Accident type, without injuries

The elevator was traveling down the shaft with four employees. As the elevator approched the bottom of the shaft, the elevator traveled pasted the landing by a few inches. This allowed the elevator doors not to open, the elevator was re-set and the elevator brought back to the surface, where the employees unloaded the elevator at approximately 2:08 pm.

The service elevator traveled past the surface landing approximately 5 feet. There were 3 employees on the elevator at this time. They were removed from the elevator with no injuries.

The elevator would not stop even with the landing. After examination it was found that a contactor was bad, this contactor was replaced at 1:33 pm.

Wheel on supply car broke causing car tto de-rail resulting in damage to 2 guide rollers. No one was injured and no other damages occurred. Was reported to MSHA 800 number as required.

While mining, loose roof rock broke a hose off of the left side spray bar and an ignition occurred from a methane bleeder. Mining was in the # 1 face of the G9 section.

Employee was lifting a sprocket with come-a-long at the de-water, when the come-a-long fell, hitting the employee on his left hand against another chain.

Employee was stepping down from tractor on his right leg. Employee felt pain in lower back. Employee did not start missing time from work until 01/04/2010.

Employee was measuring a timber when the rib rolled stricking him from behind and knocking him down.

Employee was coming off the plant elevator and hung left foot on elevator door causing him to fall on right arm and elbow.

Employee was washing down floor at the pre-plant when the water hose slipped from his hand and a metal fitting hit employee in the nose and right cheek. Broken Nose

The operator was boarding his dozer at the beginning of the shift, his foot slipped off the cat track causing him to fall to the ground, fracturing his left arm. Working conditions were dry and it was daylight.

Employee was leaning over to install rib bolt when the employee felt sharp pain in the middle of back.

Employee was taking belt frame down the slope and slipped and fell on his left hip area and also landed on his SCSR.

Employee was driving the #47 tractor, when he hit a bump in the roadway, hurting his neck.

Employee was opening air valve, when the air hose jumped and mashed left hand on pipe. Laceration to palm of hand that required sutures.

Employee was helping put a rub rail on the miner, when the employee felt pain in right elbow.

Employee was getting into #17443 shuttle car when employee struck his left forearm on a piece of metal causing laceration to forearm.

miner was watching high voltage cable pull out reached to flip a loop of cable and it pinched his finger.

miner was removing belt structure and dropped a roller catching his right hand middle finger between the roller and the belt rail.

Employee was walking down the long wall face when he slipped on a rock and fell on right arm. Employee did not start missing time from work until 11/17/2009 because of surgery on his right elbow as a result of the accident that occured on 08/04/2009.

Miner was hanging high voltage cable on a ladder with two co-workers holding the ladder. The cable fell, knocking the miner off the ladder.

miner was operating a ram car on a grade. the miner did not put the ram car into first gear and it gained speed. the miner rubbed the rib with the ram car to slow it down and was jostled around.

Miner was walking down face line and walked into shield #133.

The miner was scaling roof rock with a slate bar. A rock struck the rock bar and it fell catching his finger between the rock bar and roof bolting machine.

Miner was lifting the high voltage cable cat head and felt something pull in his left shoulder. Miner began missing work on 1/20/2009.

Miner was knocking the corner of a brattice out when a rock began to fall. The miner turned and ran, tripped and fell.

Employee was walking toward the headgate when a rock fell out between the shields and hit employee on the right forearm. Laceration to right forearm that required sutures.
