Accident type, without injuries

WHILE CUTTING COAL IN THE #1 ENTRY INBY #23 CROSSCUT ON THE E-26 SECTION AN IGNITION OCCURRED AT THE FACE. IT WAS ORANGE IN COLOR AND LASTED SECONDS. NO METHANE WAS DETECTED. MINING STOPPED AND THE AREA SECURED. MSHA WAS NOTIFIED.

The Hoist Car at Crabapple slope was inoperable for a period of time. It was determined that the speed sensor was defective. The speed sensor was replaced.

A roof fall 16 feet long 16 feet wide above the bolt anchors in the number 9 entry at 84 crosscut in a 4 way intersection. Ventilation was not affected. Passage was affected but the number 9 entry is common with the number 10 entry which is the normally traveled entry.

An unintentional roof fall occurred in 4 West Mains area at 24 wall between #1-2 x-cut and in the #1 entry. The fall measured approx. 8ft high by 16ft wide by 60 ft long. There was no injuries or equipment damage as a result of this incident.

An unintentional roof fall occurred in 4 South Mains area at 28.5 wall in the #6 entry. The fall measured approx. 8ft high by 40ft long by 20 ft High. There was no injuries or equipment damage as a result of this incident.

Dry Ridge elevator stopped moving in the shaft with an employee on board. The employee was not in any danger but was entraped for about 45min

The Dry Ridge Elevator was inoperable for an extended period of time. No one was on the elevator at the time of the incident. It was found that the door sensor on the outside door malfunctioned. The sensor was adjusted.

The Crabapple elevator was not functioning properly for an extended period of time. The elevator was down on 5/3/13 at 5:50pm. The elevator was functioning on and off for the next 3 days. MSHA was notified each time the elevator was down. The encoder bracket was loose causing a drive fault. The elevator was operational 3:30am (5/6/13).

An unintentional roof fall occurred in the intake entry of 1 J miner section. The fall occurred inby the #23 xcut. The fall measured approx 50 ft long by 16 ft high and 9 ft high. No injury or equipment damage as a result of incident

Employee was walking when employee tripped over a piece of clay causing him to fall and strike his left arm and shoulder against the rib. Resulted in a dislocated shoulder.

Employee was walking on top of a tram road rut when the rut gave out causing the employee to roll his right ankle. Employee began missing work on December 26, 2013.

Fall from machine

Employee was operating a forklift on PA st. rt. 231. The fork lift dropped off the rt side of the road. Employee could not stop in time & the fork lift rolled over. Employee was wearing his seatbelt, but when he realized the vehicle might upset, he removed the seatbelt and attempted to jump clear. He landed on his left elbow and rib cage, bruising both.

WAS INSTALLING A BRIDGE IN THE F-25 SECTION WHEN HE CAUGHT HIS RIGHT PINKY FINGER BETWEEN THE BRIDGE AND THE COAL RIB AND FRACTURED THE FINGER.

While working in the BMX 3 East mains section, an employee was spraying water on a rock pile to minimize dust when a piece of rock came off of the miner head and struck him on the right cheek causing a laceration.

While sheaving a ram jack onto the Longwall drive motor, the jack got caught on a post. Employee approached the area and the jack kicked sideways and struck his right leg.

Was walking belt and bent over to examine the bottom belt. When he stood back up he ran into an 8"" water line that crossed the belt and fractured his nose.

While walking in the BMX 1A Tailgate section, employee stepped into a haul road rut and twisted his left ankle.

Injured employee was hooking a tow strap to the Miner Cable to pull slack with the Ram Car when he felt a sharp pain in his lower back and groin area. The employee was handling a cable. The employee began to miss work on 11/26/2013.

Was helping remove a steel stuck in rib when he felt pop in his right shoulder. He didn't report it to anybody til next day. Doctor did MRI and removed from work on 10/28/2013.

Employee was lowering a shaft down into a cell and when the slack of the chain came out of the strap his right thumb got caught in the slack of the chain.

A piece of rock fell from where the rib meets the roof and struck the employee on the head and neck.

Employee was picking up a ventilation tube when he lost his balance. In trying to regain his balance he grabbed the rib protector that at the time was being lowered causing fractures to the ring and middle fingers of his right hand.

Employee was changing drawbars from 4 foot to 8 foot when the drawbar slipped and fell on to his right wrist causing a fracture.

Walking out of face and stepped over a rock, slipped, fell and fracture his left ankle.

employee was hanging wet, muddy canvas in a 8 1/2' high face area and felt a pain in his groin. hernia

Two employees were lifting a flipper plate on the longwall with two slate bars. One slate slipped and caused all the weight to pinch the employee's right ring finger and fracture the tip. Started missing on 9/17/13

Employee was walking down steps when a raccoon jumped on his back. In trying to remove the raccoon, employee fell down steps and his head struck a welder that was parked at the bottom of the steps. Employee was wearing hard hat but it fell off as he fell down steps.

