was mining in 2 East section in 2 to 3 entry at #6 cross-cut when there was an alleged ignition of gas or dust. The reported flame was 20 seconds long and orange yellowish in color.no injuries.
The Dry Ridge Elevator was inoperable for an extended period of time. A cable in the bottom door switch was shorting out causing a fuse to blow when the elevator reached the bottom platform. **** no injury or entrapment
There was a roof fall in the #2 Entry of 4 South Mains at 13 1/2 wall, which did not affect ventilation. The fall measured approximately 100' long, 15'w and 10' high. *No injuries or equipment was damaged as a result of this incident.
The Crabapple elevator was inoperable for an extended period of time. No injuries were a result of this incident. The door switch needed replaced.

Broken strands were found in the hoist rope at Crabapple slope, which caused the hoist to be out of service for an extended period.

A call was made to MSHA HOTLINE in error that the Enlow hoist was down. What happen was the belts running over top the hoist house had structure break loose and the hoist was shut down as a precaution.
Pulled slope car out of the mine and found loose wraps on wheel. After trying to un-spool the cable they found that the cable was damaged. New cable and termination was done.
Elevator would not move from top landing due to bad sensor.
found pinch cable during regular inspection and changed same. Later found a bad switch and changed switch.
Elevator slowed down and then stopped and won't move.
The Crabapple elevator was not operational for an extended period of time. The Electrical door switch on the bottom of Crabapple shaft needed adjusted. There was no physical damage to the elevator, no injuries and no entrapment as a result of this incident.
A roof fall occurred in the north mains at the 59 x-cut, number 8 entry for a distance 16 feet just in the intersection. This did not impede travel, effect ventilation, or result in injuries. It did fall above the anchorage zone. Travel routed around fall.

The Dry ridge Elevator was not operational for more than 30 min. The rope grabs on the top of the elevator shaft needed readjusted. Nothing on this elevator was damaged nor was there anyone trapped or injured due to this event
The Dry Ridge Elevator stopped in the shaft which entrapped one miner. There was no damage to the elevator. The elevator stopped due to the door switch on the bottom of the shaft vibrating open. There were no injury as a result of this event
While he was miner rib bolting, the scroll auger came out of the rib while he was backing the starter steel out of the rib and the steel struck him below his left eye.
Employee was lifting a jackhammer and felt a pain in his back.
walking down ramp at a-7 inlet while firebossing and felt pain in left ankle when stepping off ramp. Started missing work on 11/15/2012.
Installing belt roller into assembly when it slipped from hand and pinch right index finger fracturing tip.
Employee was adjusting the forks on a high lift in Crabapple Yard and pinched his left little finger.
A BMX employee was preparing to exit #34 Brookville mantrip that was parked at 59 Cross-Cut on the East Mains haulage. As he began to exit the operator's compartment his left heel caught a 1"" lip that was located on the first step out of the operator's compartment. He twisted his left foot during his dismount.
crew was moving track switch timber, everyone threw timber down unexpectedly as employee was still holding on and he pulled a muscle in his back.
Employee was struck on the head and face by a rock that fell from between the roof straps.
While working as a center bolter in the BMX 1 A TG section, employee was raising the drill head to meet the bottom of the drill steel. When the steel contacted the drill head, the steel kicked towards his left hand that was holding the drill steel.
Employee was recovering ventilation tubes in L Tail return. When he was reaching up to unchained tube, he slipped and braced himself with his left hand against the rib. The tube fell and struck his left wrist.
A jib boom hinge pin on a 50 ton crane knocked a 2.5""x2.5"" pc of angle from #1 breaker bldg causing angle to fall to the ground. The injured EE was standing near the base of crane and the angle stuck him in the neck/shoulder area fracturing his clavicle and straining vertebrae in neck. EE was taken to hospital in Morgantown, WV and released the next day. This is a LTI.
while the shear was cutting out at the headgate, the employee pulled in #5 gate shield and #6 line shield. As the shear trammed toward the tail, the employee was standing on #6 shield and attempted to pull down on the swivel assembly when the bracket broke away from the shield canopy striking the employee on his forearm causing a fractured arm.
injured was closing a shut-off valve on 2"" line when line broke striking the injured in head and left hand.
A BMX employee was removing a race off of the digging arm post on the loader that was located in the 3 East Mains section, 3-4 Cross-cut. The employee was striking the race with a ball peen hammer while he was assisted by another employee that used a slate bar to apply up pressure, when suddenly the race broke free and struck employee in the left side face/mouth area.
