Accident type, without injuries

An unintentional roof fall occurred in F26 Longwall section at 16 wall in 2-3 crosscut. The fall measured approx. 10ft high by 30ft long by 12ft wide. There was no injuries or equipment damage as a result of this incident.

During regular inspection of the Crabapple slope hoist rope, a kink in the rope was discovered. The hoist was taken out of service and the rope was reterminated.

An unintentional roof fall occurred in F27 Longwall section in the #2 track entry at approximately 20.5 wall. The fall measured approximately 8ft high by 30ft long by 12ft wide. There was no injuries as a result of this incident.

Sparta Elevator was down from approximately 5:20 AM to 6:05 AM due to a faulty door bumper. Elevator was in and out of service.

An unintentional roof fall occurred in F26 Longwall section at the headgate at 25 wall in the #1 belt entry. The fall measured approx. 13ft high by 23ft long by 12ft wide. There was no injuries or equipment damage as a result of this incident.

There was a kink found in the hoist rope 6 feet from the slope car. The kink was removed and rope reentered into hoist car.

During non destructive inspection of the hoist rope it was found that the minimum diameter was below 6%. The hoist rope was changed.

An unintentional roof fall occurred in F26 Longwall at the headgate 77+00 in the #1 entry. The fall measured approx. 16ft wide by 10ft long and approximately 8' high. There was no injuries or equipment damage as a result of this incident.

At approximately 11:10 PM, E28 longwall intersected an uncharted gas well at 82 shield at 116+45. *Accident Code for Inundation was chosen because there is no option for uncharted gas well.

A 20' X 16' X 16' area fell above the bolt anchorage point in the middle of the crosscut at 188 wall 3-4 entry in the 3 North Mains. There were no injuries or equipment damage as a result of this incident.

An unintentional roof fall occurred in F26 Longwall section at the tailgate @ approximately 86+67 to 86+40. The fall is approximately 27-30' long, 16' wide and 15' high. There was no injuries or equipment damage as a result of this incident.

An unintentional roof fall occurred in E27 Longwall section at 4+18 in the #4 belt entry. The fall measured approx. 16ft wide by 25ft long and 16' high. There was no injuries or equipment damage as a result of this incident.

During the weekly inspection of the 7N #1 capsule it was determined to be non-operational. The cause of the capsule being non-operational was determined to be from the guide rails in which it travels in the shaft being bent and distorted which would not allow it to travel the entire length of the shaft. There were no entrapments or injuries due to this occurrence.

The Dry Ridge Elevator stopped while in motion with an employee on board. The elevator was stopped for 45 min. The door switch at the bottom of the shaft became dislodged.

Oak Springs hoist was thought to be out of service. It was later realized that the touch screen panel for the hoist was not functioning. The hoist still ran in manual and could be operated. The hoist was not down.

Communication between the motor and gearbox encoders for the BMX Portal Hoist/Cage malfunctioned causing the cage to be inoperable. There were no injuries or entrapments as a result of the occurrence.

An electrical component inside of a door switch malfunctioned on the Patterson Creek Portal Elevator. The elevator cage was stuck at the surface landing and was inoperable until the door switch was replaced. There were no injuries or entraptments as a result of this occurrence.

During the 6 month inspection of the 8 wire ropes on the crabapple elevator, 1 of the ropes showed greater that a 6% reduction in diameter. The Elevator was taken out of service for repair.

At approximately 4:10 AM, E27 longwall intersected an uncharted gas well at 184 shield at 48+61. The hole had an 8"" casing that was cut through cleanly before the casing was noticed. Minimal methane was detected around the hole. *Accident Code for Inundation was chosen because there is no option for uncharted gas well.

An unintentional roof fall occurred in the #2 entry (track entry) of 4L section at 93+25(Inby 36 xcut). The fall measured approx. 7ft high by 12ft wide by 30ft long. There was no persons injured or any equipment damage as a result of this incident.

MSHA inspector found a kink in the hoist rope during a regular inspection and inspector took the hoist rope out of service.

An unplanned roof fall occured in the 4L section at 35 crosscut 2-3 entry (longbreak). The fall was approx 60 feet long by 9ft high. There were no injuries as a result of this incident.

