An Unintentional roof fall occurred in the 5L section. The fall occurred in the belt entry at 91+72 it extended to 91+50. The cause of the fall is unknown, there were adverse geological conditions in this area in the form of layer shale. No injury or equipment damage as a result of this occurrence
Employee was on the Dry Ridge elevator conducting a regular inspection when the elevator stopped due to an over travel fault. The elevator was not operational until the fault was reset by outside employees. The elevator was down and the employee was stuck for an extended period of time.
During regular inspection of the hoist rope, damage to this cable was detected and the decision was made to change the rope out. This made the Crabapple hoist out of service for an extended period of time. No specific reason was found that caused the damage to the rope. (normal wear)
Pleasant Grove Elevator was down due to a faulty door switch.
Elevator doors would not operate. During investigation found that the elevator went into over travel.
The elevator at Patterson Creek portal was out of service due to a faulty bottom landing door switch. The switch was replaced and the elevator returned to service.
Two persons were entrapped on the Sparta elevator when while riding in the elevator it stopped approximately 42' from the top landing due to the bottom hallway door latch being out of adjustment. The fault was reset on the elevator and it was called to the top where the persons exited the elevator and the door latch arm was adjusted and the elevator was tested.
The Sparta elevator was down approximately 42' from the top landing due to the bottom hallway door latch being out of adjustment.
The Oak Springs hoist was taken out of service due to damage to the hoist rope.
An unplanned roof fall occurred in the 6L section in the #2 entry just out by 33 x-cut. The fall measured 15ft long by 6ft high by 10ft wide. The opinion formed during the investigation was that the fall was caused by adverse geological condition, in the form of a cutter running up the left bolt line.
Sparta Elevator was down due to the bolts breaking on door latch.
An unintentional roof fall occurred in E29 LW Section in the 2-1 crosscut at 17 wall. The fall measured approx. 10-12ft high by 50ft long by 16ft wide. There was no injuries as a result of this incident. Condition contributing to the roof fall is adverse geological conditions in this area.
An unintentional roof fall occurred in the 6 south area of the mine. Fall occurred in the #1 entry inby 57 xcut. Fall measured approx. 30ft long by 6ft high by 16ft wide. No injury or equipment damage. *It was determined that there were unusual adverse geological conditions in this area that caused the immediate roof to separate from material above the anchorage zone.
The Oak Springs hoist was taken out of service due to the cooling motor needing to be replaced for the hoist.
The safety switch for the buffer jack was out of alignment. The elevator hoist was out of service on the bottom landing until the safety switch mounting bracket was adjusted. There were no injuries or entrapments as a result of this occurrence.
The Crabapple Hoist car was not operational for an extended period of time due to Battery connection.
This form is being submitted to abate citation 9075552. We experienced an overflow of refuse slurry material at the area #1 impoundment. There were no injuries or equipment damage. The flow was fully contained on site. Our investigation to date does not indicate that this meets part 50 reportable requirements.
The Oak Springs hoist was taken out of service due to damage to the hoist rope.
MSHA was testing the 3N6 escape capsule. It was lowered to the bottom and when it started to raise back up, the capsule got stuck between the guide cables. The capsule was out of service until they got it released from the cables.
There were broken strands on the Crabapple Hoist rope found during routine inspection. The hoist was immediately taken out of service.
Pleasant Grove Elevator was down due to a faulty door switch.
During regular inspection of the Hoist Car, a bad wheel bearing was found. The Crabapple Hoist car was taken out of service to repair.
An unintentional roof fall occurred in F28 TG Miner Section in the #1 entry just inby 25 wall from spad #62+21 to 62+53. The fall measured approx. 12ft high by 32ft long by 12ft wide. There was no injuries as a result of this incident. Conditions contributing to the roof fall include adverse geological conditions in this area.
The BMX Portal elevator hoist was out of service due to normal wear to the linkage on the inside door causing the doors to malfunction. No persons were injured or entrapped due to this event.