Employee states he was erecting a boom on a crane & fell on to the crane track fracturing his R wrist, damaging ligaments in his L knee and spraining his L ankle. The employee didn't seek medical treatment until 8/5/12 & this office was not notified of the incident until 8/8/12 when the employee was taken off work by the treating physician. This is a lost time injury.
Employee was stepping over a ditch that had been dug to drain water from a work site. The ground was wet and slick from the rain. When he stepped over the ditch and his foot landed on the other side, he slipped and hit his arm on a rock hurting his shoulder area.
Injured was hanging a shuttle car cable and felt a pain in his left bicep. Did not miss any work until 09/24/2012.
Employee stepped out of tool trailer and twisted his foot on the large rocks that were used as back fill. He fell and hit his head on a tamper. He continued to work after the accident. Then on 7/26/12 he didn't return to work for his next shift. He is now claiming he is having headaches.
WAS RAISING TRS TO INSTALL RIB MESH ON RIGHT RIB. A ROLL OF RIB MESH ROLLED OFF THE TOP OF BOLTER STRIKING THE EMPLOYEE ON THE RIGHT SIDE OF THE HEAD, ARM, & SHOULDER. DID NOT MISS ANY WORK UNTIL 08/30/2012.
While working at BMX, 3 East Mains Left Side Section as a roof bolter, a piece of rock fell from the where the roof and rib meet measuring 40""X25""X4"" struck employees' top of the left foot. Employee received sutures.
Hooking up safety chains to rock dust tanks and felt pain in left shoulder. Started missing work on 10/12/2012.
Employee was carrying a 20ft long piece of pipe and tripped over track rail and fell on back.
Employee slipped while installing a 16ft cable bolt and injured his left leg. He was diagnosed with a chip fracture on his left ankle.
Installing 12' cable roof bolts in BMX 3 East right mains section. Began to install a bolt, it became lodged halfway up the drilled hole. He tried to pull the bolt out and struck left wrist against square plate causing a laceration.
on date above employee was removing belt structure on longwall headgate and felt pain in his right shoulder. He worked until 09/10/2012 at which time he had surgery.
While cutting a piece of face chain off of the longwall, he twisted his body and his foot did not twist causing pain in right knee.*** Began missing time on 5/1/2012
Employee was side bolting in the 1 A Bleeder Section at BMX while the miner was being reposition the employee was putting the drill steel up through the roof strap opposite of the miner operator, when the strap caught the operator's side rib. His right middle finger was pinched between the strap and drill steel.
Employee was standing to the front of the trailer to the side of the tongue, no work was being done or any adjusting of the jack. As he was standing to the side of the tongue the jack broke and bent to the side shifting the trailer and sliding it causing the tongue to fall on his right foot. He sustained two fractures on his right foot and dislocated 3rd right toe.
Employee was struck by a rock that became a projectile from a 4 inch airline that was being charged and cleared after installation. Injured has two fracture fingers on right hand. Employee was standing approx 85 feet inby the end of the airline that project a rock estimated the size of a 4 inches diameter.
Working as a shuttle car operator in the 1A Bleeder section, in the #5 Entry. The EE was taking some rib bolts up to the miner when a piece of top came out of the roof, struck him on the right side hard hat and slid down the right side of his body and landed on his right ankle.
A rock fell from the roof and struck the ventilation tube. The Tube then fell and struck EE on top of the head
Employee tripped and as he was catching himself he felt a pain in right shoulder. Employee returned to work his next scheduled shift. After results of his MRI he was taken off of work on 3/29/2012.
While removing a shaft from a froth cell, travel dolly got hung up. When employee went around front of cell to free dolly, employee's lanyard got caught on the reducer shaft causing employee to be pulled into the reducer.
While helping mechanic with a blown hose on miner in BMX left side mains, miner operator was struck on hard hat by a piece of rock (30""x14""x1 1/2"" thick) that fell from in between roof straps.
Shoveling pieces of slate or rock. When he picked up to throw back on belt felt shoulder pop.
Employee was using a com-a-long to fix a broken chain on the mother #2 belt at 29 1/2 wall. While tightening the chain he felt pain in his groin area. Employee finished shift. Later diagnosed with a Hernia.
While working at BMX in the 1 A Tailgate section employee was filling the rockduster hopper on the loading machine in the #2 entry, when he was struck in the lower left leg by a piece of rib measuring 4' long X 2' wide X 12"" thick.