The dry ridge elevator stopped while carrying 12 miners. It was discovered that an incorrect relay was installed on the bottom door switch. It was replaced and the miners were out of the elevator in 55 minutes.

Pleasant Grove Elevator was down at approximately 10:28 PM due to a faulty electrical governor.

The crabapple hoist was out of service for an extended period of time. The axial was damaged during hoisting. This required the mine to take the hoist out of service to repair. No employees injured as a result of this incident.

While in the process of making a weekly exam the Mine Examiner discovered a roof fall in the #1 Entry of the 3 East Mains. The fall was located 10' outby 42 wall and traveled outby 60 feet, there were no intersections affected. There were no persons injured or equipment damaged as a result of the fall.

Sparta Elevator was down from 1:10 AM to approximately 3:00 AM due to a faulty top door latch.

Employee was using a drill when it slipped and contacted right index finger causing a laceration. Employee finished employee's shift.

Employee was installing a hose in the monorail system in E28 longwall. EE was using a chisel to help assist getting the hose together. When EE swung the hammer, EE missed the chisel and struck left thumb. EE was diagnosed with a fracture to the left thumb.

Employee was pulling a submersible pump out of a sump and felt a pain in the groin. EE continued to work all scheduled shifts. EE was diagnosed with a hernia on 1/5/2017. EE was taken off work after being seen by a surgeon on 1/11/2017. Lost work days will begin counting from 1/11/17.

Fall from machine

Employee fell from a drive base (2.5 to 3ft high) and caught self with right hand. Diagnosed with a fractured ring finger on right hand

Employee stepped out of the operator's compartment of a diesel locomotive and twisted right knee. Diagnosed with a sprained LCL and started missing work on 12/07/2016.

Employee was attempting to load a battery into the scoop bucket with a winch. In the process, employee got foot caught between the scoop bucket and battery causing an injury.

Employee jammed left wrist when reconnecting drill steels.

While removing belt structure, employee's finger was pinched between a belt chair and the tailpiece.

Employee was handling the miner cable on 11/9/2016 when EE felt a pain in the groin. EE continued to work all scheduled shifts until being diagnosed and taken off of work on 1/27/2017 with a hernia.

While moving a roll of rubber employee felt a pain in right shoulder. First day of work missed was 12/29/16.

The injured employee and another employee were picking up a bent dozer blade. In the process of removing it off of the face, they leaned it against the handrail, pinching the employees right middle finger. EE received 12 sutures.

Employee was walking down the face and looked down to where ee was walking and hit head on a shield that was 6 inches lower than the other shields. Employee was knocked backwards and fell with left leg flexing under ee body. Medial meniscus tear of the left knee joint/prepatellar bursitis left knee joint.

Employee was side bolting in 3 North Mains. Employee used the step on the miner to reach the roof. While on the step, employee lost balance and caught self with left arm. Employee felt pain in left shoulder. Previous left shoulder injury. LTA 4-24-17.

The injured employee was roof bolting on the side of the miner on a gate development section. EE was in the process of adding a second drill steel and used the drill feed lever to raise the bolter head up to the bottom of the steel. When the drill head contacted the steel, the steel bent pinching thumb between the steel and the mast, causing a laceration.

While employee was bolting on the return side of the miner in the 4A section in the #3 entry. The employee was installing a 6' fully grouted bolt when the bolt head popped off the wrench and an unknown object contacted the employee's right pinky finger causing a cut laceration requiring 7 sutures.

Employee lifted a bag of rock dust to fill a can duster when they felt a pain in lower back. LTA 10/26/16

Employee was standing at #3 shield on E27 longwall face, when the face chain started and a 3/8"" wire rope tightened and knocked employee to the ground and hitting left shin on headgate.

Employee was rib bolting in F28 tailgate, when a rock came down from the roof / rib and hit employee's left thumb while their hand was on the bolt wrench. Received 13 sutures.

EE was side bolting in E29 installing a new roll of mesh on the miner. EE had to lower strap rack down in order to move the roll past the roof strap already on the miner. As ee stepped down off the miner, the miner sumped forward and the bolt tray hit the strap and caused it to go outby. The roof strap hit the employee behind left ear and on the back of neck.

While walking toward the miner EE slipped, fell, and twisted back.

