The Slope hoist rope was damaged and taken out of service for more than 30 minutes. This prevented equipment from being hoisted out of the mine. It had a kink in the wire hoist rope and needed taken out of service to re-teminate the wire rope. How the kink occurred is not known. The strands were not damaged and no injuries or entrapment as a result of the kink in the rope.

The Bailey (BMX Portal) Elevator was out of service due to an over travel fault. No persons were injured due to this event.
Enlow Fork #1 hoist elevator was down out of service for more than 30 minutes. A wire in the trailing cable was damaged.

An unintentional roof fall occurred in the 14 I Tailgate entry between 18 x-cut and 19 x-cut. (Spad location 44+60) The fall measured approx 40ft long and 9 ft high by 16ft wide. No injury of equipment damage as a result
An unintentional roof fall occurred in the #2 x-cut of the 14 I longwall section, the Spad location is 2+70. The Fall measured approx. 23ft wide by 20ft long by 8-10ft high. Not injury or equipment damage.
The cover preventing water from coming into contact with the inspection remote was not in place. This allowed water to contact the electrical components and shut the hoist elevator down. The hoist elevator was down more than 30 minutes.
A roof fall occurred above the anchorage point on the C-track at 63 wall. No injuries, no affect to ventilation, escapeways not affected and no damage to equipment.

Roof fall occurred on the E-22 Longwall Headgate. The convergence extended from the #1 shield outby approximately 30 feet the entire width of the #3, belt entry. The longwall face is sitting at approximately 30 x-cut. The Converged area did not drop to the mine floor and left and opening 3' by 3' on the walkway side. Mine was idle at the time of the fall. No injuries.

The buckland emergency hoist was inoperable for an extended period of time. The drive card was replace. No injury.
An unintentional roof fall occurred just outby 6 x-cut in 4 south mains. This fall occurred inby the 2 east section four way, in the old I track entry. The fall measured approx. 60 feet long by 16 ft wide by 8 foot above anchorage. No injuries or equipment damage as a result of this incident.
An unintentional roof fall occurred in the #3 entry of 5 H. This fall occurred at 6 x-cut. The fall measured approx. 18ft long by 16ft wide by 10 ft high. There were no injury or equipment damage due to this incident.
A roof fall above the anchorage point occurred on the 1 North #2 belt at #40 crosscut in #5 entry. No injuries resulted from the fall. Ventilation was not affected. Damaged equipment included belt structure. Dimensions of the fall were approximately 15.5 feet wide, 16 feet tall, 30 feet long.

The Dry Ridge Elevator was unoperational. The elevator stopped in the shaft due to a govenor switch alarm. There were 3 employees on the elevator at the time it stopped. No injury's as a result of this incident
The hoist elevator at the Enlow Fork Mine #1 portal was out of service for more than 30 minutes. The programming system was not communicating with the hoist to allow the elevator to move up or down the shaft. The P.O.C. was reprogrammed and the evelator put back in service. No one was on the elevator at the time of incident. No injuries.
The scissor hinge on top of the Pleasant Grove elevator / hoist door weld broke loose and fell down the shaft. This allowed the elevator / hoist door to open while in use. Elevator / hoist immediately removed from service. No entrapment or injuries as result of this incident. Elevator back in service at 2:27 PM.
The Crabapple elevator was inoperable for longer than 30 Minutes.

