An unintentional roof fall occurred in the 4 West area, #3 to #4 entry x-cut. The roof fall was approx 45' long, 10' high, and 16' wide. There was no ventilation interruption, equipment damaged, or injury as a result of this incident. This fall will not be cleaned up.

The Crabapple slope hoist was not operational for an extended period of time. There was a bad breaker that was replaced and put back into service. This slope hoist is not used as any type of escapeway.

The slope car was moved off of the track by a high lift. During transport some of the wiring was damaged causing the cars electrics to short circuit and damage the breaker for the car. The car was sitting on the surface and not in use at the time of incident. No one was injured or entrapped as a result of this occurrence.

The Patterson Creek Portal Elevator was out of service due to a power outage that caused a fault in the system. Seven employees were entrapped due to this event and when the elevator began working all seven employees were accounted for.

In process of making weekly exam; Mine examiner discovered a roof fall at 5 wall of the number 1 entry of 3 east mains. The roof fall was a failure in the intersection and partial failure of 1 to 2 cross cut. There were no persons injured or equipment damaged as a result of this fall.

An unintentional roof fall occurred in the #1 Belt entry of 1 L section at 41 crosscut. The fall measured approx. 60ft long and 16ft high by 16ft wide. No injuries as a result of this event.

An unplanned roof fall occurred in the #3 Entry of 1L just inby #45 x-cut. The fall measured approx. 12' wide x 6' wide x 18' long. There was no equipment damaged or injury as a result of this incident.

Oak Springs hoist was taken out of service due to a malfunctioning brake on the brakeman car. The brakeman car had a damaged wire underneath the car on the outby end that caused one of the brakes to malfunction.

During routine inspection it was found that the slope car brake was not working properly. After further investigation it was found that a ground wire was broken. The slope car was removed from service and the ground wire was repaired.

An unplanned roof fall occurred in the 1L mining section at 42 wall. The fall measured approx. 45ft long by 16ft high by 16ft wide. The fall occurred in the belt line, #1 Entry. There were no injuries or equipment damaged as a result of this event.

During a scheduled inspection, a kink in the slope hoist rope was discovered. The hoist was taken out of service and the rope was re-terminated. No injuries as a result of this event

The Enlow Hoist was taken out of service because the scatterhorn was not functioning. There was no damage done to the hoist. The hoist was still functioning properly.

East Finley elevator broke down to the brake pick needing adjusted. No one was abroad the elevator at the time. The elevator was at the bottom when the incident occurred.

Hoist was taken out of service due to a kinked rope (4/7). It was fixed and tested (4/8). The car was pulled to the top and car pulled past top limits before hitting switch and final limit switch that failed to stop car. Car was pulled into the guide roller at the end of the track. *The brake coil was replaced on 4/5 while the hoist was under the supervision of Frontier Kemper.

The electrical switch that indicates the brake position was out of adjustment due to the mounting screws being loose.

Water got into the slope car control box causing the PLC to fault. Fault in the PLC caused the brakes to lock up on the car while hoisting.

The crabapple elevator was not operational for an extended period of time. As the elevator approached the top landing, vibration was detected. It was investigated and found that the Main elevator drive needed to be replaced.

An unplanned roof fall occurred in the 1L mining section at 38 X-cut. The fall measured approx. 32 ft long by 10ft high by 26ft wide. The fall occurred in the return intersection. There were no injuries or equipment damage as a result of this event

NOTIFIED AT 12:15 AM THAT THE EAST FINLEY ELEVATOR WAS DOWN. MAINTENANCE WAS NOTIFIED AND REPAIRS STARTED. AT 1:30 AM ELEVATOR WAS BACK IN SERVICE. BOTTOM DOOR MOTOR WAS REPLACED.

A CO ALARM WAS NOTICED AT THE 2 EAST MAINS TAIL PIECE AND THE SECTION FOREMAN WAS NOTIFIED. HE FOUND FLAMES COMING FROM AN INTERNAL BEARING ON THE SECTION TAIL ROLLER. HE USED A 1/2"" HOSE AND EXTINGUISHED THE FLAME IN LESS THAN 3 MINUTES. THE FIRE NEVER LEFT THE BEARING HOUSING. THE HIGHEST CO READING WAS 12.5 PPM.

Was mining in #2 entry of E26 section 24 feet from center of 27 x-cut when ignition occurred. it was orange in color and lasted 3 seconds and was extinguished by the sprays. The miner was cutting at mid face just under a binder.

Fall down stairs

Employee slipped on the last step of a stairwell hitting and jarring his back.

