Accident type, without injuries

An unintentional roof fall occurred in the Oak Springs Bottom area located in #1 Entry at 119 Wall. The fall measured approximately 35'X16'X16'. There were no injuries as a result of this incident. Condition contributing the roof fall is adverse geological conditions in this area.

The H1 Tailgate section experienced a face ignition in the #3 Entry Survey Station #108+79. During the investigation MSHA cited 3 sprays inoperative in the 5-spray block located top right-side cutting drum wing. No injuries occurred.

The elevator at Patterson Creek portal was out of service due to a power fault causing a comm loss to the encoder reader which caused a fault malfunction in the computer processor. The computer processor had the faults cleared in the programming and the elevator returned to service.

An unintentional roof fall occurred in the #2 Entry of the 6 South Mains at 58.5 wall. It measured 7' high and 35' long and 16' wide. The fall was caused by adverse geological conditions. The fall occurred out-by the active section with No equipment damage or injury as a result of this event.

The brakes on the hoist car were not releasing. The CR relay was replaced and the brakes were then functioning correctly

There was communication loss between the top and bottom of the slope. A Circuit board was replaced in the PLC for the fiberoptic communication.

The Dryridge elevator stopped inadvertently with two guys aboard. The contactors on the Bottom door switch came apart causing the elevator to stop in the shaft. No injuries or equipment damage as a result of this incident. The bottom door switch was changed

During regular inspections a slight kink was found in the hoist rope 13' from the hoist car

The main power/comm cable that travels with the elevator was discovered to have some wear. The cable was replaced. This caused the elevator to be inoperable for several hours.

The Crabapple hoist became inoperable for an extended period of time. The Hoist rope over lapped itself on the hoist drum causing it to kink. The rope was changed out to eliminate the kink.

The 7 North #2 Capsule at the 7N2 shaft was out of service due to a malfunction with the VFD. The VFD was replaced and the capsule returned to service.

An unintentional roof fall occurred in 7 North Mains 44 to 45 Wall #1 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.

The BMX slope hoist was taken out of service when the ABB drive malfunctioned. The ABB drive was repaired and the hoist went back into service.

An unintentional roof fall occurred in a crosscut along 4 west belt line. The fall occurred at 45 xcut. The fall was approx. 40ft long by 20ft high by 16ft wide. This fall was caused by deteriorated geological conditions. There was no injury or equipment damage as a result of this incident.

The BMX slope hoist was taken out of service when the brake sensor alarm malfunctioned. The brake sensor alarm was repaired and the hoist went back into service.

The Dry Ridge hoist was inoperable for an extended period of time due to a faulting door switch. The door switch was repaired and the elevator was put back into service. No one injured or entrapped.

The elevator at BMX portal was out of service due to the brake drum release solenoid malfunctioning. The solenoid was replaced and the elevator returned to service.

The 1A Capsule at the 1A shaft was out of service due to a malfunction with the lightning arrester for the cable to the bottom landing switch. The lightning arrester was replaced and the capsule returned to service.

The BMX slope hoist was taken out of service due to a malfunction with the overspeed sensor. The control was reprogrammed and the hoist went back into service.

The 1A Capsule at the 1A shaft was out of service due to a malfunction with the bottom landing switch. The switch was replaced and the capsule returned to service.

3 persons were entrapped due to a faulty VFD firing card located on the Pleasant Grove elevator. They had been entrapped for approximately 5 hours and 38 minutes. The Mechanics replaced the VFD firing card and re-programmed with new parameters. No employees were injured.

An ignition occurred in the E32 Continuous Miner section, located in the #2 Entry at Survey Spad# 101+34. During the investigation MSHA cited missing/worn bits on the continuous miner head that may have contributed to the ignition.

A deformation was found in the Crabapple hoist rope during regular examination. The slight deformation was discovered 15ft from the hoist car. The hoist was taken out of service for the rope to be cut and re-terminated.

An unintentional roof fall occurred in 3 North Mains 13 to 13 1/2 Wall #0 Entry. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the 8' bolts to fail at there couplers.

Struck by flying object

Employee was repairing a loader chain link located in the 2 South Mains. While using a hammer, a piece of metal broke off the chain striking and puncturing EE's right leg above the knee. EE is scheduled to have the piece removed on 12/17/20.

Fall from machine

Employee was standing on the toes of a shield to advance the face, lost balance and fell backwards and right knee twisted causing pain. Diagnosed with a tibial plateau fracture and torn ACL / meniscus on 11/9/20.

