Accident type, without injuries

On December 30th 2014 at 3:15 am the small elevator at the shoal creek portal went down with no one onboard due to a bad deceleration switch. On 12/30/2014 at 8:15 am the deceleration switch was replaced and the elevator was put back in service.

The small two man elevator went down due to break solenoid.

The Camp Creek hoist faulted out showing E126 circuit breaker open fault. The circuit break was reset and the elevator was put back in service at 3:05 pm.

The small two man elevator went down on coupling fault. The elevator was repaired and put back in service at 9:00am on 09/05/2014.

The elevator at the Shoal Creek portal stopped just below the surface landing as it was being sent to the bottom. The elevator was not getting a run command from the drive. No one was on the elevator at the time and the elevator was repaired and 2:20 pm.

The D.C. circuit breaker to the service hoist drive motor knocked and would not reset. The hoist was examined and the service hoist was put back in service at 8:30am on 04/15/2014.

While burning bit lugs on the head of a continuous miner a gas ignition occurred on the right rib beside the miner head. The miner was located in the #4 entry on west main section approximately 30 feet out by spad 96+4.

At approximately 2:35 pm the service hoist went down due to the north gate switch at the bottom of the shaft. Switch was repaired at approximately 3:30 pm and the service hoist was put back in service.

Employee was taking down a spool of cable and mashed left pinkie finger. Did not start missing time from the injury until 12/11/2014.

Employee was installing a feed box in the prep-plant and as the employee was sliding the box in place his right hand got caught between the structure and the feed box.

Over-exertion in pulling or pushing objects

Employee was using a rock bar while attempting to remove a rock from the tray on the long wall when he felt something pull in his felt groin area.

Employee was moving bottom belt rollers when he felt a pain in middle of back.

Employee was getting rock dust bags when he felt a pop in lower back.

Struck by falling object

Employee was removing a low trac from the north main belt line at crosscut 53 to make repairs to the canopy post leg. When he got it through the double doors the canopy post came off and hit employee on the right shoulder. Employee did not start missing time from this injury until 1/08/2015.

Employee was working out of the bucket of a loader and as he went to turn employee felt his right knee pop.

Employee was pulling tailgate shields when a rock fell hitting the shear and then hit employee in the legs.

Employee was moving a pump when his foot got caught on the bucket of a skid steer causing him to twist his right knee.

Employee was relocating the end of the come-a-long when the ratcheting end of the come-a-long fell hitting the top of his left hand.

Employee was busting some rock up on the longwall face area when a piece of rock came up and hit the end of his left index finger.

Employee was helping load a 55 gal drum onto a EDV ride when his left fingers got caught between the ride and the drum.

Employee was helping install a boom on a miner when the chain broke and the chain hit the employee on the right foot.

Employee was checking a pump when the discharge line blew off hitting employee in the right knee. Employee did not start missing time from the injury until 04/07/2014.

Employee stated as he was walking at crosscut 38 on west main belt his left knee gave out and he fell hurting his left shoulder and lower back.

Employee was climbing a ladder when ladder slipped and employee fell scraping left shin and twisted right ankle.

Employee was welding on long wall tailgate drum when a rib rolled hitting employee left hand.

Walking around the bottom area of the service shaft and stepped on a rock and twisted right knee.

Employee stated that as he was getting out of a 3.5 loader he felt the loader move and as he went to jump back in the loader he hit the steering lever and his left ankle got caught between the loader.

Employee was working inside a longwall shield and pulled a muscle in upper back.

Employee was removing hydraulic hose at #45 shield and as another employee removed the staple form the hose the hose hit the employee in the leg.

Employee was carrying a crib block to work on a relay bar and tripped over a rock and felt strain in lower back. Employee did not start missing time from the injury until 11/25/2014.

Employee was helping remove a drip pan from a belt discharge when part of the drip pan hit the employee in the back of the head.

Employee was prying on a bottom belt roller to remove it when he felt a pain in his upper back.

Reportable only: Two employees were located at the section feeder. There was an altercation between the two employees and employee was hit on the right side of the head with a shovel by the other employee. Employee was hospitalized for observation.

Employee was checking the PLC on west 1 belt and as he turned around he slipped and fell landing on his SCSR. Employee did not go to the doctor until 09/30/2014.

Reportable due to sutures. Employee was removing wood pallets and metal bands when a metal band cut his right forearm.

Reportable only. Employee was removing a screen from a 13 hp pump with a hammer. When the employee hit the screen with the hammer a piece of the metal struck employee in the upper right thigh.

Reportable due to sutures. Employee was installing on bottom belt roller when the roller slipped out of his hand and the employee cut his left index finger on a metal burr on the roller.