WAS RETRIEVING A ROOF STRAP FROM TOP OF MINER WHEN THE SUMPING MINER SQUEEZED HIS FOOT BETWEEN THE BOOM AND MINER FRAME. STARTED MISSING WORK ON08/29/2013.

Employee slipped while crossing the longwall pan line and felt a pain in his left knee. Later diagnosed with a torn ACL. Worked all scheduled shifts until he was taken off work on 9/24/2013.

Crew was loading empty rock dust sled unto a supply car using a scoop, car was parked at end of track and was being loaded from inby end of car. Scoop became stuck on supply car and employee went to check when he became caught between the car and bucket of the scoop. Accident was investigated by MSHA, Pa. DEP, GMS, and Consol Energy

A chute was lifted by overhead crane with wire chokers & clevis's, chute was lowered back to the bottom to reattach chokers to adjust angle. The employee was inside the chute passing the chokers across, the chute slid down the slope appro. 16 ' stopping at the slope bottom. The employee attempted to move with the chute but it came in contact with his right foot and leg.

Employee was removing tubes when he fell, striking him on his left arm area, which caused him to fall over the tube laying behind him on the ground. As he was falling his hand struck the ground, he felt a tear/pain in his left arm/bicep area. EE began missing work on 7/29/2013 making him a lost time accident. EE will be having surgery on 8/6/2013

Employee was traveling on the haulage when his motor lost power and the brakes locked up. The sudden stop caused him to feel a pain in his neck. Lost Time effective 4/28/14.

In the process of cleaning out the dust box the employee lost his balance and fell forward into the bolter controls and activated the rotation which closed the dust box door on his right ring finger causing a laceration.

Was removing belt structure on the f-22 longwall when he stepped in a muddy hole and slipped causing his knee to twist. Worked until 10/16/2013 and then started missing work.

Employee was walking down the pan line slipped and fell and felt pain in his right knee. Employee continued to work until 12/5/2013. Employee started missing work on 12/5/2013.

Struck by falling object

While working as a rib bolter in the BMX Mains Right Section a piece of Rib measuring 56"" X 30"" X 14"" struck the employee on his left side and ankle. This caused a fracture to the employee's left ankle.

Employee was walking along E track between 66 and 67 block, tripped and fell striking his left knee and left elbow against the mine floor.

A rib bolter operator located on the left side of the miner was in need of a longer drill steel wrench and motioned to have the opposite side rib bolter to throw it over the top of the miner. When the employee threw the wrench over the miner he was unaware of the tubers position and the wrench struck him in the right temple.

Employee was walking in the #3 entry of the 16I Longwall when he felt a pop in his left foot. He was diagnosed with a fractured bone in his ankle. Management is continuing to investigate this incident.

While attempting to lift a 1250HP motor, using a forklift and a chain. The injured employee was holding the chain as the end loader operator was positioning the forks of the end loader under the chain. The left fork pinched his left little finger against the chain. Causing a laceration that required stitches to close

He was hosing out drip pan on head of 3 north #1 while standing on a step ladder and fell on his left shoulder when he lost control of the hose and was hit in the face.

BMX employee was lowering his clothes basket at the beginning of the shift when the chain slipped through his right hand causing a laceration to the palm of the right hand.

Employee was working in the BMX 2 A Headgate section. After the crew finished pulling some miner cable, he closed the cable arm towards the boom pinching his left ring finger between the boom and the cable arm.

Employee was bending over toward his right side to pick up two standards, one in each hand he felt pain in the right side of his back. He did not start missing work until 05/06/2013.

While working as a miner helper in the BMX 3 East Mains Left section a piece of roof rock fell through the roof mesh and struck employee in the right lower leg.

While operating the head gate drum of the 16I longwall a piece of rock flipped off of the drum and struck the employee on the left hand. Causing a fracture on his left hand.

Employee was shoveling on the E-24 Beltline when she felt pain in her lower left back and upper leg. Started missing work on 7/10/2013.

Employee had the Hi-lift Jack under bed rail to take slack out of the belt chain .After the belt chain was adjusted he began to lower the jack. The handle from the jack struck him under his right jaw. He broke the front tooth & received a cut on his tongue requiring 7 stitches.

Employee was pulling miner cable and water line when he felt pain in his right groin area. Diagnosed with hernia.

Walking down entry stepped on the edge of a depression and rolled his right ankle.

While center bolting in l-Tail, employee finished drilling a 16ft hole and began taking the drill steels out of the hole, when he removed the first one with his left hand he lost his grip on the other three that was in the roof. The other three subsequently fell and struck his left thumb causing a laceration and fracture.