While installing cover plate on tailgate drive employee's r thumb became pinched.

Employee was in the process of unloading belt rollers located in the 2 East Mains 37 wall, when employee's fingers became caught between two rollers, resulting in laceration. Employee received ten sutures.

Struck by falling object

Injured employee and a coworker were removing rollers and cradles from the belt. When pulling the cradle came loose striking injured employee on the right foot

Employee was assisting loading a scoop battery and as EE stepped back, EE slipped causing EE to fall down. EE felt pain in EE's left knee. Diagnosed with a tibial plateau fracture on 10/22/20.

Employee was walking across a plank bridge and caused injury to the side of foot.

Employee was lowering a set of jars into the V-door when got fingers caught causing injury.

Employee was pulling a beam into the scoop, slipped, and the beam fell on EE's arm.

Struck by flying object

The drive chain on the side bolter broke and the injured employee was struck by a link in the chain on EE's left hand. This caused a small fracture and a laceration that took 3 stitches to close.

Employee was walking towards bolter station when they stepped on a rock and felt a pain in right knee. Employee was diagnosed with a torn ACL on 9/30/20.

While performing fireboss examination employee was climbing over gob plate located at the G1 longwall tail, employee slipped and went to catch self, got arm caught and felt pain in right shoulder. Injured was diagnosed with a torn labrum of the right shoulder and started missing work on 10/12/20.

Employee was using a hydraulic jack to adjust the track haulage. The bar slipped out of the jack pocket causing them to fall against the jack. Employee pinched right thumb between the bar and the jack pump cylinder. This caused a fracture to the right thumb.

Fall onto or against objects

Employee was performing EE's fire boss examination on the slope belt, when EE slipped and fell. Causing injured to land on EE's M-20 Self Contain Self Rescuer and injuring EE's lower back. EE was diagnosed with a herniated disc and started missing work on 10/13/20.

Employee was unhooking pump hose and slipped on freshly graded rip-rap causing employee to roll ankle.

Over-exertion in lifting objects

Employee was lifting a roll of weed fabric, when employee lifted they felt a pop in abdomen. Employee was seen at and diagnosed with a hernia on 07/20/2020. Employee was allowed to return to work the same day at normal job. Employee has worked scheduled day up until 08/25/2020 when they had to have surgery to correct the hernia.

While throwing a track switch on 7/14/2020. The tension on the track switch caused EE to lose balance when EE felt a pain in back. EE continued working all scheduled shifts until 4/15/2021 when EE was taken off work for related issues with back. Lost time began 4/16/2021.

Struck against a moving object

EE was hauling material to dump area in the A40F Volvo Rock Truck. EE hit a small bump in the road and hurt EE's lower back and hip. EE notified another EE. An incident report was completed and EE was then taken to MedExpress.

Over-exertion in lifting objects

IE and another EE were team lifting a 187lb plate in the shop onto a table, when IE pulled a groin muscle.

Struck by falling object

Employee was rib bolting on the right side of the 4B miner. While drilling a piece of rock / coal came out of the rib and struck the employee. Employee received seven sutures to the left side of head.

Fall onto or against objects

Employee was removing a drain plug on the right side of a final drive. When drain plug broke loose employee stumbled back and ran shoulder into the tracks of the dozer

Over-exertion in pulling or pushing objects

Employee felt a pain in left shoulder when EE was pulling a roof bolt off of the miner. EE continued to work all scheduled shifts until being diagnosed with a torn rotator cuff with subsequent surgery to repair on 9/9/2020.

Caught in, under or between a moving and a stationary object

While rib bolting in the 5 south section, employee pinched EE's ring finger between EE's stored drill steel and the guarding that protects the main controls. EE received a fractured ring finger on EE's right hand.

Flash burns (welding)

Employee was working on a pan line using a cutting torch when EE suffered a flash burn. When the flash took place it went behind EE's welding hood causing minor burns to EE's nose, neck and lips.

While traveling through a mandoor located at 1 South Belt, employee's foot caught on the door frame. When employee caught self from falling, the door impacted employee on left knee causing it to bend awkwardly. Employee was diagnosed with a sprain to the left knee.

While operating a locomotive, the windshield on the locomotive inadvertently flipped up from the down position. The injured employee's fingers were resting on the hinge of the window frame and were pinched by the window frame.

Struck by powered moving object

While switching out cars located in 1 South 4 wall track switch, the injured employee thought the switch was thrown for the strait. The cars unexpectedly went into the switch, made contact with the employee, resulting in four staples to the head and a fractured vertebra.