Pleasant Grove elevator was stuck at the bottom of the shaft and would not respond when activated. The DC contactor went out on fault. DC Contactor replaced to return to operational capacity.
An unintentional roof fall occurred at 6 1/2 x-cut in 4 South. The fall measured 16ft wide by 20 ft long by 12 ft High. No injury or equipment damage as a result of this incident.
The East Finley Elevator was down due to the door hold button Malfunction. A wire came off and the unit would not function properly. No damage to the elevator and no one was on it at the time of incident.
The slope hoist blower motor quit running thus taking the slope hoist out of service. This prevented personnel from being hoisted out of the mine. No injuries as a result of this incident.
A scoop was being hoisted out of the Enlow Fork mine slope. The scoop was secured to dollies and the chains broke at the 1200 foot mark. The scoop slid off the dollies and continued to slide to the 1800 foot mark, having jammed into a man hole. No injuries or damage to the slope hoist.
The latch on the outside door at the bottom of the elevator shaft would not latch in order for the elevator to be operational.
Miner entered the elevator, when he pushed down button, a power failure occurred (power company lost grid). The elevator drifted upward 2 to 3 feet above normal stopping point. The elevator lost it's positioning and had to be re-set. Once the power was restored the elevator re-set and positioning was re-established. The elevator moved and the miner was able to exit. No injuries.
At approximately 12:05am on January 5 2011, an unexpected roof fall was reported to mine management. The location of this fall was #63 x-cut, mother #3 beltline in area 2 of the mine.

