While preforming the required exams of the Crabapple Hoist rope, an area of the rope showed signs of deformation. The Hoist was taken out of service for the cable to be re-terminated.

The Crabapple Hoist car derailed as a result of ice build up on the slope track. The car was hoisted up the slope which caused damage to the Contactor Panel. The car was taken out of service to repair the damage.
The Crabapple Slope Hoist was taken out of service for an extended period of time due to deformation in the hoist rope. Possible cause for rope deformation - slack was put into the rope at the bottom of the slope causing the rope to double back on itself. The hoist rope was re-terminated to repair the deformation.
During an MSHA inspection of the 6 South #1 escape capsule, on the 3rd trip of the capsule, the capsule got caught up on the guide cables at the bottom on the south side of the shaft. One of the cables was broke. The capsule was retrieved and the hoist was taken out of service to repair the cables on the bottom of the shaft.
3B continuous miner section hit an uncharted gas well in the #2 entry just outby 45.5 wall.
An unplanned roof fall occurred in the #1 entry of 6 South area. The fall occurred at 102 crosscut in a return entry. The fall was caused by adverse roof conditions that affected the anchoring zone of the primary roof bolts. The fall was approx. 120 ft long by 16 ft wide by 12 ft high. No equipment damage or injury as a result of this fall.
An unintentional roof fall occurred in the 2 East #5 entry at 99 wall. The fall measured approximately 14 high x 25 wide x 30 length. The immediate roof consisted of laminated shale material. Adverse geological conditions and horizontal stresses contributed to the failure. No injuries occurred because of this incident and no compliance issues were found.
The crabapple hoist was out of service an extended period of time. It was found that 2 safety central relays needed changed to repair the hoist. A possible cause was a power outage.
There was an unplanned roof fall in 4 west outby area at 22 crosscut between 2 and 3 entry. The belt nor the track entry were affected. The fall is estimated to be 40' long 16' wide and an unknown height.
There was an unplanned roof fall in 6 South section #8 entry 104 crosscut. Adverse geological conditions caused the roof to separate and break. It was measured 12' high x 60' long x 16' wide. ""The conditions contributing to the fall in 6 South mains area was Adverse Geological conditions in the form of a fault line possibly caused by a stream valley""
The crabapple elevator was taken out of service due to faults from the leveling arm system. The leveling arm had seized up and needed to be lubricated to return to normal operation. The elevator returned to service at 7:50PM.
There was an unplanned roof fall at 4 West 20 crosscut #2 belt entry. Adverse geologically conditions caused the roof to separate and break. It measured 22' wide x 76' long x 16-18 ft. high.
An unplanned roof fall occurred in the 11L area at 8 crosscut #1-2. Adverse geological conditions caused the roof to separate and break. The roof fell at the crosscut connection of #1-2 crosscut and into the #2 entry. It fell 60' in length and 20' wide in the track intersection.
An unintentional roof fall occurred in 3 North Mains 12 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the 8' bolts to fail at their couplers.
An unplanned roof fall occurred in the 6 south area at 49 1/2 wall in #4 entry. Adverse geological conditions caused the roof to separate and break. The roof fell at the crosscut connection of #3-4 crosscut and outby entry. It fell 15' into the 4-5# crosscut and inby entry.
The Dry Ridge Elevator was taken out of service for an extended period of time to address equipment fault alarms. The Main Drive computer as well as all encoders were changed to correct the issue
The Crabapple elevator was taken out of service for a period of time due to drum warmth. Industrial Elevator (repair contractor) found that the brake rods were not fully releasing. They adjusted the rods so that there was no longer contact during operation.
The elevator at BMX portal was out of service due to the control transformer malfunctioning. The transformer was replaced and the elevator returned to service.
The Crabapple elevator was taken out of service for a extended period after it began to operate abnormally. The main drive computer was changed to correct the problem. There was no escapeways affected and no entrapment as a result of this incident.
An alleged face ignition was reported on the H2 section while mining the #3 Return entry. During the investigation Sulphur balls were found in the face area. MSHA cited two missing/worn bits on the right side of the miner head, and the methane monitor knocked power at 1.5% but stopped spanning at 2.2%. No injuries occurred.
Hoist was descending when mantrip went off track due to ice buildup along slope track. The rope had slack when mantrip derailed causing a kink in the rope. Rope was re terminated, and ice removed. No damage to the rails were observed. Ice buildup was due to bad pump and frozen line. Pump was replaced to correct condition.
Employee was cleaning rock out of the cable trough on the longwall panline/ A piece of rock fell from between the shields and struck their left hand. Employee went to the hospital after the shift and was diagnosed with a fractured bone in the hand.
Employee tripped and fell while walking past the loadcenter in 2K section. Employee used right arm to catch self and felt a pain in right elbow. Employee was diagnosed with a fractured Distal Humorous (small bone in elbow)
Employee was lifting a Barrel Claw to attach it to a 55 gal drum of oil to unload the barrel from a supply car when they felt a pain in left shoulder. Employee continued to work all scheduled shift until they were taken off of work on 1/19/2023 for shoulder treatment.
Employee was operating a locomotive and struck a broken rail which caused EE to d-rail. When EE came to a sudden stop EE struck EE's mouth on the inside of the operator's compartment causing a laceration.
Employee was changing out a roller on the beltline and it slipped out of EE's hand. EE went to catch it and got finger caught between the roller and a crib causing injury.
While walking outby from the face past the loading machine, employee tripped on the cables and fell to the ground, landing on right knee and shoulder.
Employee was cutting the tabs on a bucket of rivets to get the lid open. EE was cutting the tabs left handed and was holding the bottom of the bucket with right hand. The bucket is plastic. So being it was cold outside the plastic was cutting harder than usual and EE was exerting more pressure to cut. When they cut through they cut their right hand.
Employee was roof bolting from right side of the miner at approximately 80+20 survey station. The upper portion of the rib became dislodged striking the employee on the back. The rock measured 9'longx 34'highx8' thick. This action caused swelling in the employees back. No compliance issues were found. No other part 50 criteria apply.

