Accident type, without injuries

A rockfall was discovered in the south mains right return B5 #2 entry. The brow fell from the side of an already excavated overcast. This was reported as required and the MSHA Inspector on site was notified.

A roof fall occurred at the #13 crosscut on the longwall. This fall occurred as a result of this area being flooded in 2006.

Roof bolter was tightening a rib pin when an ignition occurred. No injuries occurred. No damage to equipment or ventilation controls occurred. No citations were issued.

Elevator was returned to service 0615 relay/coil was replaced, miners were not on the elevator during this time.

Methane ignition occurred in the #5 face of the North Mains section. Ignition occurred under the rippers when cutting bottom & self-extinguished. No injuries, no damage to equipment & was reported as required. MSHA was onsite investigated & determined compliance with no citations issued.

The DC bridge control failed which controls voltage for the brake release. This unit was replaced. Four miners were on the elevator until 5:55 AM.

During an examination of the ventilation system a roof fall was discovered in the North Mains left return number two entry crosscut thirty two. The roof fell several feet above the anchorage. MSHA was notified.

Miner was riding mantrip and the mantrip backed into a timber, it fell and struck the miner's lower leg.

Miner was loosening mounting bolts on a fan tower of a 3.5 loader on the supply yard when the socket slipped and miner felt a sharp pain in his shoulder.

Employee was sitting on a pipe that was suspended with chain falls. He was cleaning some of his tools. The pipe shifted when someone else hit it and smashed his foot between the pipe and the floor.

EE had been pulling on a chain fall for several days and on this day, he started to complain about his arm hurting and going numb. We found out later that he had a pre-existing arm injury we were unaware of.

Drill steel hung in hole, He pulled on steel with one hand, had other hand on jaws. Co-Worker accidently hit controls and pinched employee's hand and fingers.