Employee was filling the hopper of a concrete machine when dry material came up under his glasses.
Employee was lowering a used CMI cone onto a skid when the chain slipped on the fork causing the cone to turn sideways and fall onto employee's right foot.
Working in the BMX Mains section, the foreman was walking around a parked ram car and inadvertently walked into a rib strap that was damaged and bent outwards striking him in the right temple area causing a laceration. Employee received eight sutures.
Installing bottom roller pinched finger ask to take after personal days. Found out 3/26/12 they wanted to put some stitches in.
employee was stacking block in the bucket when one block from the top of the pile came down and fractured his finger
EE was rigging hopper plates, as he was leaning between 2 plates to rig them, the 12 plates that were leaning the other direction shifted and pinned him between 2 plates. Two employees retrieved a pry bar and removed him, he sustained a Fractured Clavicle.
EE and another employee was attempting to move a piece of slate that had fallen against a rock-prop. When they lifted to slate they realized they could not handle it because of its size and set it back down, as they did ee caught his right ring finger between the slate and the rock-prop smashing the tip of his finger. (fracture)
employee was loading empty pallets onto an empty general car when he mashed his finger with a pallet. (removed his finger nail and required stitches)
Employee stated he was lifting two 12"" x 12.5"" x 1/2"" plates and twisted around to lay them down in a man basket. The employee then states that he felt a slight ""twinge"" in his lower back that got worse during the day. Employee received medical treatment the next day and was placed on modified duty for two weeks. There is no lost time associated with this injury.
Employee was re attaching a pole harp when the pole came out of the pole dog and struck him above the left eye. (5 stitches)
Miner was using Hilti DX 460 powder actuated tool. He jumped down with the tool in his hand and the gun misfired or was unintentionally fired at him. There is a hole in the lower left of the back of his hard hat and he suffered an occipital contusion.
Person was installing grout on a silo wall and a stone size piece of concrete fell from the roof striking him on the hand resulting in a fractured bone in the hand.
EE was changing out worn screen springs, the lifting eye on the screen side broke allowing the screen to fall back to it original position pinching the employee's left hand in the spring coils
While stepping down from working on the belt line he miscalculated the height he was stepping down and pulled and twisted his right arm & shoulder which he was using to steady himself.
Employee states that he was hammering a beam into place and the hammer struck another beam causing the employee's finger to get caught between the beam and the hammer handle, fracturing the tip of the small finger of the left hand and tearing the nail from the nail bed, requiring stitches. Medical treatment administered and the employee returned to full duty the same day.
employee was carrying a post and a staple sticking out of the post caught his glove and cut his finger. (laceration with stitches)
Employee was striking a hammer on a water line coupler and a piece of metal from the hammer struck him in the upper left arm. The employee worked his regular shifts until 10/16/12 when the metal was removed and he received 3 stitches. He was released for full duty and returned to work 10/17/12
A BMX employee was working in the 1 A tailgate section helping to trouble shoot an oil leak on the scoop. The scoop bucket had the tip duster attached to it. Once the scoop bucket was lowered onto the ground, the scoop winch rope became tensioned and unhooked from the left side of the tip duster causing the winch rope hook to strike the employee on the rear left area of the head.
Working as a roof bolter in the BMX 1 A Headgate section, a piece of rock measuring 2'X8""X3"" fell from the roof and struck employee in the lower left forearm.
While operating the Shearer a rock flipped out of the pan line chain and struck the employee on the right hand causing a laceration.
Employee slipped and fell, he landed on a car coupler hood and injured is rib.
Miner was loading powder working off a ladder standing on the 3rd rung. He was warned of a small loose rock and jumped backwards off the ladder. In doing so, hard hat fell off and the rock struck him on the top of the head.
Employee got his right thumb pinched between the mast jack on the miner and the header plate on the mast jack. Employee suffered a fracture to the tip of his thumb. No lost time.
A wire rope was being used to haul trees up a hill. Employee was attempting to remove rope from fallen tree by removing the clevis. The tension from the wire rope being bent around the tree caused it to spring back and strike employee in mouth removing a tooth.
While removing a roof strap from the strap rack on the miner in 16I section, the employee got his finger caught between the stack of roof straps and the roof. This incident caused a laceration on his right middle finger that required sutures to close.
Helping an experienced miner bolt along the belt line with a gopher drill when drill extension wrench got stuck on bolt head, then fell down hitting helpers fingers.
Was diagnosis with right carpal tunnel syndrome on 3/1/2010. started missing work for same on 11/08/2012.
