An unplanned fire which was extinguished within 5 (five) minutes occurred on a 650 battery receptacle located in the F24 recovery section at the battery charging station located in #3 Entry at 3 wall. There were no injuries as a result of this incident. The right battery receptacle and the charger cable plug were damaged.
The Crabapple slope hoist was not operational for an extended period of time due to electrical issues found during normal inspection. No employees trap or injured as a result of this incident
The Dry Ridge Elevator was not operational for an extended period of time. The rope grabber oil level became low causing a fault. No injury as a result of this incident
Oak Springs hoist was out of service due to a malfunctioning blower motor.
An unintentional roof fall occurred in F27 miner section at 25 wall track chute in the #3 entry. The fall measured 16ft wide by 8-7ft long and 8-9' high. There were no injuries or equipment damage as a result of this incident.
The Oak Springs hoist was out of service due to a loose shaft on the blower motor. The hoist was put back in service at 12:35 PM.
The Crabapple elevator was not operational for an extended period of time. The ABB relay which is a field current that switches the breaking system from AC power to Battery backup failed due to under size. No one was injured as a result
While in the process of making a weekly exam the Mine Examiner discovered a roof fall in the the #1 Entry of 3 East Mains. The fall was located between 42 and 42 1/2 wall instersections of the entry. There were no persons injured or equipment damaged as a result of the fall.
There was a roof fall that in the #1 entry of the 4L section. The roof fell 16-18 feet in height and approximately 50 feet in length at 12 wall. There were no injuries as a result of this event
A roof fall occured in the #7 Switch on the Crabapple slope bottom. The fall occured in a 3 way intersection and measured approx. 25ft long by 12ft high by 25 ft wide. There were no injuries as a result of this event.
Oak Springs hoist was out of service due to a faulty low oil level switch.
A unplanned roof fall occured in the crosscut between 2 and 3 entry at 44 x-cut in the 3L section. The fall measured 16ft wide 10ft high and 50ft long. No injury or equipment damage as a result of this incident
An alleged ignition of gas occurred on the 2L longwall at the 259 shield while mining toward the tailgate. No injuries or equipment damage as a result.
An unintentional roof fall occurred in F26 miner section at 19 wall #3 entry. The fall measured 16ft wide by 65ft long. There were no injuries or equipment damage as a result of this incident.
Pleasant Grove Elevator was down at 11:00 AM due to a repair being done on the conduit for the control cable.
Pleasant Grove Elevator was out of service at 5:00 PM for a broken door sensor. The door sensor was replaced.
An unintentional roof fall occurred in F25 LW section between 34 and 35 wall in the #3 entry. The fall measured approx. 16ft wide by 40ft long and 8-9' high. There was no injuries or equipment damage as a result of this incident.
An unintentional roof fall occurred in F26 miner section at 17 wall between #1 & #2 entries. The fall measured approx. 16ft wide by 25ft long. There was no injuries or equipment damage as a result of this incident.
During regular inspection it was found that one of the magnetic breaks failed when tested. The Hoist car was taken out of service for repair
Pleasant Grove Elevator was down from 12:50 PM to approximately 1:20 PM due to the over travel sensor needing reset. Seven miners were aboard the elevator at the time. No one was injured during the incident.
Had a power failure that caused the elevator processor to fail. The elevator cage was stuck at the surface landing and was inoperable until the processor was reprogrammed. There were no injuries or entraptments as a result of this occurrence.

An unintentional roof fall occurred in F26 miner section at 22 wall between #2-3 crosscut. The fall measured approx. 16ft wide by 60ft long. There was no injuries or equipment damage as a result of this incident.
Damage to the Bailey Slope hoist rope was found
Speed sensor wire for the slope car ""snagged"" on an unknown object causing it to sever.
Control board for the automatic doors failed causing the doors to function improperly while on the landing at the surface. There were no persons injured or entrapped due to this occurrence.
The 3 North #6 escape capsule was out of service due to ice forming on the rails and wheels.
Pleasant Grove Elevator was down intermittently from 6:30 AM to 8:00 AM due to the door brackets and rollers needing adjusted.
