An ignition occurred in 6A miner section caused by an abnormal rock intrusion.
The elevator at Patterson Creek portal was out of service due to the governor cable malfunctioning. The cable and sensor were cleaned and the elevator returned to normal service.
The drum on the hoist was out of sync with the hoist statistic screen. The hoist was re-programmed to the proper settings.
5 South #3 escape facility hoist was inoperative for a time due to a faulty up over travel switch blowing a fuse. The fuse was replaced and problem fixed.
An unintentional roof fall occurred in 3 East Mains 23 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and the strata shifting.
The elevator was inoperable for an extended period due to an over travel fault. It was found that the elevator did not recognize one of the landing sensors. The sensors were cleaned or replaced and the elevator was recycled. No one was on the elevator at the time.
The screen for the central processor unit became inoperable on the hoist.
An unintentional roof fall occurred in 3 East Mains 2 Wall #1 Entry Intersection. There were no injuries as a result of this incident. Conditions contributing to the roof fall are humidity in the area and adverse roof conditions.
While ascending the slope track, the slope car derailed. No damage to the slope track rails or the hoist car was observed.
The Crabapple hoist was taken out of service for an extended period of time due to damage to the hoist rope. During regular inspection a small kink,or deformation, was found on the hoist rope approx. 15 ft from the hoist car. The cable was cut off and re-terminated.
Unintentional roof falls occurred in 3 East Mains 10 Wall #4 Entry Intersection and 11 Wall 3-4 Crosscut. There were no injuries as a result of this incident. Conditions contributing to the roof fall is humidity in the area and adverse roof conditions.
Unintentional roof falls occurred in 3 East Mains 5 Wall 4-5 Crosscut; 3 Wall 3-5 Crosscuts; 1-2 Wall #5 Entry. There were no injuries as a result of this incident. Conditions contributing to the roof fall is humidity in the area and adverse roof conditions.
The Sparta elevator became inoperable on the bottom. There were no personnel on the elevator. The clutch wheels for the bottom door was out of alignment and needed repaired.
Set screw for the door pulley came out causing the doors to not function.
The Sparta elevator became inoperable at 7:05pm. After investigation it was determined that a broken wire from the junction box to the remote pendant for the emergency stop caused the problem.
An unintentional roof fall occurred in 3 North Mains in #10 entry from 9 wall to 11 wall. The fall measured approximately 12'-15' high by 16' wide and 450' long. There were no injuries as a result of this incident. The event occurred due to humidity in the area and a lateral shift which caused the 8' bolts to fail at there couplers.
An unintentional roof fall occurred in the 4 West area of the mine. The original fall was located in the # 4 entry, Later in the shift it fell in the #2 entry, which is the belt line. During the investigation is was determined that a slip was running through that area that caused the roof to fall above the anchorage.
The safety switch on top of the elevator tripped, causing a fault. This caused the elevator to stop while in motion.
An unintentional roof fall occurred in the G1 Head Gate section at 49 wall in the #3 entry and into the 3 to 2 crosscut. The fall measured approximately 18' high x 31' long in the #3 entry x 37' long into the 3 to 2 crosscut. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure.
The crabapple hoist was taken out of service for an extended period of time. The hoist rope was found to be near the minimum tolerance and the decision was made to remove this area of the rope.
An unexpected roof fall occurred in the track entry in 6 South section at 59 X-cut. This incident was caused by adverse geological conditions in the form of a slip that was identified in this area after the fall was located. The fall measured approx. 30 ft long by 20 ft wide by 10 ft high. No equipment damage of injuries as a result of this event.

The elevator at Patterson Creek portal was out of service due to a malfunction with the slow down switches. The elevator was reset and returned to normal service.
An unintentional roof fall occurred in the 3 North Mains at 188 wall in the 3 to 4 crosscut. The fall measured approximately 20 high x 16 wide x 30 in length. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure. This area was previously supported with posts and no injuries occurred as a result of this incident.
An unintentional roof fall occurred in 3 North Mains in #2 entry from 11 1/2 wall to inby 10 1/2 wall. The fall measured approximately 12-15' high by 16' wide and 300' long. There were no injuries as a result of this incident. The event occurred due to humidity in the area and a lateral shift which caused the 8' bolts to fail at there couplers.
