Pleasant Grove portal elevator would not move from top landing due to doors not closing with door switch. The switch was cleaned by mechanics and operations resumed at 6:55 A.M.

An unplanned roof fall occured in the 13 I longwall section in the belt entry @ 27+40 and extended to 27+90. No injurys or equipment damage as a result of this incident.

One of the jacks on the rope gripper was seeping oil and bled off until hitting a limit switch, shutting the elevator off. No one injured or trapped as a result of this incident.

An unintentional roof fall occurred in the 13I section in the intake entry. The fall occured at 13 crosscut and measured approx. 70ft long by 16ft wide by 12 ft high. No injuries or equipment damage due to this incident.

There was an unitentional roof fall that occured in the 12I section at #2 x-cut. The fall measured approx. 12ft wide, 30ft long and 9ft high. This fall fell on top of an unattended 630 DBT scoop. ******No damage to the scoop or injuries as a result of this incident

An Unintentional roof fall occured in the 15H longwall section. The fall occured in the intake intry between 35 and 36 x-cuts. the fall measured approx. 65 ft long, 9 Ft High and 16 ft wide. *** No injuries or equipment damage as a result of this incident

Breaker tripped at Enon Sub Station while elevator was ascending at plesant grove portal. Power was lost to elevator causing four employees on board to be stuck on elevator for more than thirty minutes.

The linkage on the elevator doors at Pleasant Grove portal misaligned, which would not allow the elevator doors to open properly interferring with the use of the elevator for more than 30 minutes. The elevator initially broke down at 2:35 PM and was back in service at 3:30 PM

A roof fall occurred on the 1 North #2 Belt, in the #5 entry at 21 1/2 crosscut. The roof fall was above the anchorage. The only equipment damaged was belt structure that was located in the entry. No injuries occurred as a result of the fall. The approximate dimensions of the fall measured 50' long x 15.5' wide and over 12' high.

The Crabapple elevator was not operational for a period of more than (30) thirty minutes. No injuries reported as a result of this incident.

A roof fall occurred in the West Mains #5 Entry at station number 1+78.01. The roof fall was above anchorage. There was no known damage to equipment and no injuries. The approximate dimensions of the fall measured 45' long, 15.5' wide and 12' tall. The fall was discovered by mine examiner at 8:49 AM.

An unintentional roof fall occurred in the return entry of the 14I section at 22 X-Cut. The Fall Measured approx 100 ft long by 16ft wide by 12 feet high. No injurys or equipment damage due to this incident.

Employee was inspecting the elevator when he inadvertently activated the governor causing the elevator to fault. Employee was entrapped on the elevator for 35 minutes. No injuries or damage to the elevator resulted from this incident.

An unintentional roof fall occured in the 1 South area. The fall occured in the belt entry outby 24 cross-cut. The fall measured 35 ft long by 9 ft high by 16ft wide. **** No injuries as a result of this incident.

An ignition occurred at 65+74 on the F-19 Longwall Face at #92 shield. The ignition was instantaneous with no timeable duration. It was self extinguished by the shearer water sprays. There were no injuries to personnel or damage to any equipment. There was a visible sandstone channel ranging from 1"" to 6"" from 90 to 135 shield.

Crappapple portal elevator was unoperational with several miners aboard for a short period of time. No injuries reported as a result of this incident.

The East Finley Elevator was down for more than 30 minutes. The elevator was down for a fault in the contactor relay. There was no damage or entrapment.

The east finley elevator was down for more than 30 minutes due to a blown buss feed circuit breaker. There was no entrapment.

Crabapple elevator was unoperational due to the breaks needing adjustment. The Elevator was able to be operated in Manuel. No injurys were reported due to this incident. Elevator manufacturer was contacted and performed maintenance.

An unintentional roof fall occurred in the 12 I longwall tailgate entry. The fall measured approx 35ft long, 16ft wide and fell above the anchorage. No Equipment, injury or equipment damage from this event.

The Enlow Hoist was down for more than 30 minutes due to a blown SCR Rack. There were 5 people aboard the hoist.

Struck by falling object

Moving structural beams in to place. Used a plate cart to move a beam assembly. Instead of lifting the beam from the cart with the available chain hoist, the employee dumped the beam from the cart. The beam caused the cart to roll, allowing the beam to move and fall on to the employee's left foot - striking his metatarsal guard.

Employee was attempting to bend bracket above conveyor belt by striking it with the sledge hammer. The hammer missed the bracket, struck roller, and bounced back, feeding it into the next roller. Employee was able to let go of the handle with his left hand but his right hand was pinched between the belt and the roller.

