An unintentional roof fall occurred in 3 East Mains #1 Entry 28 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.
An unintentional roof fall occurred in 5B LW #1 Entry 24.5-25 Wall. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof. The fall area measured approximately 26' L x 12' W x 17' H.
An unintentional roof fall occurred in the 2 South Mains at 16 wall in the #11 return entry. The fall measured approximately 16 high x 16 wide X 16 length. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure. No injuries occurred as a result of this incident and no compliance issues were found.
While ascending the slope track, the slope car struck a crib causing derailment and damaging the brake. The brake was repaired, and the hoist returned to normal operations. No damage to the slope track rails was observed.
The elevator at BMX portal was out of service due to a blown hose on the brake pump. The hose line was replaced and the elevator returned to service.
A face ignition was reported on the H2 section while mining the #3 Return entry Survey Spad #66+78. During the investigation Sulphur balls were found in the face area. MSHA cited two missing/worn bits on the left side of the continuous miner head. No injuries occurred.
An unplanned roof fall occurred in the 4 west area at 26 x-cut. Adverse geological conditions caused the roof to separate and break. The roof fell 10ft from the track entry (#3 entry) through the crosscut to the belt, bridged over the belt entry and continued falling in the crosscut from the belt (#2 entry) towards the adjacent entry (#1 entry).

An unintentional roof fall occurred in 3 East Mains #2 Entry 80 Wall intersection. There were no injuries as a result of this incident. An unintentional roof fall occurred in the G2 Longwall inby 11 crosscut #3 entry, approximate spad 122+10.52. The fall was approximately 15' Long X 8.5' Wide X 16.5' height. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the failure. The fall was just at the longwall face at the time the fall occurred.
An unintentional roof fall occurred in the 9L section. Adverse geological conditions in the form of layer shell rock separated causing this occurrence. The fall occurred at 12+00 on the future longwall face. The fall measured approximately 30ft long, 16ft wide 20ft high. No injuries or equipment damaged as a result of this incident.
An unintentional roof fall occurred in the H1 Tailgate Continuous Miner section in the #3 return entry located at 49.5 wall intersection. The fall measured approximately 6'1' high x 8' wide x 30' long. Adverse geological conditions contributed to the failure. There were no injuries as a result of this incident.
The elevator at Patterson Creek portal was out of service due to a short power outage which caused the safety relay bar/switch on top of the elevator to activate. The breaker and switch were reset and the elevator returned to service. The miner onboard the elevator exited safely once it returned to service.
An unintentional roof fall occurred in 9L section. The fall occurred at 17 wall crosscut, approx halfway into the crosscut between #1 & 2 entry through #2 & in the crosscut 2-3. The fall was approx 120' long by 16' high & 16' wide. A site specific mine plan was submitted to rehab the area. *Adverse geological conditions caused the immediate strata to separate & subsequently fail.
While performing routine maintenance checks on the 5 South #4 escape hoist, it was discovered that there was an electrical issue with the processor on the PLC. The escape hoist was taken out of service while repairs were made.
An unintentional roof fall occurred in 7 North Mains #3 Entry 45 1/2 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.
An alleged face ignition was reported on the H1HG section while mining the #2 entry. An investigation revealed that small areas of pyrite and Sulphur balls were found in the rib and face areas. During mining these pyrite areas were struck and may have caused sparks that contributing to the face crews assessment that ignition occurred. No violations issued during the investigation.
An unintentional roof fall occurred in the G2 Longwall section 2-3 crosscut at 39 wall Spad# 107+10. The fall was approximately 16' wide x 50' long x height unknown. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the failure. The fall was just at the longwall face at the time the fall occurred.
An unintentional roof fall occurred in the H1TG section at 49 wall in the 1 to 2 crosscut into #2 entry migrating into 2 to 3 entry. The fall measured approximately 66 Long x 24 Wide x 16 High. No injuries resulted from the accident. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure.
An unintentional roof fall occurred in 7 North Mains #1 Entry 46 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.
The BMX slope hoist car was taken out of service when the Main Breaker malfunctioned. The main breaker was replaced along with applying dielectric grease to the power plug connection points.
2 persons were entrapped as a result of the Pleasant Grove elevator automatic door linkage that connects to the motor pulley when it had broken due to standard normal wear. They were entrapped for approximately 30 minutes. The linkage was replaced with new to prevent re-occurrence. No other part 50 criteria applied.
The 1A Capsule at the 1A shaft was out of service due to a malfunction with Up Overtravel electrical switch.