A unintentional roof fall occurred in the 6 South area of the mine at 5 x-cut, in the #7 entry. The fall was caused by adverse geological conditions in this area. The fall measured approx. 63 ft long and 10ft high.
2 persons were entrapped as a result of Sparta elevator stopping unexpectedly, approx. 20' from bottom landing due to a bracket on the retractable door edge system being broken. They were entrapped for 1 hr 49 min. Mechanics had to reset the fault & elevator continued to surface. Once at top landing the persons exited the elevator & the part was replaced.
The Sparta elevator stopped unexpectedly approximately 20' from the bottom landing due to a bracket on the retractable door edge system being broken. Two persons were entrapped as a result of this incident.
Routine maintenance was done to the Dry ridge elevator. After the maintenance was complete the elevator would not operate normally for an extended period of time. It was found that a fuse was blown and would not allow the elevator to operate. No injuries as a result.
The elevator at Patterson Creek portal was out of service due to the door bracket failing. The door bracket was repaired and the elevator returned to normal service.
The Pleasant Grove elevator became inoperable at the surface landing. There were no personnel on the elevator. A link on the chain for the sprocket on the automatic door ""kinked"" which in turn did not allow the automatic doors to close fully.
A roof fall occurred in the E-29 LW section. The roof fall was located on the face and in the #3 belt entry. The fall was approximately 3' in front of the #2 & #3 shield tips and extended outby in the beltline to just inby the crusher. The immediate roof consisted of a laminated shale type material. It fell approximately 7' above the anchorage of the 8' bolts used in the area.
A coil for the electric brake malfunctioned, prohibiting the brake from working properly. This was found during the inspection of the car.
The motion control encoder malfunctioned causing the elevator to stop while in motion.
The elevator at Patterson Creek portal was out of service due to slack in the governor rope. The governor sensor was adjusted and elevator returned to normal service.
The Dry Ridge Elevator was inoperative for a time due to an electrical fault that was reset.
A fault of the governor slack switch and a rope gripper fault occurred on the Sparta elevator which interfered with the use of the elevator for more than 30 minutes.
A kink was found in the Crabapple hoist rope during regular inspection. The Kink was found 16ft from the hoist car. The Hoist was taken out of service and reterminated.
An unintentional roof fall occurred in 3 East Mains in the #0 Entry at 2 Wall intersection. The fall measured approximately 8-10' high by 16-30' wide and 90' long. There were no injuries as a result of this incident. *Conditions contributing to the roof fall include adverse geological conditions in this area.
Dry Ridge elevator was not operational for an extended period of time. It was reported that a wire vibrated off of the outer door switch at the bottom of the elevator.
The breaks on the hoist car were found to be sticking during a regular inspection. The hoist was taken out of service to change the breaks. While dropping the car during the inspection drop a kink in the hoist rope was found. The hoist was again taken out of service to reterminate the rope.
The BMX slope hoist was out of service due to a bad brake sensor. The brake sensor was replaced and the hoist returned to service.
The Chestnut Ridge Escape capsule was out of service for a extended period of time. The chain for the cable handler was found to be damaged. The cause of the damage is unknown. The damage was found during normal inspection.
The elevator at Patterson Creek portal had a damaged bottom landing door switch. The switch was replaced and the elevator returned to normal service.
An unintentional roof fall occurred in the E-29 longwall section in 1-2 cross cut at 18 wall. The fall was approximately 20' high x 16' wide x 36' long. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the failure. The fall was just inby the longwall face at the time the fall occurred.
The crabapple portal elevator was not operational due to a faulty door limit switch.
An unintentional roof fall occurred in the E-29 Longwall section in the #2 track entry at 19 wall intersection. The fall measured approximately 20' high x 16' wide x 16' wide. The immediate roof consisted on laminated shale type material plus adverse geological conditions contributed to the failure. There were no injuries as a result of this incident.
The bottom door latch switch froze and interfered with the use of the elevator for over 30 minutes. The switch was replaced allowing the elevator to resume normal operations.
The hoist drum hydraulic hose that controls the brake pressure developed a leak which caused the hoist to be inoperable for over 30 minutes. The hose was changed and the hoist returned to normal operations.
The BMX slope hoist was out of service due to a bad brake line for the hoist drum. The brake line was replaced and the hoist returned to service.