Pleasant Grove Elevator was down due to the malfunctioning alignment stop on the inner door.
Pleasant Grove Elevator was down due to a bad wheel. Vibration was felt and elevator was inspected until it was determined a wheel needed replaced.
The Dry Ridge Elevator was not operational for an extended period of time. No one was trapped or injured with this incident.
While mining in the #8 entry of the 7 North Right section an ignition occurred while sumping the continuous miner head just below the binder. Two bits were found damaged on the return side of the head in the inner ring of the drum extensions. No persons or equipment were injured/damaged as a result of the ignition.
A mantrip traveling out of a switch collided with a mantrip traveling to the straight track. A passenger in the mantrip received crushing type injuries to his right foot. (per phone call both toes amputated completely due to the injuries)
Employee was in the process of lifting a 16 foot I beam, along with a five other employees, onto the ATRS of the rib bolter when EE felt a sharp pain in EE's lower back. LTA as of 1/11/2016.
Employee pinched his left finger between the belt and a belt winder while repairing the Reclaim belt.
Employee was on the mine floor working on guarding at a conveyor belt tailpiece when another employee stepped on the back of his foot (heel) causing his left ankle to twist. Caused a strain/sprain to his left ankle, knee and hip.
Employee was slipped off a shield toe on E26 longwall face and sprained his left ankle. He lost his balance and fell into the handrail and also felt a pain in his left shoulder. Made aware on 1/4/16 employee became a LTA for his right ankle injury.
Employee was struck in foot by forklift tire.
Worker was helping to clean a conveyor spill. He picked up a rock to place it on the belt and caught his finger between the rock and other material on the belt.
While cleaning off trash from a underground mine car on the surface at the hoist area. The injured employee picked up a empty wooden pallet and proceeded to place it into a trash bin and felt pain in the lower back.
Employee injured his right shoulder as he fell going through a set of air lock doors. He opened the set of doors and stepped on a piece of cement block that was about 12 x 12. He was falling and tried to catch himself and put out his right hand/arm. The force of him falling caused the injury to his right shoulder.
Employee was bolting in F27 miner section when a piece of rock fell hitting the slider tube, bending the bracket which held the slider tube in place. The slider tube then struck him in the upper body knocking him to the ground, spraining his right knee. Diagnosed with a torn patella tendon.
Employee was helping carry water pipe and was transferring to other shoulder. During transfer, employee shoulder got injured and popped out of socket
Employee was getting off a forklift when he stepped into a hole causing his foot to roll. As a result, employee suffered a broken bone in his left foot.
Employee was making up a bolt and a piece of rock fell from the arch of the roof and rib and between roof straps and struck him on the right index finger causing a fracture in his index finger
Employee was moving the monorail dollies at the outby inchworm in F25 longwall. As he was moving the inchworm, a 1/2"" hose that was plumbed to the inchworm got caught on a top roller. The hose pulled out of the coupler causing emulsion to suddenly spray him which caused him to fall to the ground.
Employee was rib bolting off of the Continuous Miner, while drilling a hole a piece of rock fell from the rib striking the employee on the right elbow causing a contusion. On 9-22-15 the employee began missing work due to this occurrence.
The employee and a coworker were sliding a 3'x3.5' rotary breaker replacement panel from a stack of panels when the EE slipped, causing the tip of his right thumb to be caught between two panels. This resulted in a laceration and fracture to the tip of his right thumb.
Employee was conducting the weekly exam of the F19 seal, when he lost his balance on uneven bottoms and twisted his right knee. Diagnosed with a patellar fracture.
Employee was using a slate bar to dislodge a top middle belt roller on the conveyer belt. When the top middle roller became dislodged the slate bar fell and struck the employees right thumb causing a contusion. On 9-14-15 the employee began missing work due to this occurrence.
Employee was carrying a five gallon can of oil, when his right foot slipped in the mud and he felt a pain in his right groin. Diagnosed with a hernia - LTA as of 8/25/15.
Employee was walking in the belt entry during a belt move when he stepped in a divot in the mine floor causing him to twist his left knee.