On 7/30/2016 the injured employee exited a loading machine ee was operating, turned towards the outby end of the machine and stepped over the cable when ee felt pain in left knee. On 3/14/2018 ee had a partial lateral meniscectomy to repair the knee and began missing work.

While cutting the end of a piece of rock dust hose, employee cut the palm of left hand.

While preforming normal work duties as a miner bolter employee felt a pain in the groin area. Employee worked all scheduled shifts until they was diagnosed with a hernia and subsequently scheduled for surgery on 2/15/17.

While cleaning the sight glass on the emulsion tank with a paper towel the sight glass broke resulting in lacerations to the left index and middle fingers.

Employee rolled right foot walking from a switch to ride. Worked all scheduled shifts until being taken off of work on 7/25/16.

Employee was walking in the panline on E27 longwall face, when ee struck head on a low shield and felt pain in neck. LTA 7/11/16.

Fall from machine

Employee was in rib bolting in 2 East mains section. EE was standing on a step on the back of the miner pulling a roof strap when EE slipped off the step and fell backwards. Fracture to left wrist.

Struck against stationary object

Employee was operating the miner in E29 tailgate section, when a rock fell on top of the miner. While moving a rock into the conveyor chain, EE cut right middle finger on another piece of rock.

While installing an A/C compressor on the engine of a CAT 777 dump truck, EE was tightening a bolt with an impact gun. While tightening the bolt, EE was supporting the compressor with left hand. The compressor shifted while tightening the bolt, and pinched EE's hand between the compressor and mount.

Struck by falling object

Piece of rock fell from the roof/rib (corner) and struck employee's finger between the rock and the rock deflector on the miner. This caused a fracture to left middle finger.

Over-exertion in pulling or pushing objects

Employee felt a pain in left shoulder after throwing a manual track switch. EE worked all scheduled shifts until taken off work pending a MRI on 6/11/16. EE was diagnosed with a strained shoulder. Lost time began 6/11/16.

Employee was operating the miner in F27 section. Was cleaning out the right side dust box when the rib bolter began to drill. This closed the dust box door on right fingers.

Employee was bolting on the left side of the miner in F27 section. While the miner was backing up, employee placed left hand on end of the rib protector and the left pinky finger caught between a rib bolt and the rib protector. Diagnosed with a fracture to left pinky finger.

While walking on loose gravel in Oak Springs yard, employee felt pain in right knee. LTA 6/7/16.

While loading roof posts into a mantrip, employee felt a pain in lower back.

While removing shovels from behind the seat of a mantrip, the employee felt discomfort in right wrist. Diagnosed with a fracture to EE's right wrist. LTA 6/29/16.

Employee was rib bolting in E29 miner section. When EE pushed the bolt into the hole, EE felt a pain in right shoulder.

Struck by falling object

Employee was side bolting on the miner in F27 when a rock fell from the roof / rib and hit employee on the left shoulder / neck area.

Employee was working as a shield person on the F25 longwall face. EE was standing on the toes of number six shield when the number seven shield pulled in. When the shield pulled in, it caught the end of a ""leg jack holder"" stored on the face. The holder shifted toward EE and struck them in the back and side of right their leg in the calf area.

Employee was walking in a shuttle car rut, the ground gave away under EE's right foot causing pain in EE's lower leg. EE continued to work until EE was taken off work on 4/4/2016

While walking employee stepped on a rock twisting EE's ankle. An x-ray revealed a broken ankle. EE first missed work on 3/10/16

Employee was going through a man door in 2 East Mains Right, when left foot slipped causing left knee to twist. MRI pending.

Employee tripped and fell, as employee was attempting to catch self, felt a pain in left shoulder. Employee continued to work all scheduled shifts until being taken off work on 5/18/16

Employee was side bolting in F26 section, when they lifted an 8 foot bolt into the hole and felt pain in their right shoulder.

Employee was walking in E27 longwall track entry, when they stepped into a muddy rut and felt pain in their right knee. MRI results pending.

Employee was walking in 3 North Right when they stepped in a rut, losing their balance and falling to the ground causing them to have pain in their right knee. Lost time on 2/23/16.

Employee was working on a shield hose, loosening a fitting with a hammer when a piece of the hammer chipped off and a sliver of the hammer became lodged in right index finger.