A weld broke on the bottom secondary door switch bracket which interfered with the normal operation of the elevator for more than 30 minutes.
The elevator at BMX portal was out of service due to a main contactor needing cleaned. The tips of the contactor were cleaned and the elevator returned to service.
The door sensor strips malfunctioned causing a delay in the availability of the Pleasant Grove Elevator for more than 30 minutes. The malfunctioning door sensor strips were removed and replaced with new sensors.
Due to the cold weather, the pump at the bottom of the Sparta shaft failed and water accumulated in the sump of the shaft causing the buffer switch to freeze.
The Crabapple elevator was not operational for an extended period of time. The inner door switch at the top of the elevator malfunctioned causing the elevator to not operate with automatic controls. This switch was changed to restore the elevator.
The Dry Ridge portal Elevator was not operational for an extended period of time. Investigation revealed a frozen magnetic switch on the elevator doors. The switch was changed to repair.
The Dry Ridge elevator was not functional for a extended period of time. A screw that holds the main contactor into place had loosened and fell between the tips. This screw would not let the contactor tips close. The screw was removed and replaced.
An alleged face ignition was reported on the F28 TG section while mining in the #2 entry. An investigation revealed that small areas of pyrite were found in the rib and face areas. During mining these pyrite areas were struck and may have caused sparks that contributed to the face crews assessment that an ignition occurred. No violations were issued during the investigation.
The E30 development section experienced a face ignition while mining in the #2 track entry. During the investigation MSHA cited two worn bits on the left side of the continuous miner head on the drum extension that they believe may have contributed to the ignition.
Injured employee was standing at the headgate of the F-27 Longwall section when the decking at the HG #5 special pan caught the toes of their left foot between the decking and the mine floor causing an injury to the toes on the left foot that required surgery.
Employee was lifting belt clamp from waist to overhead. EE used improper body mechanics while lifting, and then ee heard a pop/crack sound and felt pain in left hip.
The injured employee picked up heavy steel rollers and frame with another employee when ee felt a sharp pain in back. Please note that the employee continued to work unrestricted until 12/20/2018. Officially off of work 12/21/2018.
While stitching a short block in the E31 CM section, the employee went to add a drill steel when a rock fell from the ATRS of the rib bolter, striking the back of employee's right hand. EE was diagnosed with a fracture to the right ring finger.
Employee injured right bicep while carrying a sand prop from the track over to the end of break stopping at the corner of the intersection upon stopping employee threw the sand prop off to the right and it got caught on another sand prop causing the injury.
Employee was carrying bag of rock dust through a check curtain in the mine. While going through the canvas along the left rib when a piece of mesh caught EE's foot and caused EE to fall on the left arm.
Employee was at the end of the track in 5A retrieving the winch rope hook from the scoop, when the hook struck them in the ribs causing a fracture.
While changing horizontal pin in the F28 Longwall employee was lining up the eye of the clevis, when the relay bar kicked up catching left foot between the bar and the side of the shield. Employee was diagnosed with a sprain to the left ankle.
Employee stepped onto the corrugated metal strap attached to the side of the miner, when the step buckled downward causing ee to fall toward the machine. Employee struck left knee against the miner. EE received seven sutures.
Employee was carrying a K-door in the F29 section when employee tripped over an Omega Block causing employee to lose balance striking the back of head against the rib.
While walking employee stepped on uneven ground and felt a pain in left knee. Employee continued working until first day missed 11/19/18 when employee was diagnosed with a torn meniscus.
Employee was releasing a belt clamp after performing belt maintenance on a section belt. As the belt was being released it moved through the clamp and pinched employee's right index finger between the clamp and a bottom roller.
Employee was trying to tram a bolter which had gotten stuck. While moving the tram levers back and forth, ee felt pain in left shoulder.
While track bolting, employee was changing steels and the mast came down on their right foot. Employee was diagnosed with a fracture right big toe.
Employee was getting down off of work area to allow another worker to sand blast. Employee had grease on work boots and stepped down onto the uneven ground when the employee lost footing and slipped. The ground was one eighteen inches lower than the work area.
While troubleshooting the suction on a center bolter employee suffered an injury to the left hand.
Employee picked up a belt roller and felt a pain in groin. EE did not seek medical treatment and worked all scheduled shift until being taken off of work on 2/1/19 to repair a hernia.