An unintentional roof fall occurred in F27 Longwall section between 26 wall and 25.5 wall at spad numbers 63+60 to 63+88 in the #1 entry. The roof strata separated above the anchorage and fell to the ground. The fall measured approx. 10ft high by 28ft long by 12ft wide. There was no injuries as a result of this incident.
A Mine Examiner discovered a roof fall in the #1 intake entry of 7 North Mains just outby 31 wall. The fall originated just inby a permanent ventilation stopping located in the entry and traveled approximately 19 1/2 long and 7 1/2 feet wide. There were no intersections affected by the fall. No persons were injured or equipment damaged as a result of the fall.
There was a kink in the hoist rope near the termination point at the hoist car. The kink was removed and rope reentered into the hoist car.
The E29HG crew experienced a face ignition while mining. It lasted about 30 seconds. There was a clay vein that went across the face and it emitted methane on the left side of the miner. MSHA also citied worn miner bits and low water pressure.
The jack rope gripper faulted due to an oil leak. The elevator hoist was out of service at the surface landing until the jack was filled up with oil. There were no injuries or entrapments as a result of this occurrence.
Sparta Elevator was down due to a malfunctioning door switch. No one was on the elevator at the time.
At approximately 11:48 AM, F27 Longwall intersected an uncharted gas well between 35&36 shields at 79+05. *Accident Code for Inundation was chosen because there is no option for uncharted gas well.
The dry ridge elevator was not operational due to a faulty car safety switch. It was replaced.
Around 7:02 pm on 3/20/17, the E29HG crew experienced a face ignition while mining. Conditions contributing to the face ignition include: a clay vein traveling across the face which emitted methane. MSHA also cited bad bits, two broken tubes, and two sprays out in the throat of the miner.
An unintentional roof fall occurred in F28 TG Miner Section in the #1 entry just inby 25.5 wall from spad #63+38 to 63+11. The fall measured approx. 8ft high by 27ft long by 16ft wide. There was no injuries as a result of this incident.
Sparta Elevator was down at approximately 8:48 AM to 9:17 AM due to the PLC needing reset on the elevator.
An unintentional roof fall occurred in the 5L section in the return entry at approx. 94+92. The fall measured 25ft wide by 6ft high by 17ft wide. There was no injuries and no equipment damage as a result of this incident
The Hoist car was not operational for an extended period of time due to a broken Axle.
An unintentional roof fall occurred in F26 Longwall section just inby 15 wall in the #1 entry. The fall measured approx. 9ft high by 23ft long by 12ft wide. There was no injuries as a result of this incident.
Due to frozen grease build up on the governor wheel, the Dryridge elevator malfunctioned and stopped with an employee on board.
Employee was pulling coupler cut-off on the general mine car when the cable broke, causing employee to lose balance and fall injuring right shoulder.
Employee was installing a sandwich plate on the E29 longwall face at #96 shield when EE experienced pain in right shoulder that resulted in a torn Labrum in right shoulder.
Employee opened silo door and felt pain in stomach.
Employee was a passenger in a mantrip when it de-railed. Employee claims that EE struck head on the roof of the mantrip. The accident is currently under investigation. The employee was considered a lost time based on information received on 12/18/17.
Employee was getting out of shuttle car when they hyperextended the left knee. Employee started missing work 12/12/2017.
Injured employee was helping to load an Irwin Diesel Locomotive wheel onto a dolly when ee caught left ring finger between the wheel and the edge of the dolly. The injury caused a fracture to the tip of finger and a laceration that required 4 sutures.
Two employees were carrying a rack wheel onto the face of the E-29 LW section when the rack wheel moved causing the weight of the wheel to shift towards the injured employee which in turn caused the IE to strain lower back.
Employee was attempting to pick up a track rail and felt pain in employees stomach area.
While unloading steel beams from a scoop bucket, employee dropped the beam pinching their fingers between the dropped beam and a beam laying on the ground.
Employee was shoveling under belt and hit solid rock and injured finger.
The employee was throwing a track switch on Pleasant Grove bottom when EE felt pain in right shoulder. EE was later diagnosed with a partial tear of the right rotator cuff and began missing work on 10/30/2017.
