Accident type, without injuries

On December 21st 2015 the service hoist located at Camp Creek Portal went down with 12 people on the hoist due to the tail ropes coming in contact with the wooden beams at the bottom of the shaft.

At approximately 4:30pm the service hoist went down due to DC fault trip. At approximately 6:00 pm it was re-set and the service hoist was put back in operation.

On Friday October 23, 2015 at approximately 6:00pm a roof fall was found in the North Main #4 entry intersection at crosscut 32. The roof fall is approximately 25 feet long by 25 feet wide and approximately 10 to 15 feet high. The roof fall is not located in the weekly examination route entry.

During an inspection by MSHA inspector at the escape hoist located at crosscut 89 on west main it was found that the AC output card was bad.

The small two man hoist located at Camp Creek Portal went down due to E-stop switch.

The small two man hoist stopped approximately 750 feet from the surface due to the control cable coming in contact with the guide roller on the hoist.

The guide rope at the escape hoist located at cross cut 89 on the west mains was found damaged. The guide rope was replaced and the escape was put back in service on 01/20/2015 at 1:15 pm.

Employee was walking to the long wall face on the head gate side when a rock fell between the roof bolts hitting the employee on the middle right side of back.

Over-exertion in lifting objects

Employee was working on the center section of 5708 loader when ee went to pick up a jack and strained lower back. Employee did not start missing time from this injury until 05/19/2016

Fall to the walkway or working surface

Employee was standing on the spill tray of the longwall hooking a chain to the spray boom in front of the #9 shield when the face in front of shield #10 rolled out causing the employee to jump backward landing on his lower back.

Struck by falling object

Employee was building a cut through brattice wall when a block he had just put on the top roll fell mashing the tip of his left middle finger between the block and the top of the brattice door.

Employee was walking with tools when he stepped off a 6x6 timber at the dewater location and twisted left knee.

Employee was hanging curtain in the #2 entry when he fell out of the bucket landing on his back.

Employee was lifting a 5 gallon bucket of degreaser out of the back of a pickup truck and felt a pain in the abdomen area. Employee did not start missing time from this injury until 10/15/2015.

Employee hit elbow while removing belt structure from tray. Employee did not start missing time from this injury until 10/07/2015.

Employee was walking the N-O bleeder when he slipped and pulled his right knee. Employee did not start missing time from this injury until 10-30-2015.

Caught in, under or between a moving and a stationary object

Employee was checking for bad rollers on the belt line when he jammed his little right finger into the belt causing dislocation to finger. Employee did not start missing time until 09/14/2015.

Reportable only due to sutures. Employee was helping set water dukes at shield #10 on I-4 longwall when a piece of scale rock fell hitting employee right forearm causing laceration to forearm.

Employee was picking up some roof bolt plates and felt a pain in right shoulder. Employee did not start missing time from this injury until 08/18/2015.

Employee was changing tailgate drum bits on the shearer when a scale rock fell causing employee to move backward and twist left ankle.

Employee was removing belt structure on long wall belt line when a belt rail fell on top of right foot.

Employee was installing belt hangers on the slope when his left hand hit belt rail cutting left index finger. Employee did not start missing time until 06/02/2015.

Employee was loading mining bits in the back of a EDV ride when he felt a pain in lower back.

Employee was walking to get tools off a man trip when he stepped on loose coal and twisted right knee. Employee did not start missing time from this injury until 02/16/2015.

Employee was unloading roof bolts from a loader bucket when he strained his right shoulder. Employee did not start missing time from this injury until 04/03/2015.

Employee was come-a-longing a rock off the hand rail on I-3 longwall panel when the chain slipped causing the rock to fall hitting employee on the right leg between the ankle and knee.

Employee was installing a roof bolt when it got stuck. The employee was spinning roof bolt trying to remove the roof bolt when it dropped out of the roof causing the roof bolt plate to hit the employee in the face area.

Employee was going through a man door when he slipped and fell hurting his right shoulder, elbow and knee.

Reportable only due to sutures. Employee was helping remove a cross under from the North main #1 belt when the chain holding the cross under can loose hitting employee on the side of the right eye.

Reportable only due to sutures. Employee was operating a long wall shear when a rock was thrown by the shear and struck the employee left hand.

Reportable only due to sutures. Employee was cleaning beside the belt when a small rock came off the belt and struck him on the right side of face.