Employee was walking through shop stepped from gravel to concrete causing self to slip and fall injuring left foot. Incident resulted in a fracture of the left big toe/foot.
While lifting Miner cable from cable hanger employee felt pain in right shoulder. Injured was diagnosed on 10/10/18 for a torn rotator cuff.
Employee was kneeling down to splice belt in the E30 section, when EE felt something shift inside the right knee. EE was diagnosed with a Torn Meniscus to the right knee on 09/18/2018 and began missing work.
Employee was roof bolting on the return side of the miner in the 5A section. While drilling a bolt hole a rock approx. 4' x 3' x 1"" came out of the roof area and struck EE's right lower leg causing a laceration, requiring sutures.
Employee was riding in the mantrip when it hit a rail and stopped. Employee got their foot caught between the seat, causing injury to ankle.
Employee was using a bolt to pick up belt clips. Employee bent over to pick up and felt a pull in lower back. Employee started missing work on 10/30/2018 and was diagnosed with lower back strain of lumbar region.
While trying to recover the mule from the mud, employee was hit in the head by a hook that broke off the mast of the mule.
Employee was operating the tailgate drum of the shearer in the F28 LW section, mining towards the tail when a rock came over the handrail out of the panline and struck the employee on the inside of the right leg just above the ankle. Incident resulted in fractures to the tibia and fibula in right leg that required surgery.
Employee was helping to remove a bottom roller from the F-28 Longwall belt. As they were lifting the bottom roller they felt pain in the lower back. Was later diagnosed with a lumbar strain and began missing work on 8/15/2018.
Employee was struck by an airbag while re-railing a derailed low-boy car. This incident caused a fracture to left leg.
While at the face a rock fell and hit EE's right leg. Original incident occurred 7/26/18 - first day missed 9/13/18.
Employee was putting links together on the conveyor chain on 5A miner. When EE swung EE's hammer, EE missed the link and hit EE's right thumb between the boom and the hammer. Employee worked up to 7/24/2018.
While walking on the Longwall setup face a roof bolt head popped off and struck employee on the right shoulder. Employee worked all scheduled shifts until being taken off of work on 7/19/18.
Employee was roof bolting on the continuous miner in the 2 East Mains Section when a rock measuring approximately 22' x 18' x 6' fell from the rib above the binder and rib protector and struck EE on the back of the neck and shoulders. The rock knocked EE to the ground and EE was allegedly unconscious for 15-30 seconds.
Employee was rib bolting on the continuous miner in the G1 HG section. While separating the drill steels with hands a rock fell from the mine roof and struck EE's left hand causing a laceration to the left ring finger that required 5 sutures.
While walking through a crosscut in the F-29 Continuous Miner section ee stepped over a piece of coal. EE right foot got caught causing ee to lose balance and fall to the mine floor and strain right knee.
Employee was operating a mantrip on the E-track just inby E-24 cross over. When EE stood up to ensure that EE had clearance to maneuver into the switch EE struck the back of EE's hard hart on a roof strap which caused EE's head to move forward. EE's forehead contacted the radio compartment on the mantrip and caused a laceration to EE's forehead that required 5 sutures.
Employee was handling belt structure and felt pain in stomach area
Employee was loading cable into scoop bucket and foot got smashed under bucket causing injury.
Employee reached for a drill steel while bolting off of the miner and felt a pain in shoulder. Employee continued to work all scheduled shifts until employee was taken off of work on 7/9/2018 to repair injured shoulder.
After conducting an exam in the tailgate ee was crossing over the gob plate when left foot slipped. This caused weight to shift and ee grabbed onto the gob plate with left hand/arm to keep from falling injuring left shoulder. On 07/19/2018 ee had surgery to repair left shoulder.
Employee was carrying rock dust on left shoulder heading toward the feeder to dust. While walking alongside a shuttle car employee slipped and reached up with left arm to catch self and felt pain in left shoulder. LTA 6/18/18.
Employee was lowering the inspection door for the headgate chain conveyor sprocket on the F-27 Longwall Section. While lowering the door to the mine floor EE caught left middle finger between a ""D"" ring on the door and a crib that was on the mine floor being used as blocking material. Suffered a fracture to the tip of left middle finger that required surgery.