Employee was pushing open the k doors on PG Bottom, when employee felt a pain in employee's right ankle. Diagnosed with a ruptured Achilles tendon.
Employee was walking in F28 HG where employee was working on a pump. As employee was walking, employee's left foot slipped down and felt pain in left hip. Lost Time on 10/16/17.
Employee stood up while walking through the plant and jammed neck on a pipe that was above EE.
The employee was standing behind the rib pinner on the intake side of the 5A Joy ED miner when a bundle of 4"" x 6"" roof plates unexpectedly fell off the side of the miner. The plates then struck the employee's left middle finger causing a laceration that required 5 sutures to repair.
On 12/30/15 the employee was lifting an I-beam injuring their back and began missing work on 1/11/2016 which was reported on a 7000-1. On 10/1/2017 the employee was in the locker room getting ready for their shift. While retrieving work boots they re-aggravated their back and started missing work again on 10/2/2017. This 7000-1 is being submitted to abate citation (9077226).
Employee was using a hammer and a chisel to remove metal from the top of a tool car when fragment of metal struck EE in the chin. Employee did not seek immediate medical treatment and worked all scheduled shifts until being taken off work for surgery to remove the piece of metal on 10/5/2017.
Employee was cleaning beltline and injured back while throwing rock dust with a shovel.
Employee was helping offload cans off a car. When the scoop backed up, it hid a bedrail that swung around and employee in the foot causing an injury.
Employee was lifting a ventilation tube, when a piece of fiberglass went through glove and into left palm. Surgery required.
Employee injured lower back while lifting parts to change equipment.
Employee was throwing track switch when ee felt pain in arm.
Employee was laying track through a mud hole in F28 HG. Employee felt pain to employee's lower legs from coming in contact with concrete mix used in the mud hole. Abrasions and burns to both lower legs.
Employee left knee was struck with CMI cone as employee was hooking it to a hoist and it slid off the cart.
A discharge line fell striking the employees R hand while the employees hand was resting on top of an airline.
Employees was walking in the right recovery chute in E28 longwall when employee slipped on a piece of belt in the walkway. Employee braced for the fall with employee's right arm and felt a pain in employee's right shoulder when employee landed. MRI results pending.
The employee was bolting on the intake side of the continuous miner, while retrieving a roof strap from the strap rack on the miner EE got EE's left hand caught between an upright standard on the strap rack and an upright standard on the bolt rack causing a laceration/avulsion to the back of EE's hand. The injury required 12 sutures to repair.
Employee was changing bits in E29 Longwall on the tailgate drum when a rock fell from face and landed on left foot.
The employee was recovering high voltage cable when the cable fell from the mine roof it struck the employee on the L wrist. The weight of the cable caused the employee's wrist to bend back fracturing L wrist.
Employee was pulling plastic steps into place while working on a motor in the underground shop. The steps caught EE's left foot and EE fell to the ground. EE felt pain in left knee. Diagnosed with ACL and meniscus damage. LTA 8/21/17.
Employee was dragging a ventilation tube when ee felt a strain in hand. EE followed scheduled work schedule until seeing the doctor on 7/12 at which time ee was taken off work. First day missed 7/12
Employee was building a block wall on top of scaffolding. While in a kneeling position the employee lifted a block to hand to another employee. When handing the block over to the other employee they felt pain down the lower R side of their back and leg.
Employee was removing old rollers from 3 North #6 belt. Employee bent over to pick up a roller and felt pain in the right hip and back. Diagnosed with a herniated disc on 9/20/17.
While going thru a man door, Employee's foot got caught and employee fell to the ground. Employee complained of back pain that radiated down into leg. Employee worked all scheduled shifts until employee aggravated it on 10-12-17. After several doctors opinions employee's injuries were considered related to the 6/22 claim and became a lost time on 10/27/2017
Employee tripped while chaining cars together and caught self with right hand on the ground. This caused a fracture to hand and wrist.