Employee was moving a belt roller out of the walkway along 4 West belt and felt a pain in lower back. EE continued to work all scheduled shifts until being taken off on 3/2/16 for herniated disks

While lifting a can of oil from a car EE felt a pain in EE's shoulder. An MRI revealed a torn left labrum. EE first missed work on 3/10/16.

While operating a mantrip on E27 track, employee struck a broken rail causing EE's left shoulder and lower lip to hit operator's compartment.

Employee was carrying a 5 gallon oil can up #1 entry to the F25 longwall face thru a water hole measuring 30'L X 5'W X 4""D when stepped on a rock twisting left knee. LTA on 7/8/16.

Employee was stepping up onto an equipment dolly when ee lost balance and twisted left knee. Diagnosed with a sprained MCL and began missing work on 12/07/2016.

Employee was offloading a bolt tray off of the scoop. In the process, a weld broke on the bolt tray and smashed employee's finger between scoop and tray.

Employee bent down to pull out a tie under gravel cars. As employee stood up, right knee buckled. Diagnosed with a torn ACL. LTA on 12/6/16.

While working underground at Bailey Mine's Reclaim Belt, employee slipped in mud while removing top platens of a sectional vulcanizing press injuring EE's left shoulder, arm, and knee. REPORT WAS NOT SENT WITHIN 10 DAY TIME FRAME DUE TO WAITING ON CLARIFICATION FROM EMPLOYEE'S PHYSICIAN WHETHER INJURY WAS PRE-EXISTING PRIOR TO INCIDENT OR PRESENTED NEW INJURY.

Employee was side bolting in 3 North Mains. Was trying to remove bolt wrench from the chuck but it was stuck. Employee pulled and wiggled the wrench and when it came out of the chuck it hit the employee in the mouth causing a chip in right front tooth.

Employees was taking down airline and when EE hit the coupler, the line discharged and hit employee in the face causing injury.

Employee was throwing garbage in a scoop bucket. Threw a track frog into bucket, a rib bolt sprung up and hit employee in mouth causing an injury

Employee was a passenger in a mantrip when it was derailed causing to strike elbow on the inside of the mantrip. This caused a laceration that needed 4 stitches to close

Struck against stationary object

Employee was adjusting the roof mesh above the continuous miner when left hand slipped and struck the edge of a previously installed roof strap. Caused a laceration to the knuckle of middle finger on left hand that required 2 sutures to repair.

Employee was laying track. EE placed a crib down while another employee used a track hammer to hit into place. The other employee missed the crib and hit the injured employee in the mouth with the hammer.

While removing a clog from a rock dust hose, a coupler came apart and struck EE on the right forearm.

While supplying the miner in E29 HG, employee was pushing straps on the miner. As EE was pushing, one got caught and forced arm into the stack, cutting right forearm. Received 11 stitches.

A rock fell from between 2 shields and struck EE's left hand causing a laceration.

Employee was loading straps on the miner when EE cut their left wrist on a strap. Received 10 sutures.

Employee was lowering tailgate of a rail truck and got finger smashed between tailgate and rail truck.

Employee was handling a track rail that was bent and it rolled over and hit foot.

Fall onto or against objects

Employee slipped while walking down a set of steps and struck head on a over hanging beam. Employee suffered a laceration above right eye. Stitches to close. No lost time.

A piece of rock fell out of the roof and struck the tailgate shearer drum, a piece of the rock flew off the drum and struck EE in the lip

Employee was changing a vertical pin on F25 longwall when they pinched their right ring finger. They received 2 sutures.

Employee had just placed a beam on top of shield to be pulled in E26. They stepped into the walker shields to get out of the way of the mule. As they stepped into the walker shield they slipped and fell. They attempted to stop their fall and caught their fingers on the bracket for the base lift jack. Diagnosed with a fracture to the tip of their right middle finger.

Employee was tossing winch rope into scoop and the safety clasp on the hook got caught on EE's finger causing a laceration.

Employee was struck on the neck by a piece of rock that fell from the roof. Employee did not miss any scheduled work days as a result of the injury. EE was treated with 8 stitches to close.

Employee was diagnosed with Carpal Tunnel illness as a result of repetitive motion, performing work related tasks. Began missing time on 12-7-2016 as a result of surgery to repair