Employee was bolting on the right side of the continuous miner in 2 East Mains. While dropping the bolter head the bolt wrench stuck on the head of the roof bolt EE installed. The bolt wrench then fell and struck EE on the upper lip causing a laceration that required 5 sutures.
After helping to secure a ventilation tube to the mine roof in the G1 Headgate section ee turned to walk away and twisted left knee. Resulted in a strained left knee.
Employee was descending an overcast ramp when ee slipped and fell to the ground injuring right leg. EE was diagnosed with a fractured tibia in right leg.
Injured employee was observing another employee repair an impellor on a Sunflo pump with a chisel when a small piece of metal struck IE on the left cheek. The small piece of metal was removed by a physician on 4/4/2018 when IE also received 5 sutures.
Employee was hosing shield toes on the F-28 Longwall section during an idle shift when EE got debris in EE's right eye. EE was diagnosed with a corneal abrasion of the right eye and began missing work on 3/26/2018.
Employee was rib pinning on the left side of the miner in 5A section when left index finger got caught between the drill chuck and wrench. Fracture to left index finger.
Injured was driving a mantrip inby on the E-track when a derailment occurred causing the injured to strike EE's left side and face against the frame of the mantrip resulting to chipping of the 8th and 10th teeth and 7 sutures to EE's mouth.
While uncoupling a car on bottom, employee pinched finger on coupler causing injury.
Employee was cleaning off the top of a shield with a slate bar in the F-27 Longwall section. While prying down on the slate bar EE caught EE's left hand/fingers between the slate bar and the handrail. Resulted in a fracture to the tip of EE's left middle finger.
Employee was using a hand grinder to remove a separator roller in a conveyor belt take-up. During the process EE struck self with the grinder that was equipped with a 4 ½"" cut off wheel on the top of left hand between thumb and index finger. The injury required surgery to repair the extensor tendon in the left thumb and index finger.
Employee was in the panline on the F-27 Longwall face hooking the panline up to shield when a rock slid out of the face striking employee on the left lower leg causing right lower leg to strike the eiko track. On 2/28/2018 employee was diagnosed with a non-displaced fracture of the fibula & a partial tear of the lateral collateral ligament in right leg and began missing work.
Employee was center bolting in the F-29 section. While removing drill steels from the roof ee pinched left 5th (pinky) finger between the connection point of the bottom two steels causing a laceration to side of finger that required 7 sutures to repair.
Stepped on a rock and twisted ankle resulting in a sprained ankle.
Report was, while retrieving a bolt from the miner, a piece of rock fell from the roof and struck ee on the head and back of the neck. Initial doctors slips and evaluation reports were not consistent with this incident description. An independent medical evaluation revealed that the symptoms were related to the incident description, therefore this incident became a LTA on 3/28/18
Employee was raising a vent tube to secure it to the roof and felt a pain in EE's groin. EE worked all scheduled shifts until diagnosed with a hernia and began missing time for surgical repair on 2/19/2018
A piece of rock fell from the rib and struck employees right leg. This caused a fracture to the right heel bone.
Employee was standing on the step on the side of the CM tightening a chain on a vent tube. Employee's right hand was positioned at the base of the rib drill and right elbow contacted the 'feed' lever causing the bolter head to move and pinch employee's right index & middle finger between the frame and bolter head. Had surgery to repair the tip of right index finger.
IE was working on construction of a vertical shaft. IE was in the process of flipping a bucket bail, on the muck bucket, to the open position and was struck in the foot by the bucket bail. Proper procedure was not followed as the bail stop was not activated prior to flipping the bail locking ears.
A miner was walking in the parking lot when EE was bumped by the right front side of a pickup truck traveling at a low rate of speed causing EE to fall and strike the back of head. EE was taken by ambulance to a local hospital of EE's choosing and was treated for a laceration to the back of the head that required 10 staples and was released.
Employee was installing a backboard on the end of a rail car when ee slipped on a track tie injuring left knee. EE was later diagnosed with a partial tear of ACL/MCL in left knee.
Employee reached over their head to turn a 6"" waterline shutoff valve and felt pain in the lower right abdomen. Was diagnosed with a hernia.
While lifting a bottom roller by its handle EE felt a pain in EE's left arm. Original incident 1/10/2018 - first day missed 2/14/18.
Employee was holding a door back from slamming and rolled an ankle. EE continued to work and did not seek medical attention until 8/7/18.