Employee was walking to talk to motormen when employee stepped on the corner of a track tie and felt pain in employee's left foot. Diagnosed with a broken 5th metatarsal.
Employee was side bolting on the miner in 2 East Mains. They were beginning the 4-5 crosscut at 86 wall. After they installed their first strap, the side bolter traveled outby just as the top inby corner fell out. The rock landed on their left foot. Diagnosed with a dislocated foot.
EE was helping unload a gang box out a pickup to the shop floor and carrying it backwards to set it down when EE caught left knee on a ratchet strap that was holding the belt press together. There was four other people helping to unload the gang box.
Employee was struck by a piece of roof/rib that they were drilling through causing them to fall forward and strike their face on the side of the miner. This caused a laceration to their right eye. 6 stitches
Employee was stepping down from the step on the left side of the miner and stepped on a piece of coal and twisted right knee. Diagnosed with a possible medial meniscus tear on 6/6/17.
While side bolting, employee was raising a roof strap to the roof when a piece of rock 20""x14""x6"" fell out from the roof and hit employee on employee's right knee. When employee fell, employee caught self and jammed employee's left wrist. Diagnosed with a blood clot in employee's right leg on 6/1/17.
While re-railing a mine car, the hydraulic jack kicked out and struck EE on the right arm/hand, causing a fracture
A nail used to secure the free end of a belting roll was allowed to remain in the belt and was pointing up as the free end was lying on the ground. The employee stepped on the nail.
While working on the suction system on the angle bolter, employee pinched right middle finger with the dust box door. The laceration required 5 stitches to close.
Employee was walking in F28 TG alongside the exhaust fan tubing. The tubing stretched from the straight of the #2 entry around the corner of the 2-3 track chute to the return. The slack in the tubing shifted quickly and knocked employee to the ground. Employee fell on their right ribs. Diagnosed with two broken ribs.
Employee was carrying mandoor when they stepped in a hole and injured knee.
At approximately 8:30am an accident occurred at the Bailey prep plant course refuse area. While an employee was walking to the dozer EE was struck by a Caterpillar road grader that was traveling in reverse.
Employee fell while traveling through a man door and the door closed on leg. As employee reached back to attempt to free self employee felt a pain in right knee and lower back. Became a lost time on 5/8/2017
Employee put both hands on the lip of a trash car when a roller that was in the car rolled and struck their pinky finger between the roller and the side of the car.
While rock dusting, employee stepped in a hold and felt a pain in employee's knee. Employee worked all schedule shift until taken off by a surgeon on May 3rd
Employee performed normal duties, on Friday 4/7/2017, of handling material. On morning of 4/8/2017 (Saturday) Employee felt pain in left shoulder. No specific incident. Employee went out for surgery 5/24/2017.
Employee was loading a 2"" water pump onto a mantrip and felt a pain in lower back.
Employee was walking from the mantrip to load center on a CM Section. EE stepped on uneven ground and felt a pain in the left knee. EE continued to work all scheduled shifts until EE was diagnosed with torn Meniscus. Lost time began on 4/3/2017.
Employee was dragging rock dust hose and felt a pain in lower back. First official day missed was 3-27-2017.
Employee was lifting block and felt pain in groin area
Employee was carrying a de-rail to be installed on the track when the de-rail unexpectedly opened up causing employee to lose grip with the left hand allowing all the weight of the de-rail to shift to the right arm injuring their shoulder. Employee was diagnosed with an torn rotator cuff and began missing work on 5/24/17 to have the right rotator cuff surgically repaired.
Employee was running ram car in 2 East mains section when ee moved a loop of miner cable in the tram road and felt a pain in lower abdomen. Diagnosed with a hernia.
Employee came in contact with a damaged area in the angle bolter cable and suffered an electrical shock which caused to fall to the ground catching self with right hand. Causing a fractured bone in right arm
Employee was examining 3 belt around 48 wall when employee took a step and strained employee's left knee. The walkway in the area was level and had no obstructions. Employee was diagnosed with a torn ACL and began missing work on 5/5/17 to have employee's knee surgically repaired.