Struck by falling object

Employee was in the process of tilting up a vertical pan line bolter. After removing the keeper pin from the trapping shoe, the bolter moved toward the employee and came down on his right leg.

Employee was carrying roof straps to the miner when he stepped on a lump of coal and rolled his left ankle. Diagnosed with a fracture to his left ankle.

Employee was struck just above the left eye by the end of a ventilation tube (slider) causing a laceration that required 11 sutures.

Employee was cutting off a belt flapper in E26 belt storage unit. While cutting with a utility knife, the knife slipped cutting the inside palm on his left hand.

Employee pinched the tip of his left ring finger between a conveyor belt bottom roller and frame of shuttle car. Resulted in a fracture to the finger.

Struck by powered moving object

EE placed heater for direct heat to thaw track/started walking away. Operator in cab w 1 track slightly elevated reached for tram stick to rotate track; coverall caught single joy stick causing Volvo to raise/swing toward left striking EE. EE fell bckwrd inside bucket/Operator raised hands; clothing still caught/Machine swings right raising & drawing in. EE leg contact w track.

Struck against stationary object

Crew was walking the primary escapeway at the end of the shift, as the employee reached the top step of the belt overcast he struck his head on a roof strap. The accident caused severe neck strain.

Caught in, under or between a moving and a stationary object

Employee was in the process of lifting up a scoop to remove the rear axle. While hooking up the hoist chain and hook to the scoop, his right index finger became caught between the scoop and the hoist chain.

Employee slipped and fell onto his right knee causing a sprained knee.

Over-exertion in lifting objects

Employee was throwing a pallet into coal car when the pallet became caught on the rib-mesh causing him to have lower back pain.

Struck against stationary object

Employee was walking to the end of the track in F25 miner section. He stepped on a track rail going into a switch and his right foot slipped off the rail causing pain in his right ankle.

Employee was installing a roof bolt when he pinched his right pinky between the shaft of the bolt and the bolter mast. Caused a laceration that required 8 sutures.

Employee was walking toward the head gate and stepped on a pile of coal at 5 shield injuring his right ankle. Diagnosed with a fracture to the Talus bone in ankle.

TTIS (Contractor) employee was working on a finger splice on a conveyor belt at the mine operator's prep yard. The splicing tables are approximately 13 inches off of the ground. The employee was in the process of stepping down off of the splicing table when he twisted his right ankle resulting in a sprain. The employee returned to full duty without restriction on 10/20/14.

Crew was pulling LW hoses from the trough using a chain attached from the relay bar to the hose. As a section of hose was being pulled out, the entire hose abruptly came out of the trough striking the employees' lower right leg. (fracture)

Employee was struck on both legs by a piece of rib that fell out of the face while he was helping bolt.

Employee was roof bolting on the continuous miner. While reaching out with his right arm to remove a drill steel from the bolter chuck a piece of rock fell from the mine roof striking him on the upper back and right shoulder. On April 15, 2015 the employee had surgery to repair the bursal sided rotator cuff in his right shoulder.

The employees were loading pipe into the cradle of a fusing machine. The operator put his hand in the end of the pipe in an attempt to lift it. The excavator lifting the pipe also had horizontal motion towards the fusing machine. When the employee lifted the pipe it slid rapidly crushing the employee's hand between the pipes. This injury required surgery to his left hand.

Employee was carrying pipe couplers down the slope area, slipped in a muddy area, employee was knocked into a belt hanger chain an lacerated the right side of face.

Employee was walking to the end of the track to check on incoming trip of shields when he tripped on a cable sticking out of a bridge resulting in pain in both his shoulders. Diagnosed with torn right rotator cuff. LTA effective 10/1/14.

While side bolting on miner, he was installing his second steel as he raised the drill head up it struck the steel causing it to bend pinching his hand between the steel and the mast.

Operating cat 657 pan to clean mud off road, went up hill to spread. Went around a parked D11. Went to go back down hill, looked back at bowl and went off the edge of a 20 ft wide cut with a 3 ft drop off and a total of four to five foot drop. Injured back.

Employee was walking with a wash down hose when he lost his footing, falling to the ground on his right shoulder.

Employee was hanging miner cable in E26 miner section, when he felt a pain in his left shoulder. LTA effective 10/2/14.

Employee was stepping down from a ladder, when his right foot slipped off the second rung causing him to twist his right ankle. Lost time effective 8/6/14.

Caught in, under or between a moving and a stationary object

Employee was attempting to reset a jabco on 3 North #5 belt, when he tripped and his wedding ring caught on the mounting bracket for the jabco. This caused a skin avulsion to his left ring finger that measured approximately one inch.