Picked up a chain fall and felt pain in back. *EE continued to work as a belt man with no restrictions. EE had surgery on March 26, 2018.
Employee twisted EE's right knee while walking on muddy/icy jobsite. The Temperature was just at freezing making some areas frozen and some areas still muddy and wet.
Employee was dragging drisco pipe and stepped on uneven ground and twisted knee. EE did not miss work until ee was seen on 3/18/2019 and it was determined ee needed surgery.
Injured was walking up over a berm
Employees was opening an airlock door so mantrips could pass through and got caught between the door and mantrip causing injury to their fingers and ribs.
Employee was working in the 2 East Mains Section. The fire suppression on a scoop outby in the section inadvertently actuated releasing Foray ABC Multi-Purpose Dry Chemical that is used in the system. The employee had a respiratory reaction (Reactive Airway Disease) that affected breathing.
Employee was trouble shooting a shield by engaging the solenoid controls on the shield, when a 2"" return hose came out of its fitting house due to there not being a staple lock installed and struck employee on the upper lip. The injured resulted in employee receiving 5 stitches.
Employee was hooking up can setter and slipped and hit finger causing injury. Original accident happened on 11/14/18 and continued to work. EE did not seek medical treatment until 11/20 and then was released.
Employee was disconnecting a 2' hose when the hose came apart and struck EE in EE's left hand resulting in 6 sutures. Employee did not miss work.
Employee was loading dust line onto a gen and was struck in the head by a coupler.
Working on shuttle car while attempting to line up tensioner bolt on a level wind box when the box shifted and caught EE's ring finger between the box and compartment. No work was missed as a result of this injury.
Employee was handling pipe and smashed finger causing injury.
Employee was handling track tie and got finger smashed between the tie and a rock.
Employee was walking through a partially hung check curtain and a small piece of rock fell from the roof/rib and struck EE on the side of the face causing a laceration. 6 Stitches to close. No work missed as a result of this incident.
Employee reached to catch a falling bundle of roof plates that caused a laceration.
Employee was driving a mantrip and they hit their face on the mantrip when it struck a rail causing injury.
Employee hit finger while using a block hammer causing injury.
Employee was cleaning under the section feeder, prepairing to move it inby when a finger became pinched between a piece of plate and a rock. Employee received 5 stitches and returned to work on the next scheduled shift.
Employee was using knife to cut belt and the knife slipped causing injury to employee's leg
Prepping for a vulcanize splice. EE was cutting off pieces of fingers or wedges of belt. EE put a new blade in EE's knife and was cutting the fingers and had EE's left arm close to where EE was cutting and cut through the belt and cutting EE's right forearm.
Employee was retracting trailer jack when left thumb was pinched in the jack.
Employee was helping with a belt splice and cut hand causing injury
Employee was walking to the mantrip when another employee called them. Employee swung around and as they turned the right hand hit a bent out rib strap causing a laceration to the webbing between the pinky finger and ring finger. Employee received three stitches.
An Employee was using a hammer to pound a pin while building the a longwall tail drive. The hammer slipped out of the users hand and glanced off of the pan line and struck another employee above the left eye causing a laceration. 12 stitches to close.
Employee was building a crib. Employee caught their finger between two crib blocks causing injury to the finger.
An accident occurred in the E-30 Continuous Miner section. While the crew was mining in the #3 entry with #26 Joy ED CM they encountered a clay vein that liberated methane that was ignited by an undetermined source. The heat from the incident caused minor burns to the forehead and nose of the return side bolter.
Employee was installing drive roller. It shifted and pinched right finger causing injury
Employee was loading a belt scraper into a ride when the cradle slipped and injured employee's pinky finger.

Employee was cutting with a knife and injured employee's thigh.
Employee was diagnosed with and compensated for work related hearing loss. The employee did not suffer an incident or injury that caused the condition to immediately occur. This employee has not missed any scheduled work days as a result of this diagnosis
While preforming normal duties as a longwall systems operator, Employee began to feel pain in wrists that continued to get worse. Employee was diagnosed with Carpal Tunnel syndrome and was taken off of work for surgery to repair on 2/6/2018
Employee suffered an medical emergency that resulted in untimely passing. Medical treatment was provide at the surface facility until EMS personnel transported ee to a hospital where ee was pronounced dead.