Employee was walking down the slope belt. Employee turned sideways and stepped over a hose and felt a pain in employee's left knee. Diagnosed with a torn meniscus on 3/7/17.
Employee was operating the tailgate side of the shear in E28 longwall mining towards the tail. At 108 shield, a rock flung off the cowl and struck employee on the right shoulder. Diagnosed with partially torn tendons in right shoulder on 3/3/17.
Employee was side bolting on the left side of E29TG miner. As ee connected the drill steels, ee applied up pressure which cause the steels to bend, pinching left middle finger between the steel and the ATRS jack.
Employee was raising a clothes basket in the crabapple portal locker room and struck the left hand on the lockers door latch causing a laceration.
Employee was stacking wooden pallets on a mine supply car when ee slipped and felt pain in lower back.
On 7/6/2017 at approximately 2:00 pm, service technician was riding in the jumper seat of a man trip at the Bailey Mine (Crabapple). When exiting the man trip, the employee underestimated the distance to the ground and hyperextended right knee. The injury was reported late afternoon of 7/10/2017.
Employee on drive crew was assembling a skirt belt and caught ring finger between an upright and a beam
Employee was changing bits on the E28 longwall shear. EE was on the head gate side of the shear and attempted to rotate the drum with another employee. EE pushed on the drum with right foot and felt a pain in left groin. Diagnosed with a hernia.
2 ee were cleaning. Debris from structure wing roller was stuck from debris. EE was on head side of structure using a slate bar cleaning on debris. Slate bar ee was using bounced off debris and point hit belt causing belt to pull slate bar. Pulling slate bar fast it pulled ee toward belt resulting in left thumb getting caught between belt & roller.
Employee was setting a rock prop when employee tripped over another rock prop in the work area. When employee fell to the ground the prop that employee was setting fell and struck employee in the back area.
Employee was loading water line into scoop and caught employee's pinky finger caught between pipe and scoop bucket causing injury to finger
Employee was assisting with changing flights on F26 Longwall face at the headgate. A rock fell from the roof / face and struck the injured on left foot. The shields were pulled in to the face in this area. Diagnosed with fractured left 5th toe on 7/24/17.
Employee was outby on the E28 Longwall belt checking dollies. EE crossed the belt to move a chain off the dollies. When EE returned to the walk side of the belt, EE tripped coming off the belt and fell onto belt structure. EE felt pain in EE's left ribs. Diagnosed with a fractured rib.
Employee was installing a rear boom jack roller on the Continuous Miner. The roller came out of the guides and pinched employee's little finger on employee's left hand causing a laceration that was given 4 stitches. Employee returned to work the next scheduled shift.
Employee was traveling inby on the E-track when the mantrip EE was operating derailed causing EE to strike EE's left rib area in operator's compartment of the mantrip. Was diagnosed with a non-displaced fracture of the 7th rib.
Employee was knocking down a block wall and hit finger with a hammer causing injury.
While cutting a piece of conveyor belt, the knife slipped and cut the right thigh. 9 Stitches to close the laceration. No lost time as a result of the injury.
Employee was cutting a joint of dust line when the blade broke and caused a laceration to hand.
Employee was setting prop and rock fell from top and hit employee on head causing injury.
Employees was loading posts onto the forks of a scoop and smashed finger between two posts causing injury.
While employee was changing bits at the headgate a rock roughly the size of 12"" Long x 6"" wide x 2"" Thick fell out between the #19 and #20 shield hitting the employee on the top of the right middle finger causing a cut/laceration; 6 sutures
Employee was working on the F-27 Longwall face when a rock fell from between the shields and grazed the left side of face causing a laceration to left cheek that required 5 sutures to repair.
While cutting a zip tie with a utility knife, the knife slipped and ee cut two fingers.
The employee was in the 3A longwall in the process of changing bits on the tailgate drum of the shearer. While the employee and a co-worker were spinning the drum down, the injured employee pinched right ring finger between a newly installed bit and the LW face. The result of this incident was 14 sutures to the right ring finger.