During a normal examination of the Crabapple elevator, a damaged spot in the training cable was identified. The Elevator was taken out of service to repair this damaged area. The cause of the damage was attributed to the age of the cable and the time that the cable has hung in the shaft.
The Dry Ridge elevator was taken out of service by mine management for and extended period of time, due to the elevators fault alarms showing the elevator was not operating normally. The rope Gripper switch was changed along with the bottom door switch and the elevator became operational. There was no one on the elevator when it was taken out of service.
An unintentional roof fall occurred in the 3 North Mains at 187 Wall 2 to 3 Crosscut. The fall measured approximately 16' high x 16' wide x 35' length. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure. No injuries occurred as a result of this incident.
One of the trailing cables became dislodged due to normal wear and was pulled out from the mid-shaft junction box. This cable was replaced in its entirety and re-entered in the mid-shaft junction box to the elevator. No injuries had occurred.
An unintentional roof fall occurred in 7 North Mains 35 1/2 to 36 Wall #1 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.
Employee was standing on the #7 shield located in the H1Headgate Longwall face. The shearer was cutting back into the snake, when #8 shield advanced it snagged the spare ram jack that was being stored on #7 shield, triggering the ram jack to strike EE's left lower ankle foot. EE was diagnosed with a hairline fracture of the third metatarsal.
Employee was bolting on longwall face and injured hand on the mast while handling drill steels.

Employee was lifting a box of splice kits to place them on top of the drive motor when employee felt a pain in left shoulder. Employee reported that the pain began to worsen the next few weeks. Employee worked all scheduled shifts between December 4th 2021 until February 16th 2022 when employee was taken off of work due to this shoulder injury.
Employee was an the process of replacing the cover on the walk side rear jack of the cat mounted tailpiece, when the tool twisted in their right hand and they received a laceration.
Employee was carrying a piece of roof mesh, as EE was walking EE stepped on a rock causing step to be unbalanced. EE felt a pain in right knee. EE continued to work normal job for all scheduled shifts until EE began missing work on 1/12/2022
Employee was recovering rail, while unbolting a set of fish plates, EE pinched left thumb between between two pieces of rail. EE was diagnosed with a fracture to the tip of the thumb.
While hosing in the raw coal transfer building, employee slipped on wet floor and struck leg against a kick plate, causing a laceration. Injury was not the result of a training or compliance issue.
Employee was pumping cribs and came in contact with the concrete mixture. Chemical burns on both legs.
Employee was using a come-along to lift an ATRS jack over the rib door on the miner when EE pinched EE's left hand between the jack and door. EE received 15 stitches.
Employee was helping pull hoses off from a turntable on scoop and was struck by the scoop bill causing injury to foot.
Employee was changing out top cradle on main line belt and pinched finger between cradle and bedrail causing injury.
Employee was walking down the pan line in 8L Longwall Section, when EE slipped. EE felt a pain in EE's left shoulder when EE caught self. Employee reported that the pain has came back over the last couple of weeks. EE worked all scheduled shifts between October 10th 2021 until February 17th 2022 when EE was taken off of work due to the injury to EE's left shoulder.

Employee was rib bolting in the 4B miner section when EE swung the rib bolter towards the rib, the bolter hit a loose bolt knocking over a stack of roof plates that fell from above the rib bolter controls striking the employee on the left forearm causing a laceration. EE received fifteen sutures.
While cleaning in Silo #4, employee slipped in grease and felt pain in knee and back. Injury was not the result of a compliance or training issue.
Employee was standing next to the can setter controls on the scoop with leg bent and foot against the rib causing a knee to stick out. Employee was injured when scoop bucket swung towards the rib causing injury to the knee.
Employee was placing spreaders on bottom forms when the one they were standing on twisted causing them to fall four feet.
WENT TO WALK ACROSS TRACK TO TIGHT SIDE, TRIPPED OVER RAIL, TWISTED LEFT KNEE.
Employee lost balance after turning on a water line and struck back against a handrail causing a strain. Injury was not the result of a training or compliance issue. Employee did not start missing work until 8/26/2021.
Employee was moving the miner cable and waterline out of the tram road when EE felt a sharp pain in EE's right arm. Was diagnosed with a Torn Bicep and was taken off work pending surgical repair.
Employee was rib bolting when a piece of rock fell from the rib and hit right leg / ankle. EE was diagnosed with a fracture to lower right leg.
Employee was changing a rack wheel when the wheel slipped and EE tried to catch it and pinched finger between the rack wheel and the relay bar fracturing the tip of EE's left index finger.
Employee was standing on a bar grating platform to hose off floor steel when the grating gave way causing employee's right foot to fall through a hole in the platform.