Employee was lying on the ground on the walk side of the belt changing bottom rollers on 6 South belt. An employee was placing a roller on the top of the top belt when the roller fell of the top belt and struck the injured employee in the left hip.
Employee was bolting on the left side of the miner. Rib bolter was swinging out the rib drill to install rib bolt when the drill encountered the roof bolters drill steel. Dislodging the drill steel from the holder and striking the employee in the right cheek. This action resulted in a fracture to the cheek bone. No compliance issues were found.
Employee was attempting to remove a keeper pin from a dog bone using a pick hammer. Employee hit the dog bone with the pick hammer and a piece(s) of metal broke off causing injury to EE's eye.
Employee was standing on the axle of a scoop between the batteries and the pivot point. After disconnecting the charging cables from the batteries, employee slipped on the axle and right foot was caught between the batteries and the axle resulting in pain. Injury was not the result of a training or compliance issue.
The mantrip that the employee was sitting in was struck by another mantrip. The injured employee braced themselves and felt a pain in right shoulder from the sudden movement. This employee worked all scheduled days until they were taken off of work on 10-4-2022.

Employee received minor chemical burns on right calf/knee area. Employee worked unrestricted until 8/23/22, began missing time 8/24/22. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training & attitude; or protective items relating to clothing, or protective devices on equipment
Employee was walking to throw the 2 East Mains switch, while employee was walking the employee rolled an ankle. Resulting in an avulsion fracture. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved. No other part 50 criteria applies.
While assisting a pan line bolter installing bolts, the injured employee's left pinky finger was caught in a pinch point when the bolter operator dropped the mast down. Injury was not the result of a training or compliance issue.
Employee was walking out of the 12J section, left foot stepped in a shuttle car rut causing employee to slip and fall. Employee felt pain in left ankle. Employee was subsequently diagnosed with a fractured bone in the ankle.