Initial injury date was 7/29/2014. Employee continued to work all scheduled shifts until 8/5/2014 when his MRI was read. Employee returned to work on 8/13/2014. Employee was hanging a vent tube when he felt a pain in his right arm.

Employee was carrying and spreading rock dust bags when she felt pain in her left shoulder. LTA effective 10/06/14.

Employee injured his left leg stepping down off the belt structure. As he came down, he felt a pop and could not put pressure on it.

Employee was helping to remove a belt clamp when he pinched his right pinky finger between the clamp and belt structure. The incident caused a laceration that required 5 sutures and surgery to repair.

Employee was helping install top rollers for a new belt installation. Another employee dropped a top roller striking the injured employee on the right ring finger. The incident caused a fracture and laceration that reguired six sutures to repair.

Based on Medical records received on 7/16/2014, the employee has been taken off of work for medical treatment. While bolting in 2L section, employees bolt wrench became stuck on the bolt head. While attempting to pull the bolt wrench off, his hands slipped, causing him to hit his left hand off of the rock protector/canopy resulting in a fracture to his left middle finger.

Employee was carrying a piece of shock mat, approximately 8 feet long, with his right arm and was opening a K-Door with his left arm when he felt pain in his lower back.

Struck by falling object

Employee was struck by a rock while rib bolting off of the miner. The rock struck his right leg. The employee worked all scheduled shift until being taken off of work on 7/30/2014.

Employee turned and sprained right ankle when stepping off a catwalk.

Employee was pulled into 102 shield as it advanced. Employee's leg was pinned between the shield toe and the pan line as he traveled in the area where shield had been batched to pull in. Compound fracture to the left leg.

Struck by flying object

Employee was running Tanks of dusters along the E Track @ E15 section. While he was checking on the #2 tank, he removed the cap and it blew his hard hat and glasses off causing dust to get in his right eye. He was diagnosed with a concussion.

A scoop struck the end of a track rail causing the rail to shift striking two employees on their legs below the knee. One of the employees was air lifted for medical treatment because of a 4"" laceration he received after striking the top of his head on an airlock door. The other employee received fractures to the tibia and fibula of his right leg.

A scoop struck the end of a track rail causing the rail to shift striking two employees on their legs below the knee. One of the employees was air lifted for medical treatment because of a 4"" laceration he received after striking the top of his head on an airlock door. The other employee received fractures to the tibia and fibula of his right leg.

While setting a hydraulic floor to roof support, as the top contacted the roof it caused the bottom to kick out striking the employees left foot causing a fracture to his 4th toe.

While carrying supplies to the continuous miner, employee hit his right ankle against the digging arm of the loader. Became LTA on 6/29/14.

Employee was loosening a rope ratchet on the 1J longwall when the ratchet moved suddenly causing him to injure his right shoulder

Employee was dragging canvas in E26 section. He lost his balance and fell, hitting the right side of his body / rib area on a ventilation tube. He was diagnosed with one cracked rib. LTA effective 6/25/14.

Employee was sliding a track tie underneath the track when he felt a pain in his lower back. (LTA effective 6/26)

Struck by falling object

Injured was carrying a sieve with a co-worker. Injured lost his grip on the sieve and it dropped onto his knee causing a laceration. The laceration required 5 sutures to close.

While installing a 16ft roof bolt, employee felt a pain in his left bicep. Diagnosed with a torn bicep muscle.

Over-exertion in pulling or pushing objects

Employee felt a pain in his right arm while throwing a track switch on Oak Springs bottom. He was later diagnosed with a torn bicep. (LTA 6/16/14)

Over-exertion in pulling or pushing objects

Employee was assisting to push a roll of belt off of the winder in the 1A storage unit when he felt pain in his left knee. He was later diagnosed with a torn meniscus that required surgery to repair on 7-24-14.

Employee was side bolting on the left side of the miner in 3 North Mains section. He was drilling a hole for the installation of an 8' bolt. While adding the second steel, the top steel bent and caught his left hand against the timber jack.

Employee was hauling a long wall shield when the scoop broke in half at the articulate connection. The employee continued to report to work until 9/14/2014 when he received a medical slip to be off work.

Employee was installing a roof strap when the strap fell off of the plank jack of the continuous miner striking his right hand. Caused a laceration to the tip of his right middle finger that required sutures to repair.

A Bailey Prep Plant employee was examining a screen in plant 2 on the third floor. As he was stepping off of the toeboard edge of the screen he twisted his left ankle.

Employee was shoveling spillage on 2 East mains belt. While moving a Wheelbarrow, it kicked over and caused the employee to twist her right knee. Became a LTA on 5/12/14.