employee was cleaning the gunite machine while the machine was running. the auger caught the injured person's glove, which resulted in a laceration to his right index and middle finger.
While walking along the longwall face and using the handrail, a rock fell from between the sheilds striking him on the left ring finger. Resulting in amputation just below left ring finger, finger nail.
Employee was carrying his personal tool box (2'x10""x10""), he reached out to set the toolbox down and felt a pain in his lower back.
While walking 14 I belt line, employee tripped and attempted to catch himself on a piece of belt structure and injured his little finger on his left hand. Employee began missing scheduled work days on November 28th 2011
While installing a rib bolt off of the Voest-Alpine miner rib bolter the bolt wrench broke and the socket end struck the employee on the right side of his upper lip. Eight sutures to close the wound.
Employee was attempting to remove a shaft from the rack wheel on 14 I long wall shearer, He got the end of his ring finger and pinky finger smashed while using a slide hammer.
Employee was attempting to shut a K-door in J- Bleeder section, when he was struck in the back of his left hand by the other door that closed inadvertantly. Employee was diagnosed with a fracture to a bone in his left hand.
While moving miner cable employee felt a pain in groin area. Diagnosed with a Hernia on 10/24/2011.
While roof bolting off the left side of miner in BMX section, a piece of black rock from the top of rib struck the employee on left hand and forearm.
F-22 HG section, track entry, 3 1/2 to 4 Cross cut, employee was helping to spot a track rail when the rail slid off the scoop, bounced, striking him below the left knee. Left Patella avulsion fracture- left patella tendon rupture.
Employee first reported a groin strain 9/16/11 but did not need medical attention at that time and continued to work. EE developed a bump which was found to be a hernia and went off work April 9, 2012 for surgery, This will be a lost time occurred.
Walking to the loader, he stepped on a rock and twisted his left knee. Original RPI on 9/14/2011. He became lost time on 10/14/2011.
Employee previously complained of pain in finger of right hand while operating a weed/grass trimmer. He was promoted to a job with no vibration and stated that he again had pain in the finger so he sought medical treatment. He was given three days off and returned to full duty.
While taking the slack out of the Joy Continuous Miner conveyor chain, he was standing on the conveyor deck between the flights at the back of the boom. He was hitting the conveyor link with a 3 lb. sledge hammer when the link came apart. As the chain slid off the tail roller it caught his left foot. He fell 4' to the bottom landing on his left side.
Employee was struck on the left wrist by a piece of mine roof while bolting in 4 West section
While working as a rock duster at BMX, Employee was cutting off a hose coupler when the utility knife slipped off the coupler cutting his left leg.
Employee was walking down F-15 intake overcast ramp when he slipped twisting his right knee
Employee was pulling a welder cart in the 2 east section and felt a pain in his groin. Diagnosed with a hernia on 7/28/2011, Employee has not missed any scheduled work days.
Employee on top of CM cleaning off rock when he slipped, caught himself with arm against rib.
While bolting on the Right side of the miner in 4 West section, employee reached for an 8 foot cable bolt and felt a pain in right shoulder. **Lost Work Days began on July 18th 2011
Carrying a return roller along 3N#2 Beltline when stepped awkwardly on right foot resulting in a distal fibula fracture of right ankle.
Re-railing a coal car used for trash with an air bag. With car up in the air, the injured hit the trucks on the car with a 4 pound sledge hammer and felt sharp pain in left shoulder. did not miss work until 1year later on 6/8/2012.
Employee was carrying roof mesh along the left side of the continuous miner, when he stumbled and caught his left hand between the rib and the continuous miner rib protector. Resulted in a fracture to the tip of his left ring finger.
Felt pain in right shoulder while lifting valve.
EE stepped in wet cement during a cementing operation at the site of a well plugging operation 5/20/11. A chemical burn developed over the weekend on his left foot. The EE sought medical attention on 5/23/11.
Crew was moving rail with pull straps (4 men) employee fell and pulled rail onto his right foot.
Employee was changing bits on the tailgate side shear drum on the E-22 longwall section. Rolled drum and his left ring finger was caught between the bit ring lug and the face. Resulted in 10 sutures.
The injured employee was helping carry a belt roller through a walk through door, he tripped over the bottom of the door jam, which caused him to twist and he felt pain in his left hip. He continued to work until he was taken off of work on 4/26/2011.
Injured Employee was carrying conduit and twisted his knee while walking. There were no obstructions. Wet and and sloping terrrain.
Employee was picking up conduit from a stack. Lifted four 3/4 inch pcs by one end and lifted up. He proceeded to walk into the pile in order to place conduit on his shoulder for transport. As he move into the stack he lost his balance and fell. He reported the fall within minutes. He said he was fine just a little sore. There were no obstructions of hindrances.
Employee tripped over a 1"" air hose and fell onto his left shoulder, causing a dislocation of left shoulder.
While working in the motor compartment of a rail mounted jeep, the compartment lid fell on his right hand causing a laceration.
Employee was bolting on the right side of the miner in 5 South section. The bolter head plugged. As he was cleaning the bolter head, the drill steel that was in the roof fell and struck him on the right fingers. Fracture to his right ring finger and 4 stitches to his right middle finger
Employee slipped on a 16' cable bolt in the 16H section and felt pain in left knee. He continued to work all scheduled shifts until 4/1/2011.
EE was removing pipe when a valve came apart causing approx. 120 psi of water to hit him.
While dragging a ventilation tube around a corner he rolled his left ankle and fell to the mine floor.
While turning off water on longwall shield spray hose, a rock fell striking him on his left hand and wrist. Result in fracture to carpal bone.