Employee was helping with track rail recovery, as rail was being pulled outby by a diesel locomotive. As the third rail was passing the switch tie the employee attempted to lift the leading end of the rail over the tie with a bar. The rail came in contact with the switch tie and moved it outby catching the employees foot between two ties. (fracture to the right ankle)

Working in muddy conditions. Employee began missing work on 7/10/13 due to a torn meniscus and will be having surgery sometime next week.

Employee was trapped between the conveyor chain and the boom of the continuous miner located in the 3 north mains section. Employee has mass trauma and multiple fractures and was transported by life flight helicopter.

Injured was loading supply cars in mine yard when he slipped on ice and fell fracturing 2 ribs.

Employee was working in the BMX 2A Headgate section as a beltman. As he walked past the tailpiece he slipped/tripped and fell striking his left cheek against the tailpiece causing a contusion.

Employee was lowering bail on muck bucket and smashed finger.

Employee was struck by the loose end of a gunite hose. Employees were trying to unclog gunite hose and when pressure was applied the end of the gunite hose whipped around striking the employee on the left ear causing a laceration. Employee received nine sutures.

Crew was pulling long wall shields, employee was connecting boom of mule to long wall shield when his left hand was caught in a pinch point between the boom and the bottom of the shield.

Employee states he felt pain in his stomach/groin area when lifting vulcanized press parts from a flat car on 7/2/13. Medical treatment was obtained but this did not become a reportable accident until employee lost time due to surgery on 8/12/13.

Employee was assisting 4 others lift one end of an 85# rail which was protruding off the side of the rail truck, as they threw the rail on the truck, the rail being repositioned bounced back very quickly and caught employees' thumb between the rail being repositioned and the pile of rails on the car.

Using rock bar to move forms, bar slipped causing EE to fall on truck bed, twisting left shoulder.

Employee attempted to remove rail pulling ""Finger"" from two overlapped rails. top rail slid off bottom rail and pinched left middle finger between the rails causing a laceration. required 10 stitches.

Injured tripped and fell while cleaning up the work site. He tripped and fell. His right hand landed on a piece of glass. The glass pierced his glove and cut his hand.

Employee was prying a plate from the cable trough on the longwall face conveyor, as he was prying the plate with a slate bar, the plate suddenly sprung free and the bar hit the employee on the side of his left foot.

Employee was working at new office/bath house for new #7 north portal. Employee cut his left index finger with 4.5"" angle grinder while cutting a 4"" vertical PVC pipe below grade. Finger was cut when his glove was caught between blade and guard while he was stopping his cut. Employee received six stitches at urgent care facility.

Three inch metal water line slid down the slope, knocking the injured to the slope floor. Injured suffered bruises to the back and shoulder. He also had abrasions on his left ear that required 3 sutures.

Employee stepped onto duckbill of 630 scoop with a 15' X 4"" steel I beam at hip height, he slipped and fell cutting his finger on beam. (required stitches)

employee was putting Inbed structure together when he mashed the tip of his left thumb, A bone chip was found and was removed 8/16/13 and he received stitches.

Employee stated that he tripped striking his left hand against a rib pin plate and pie pan causing a laceration requiring 8 stitches.

Employee was bolting on the side of the miner when a piece of rock fell from the roof striking the employee on the right side of the face resulting in nine stitches to a cut below his right eye.

Employee was side bolting on miner in 5 South Left mains. He put his hand on top of the guard used to cover the torque gauge, and the tip of his left ring was pinched between the guard and the mast jack.

The employee was working in the BMW Mains Left Section when a piece of rock measuring 18""X18""X1"" came out of the top and struck him on the right forearm.

Employee was walking near the end of the track and tripped over the rail causing him to fall. The employee put out his hand to attempt to break his fall and injured his left thumb. X-rays taken at the hospital showed that the employee had a dislocation to his left thumb.

Employee was recovering rail in 16I LW. While trying to remove the rail links between the rail, the rail needed to be raised to order to release pressure between the link and the ground. As the employee was removing the link, the rail slipped out from the bar and pinched his left index finger between the rail and a rock.

Miner was carrying materials down 14 degree slope from mine car to bottom station. He slipped and his ankle rolled. He was evaluated at the hospital and diagnosed with a bone fracture to his foot and possible ligament damage.

Employee was cutting plastic covers with a utility knife. When walking to reposition himself he tripped on the pallet and stuck himself in the left hand with the utility knife that he was carrying in his right hand with the blade open. Employee received 3 sutures in his left hand. Employee returned to work on his next regularly scheduled shift.

Injured was installing 8' bolt off left side of miner. he finished drilling with 5' starter drill ,stopped and attached short driver and pushed into the roof while rotating. He noticed the steel bending and left off and grabbed the steel. The steel sprung,striking left hand between thumb and index finger.

Employee was walking to the back of the center bolter located in the 3 North Left Section when a piece of rock fell from the roof striking the employee on his hard hat causing a laceration to his forehead above his right eye. Employee received 4 staples and returned to work on his next regularly scheduled shift without missing anytime.