Cable was too short for the task, powerclimber rode off cable, basket fell. Laceration on face, head trauma.

Employee strained his right calf muscle while reaching to latch a cable hanger.

Employee was driving in a wedge for a roof support can when he felt pain in his right shoulder.

An employee and injured employee were moving an adjustable top rail to the 30 belt conveyor. As they were descending stairs, EE set his end down to reposition himself. IE was at the top of the stairs and felt the rail move. IE tried to reposition his hand when his finger got pinched between the rail and the top step.

Employee was carrying (2) 5 gallon cans of oil past the stage loader and onto the LW face in front of the gate shield. His right leg got stuck in the mud when he turned and twisted his right knee. He was later diagnosed with a torn meniscus. LTA effective 5/14/14.

Employee was cleaning off the top of the LW shearer when a rock fell from the mine roof and struck him on the tip of his right index finger. Resulted in an open fracture and laceration that required sutures to repair.

Employee was struck in the nose by a bolt wrench while installing a bolt off of the miner.

Employee slipped while walking which caused a fracture to his fibula.

Employee was changing a bolter tire with a 3/4 ratchet. When the lug nut suddenly loosened he felt a pain in his left shoulder. He continued to work all scheduled shifts until he was taken off work on 4/28/14

Employee was walking through door at C2 and struck his head on the top of the door jamming his neck. Employee finished his shift and did not seek medical attention. Continued working until 9-23-14 when he was taken off work following an MRI - diagnosed bulging disc with nerve damage.

Employee was installing 12"" PVC drain pipe on top of coal silo. As he was raising the pipe with a chain hoist, the pipe slipped and struck him on the back on the neck/shoulder area. Note that employee did not report this until the following day.

Employee was running the rock dust tanks when the fill valve opened unexpectedly causing a release of pressure causing the employee to fall resulting in a fracture to his left thumb.

Crew of three were moving refuge chamber with a scoop, the chamber was hooked in the scoop bucket with the winch rope hooked onto a 3/8 inch chain which was on the top of the chamber in a V shape so the chamber could be lifted up. After the move the bucket was lowered and the winch rope broke striking the employee in the lower right arm with the rope and hook.

Employee stepped on a rail tie and rolled his left ankle resulting in an avulsion fracture to his ankle.

Employee was lifting a vent tube to install on the slider when he felt a pain in his shoulder.

Employee was dismounting operator's compartment of locomotive, stepped on track rail and rolled his left ankle, resulting in a sprain. Employee began missing work on 1-21-14.

Employees were loading cans into the can setter, they had just shortened the chain that held the can setter and were taking out the slack by moving the scoop ram out when the chain broke and hit the employee in the lower groin.

employee was carrying crib blocks along the track heading and tripped on a track tie. As he fell, one of the crib blocks caught his right middle finger against the top of the rail. (fracture and laceration with stitches)

Employee was building a stopping, and while attempting to place a stopping block on the stopping wall while working from a ladder, the ladder kicked out and the employee fell to the ground and the stopping block struck him in the left side of his head. The employee received 10 sutures.

While standing on a supply car the employee picked up an armored track tie and placed it on his shoulder and attempted to step off the car, lost his balance and fell. He attempted to throw the tie out of the way, his left ring finger was caught between the tie and the side of the dumpster. (Fracture and laceration requiring stitches to his left ring finger.)

Installing a 16' bolt with gopher drill, bolt was not going in hole straight and drill started to push away, employee tried to push drill back, driller let off pressure and bolt came down and struck thumb that was on frame of drill.

During staple installation a piece of metal fragmented and struck the employee on the left wrist. He then tried to lower the bolt and raise it back into the hole rotating it. The rotation caused the bolt wrench to come off the drill head striking the bolter and then striking him in the mouth.

He then tried to lower the bolt and raise it back into the hole rotating it. A piece of rock fell from the roof, between 2 longwall shields and struck the employee on the left arm causing a laceration that needed 6 sutures to close. No lost time as a result of this incident.

AT APPROXIMATELY 10:45 AM AN EMPLOYEE BECAME ILL AND COLLAPSED WHILE ASCENDING A SCAFFOLDING AT THE BAILEY PREPARATION PLANT AREA. HE WAS ATTENDED TO BY PERSONNEL ON THE SCENE AND TRANSFERRED TO AN AMBULANCE AND TRANSPORTED TO WASHINGTON PA HOSPITAL WHERE HE WAS PRONOUNCED DEAD. CAUSE OF DEATH IS CURRENTLY UNKNOWN PENDING AUTOPSY RESULTS.