While shoveling the slope belt, employee felt pain in his right knee. Employee worked all scheduled shifts until going off work on Jan 7 2011.

Fall onto or against objects

Employee tripped on debris and fell forward attempting to catch himself he struck his left forearm on the control panel of the MSA rock dust tanks. This resulted in a fracture to his ulna bone.

Fall from ladders

The employee was ascending a ladder to tie off the ladder. The ladder was on to flat of an angle and the employee did not seek assistance to hold the ladder while tying it off. The ladder base slid out - the employee reached out landing on his left arm.

Fall onto or against objects

Employee was removing pin from monorail. Rib rolled into entry. Employee was startled, moved quickly slipped and fell onto rolled out rib injuring his left thigh area. Diagnosed with left thigh contusion. Started missing work on 10/26/2010.

Employee was stepping up onto a continuous miner, when he lost his balance and fell backwards, landing on a section foremans walking stick. Diagnosed with a fractured coccyx (tailbone) and right axilla abrasions.

While passing belt rollers between the belt in 14I section, employee got hit in the left knee with the roller. Lost work days began 9-27-2010

Employee was performing longwall shield maintenance when he got his left foot caught between the panline and toe of the longwall shield. Initial diagnosis was contusion of the left foot.

While working in a muddy area in 15I section. As he pulled his foot out of the mud he felt a pain in his left knee. Employee continued to work. ***Became a Lost Time on 9/22/2010

Employee received a splinter from a fiberglass ventilation tube, while handling the tube. Splinter entered the palm side of his left hand just below his index finger. Employee was treated for cellulitis and began missing work on 9-9-2010.

While helping to center bolt in BMX rehab area, he was pushing 16' rope bolt into hole and felt pain in right shoulder, neck and back. Employee began to miss work on 10-02-2010.

Employee was handling cables, lost his balance, right foot stuck in mud causing him to strain right knee. Started missing work on 8/16/2010.

Employee was walking in the 14H section and rolled his ankle, which put added pressure on the inside of his foot. Employee experienced a fracture to the 5th metatarsal on his left foot.

Employee was helping to change bits on continuous miner. Pulled a rock off of the head striking himself on top of the left foot. Diagnosed with fracture to the 5th metatarsal and 5th toe of the left foot.

Employee was recovering chain used to hang ventilation tubing from a roof strap when a rock fell from between roof straps striking employee on top of the left foot. Employee was diagnosed with fracture and ligament damage to the left foot.

Fall from machine

Employee was riding into the section on a brookville jeep. Employee exited off the jeep falling and injured left forearm and right shoulder.

Over-exertion in lifting objects

Employee strained right shoulder while picking up a 5 gallon bucket filled with pieces of chain. Diagnosed with right shoulder and lumbar strain. Employee started missing work 8-10-10 due to the injury.

Employee was bolting in 16H section and a piece of rock fell from between the straps and pinched his left finger between the rock and the roof plate. Lost work days began July 5th 2010.

Employee was bolting on the intake side of the miner in 16H. A rock fell from the end of the roof straps and struck him on the shoulder. Causing fracture to clavicle.

Employee pinched his left hand between a piece of belt structure and the rib while assisting with a belt move in 16H section ***Employee began missing time on June 7 2010

Employee injured left elbow when he slipped and hit elbow against handrail while shoveling sludge.

Employee was struck in the mouth while trying to unclog a rock dust hose.EE needed 8 stitches to bottom lip.

EE was moving an electric motor across the prep plant floor on a flat cart. The EE was pulling the cart by the handle while 2 co-workers pushed the cart. The handle came off of the cart causing the EE to fall against the plant wall striking a wall mounted fire extinguisher.

While bolting on the side of the miner in 14I section, a piece of rock fell from between the straps and struck this employee on the right forearm, causing a laceration. ***8 stiches to close laceration.

Employee was bolting off the miner in 4 West. As he was pushing the roof bolt into the hole, the bolt coupler got stuck on the hole in the channel. The employee reached over to push the bolt free from the channel and it sprung catching his left hand between the bolt and the plank jack.

Employee injured right hand while using a board to knock a reducer off of a shaft, as instructed by Consol supervisor.

EE was getting out of the #25 shuttle car in 13 I section. As he exited the machine his right boot got caught in the clean out hole in the operators compartment, he felt pain in his right knee. ****Lost work days began 4-22-2010 ****

Struck by falling object

While bolting on the left side of the miner in 14I, drill steel fell out of the roof and struck the employee on the left index finger. 5 sutures to close.