Employee was mucking at the Enlow 2 South #1 Airshaft Site. During the process of switching lifting hooks from one muck bucket to another employee slipped on the wet shale and rock. This caused a rolling of the right ankle beyond normal range of motion.
Employee was walking to the mantrip when EE stepped into a hole and felt a pain in right ankle. Resulting in a sprained ankle and a small avulsion fracture.
While operating the Longwall shearer employee tripped and fell onto right arm. Employee returned to work next scheduled shift and worked all required shifts until being taken off for surgery to repair shoulder on 10/6/2021. Lost work days will be counted from 10/6
Employee's right foot got caught on a piece of mesh causing EE to trip. Resulted in a fracture tibia / chip fracture on base of tibia.
Employee was setting a k-door frame and caught right index finger between the horizontal cross member and the vertical support beam causing a fractured finger tip.
Employee was side bolting on operator side of Continuous Miner located in the H1TG section. The employee had next 6' bolt to install resting up against rib protector door. When reaching for said 6' bolt with left hand, a rock from the top fell striking the left hand against the top of the rib protector door. EE was diagnosed with broken finger.
Employee was lifting a ventilation tube and felt a pain in lower left abdomen. 6/14/21 Employee released return to work with no restrictions. 6/21/21 Employee was diagnosed with a hernia and taken off of work.
Employee was rolling a rail when the rail rolled back causing the slate bar to hit employee in the chest/ear.
Employee was bolting off the miner when a piece of rock fell striking left calf. Employee suffered a laceration and received stitches to repair.
Employee was side bolting along the Continuous Miner located in the H1HG CM Section. EE was in the process of hanging rib mesh when a rock 1x2x2 fell from the top striking the injured on the nose. EE received four sutures.
Employee was drilling when a Rock (Fire/Clay/Mud Mixture) fell from below Panning tin and struck EE on the hard hat and glanced off EE's left shoulder. EE was done collar piping and was drilling on the 4 row when the accident occurred. Rock measured approximately 16""x21"".
While working along the left side of continuous miner in the H1TG section #1 entry. Employee was in the process of supplying miner when a rock slid from where the rib and roof meet striking the injured in the back of the head. EE was diagnosed with a concussion.
Employee was attempting to throw a rock on a belt when a splice caught EE's glove causing EE's thumb to be caught between the roller and the belt causing injury.
EE was pushing the drill rig arm forward while setting a collar pipe in the hole EE just drilled. EE's foot slipped and right shoulder popped out of place. EE had the collar pipe in left hand and was pushing the rig with right hand when EE slipped. As EE slipped and caught self EE's right shoulder became dislocated.
Employee was on the face at the G2 longwall section at around #4 shield, when a rock approximately 2' long x 1 ½' width x 6"" to 12"" thick fell between #3 shield and #4 shields, reflecting off the headgate drive and striking EE on the right leg. Incident resulted in fractures to the fibula and tibia in EE's right leg.
While bolting off of the miner, a rock fell from the top and struck a hydraulic hose while the employee was raising the head. The fitting on the end of the hose broke and the hose struck the employee on the right foot. Employee was taken off of work for a contusion. No laceration, nor fracture.
Employee was shoveling the tail of the slope and felt pain in the shoulder when EE raised the shovel to dump onto the belt. Note: Employee worked until Aug 23, 2021 when EE was diagnosed with a tear and had to have surgery. EE has been off since that date.
Employee was carrying a bottom belt roller when EE dropped it down EE caught their fingers on the belt rail causing lacerations to EE's middle and ring fingers.
While installing a cable bolt into a previously drilled hole it became stuck in the mesh. When EE adjusted the bolt to free it, their thumb became pinched between the bolt and the ATRS jack of the miner causing a laceration.
Employee was recovering vent tubes in the 5B HG section from 4 Left entry. While pulling a tube up off the mine floor, EE lost balance and fell towards the rib. While falling forward EE stuck right hand out to brace self, when EE's hand struck against a rib strap. Diagnosed with a fracture to the right 5th finger on 2/10/21.
Employee was operating Shearer in the G2 longwall. EE's left hand was resting on the handrail. A rock came up the pan line from the tailgate contacting EE's left hand. When EE attempted to move hand out of the way it became pinched between the rock and the top of the shield causing lacerations to EE's left fingers. EE received 6 sutures.
The employee had just arrived at the Dry Ridge portal and was walking from their vehicle to the bath house when they slipped on ice, falling and landing on left elbow. X-rays would later reveal a fracture of the ulna bone.
Employee smashed finger between two blocks causing injury
Employee was pumping cribs and came in contact with the concrete mixture. Chemical burns on both feet and calves.