While bolting in the BMX section employee bent down to remove material away from the dust box door when a piece of slate fell from the top and struck the tip of his right thumb.
After raising jock on canopy of bolter, he got in the operator compartment to tram machine. When he began to tram the front of the canopy slipped off the chasis and fell striking him in the right shoulder and arm.
Employee was moving tool cart with another employee. While moving cart, cart slid sideways smashing employee's left hand against a FCC screen.
While pulling a rib drill out of the locked position, Employee inadvetenly hit the rotation lever for the drill which caused injury to his left little finger
Employee was steadying a muck bucket and smashed finger between loaded bucket and empty bucket.
Cutting water hose with belt knife, slipped cutting thru gloves and cut base of left thumb.
Employee was struck in face with shotcrete hose.
EE and another employee was carrying a sand prop when ee lost his footing and fell, in the event to catch himself he run his left hand across the flat sharp edge of the prop he was carrying. EE suffered a laceration to his left hand in the thumb area requiring 5 stitches.
Employee was walking on a plywood plank and it collapsed.
While walking to engage the mud pump, EE's foot became entangled in hydraulic lines causing him to nearly fall. His right knee struck a 4"" valve on the mud pump causing a soft tissue injury and an abrasion accompanied by some swelling.
While changing a turnover roller the bearing housing turned, catching the right thumb between the housing and the housing base, crushing the tip of the thumb.
While working in the supply yard, employee was unloading junk off a supply car. While pulling metal bands out from under k-doors, the metal cut through his glove and cut the palm of his right hand. He was taken to the hospital where he received 4 sutures. Released to return to work.
Employee was carrying block to build a wall when he stepped on a rock and rolled his ankle
While loading a piece of rib material into a scoop bucket, a rock fell from the roof and struck his left middle finger, received 21 sutures.
EE was grinding when wheel caught Z girt and kicked back
A piece of rock fell from between two roof straps and struck this employee on the back of the head, resulting in a laceration that took 6 staples to close. Employee did not miss any work due to the injury
While attempting to seat a bearing into a scoop articulation plate, the bearing did not seat properly, he struck the bearing with a hammer. A piece of metal sheared off the bearing and lodged in the right side of abdomen. He was seen at the hospital and it was determined the metal was best left in place. Employee had medical treatment to have the metal removed on 09/02/2011.
Employee finger caught in wench
Employee stood up on the bench in the bathhouse to put his boots on top of his locker, lost his balance and grabbed the inside of the open locker. His left hand slid door down the side of the locker and his left ring finger caught on the door latch. This resulted in a laceration requiring 6 sutures to close.
Crew was installing PVC conduit (6"") in an open trench. At the end of an existing pour there were rebars exposed with mushroom caps installed for protection. One cap was knocked off when an employee slipped on muddy ground and fell into the rebar, cutting his lip and nose. First aid was given and employee was taken to the hospital Express Care as a precaution.
While hammering the split set driver off steel rod, piece of hammer broke off and embedded into left inner forearm.
EE was in the process of identifying 20 amp 277 volt circuit to verify if it was energized and if so to de-energize lockout and tag the circuit. He grabbed the 12 gauge MD pig tail cable and other hand contacted light fixture, sustained an electric shock. EE sustained small burn on his right hand and experienced chest pain. Transferred to local hospital for monitoring overnight.
The worker attempted to lift the pump in order to relocate it, and a loose part of the pump flung up and struck the worker on the right side of his face. The area was wet and muddy with an uneven bottom. The 24 inch flexible line was stuck and it broke free. He was approximately 150 feet from the face of the slope.
Was using a cobra drill when the dust discharge line became blocked, employee removed discharge hose and tried to clear the blockage in the discharge port with his finger, fellow employee holding the handle accidently touched the trigger cutting the end of the first employee's finger
Employee was retrieving a tool from the section parts car and pinched his right ring finger between the tool and the parts car. Resulted in a fracture to the tip of his right ring finger and a laceration that took 3 stitches to close.
While removing the valve back cover on the 4 south miner, the raised lid fell suddenly and struck his index finger on his left hand. Employee received 3 stitches the under side of his left index finger
Employee wrapped arms around a 5 gallon bucket to carry it. When setting it down, a sharp edge lacerated his arm.
Employee was dragging a rock dust hose in the track entry of 2 east. He was walking backwards as he was pulling the hose. He tripped on a track tie and fell on to his M-20 Self rescuer causing a fracture to the 12th rib on his left side.
Employee was carrying two 1/2 ton come alongs, fell forward, striking his lower lip and jaw. This caused him to bite through his lower lip. Resulted in six sutures. Employee returned to work next scheduled shift.
Pulling on water and air hose on steep slope - (Oak Springs). There had been light rain and the temperature was cool - 37 degrees F.
Employee was carrying a barrel for a pump when he tripped on a clamp for a pipe and the barrel smashed the little finger on his left hand.
Diagnosed with bi-lateral carpal tunnel syndrome.
Employee was diagnosed with bi-lateral Carpal Tunnel Syndrome and company notified on 9/20/2011. No surgery has been scheduled. Employee is currently working as scheduled.
The mine has experienced a death in the 1 East Bunker Control room. Employee was sitting in a chair in the bunker control room when his lost consciousness and stopped breathing. EMS was notified, transported to the hospital and later pronounced dead by the coroner. Cause of death Cardiac Infarction. This is not a mine related injury. MSHA report no contributing factors.
