exertion in pulling or pushing objects

Employee was repositioning jack pad with pry bar, heard pop felt pain in left elbow.

Pushed bolt back into cutting edge with hand while impact was ran, impact spun bolt and cut through EE's glove cutting right middle finger.

Caught in, under or between running or meshing objects

Employee was repositioning right leg for leverage on the grinder when right foot slipped. While slipping they reached for the handrail in front of them to maintain balance which caused the grinder to turn sideways and cut into right thumb.

Over-exertion in pulling or pushing objects

Employee was closing the 1 pump valve at the 7 booster to balance flows at the plant. EE went to the 7 Booster to close the valve with a pipe wrench. After the valve was closed, EE walked to check the pressure gauge and felt pain in EE's right shoulder and arm.

While employee was performing work on the press, a cathode shifted causing a laceration to right hand.

EE was placing 4 cable bridal ring onto crane hook. Crane hook safety latch pinched right middle finger.

Employee was removing right wheel tires on Haul Truck when arm was pinched between reaction arm and wheel assembly.

Employee was stripping copper. When EE squatted down to pick up an air hammer and felt pain in EE's right knee.

Employee was trying to move an impeller from 1 pallet to another, when lifting it they heard their lower back pop .

Truck shop employee fractured pinky finger as the employee was removing a starter from a pickup truck.

Over-exertion in lifting objects

Employee was replacing bags in Number 3 Filter. As employee lifted and removed each sector and cut off each bag and replaced it with a new filter bag employee felt pain on left side of back.

Employee was walking through some mud and debris on the wash pad when EE slipped and twisted hurting EE's back.

Over-exertion in wielding or throwing objects

Employee was swinging a double jack to drive out a pin on 324 grader. EE needed to switch to swinging left handed because of the awkwardness of pin. Later in the day EE started to feel pain in EE's lower back. The employee did not report the injury until the next day.

Caught in, under or between a moving and a stationary object

While tossing a 15 lb dust filter cartrage over a handrail employee smashed left ringfinger between a light post and the filter cartridge. Immediately felt the pain and blood run down finger. When employee removed glove they saw that finger tip was cut and bleeding and reported it immediately.

Over-exertion in lifting objects

While Shoveling employee has been feeling a strain in left elbow. Employee feels that the pain is from prolonged exposure to shoveling. Employee is new to these tasks.

Fall onto or against objects

Employee tripped over a chain while holding a sharp bolt and cut hand

Over-exertion in wielding or throwing objects

Employee was swinging a 15lb sledge hammer to drive in bars and strained a muscle in EE's upper middle back.

Struck by flying object

while rotating SAG mill to perform a gear alignment, a mill ball bounced out striking IE in mouth requiring 3 sutures.

Struck against stationary object

Employee was attempting to remove bolts from the lid of a Mine owned water truck, using a battery operated impact. While attempting to loosen a bolt, the employee's left index finger was lacerated between the lid and the nut.

Used power tool on exhaust flange and rust and debris went over safety glasses and into Left eye.

Employee was backing off the rock breaker when they felt a tug on pant leg. Employee examined the leg and noticed a metal splinter. Splinter was removed and required one suture.

Employee was cutting sandbags and the knife employee was using slipped off the material and employee cut left hand.

Employee was hand stripping copper in the stripping station using a bar. The bar slipped and hit the employee in the chin. EE received 9 stitches.

Struck against stationary object

Employee was walking down the stairs to the South SAG on the grind floor. Employee stepped on a rock on the grating just below the last step of the staircase on the main floor decking. This caused employee's right foot to roll to the side and employee fell to the grating. Employee also heard ankle pop.

On 12/3/2020 Employee was diagnosed with a standard threshold shift by a Dr. in the right ear, this has not been deemed work related at this time.