Employee was helping to reconnect a cat track on a continuous miner when he pinched his fifth finger (pinky) on his right hand between the top of the cat pad and the bottom frame of the continuous miner. Caused a laceration that required sutures.
The employee was operating the Longwall Shear, while walking along the face the employee stepped onto a shield toe and twisted his right knee. Diagnosed with a torn MCL.
Employee was rib bolting off of the continuous miner in the 7 North Mains Right Section. When preparing to start drilling a bolt hole a piece of rock fell from the rib striking the employee in the left ear, causing a laceration requiring 12 sutures to repair.
Employee was installing a track switch at 45 wall in 2 East Mains. While pulling an 85lb point, one of the side jaws got caught on a tie and he felt a pinch in his leg / groin area. Diagnosed with a hernia on 8/4/15. LTA as of 8/12/15.
Employee stepped down off of the stageloader and twisted his left knee
Injured and 2 co-workers were jostling a 10 foot long piece of aluminum monorail beam on the 4J section belt entry. When he felt a pain in his right groin area. The injured employee was supporting the monorail beam as his coworkers were fastening and hanging the ends of the beam.
Employee was helping to remove L.W. high pressure hoses when he was struck in the left knee by a 3"" staple lock coupler.
Employee was standing on step of the over cast which was approximately 3 feet from the surface below working on water line. As employee was reached out to adjust the manifold on the water line employees feet slipped off of step. This resulting in employee landing with all of his weight on his left leg.
Employee was loading cable into the scoop bucket and got foot smashed by scoop bucket.
Employee was on his knees, lifting a bottom roller into the bottom assembly on 3 North #5 belt. As he finished lifting the roller, he felt a pain on the right side of his lower back.
Employee had a drill head strike his foot.
Employee was struck on the left ankle by a piece of falling rib.
Employee was unloading rock dust bags from the outby end of a shuttle car when a rock fell from between roof straps striking him on the back of the right shoulder. He fell to the mine floor awkwardly and injured his left ankle and leg. Resulted in a fractured tibia/fibula and a dislocated ankle.
While carrying a track tie, employee stepped into a hole resulting in knee pain. Employee was originally injured on 4/20/15. He did visit the doctor and had various tests done on his knee. he continued to work until 5/29/15 when he had another doctor visit and was taken off work.
Employee was walking down overcast steps and slipped, causing his knee to buckle. He did not miss any time until he had surgery on 7/2/15.
Employee was walking toward the miner when he stepped off the coal pile and felt pain in his lower back. LTA as of 5/4/15.
Employee was struck on the right ear with a roof strap while attempting to clean off the top of a miner causing a laceration.
Employee was installing belt clips with angle bolter in 3 North Left Mains. While in the process of adding a steel to his six foot starter, the suction hose caught the rib. He reached across the head to free the suction hose with his right hand when the drill steel fell from the roof and pinched the tip of his left index finger between the steel and the drill pot.
While making a splice on F25 belt, employee was holding the belt clamp in place when the belt tension was released. The clamp moved and his right index finger became caught between the clamp and the belt structure.

Employee was walking in the hoist/yard area when he slipped on ice causing him to fall injuring his right shoulder.
Employee was injured when he slipped and fell into the operating longwall face chain.
IE had just finished raking refuse material from under the slope belt and positioned himself to shovel material on to the belt. An accumulation of coal and water washed back off of the slope belt and a piece of slate came off of conveyor striking IE in right side of head and face.
While rolling a roll of mine belt, the employee attempted to stop the belt from rolling and felt a pain in his left shoulder. The incident was reported on 2/24/2015. Employee continued to work all scheduled shifts until being taken off for surgical repair
Employee was center bolting, while walking by the bolter he stepped on the power cable straining his right ankle.
A Bailey Prep Plant employee pinched his left ring finger while in the process of changing out a CMI basket.
Felt pain in lower back while sliding bundle of steel
Employee was helping to carry a conveyor belt bed rail with another employee when he tripped and fell striking his left leg just below the knee on a metal rib strap that was on the mine floor. Caused a laceration that required 7 sutures to repair.