While removing a rock from the handrail on the 9L longwall face. The rock shifted and EE's middle and ring finger were caught between the rock and the shield causing a laceration. EE was assessed at Ruby Hospital where EE received 6 stitches.
Employee was flushing lines and left leg and foot came into contact with silicate material. Injury was not the result of a training or compliance issue.
Employee stepped on the rail, EE's foot slipped, and EE felt a pain in EE's left foot. EE was examined at Wheeling Med Express and was diagnosed with a fractured 5th metatarsal.
The employee was loading supplies when EE miss stepped on uneven road material. EE felt a pain in left ankle. On 6/12 EE went to Med Express to get it checked and was diagnosed with a fracture to the left fibula.
Employee pinched right thumb between a slate bar and a belt crossover while installing an arch on #5 mother belt. Employee received ten stitches to the thumb.
Employee was assisting 3 other employees lifting a DB box to release it from the chains that it was hanging on. EE felt a pain in EE's left bicep. EE continued to work all scheduled shifts. The pain in EE's arm did not get any better so EE saw a doctor on 4/20 when EE was diagnosed with a torn Bicep tendon that would need surgery to repair. Lost time as of 4/20/2022.
Employee was throwing frogs into coal car and got finger caught between frog and garbage car.
While shoveling loose coal in the prep plant, employee felt a pop and pain in left shoulder. As they fell down due to pain, left foot got caught and employee twisted the left knee as they fell down. Injury was not the result of a training or compliance issue.
While employee was stepping off the platform of a track mounted bolter and struck EE's knee against a piece of guarding. Injury was not the result of a training or compliance issue.
Employee was handling a piece of steel grating near the slope tail, and unexpectedly, the grating struck employee in the mouth/head area causing injury. The injury did not involve PPE issues, protective devices, training or attitude issues or mining equipment.
Employee was throwing the miner and loader cable and the water line out of the walkway in 11L and felt a pain in the left knee. Employee continued to work all required shifts until being taken off of work for surgery on 11/30.
Employee was bolting on the left side of a CM miner, and experienced suction problems. Employee was cleaning the lower seal on the dust box, when EE's hand was struck by the dust box door. Suffered fractures to the 2nd and 3rd metacarpal.
Employee was involved in the removal of track rails on 11J longwall section. A cable was connected from a mantrip to the track rails with a clevis. While pulling the rails with the mantrip the clevis came loose and became a projectile. The employee was struck in lower left leg by the clevis causing fractures to the leg.
Employee was side bolting in the 12J Section, as EE was retrieving 6 foot bolts from on top of the rear of the miner EE's right foot slipped off the step. EE's foot got caught in the miner cable and EE felt a pain in EE's right ankle. EE continued to work all EE's scheduled shifts until EE began missing work on 2/25/2022.
Employee was walking around the tail of the slope and EE stepped wrong and rolled ankle causing injury.
Employee was transporting supplies through the mine on a motor, when the supplies caught an air hose hanging against the mine roof. The hose fell and hit the employee in the face. Surgery for a deviated septum on 6/7/22.
Employee operating the outrigger on a leased truck and inadvertently had EE's r. hand in the hinged area of the outrigger when putting it up. Pinched EE's r. ring finger in the hinge of the outrigger.
Employee working on impact channel. Channel rolled smashing employees left hand. Hairline fracture of wrist resulted.
Employee was bent over stacking block along the rib in preparation for building a stopping. Another employee unfastened the hooks holding the miner water supply hose. The hose fell down and struck the injured employee in the back of the neck. Injury was not the result of a training or compliance issue.
Employee was bolting on the left side of a CM Miner. Employee was standing behind the rib protector door, after starting the miner head, a rock flew out over the pan and struck the employee's left cheek resulting in a laceration. The employee received 6 sutures. No compliance issues were found.
Employee was pulling bits from shearer using a hammer and bit puller and accidentally was hit by a piece of metal in the arm causing injury.
Employee was using a pry bar to move a roller. The bar slipped out from under the roller causing EE's hand/finger to be caught between the bar and the belt structure causing injury.
Employee was bolting on the miner and EE's left hand was caught between the ATRS jack and the frame of the machine. The ATRS pad slipped off a ledge when employee reached for a drill steel. Employee was treated for a fractured little finger and received 3 stitches, release to return to work.
Employee was cutting a flapper on the belt and the knife accidentally slipped out of their hand and came in contact with their forearm causing injury.

Employee was removing tangled up belt when the knife slipped causing a laceration to right leg. They received 2 stitches.
Employee was sitting on the cable trough waiting for tools to service the longwall shearer in 9L section. A small piece of rock fell from between the shields, bounced off of the head gate drum and struck the employee on the right leg. This caused a laceration that used stitches to close. Employee did not miss any scheduled work as a result of the injury.
Employee was hanging a crib bag with a pogo stick and cut EE's right pinky finger on a metal roof strap. Received 2 sutures. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
While lifting a belt to change a roller the connection point failed causing the belt to fall suddenly striking the employee. Employee received a cut to nose that required stitches to close. Employee returned to work next scheduled shift.
Employee was replacing a magnetic mount on a tracking pad to attach the pad to a tool box. The pad flipped expectantly and pinched employee's left thumb between the magnet and the tool box. Employee went to a doctor on 7/5/2022 and was diagnosed with a fracture to the thumb. The employee continues to work all scheduled shift.
Employee was cutting off old belt in 4B storage unit. EE was in the process of changing blades on EE's razor knife. EE left the razor knife on the ground with the blade out. After getting a new blade out from bag, EE put hand on the ground and the razor knife to get up and suffered a laceration to right index finger which resulted in eight sutures.
Employee was attempting to remove a crib from underneath a non-powered pneumatic conveyor. As they were adjusting the conveyor, employee's finger was accidentally got caught between the crib and conveyor causing injury.
Employee was attempting to cut belt with knife and lost balance and inadvertently cut right thigh causing injury.