Working from aerial lift truck to hang a 24-ft wooden, cross arm. Bolted the cross arm to the pole and went to move away. Cross arm slid and struck right hand which was controlling bucket. Hand moved controller and bucket moved forward. Caught employee's left lower arm/wrist between the cross arm and the bucket of the aerial lift truck.
Employee was cutting a window opening in between the cables of a conveyor belt to put piano wire in to be able to strip the belt to do a vulcanize splice. While cutting the belt window opening employee was leaning on left hand and knife slipped resulting in cutting left wrist. Employee had to get nine stiches.
Employee pinched right pinky finger between hopper and handrail
Employee was leveling a belt line at nine and half block. EE was using a Hi-Lift jack. EE was releasing the jack and EE's hand slipped off of the handle and hitting EE on the right side of face, resulting in some fractures. All employee's are being retrained on this jack. No other part 50 criteria applies.
Employee was using a razor knife to clean off the end of a slick line and the knife slipped causing injury to forearm.
Employee was carrying tools from the stage loader on the tight side, when a rock fell from rib striking EE's left foot. Rock Measured 25"" L x 5 ½"" w x 4"" thick. EE finished EE's shift and worked for two weeks before being diagnosed with a fracture on 12/3/21. Section was idle when accident occurred.
Employee injured a finger in a door mechanism while moving a steel mechanism from a box to the table to complete assembly. A door on the mechanism with pivot plates caught their gloved hand, pinching it, causing a laceration to the right hand little (fourth) finger. The wound required sutures.
Employee was struck on the face by a rail causing injury.
Employee was operating the head gate drum of the shear in 5B LW when a rock came off the drum and hit the employee on left hand. Employee received 3 stitches to left index finger.
Employee was struck in the left ear while advancing trunk lines. Causing a laceration which resulted in 13 stitches. The employee returned to work next shift with no lost time.
Employees were cutting bolts off belt structure. When employee hit the bolt with a hammer, the bolt flew off hitting another employee in the face causing injury.
The employee was installing a K-door frame. The extension collapsed when employee stood up and EE's finger was caught between the extension and mainframe. This resulted in a fracture to EE's left thumb.
Dropping belt structure (bottom belt cradle) on the reclaim belt. Employee pinched left index finger between the cradle and the bedrail. Employee received stitches to close a laceration. Employee did not miss any schedule work time as a result of this incident.
Employee tripped on track tie and struck knee on a spike puller causing injury.
The employee was struck by a 5'roll of the liner, as the liner unrolled itself down the slope. EE was struck by the liner to the right side of head and neck area. The employee was given first aid measures and finished the shift.
Employee was helping put together a belt take up unit in the 11L. As they were inserting a pin into an adjustment hole they inadvertently rested left ring finger into another adjustment hole. The structure moved suddenly and the tip of finger was pinched. Employee received stitches as a result. No lost time due to this incident.
Employee was using a claw hammer while putting together hose on the monorail. EE hit the hammer with a pick hammer to line up holes for the staple and a sliver of metal came off causing injury to EE's abdomen area.
Employee tripped while walking to the miner. EE grabbed the rib to catch balance and a rock fell striking EE's left thumb. This resulted in 6 stitches. No lost work days or restrictions from this injury.
Pushing a rubber guarding up on the miner when employee struck hand against a roof strap. Resulting in a laceration that required 9 sutures.
Employee pinched finger while helping change out a belt roller. Received 5 stitches, and x-rays revealed a small fracture.
Employee was setting up the can setter. It shifted and pinched employee's finger causing injury.
The employee was cutting rib mesh. The knife slipped and caused a laceration on EE's left forearm. EE received 13 stitches.
Employee was holding a 6 foot piece of 48 inch corrugated plastic pipe steady while another employee cut a 8 inch notch in the end with a chain saw. The saw kicked the bar out of the cut and the operator struck the left gloved hand of the second employee. Injured employee was seen at Med Express and received three sutures to close a laceration on index finger.
Employee was placing wooden blocks under a cranes outriggers before they were extended. When EE threw a block under the outrigger, EE misjudged the distance and smashed the left pinky finger of their gloved hand resulting in a small laceration to EE's finger. Employee was seen at Med Express and was given 3 sutures with a full release to return to work.
Employee was loading T-3 rib straps onto the miner unit when EE cut their left index finger on the edge of a T-3 strap. This event resulted in 5 stitches to the left index finger.
Employee began to feel pain in both wrists while performing normal duties of an outside mechanic. Employee reported no specific incident. Diagnosed with carpal tunnel syndrome and was scheduled for surgery. Lost time began 9/23/21.