Miner was welding a patch on a Sump when the clamp that was holding the patch on came loose and struck the miner on upper lip.
Employee was walking by E29 longwall head drive motor when the mesh tore and rocks fell out, striking the left hand. They received 16 sutures.
Employee was handling belt cradle and smashed finger causing injury.
Employee was using a ratchet to tighten a bolt on the shearer spray arm. The ratchet slipped off of the bolt head causing ee to strike knuckles on the frame of the machine. This caused a laceration on right ring finger that 3 stitches was used to close. No lost work days
The employee was scooping an entry with a 580 scoop, and was attempting to remove a rock with the bucket by going underneath it. While in this process the front scoop tires where lifted off the ground, then the bucket came off the rock causing the scoop to bounce down. During this motion the employee bounced in the cab and bit down on teeth, resulting in a chipped tooth.
While the employee was operating the remote control for the Continuous Miner, a rock fell from the roof and struck employee's left thumb. Diagnosed with a hairline fracture of the left thumb 7/27/17.
EE and a coworker were on the industrial elevator at Enlow Mine. EE's fingers were pinched between the door of the elevator and the door frame for the elevator door. EE lost the fingernail on and fractured a phalanx in left ring finger.
Employee was center bolting in F28 HG. EE was installing roof bolts in a turn out area. After drilling the hole, EE pulled out EE's steels and a rock fell from the roof and hit EE's left arm. Rock was approximately 2'x3'x3.5"". It caused a laceration to EE's left forearm which required eight sutures.
Employee was changing a bottom roller. While lifting the new one up into place it slipped out of employee's hands. Employee tried to catch the falling roller when it pinched their arm between the roller and a jack. Laceration, 8 stitches to close
Employee was side bolting in E29HG and while raising the steel to the roof a rock came out from the roof, bounced off the strap and struck the employee. Rock hit employee on the left side of employee's face and employee received two stitches above employee's left eye.
While rib bolting off of the side of the miner, employee pinched left thumb between the bolt and the bolter head. Employee received 6 stitches to close the laceration. No lost time.
EE was carrying items and stepping off of a catwalk to the floor when EE's right foot slipped on the wet floor and overstretched EE's right groin. EE did not report it because EE was afraid of getting fired. EE did get fired on 5/26/2017 do to a D/A test, that is when EE filed a claim. EE was off two scheduled days before coming back to work on 5/10/2017
A piece of metal chipped off of a hammer head while employee was using it and struck employee in the left arm. Piece of metal was remove and one stitch was used to close the wound.
While removing a flight from a loader conveyor chain, a piece of metal flew off the flight when struck with a sledge hammer and hit the employee under bottom lip. EE received two sutures.
Employee was helping to remove a rock that was stuck in the LW face chain/panline. While lifting the rock using a come-a-long the rock unexpectedly pivoted and caught employee's left foot between the rock and the face side of the panline handrail. On 3/29/17 employee was diagnosed with an avulsion fracture of the navicular bone in left foot.
GMS employee was bolting and the drill steel fell out and caught employee's hand between drill steel and drill handle causing injury to hand.
Employee was loading a heavy equipment part into service truck, ee grip on the part slipped, causing the part to fall on the tailgate of truck. EE right hand was in between the part and the truck tailgate as the part fell, cutting right hand middle finger.
Employee was pulling the safety bar from under plating and when the employee was pulling it out there was another safety bar directly behind EE against the rib and finger was pinched in the process.
The operator was notified on 11/15/2017 that the employee was awarded compensation for Noise Induced Hearing Loss.
Employee began to feel pain and numbness in both arms and hands. Employee was subsequently diagnosed with Carpal Tunnel Syndrome and scheduled for surgery to repair on 12-6-2017.
Employee began feeling pain in EE's left wrist while performing EE's normal duties. EE was diagnosed with carpal tunnel syndrome and continued EE's normal duties until EE had surgery and became a lost time on 11/18/19. EE's carpal tunnel was due to repetitive motion and it was determined that it was work related.