Felt pain in his side under his rib cage after moving platens for vulcanizing press.
Employee was walking in the 7 North Mains Section around the feeder when he tripped on a 5 gallon can of hydraulic fluid causing him to fall and strike his left elbow on the mine floor. Resulted in a radial fracture of the left arm at the elbow.
employee was standing behind scoop (just closed K door) scoop was loaded with cans with chain wrapped around the cans. Chain broke 3 links from hook and hook and links flew back and struck the employee behind his right ear.
Employee was installing Air Lock Doors outby the Longwall Headgate. Injured employee and another employee went to set the leg of frame upright and the top jack slid down striking injured employee on left thumb.
Employee was cutting a piece of pipe when he slipped and cut his leg.
Injured was standing on the second rung of an 8 foot step ladder removing a Victaulic coupler from two 8 inch diameter sections of aluminum water line when he lost his balance and fell to the floor causing a laceration requiring 7 staples to close.
Employee was rib bolting in E29 section. After drilling a hole, he went to remove his auger from the wrench and his right hand slipped across auger cutting his right index finger on a burr. He received eight sutures.
While removing a large haulage shaft off of the top of the longwall shearer, he lost his grip and caught his finger between the shaft and the handrail
Employee was side bolting in F26 miner section. While he placed a roof bolt into the wrench, a rock (8' x 5' x 3') fell from the roof and hit his left arm. The rock fell from the end of the wire mesh and struck him on his left forearm causing a laceration. Employee received 3 sutures.
While traveling, the jeep that the employee was operating went into a switch and struck a park locomotive. This caused him to strike the left side of his body on the arm rest. Employee did not mis any time as a result.
While carrying a beam, he slipped and fell and the beam landed on his right wrist causing a laceration.
Track mounted mantrip was traveling out of the mine to the portal when it was involved in a collision with a mantrip that was being switched out to give haulage clearance. Upon impact the IE was struck in the upper lip by the bill of another miners' hardhat and received 5 stitches to close laceration. There were a total of 9 miners in the mantrip including the operator.
Employee was lifting bags of shotcrete and strained a muscle in his back.
Injured was shoveling along the tight side of the 4L belt between 71/2 - 8 crosscut when a small piece of slate fell from the roof striking the employee on his hard hat and left ear. This caused 3 small lacerations on employee's ear which required 11 sutures. The area had been meshed between bolts.
Employee was operating the miner in 3 North Right Mains, when a rock fell from the face and struck the slider tube. The slider came down on top of the employees hard hat and caused a laceration to his forehead. He received 6 sutures.
Employee was passing through door and tripped and fell. He tried to catch himself and hurt his hand.
Employee was walking in the underground wash down pit when he tripped on a cap block laying against the rib which caused him to fall and received a laceration to his forehead. He received 12 sutures.

Employees were placing railroad ties to install a switch, injured employee caught his finger between two ties causing a laceration. (required stitches)
Employee slipped while unloading supplies out of a coal car, he struck his face off of the side of the car causing a laceration.
Employee began feeling pain in his shoulder while bolting. This incident is believed to be related to repetitive movement. Incident is still under investigation. Employee did not miss any scheduled work days until January 4th 2016
Employee was diagnosed with Carpal Tunnel Syndrome. Lost Time 9-30-15.
I was notified today 8/24/2015 that this employee has been diagnosed with hearing loss. The hearing loss was considered work related and the company subsequently settled with this employee
I was notified on 5/21/15 that the company has settled with this employee for work related hearing loss.
Received notification from MSHA on 03/15/2015 that miner (MIIN:M09850762)had exercised his rights as a Part 90 Miner under Title 30, Code of Federal Regulations due to medical examination indicating sufficient degree of pneumoconiosis.
Working outdoors changing out roller on conveyor; frostbite
Employee was walking on third floor of plant 2b when a screen center strap fell through a hole chase from above striking the employee in the hard hat and right shoulder. Center strap weight approximately 10 lbs and fell approximately 1 1/2 feet. Muscle pain was the only thing found at the hospital for the injured employee.
