Caught in, under or between a moving and a stationary object

Employee was removing a loosened nut by hand when the pipe shifted pinching his right index finger between the nut and the Victaulic clamp. DX right index finger partial amputation.

Employee was removing rocks from a feeder using a specialty tool designed with a hook on the end to dislodge the rocks. While pulling up on a rock, the hook slipped from beneath the rock causing the tool to move upwards, pinching the employees left middle finger between the metal tool handle and the feeder chute.

The trip cable on ES01 caught on RT560 pulling the sweetheart from the banjo, causing the sweetheart to break free and go through the back window striking the operator.

Struck against a moving object

Employee was blading the ground around the #518 shovel. While in motion, the employee removed his seat belt and stood up to stretch his legs. While standing up, the blade caught a hard knob causing the employee to hit his head on the air-conditioning unit inside the enclosed cab. Employee sought medical treatment on his own 2 days later.

Over-exertion in pulling or pushing objects

Employee was up on the #516 shovel preparing to pull the ladder up. The employee felt a pull in his lower back as he pulled up the ladder. Began losing time on 8/29/2015.

Fall onto or against objects

The top of the scale was wet due to the rain. The step was hooked on to the scale. The step was about 12 inches narrower than the opening in the railing. The driver had parked where he had to maneuver around the door, the hook and the railing. There was no anti-slip surface on top of scale.

Over-exertion in pulling or pushing objects

EE was removing a drain plug at the bottom of hydraulic tank for a loader. When the plug broke loose the employee felt a popping sensation in shoulder. This injury has been treated as first aid and employee was sent for an MRI when it was determined that surgery was needed. On 04/14/2016 were ee surgery and the first day of lost time.

Fall to the walkway or working surface

CTI driver got out of his truck & placed wheel chocks around his tires. Roughly around 0840 he turned & walked toward the copper concentrate stock pile for a product sample. Before getting to the sample pile he slipped on wet material, injuring left knee. The material did not appear to be slippery but was wet due to a previous rain storm. Diagnosis left knee strain.

Over-exertion (Not Elsewhere Classified)

EE claimed with the Industrial Commission a back injury 6/6/2015. There was no report of injury to us, and no cause of injury. We denied the claim. EE had surgery. We sent ee to an Independent Medical Exam and it was ruled compensable. We received the report 10/16/2015 from the IME doctor.

Employee was struck by pipe on the right leg causing injury.

Struck by falling object

While installing a new air cylinder, the air cylinder fell striking employee's right index finger. Dr. dx minimal displaced fracture.

Over-exertion in pulling or pushing objects

Employee was attempting to remove the dipstick to check the oil during his pre-operational inspection of the haul truck. While he was struggling to remove the dipstick he felt pain in his shoulder.

Employee cut his left hand above base of thumb while trying to remove a splice jacket on a cable.

Employee was getting out of the service truck when he twisted wrong and felt a pain in his back and down his left leg. Company notified by doctor on 11/30/2015 of duty restrictions.

Hammering a piece of metal when a sliver came off and went through glove into left hand, palm.

Struck by flying object

Employee removed his welding hood and was putting on his hard hat when a foreign object got into his left eye.

Employee was cleaning up around lathe work area when he cut his finger on some metal shavings.

Caught in, under or between a moving and a stationary object

Employee was working on a plugged up Syntron feeder (4) ee was using an air lance to bring down the feeder, while doing this the feed came down struck the air lance pinching left index finger causing a laceration on the index finger

Fall to the walkway or working surface

Employee was manually tightening bolts on a casing clamp using a 3/4"" drive socket and 18"" breaker bar, when the clamp was jerked the casing to rotate and the socket slip off of the bolt. Employee lost his footing and fell backwards striking the bumper of the pipe truck with his right buttocks, he then stopped his fall with his extended arms and hands to the ground.

IE was moving a 240# wheeled portable fire extinguisher into a MCC up a 3-4 step stairway. The wheeled FE moved the wrong way and IE move to quickly stabilize the unit. This caused a pain in his back that was not reported to his manager until the morning of 2/2/15.

Employee was cutting a rubber hose off of a water tube for the engine when the utility knife slipped and cut his right leg.

Employee pinched finger between 2 metal objects while replacing the cutting edges on the Caterpillar 420 grader.

Employee was removing a light bulb using an extended stick. While twisting to remove the bulb from the hugger with right hand, the bulb broke; causing laceration to middle finger.

Hydroblasting stairway inside of shovel under very cramped conditions, waterjet penetrated boot injuring toe. Steel toe boots were in use.

On the above date, employee reported for audiogram in compliance with the Hearing Conservation program and a standard threshold shift was noted. The audiogram demonstrated a standard threshold shift with an average loss of 10dB or greater at 2,3, and 4kHz in the right ear from the initial baseline and an average hearing loss more greater than 25dB at 2,3 and 4kHz.

Employee was walking on rebar when he slipped, fell and punctured his hand on a piece of tie wire.
