BASED ON THE INFORMATION RECEIVED FROM THOSE PRESENT IT APPEARS THE ALLEDGED IGNITION OCCURRED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEAR) IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING HARD ROCK AND COAL IN ADVERSE GEOLOGICAL CONDITION.

BASED ON THE INFORMATION IDENTIFIED BY THOSE PRESENT, IT APPEARS THE IGNITION OCCURRED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEAR) IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING HARD ROCK AND COAL IN ADVERSE GEOLOGICAL CONDITION AT SHIELD #174. MSHA NOTIFIED.

Based on the info received from those present it appears that the ignition was caused by a spark from the cutting machine (shearer) ignited an unknown methane bleeder while cutting under shearer. MSHA notified.

Based on the information how received from those present it appears that the ign. tron was caused when the spark from the cutting machine shearer ignited an unknown methane bleeder while cutting hard rock land coal at shield #104 with headgate drum. msha was notified timely.

Shoal Creek Mine experienced an unplanned roof fall on the B-11 section in the No.3 entry last open crosscut. The Fletcher drill was in by the roof fall and was blocked. This was above anchorage of the bolts.

BASED ON THE INFORMATION IDENTIFIED BY THOSE PRESENT, IT APPEARED THE IGNITION WAS CAUSED WHEN SPARKS FROM THE CUTTING MACHINE (SHEARER) TAILGATE DRUM IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING COAL/ROCK. THE FLAME LASTED LESS THAT 3 SECONDS.

BASED ON THE INFORMATION FROM THOSE PRESENT, IT APPEARED THE IGNITION WAS CAUSED WHEN THE CUTTER BITS WERE TRIMMING ROOF ROCK AND A SPARK IGNITED AN UNKNOWN METHANE BLEEDER.

Based on info received it appears the ignition was caused when the sparks from cutting torch caused a piece of slag to travel from the off-side of the machine to the coal face and ignite an unknown methane bleeder.

Employee stated his left knee caught belt rail. He told operator of utility loader to lower bucket; instead, the operator raised the bucket and the employee sustained a fracture.

EE STATED EE AND COWORKER DISCONNECTED THE 4"" WATER LINE FROM DRISCO LINE, IN PROCESS THE WATER STARTED BLOWING OUT AND CAUSED EE TO FALL AND HURT SHOULDER.

WHILE WORKING IN TAILGATE ENTRY, EMPLOYEE WAS IN THE PROCESS OF CUTTING DRISCO PIPE WHEN A SECTION OF COAL RIB FELL STRIKING HIM ON THE HARD HAT.