IE attempted to throw bedrail thru door and smashed EE's finger between bedrail and door frame. Miner was fully trained, wearing EE's PPE and was in compliance with the law.

While side bolting off the non-tube side of miner. Employee was setting the roof strap when a rock approximately 5'x4'x3' fell from the roof hitting the employee in the back of right knee/calf, pinning leg between the miner step and the rock. Employee was diagnosed with a contusion of the right lower leg and Edema & Bruising.

Employee went to step up on bench at EE's locker in the Archer portal locker room. When EE stepped down EE's left foot landed on EE's supply bag which caused EE's foot to roll, triggering pain in EE's ankle. EE was diagnosed with a left foot lateral sprain, as well as a left fifth metatarsal fracture.

Employee was working on the monorail system in the E31 Longwall. As they were hitting the stiff-arm pin for removal, the stiff-arm simultaneously dropped pinching the tip of the employee's left thumb. The employee then quickly pulled back their left hand from the stiff-arm. Resulting in a degloving and fracture of the tip of the left thumb.

Over-exertion in wielding or throwing objects

Employee was setting posts and swinging a sledge hammer and felt pain in lower abdomen. Employee did not miss work until 1/27/2020 when it turned out that employee needed surgery for a hernia.

Over-exertion in lifting objects

Employee was unloading J-Crib bags out of a scoop bucket and felt pain in lower back. The miner was fully trained, using the proper PPE and complying with the law.

Struck by falling object

Employee was carrying a wing roller to change out. EE was carrying the roller up a set of steps and on the last step EE tripped and fell on the landing and the roller landed on EE's left thumb, resulting in fracturing EE's thumb and lacerated it also. EE was carrying the roller on EE's right shoulder.

Over-exertion (Not Elsewhere Classified)

Employee was riding in a mantrip and they spurred up and the employee was getting out of the mantrip with right leg first and right foot slipped on the loose material and extended right knee. EE continued to work and at the end of the shift knee became stiff and sore. EE had it checked out and has bursitis and had to have it drained.

Employee was helping another employee change a roller that was heavy and in an awkward position while trying put the roller in the frame work. The roller slipped out of their hands. Employee tried to hold the roller from falling and pinched EE's left ring finger tip against the frame work.

Struck against a moving object

Employee did not have a seat belt properly worn, causing them to strike their head off the front of the cab. The result was 9 stitches to the forehead.

Employee was helping set belt drive frame and got hit with a jack handle causing an injury about EE's eye.

Struck by falling object

While operating the miner, a rock came down between the mesh and struck EE on the left middle finger. EE received ten sutures to the finger.

While profiling belt structure employee was using roof bolt plate to maintain balance when employee's pinky slipped inside the hanger eye of the plate and caused a laceration.

Absorption of radiations, caustics, toxic and noxious substances

Employee was standing approx. 6 feet away from crib pumping activities and was inadvertently sprayed in the face with crib concrete material.

Employee was riding in the mantrip and hit EE's head on the SCSR cache causing injury

Employee was changing rollers and a cradle fell off structure causing injury to EE's wrist

While walking the 109 seals employee struck head on a roof strap, causing EE's hard hat to dislodge. As injured began to stand up EE struck head on same roof strap causing a laceration to the head. EE received six surgical staples. EE was treated and released back to work on 4/14/20.

Fall onto or against objects

Employee slipped and fell while holding a metal regulator panel. The panel cut right ear when employee fell causing a laceration that was stitched to close. The employee received 5 stitches. No lost time.

Employee was unloading a hydraulic water pump and the pump fell backwards squeezing and lacerating employees third left finger.

Employee was separating the forks on the scoop and got EE's finger pinched between the fork and the frame of the fork assembly.

Struck by flying object

Employee was grinding rip clips on the bottom of the belt with angle grinder for approximately 30-40 minutes. Employee was wearing safety glasses during operation grinding. Employee and Supervisor both noted that high winds contributed to this accident.

Employee began feeling pain in both wrists over the past few years while performing employee's normal job duties as a miner bolter. The pain has since increased. Employee was diagnosed with carpal tunnel and has been taken off for surgery to repair.

While performing normal duties as a roof bolter employee began to feel pain in right shoulder. The pain increased as employee continued to work all scheduled shifts. There was no single incident that caused this specific pain. Employee began missing time as a result of this shoulder pain on October 23rd 2020.

Fall to the walkway or working surface

Employee was walking to their car to retrieve an item - slipped on a rubber mat over a metal grating and fell to left knee. Small laceration and visible bruising to knee area
