Accident type, without injuries

Slope car was hoisted out of mine, when car was inspected on surface it was found that the brake sensor wires were broken and one of the brake sensor brackets was bent. The car was immediately taken out of service. Although exactly unknown in all probability a patch of ice approximately 100' down the slope snagged the brake sensor wires causing the damage to the slope car.

Accident type, without injuries

The East Finley Elevator was down for more than 30 minutes. The elevator was down due to the control power that controls the doors had to be recycled to open the doors. There was no damage to the elevator. There was one person on the elevator at the top landing for 35 minutes.

EE working in a chute - removing scaffold supports. Employee was stabilizing self from rigging above his foot slipped, employee caught self with right arm strained shoulder

EE entered prep plant - walking on smooth walkway and rolled left ankle

Over-exertion in pulling or pushing objects

Employee was installing a new tram chain on the angle bolter in 4 South left section. As he was pulleing the old chain he felt a pain in his lower back. Lost work days began 12-28-2009

Struck by falling object

Employee was side bolting in #3 entry, F-19 Section when a piece of rock measuring 24"" x 47"" x 1/2"" to 4"" thick fell and bounced off of the ventilation tube striking him on the head resulting in a fracture to the C-5 vertebrae of the neck.

While working on the 12 I longwall face, the employee got his left foot caught between the panline and a toe of a shield.

Struck by falling object

EE was on tight side of the stageloader to take down a section of monorail in #1 entry, F-18 longwall section when a piece of coal/rock that measured 7'x 27"" x 12"" thick fell from the rib and struck him on the left leg.

Employee was pushing a pilot of metal stopping out of the back of a truck when his hand slipped causing him to lacerate his right hand. 5 Suters to close