Struck against a moving object

Employee was a passenger in a mantrip. EE was parked in a switch when an oncoming mantrip jumped track and struck EE's mantrip. Employee felt pain in EE's lower back. EE continued to work all scheduled shifts. EE has continue to treat for lower back pain and has been taken off of work for surgery on 6/24/2019

Struck by falling object

Employee was bolting on the right side of the continuous miner when a rock fell out of the rib above the rib protector striking the employee on the back of the neck. Employee was diagnosed with a non-surgical fracture of the C6 vertebrae in his neck.

Fall to the walkway or working surface

Two workers were pulling a concrete hose to clean out and the hose got caught on plumbing pipe and when that happened it stopped and pushed or threw them to the ground. One employee went sideways and twisted (sprained) his left ankle.

Over-exertion in wielding or throwing objects

Employee was throwing a piece of rock on to the E-20 Belt at #5 wall and felt pain in his back, neck and left shoulder. Employee began missing work 1/11/2010.

EE was walking #4 Belt South Mains when he heard a noise behind him. EE looked over his shoulder when he turned back around he walked into a fire valve, striking his upper lip requiring 12 sutures.

Employee was building crib, was putting wedges on top of crib to tighten when he grabed a loose crib and fell backward and stepped on a crib laying on the ground and rolled his ankle.

Employee was holding curtain up so the scoop could get through, his foot slipped into a rut in the roadway and the scoop ran over the end of his right foot.

Fall from ladders

Area wet and muddy. The injured man was on a ladder. He slipped and fell on his left side resulting in a fracture to his left wrist.

employee was trying to pull a bent roof bolt out of the top before his set up. his weight went back on his right ankle (causing a stress fracture)

Employee was turning the 2C fine return sieve. While raising the sieve into place after turning employee caught his left index finger between the feed nozzle and sieve holder.

Employee was the left side miner rib bolter. Stepped up on the left rear corner of the continuous miner. Miner operator dropped the tail boom. Boom came down on employees right big toe. Resulted in a hair line fracture.

Crew was slewing rail off of ties using slate bars, employee strained his back.

Employee slipped and tripped and fell on top of a roof bolt plate which cause a laceration to his right leg. 4 Stiches to close laceration

Drilling a hole in the roof to install a bolt in a strap and a piece of stone fell from the roof and struck him in the upper lip. 3 sutures to close.

employee was attempting to hook up scoop bucket, scoop tire pushed up onto his left foot

EE was tightening a bolt using a ratchet/socket assembly. Socket slipped, EE struck elbow in screen side that he was repairing.

While breaking a tool joint (drill pipe) the injured employee's 3rd & 4th fingers of the left hand were struck between the opposing tong jaws of the drill pipe tongs. The superintendent was pushing the tongs into position to attempt to latch them on the tool joint. The EE suffered fractures to the metacarpal phlanges of both fingers & lacerations requiring sutures

While the employee was using a pipe wrench to loosen 1.9 inch grout tubing, the wrench slipped and the employee's 5th finger on the right hand was lacerated as it was struck between the wrench and the tubing elevator. seven stitches were required to close the laceration.

Beltman was pushing a top assembly off a bed rail, when top roller assembly rolled, pinching his left ring finger between the top assembly roller and the top belt. Employee received 8 sutures.

Employee was installing a 16ft cable bolt using a center bolter in 15H section. As he was spinning the bolt to mix the resin, a rock fell hitting the bolt plate, spinning off striking him in the mouth. Employee received 2 stitches inside of his mouth.

Employee was installing a roof bolt, as he was pushing the bolt in the hole the bolt stopped. He shook the drill head to unstick the bolt and the bolt bent and jammed his finger into a nearby crib. Resulting in a fracture to the tip of his little finger.

Setting top belt structure on bed rails, top structure was pulled too far causing it to come off the bed rail mashing employees right thumb.

While walking the 14I belt line the employee turned to change direction and fell, he struck the back of his head during the fall. *** 4 staples to close laceration.

Injured was struck by lift apparatus swinging from hook of boom truck.

EE was using a utility knife to cut plastic mesh at the E-22 Belt Drive and while drawing the knife downward his left index finger came in contact with the plastic mesh causing a laceration requiring 7 sutures.

Employee began missing time on 12/20/2010 due to carpal tunnel syndrom. He under went surgery on this day to repair. Condition was deamed to occure over an extended period of time at the coal mine. This is considered an Occupational Illness.

Employee was diagnosed with bi-lateral carpal tunnel syndrome on 3/02/2011 and began missing work on 4/14/11 because of corrective surgery.
