Accident type, without injuries

The main hoist at Camp Creek Portal went down due to a problem with the hydraulic brake accumulator. The hydraulic brake accumulator was repaired and the hoist was put back in service at 2:54 pm on 07/26/2017.

Lose power on the camp creek portal side of Shoal Creek mine. The power lose effected the #7, #8, bath house and camp creek hoist. The hoist was in operation at the time with one person on the hoist. Power was restored and the person was removed from the hoist with no injuries at 8:19 am.

At 7:05 pm on July 10, 2017 a gas ignition occurred on the H-3 section in the #3 entry while the continuous miner was mining.

The guide rope for the escape hoist located at cross cut 89 on west main was found damaged during a weekly inspection of the hoist. The damaged guide rope was repaired and the hoist was put back in service at 11:40 am on 07/07/2017.

The service hoist at the Camp Creek portal went down at 4:20 pm due to a power outage caused by an electrical storm in the area. After examination of the hoist it was determined that the hoist would not operate due to a bad rope slip switch. The rope slip switch was replaced and the hoist was put back in operation at 8:14 pm.

The service hoist at the Camp Creek Portal went down at 11:37 am due to a bad power supply for the surface I/O rack. The power supply was repaired and the hoist was put back in service at 12:16pm on 06/26/2017.

At approximately 9:25 am a small fire around 1 foot in diameter was discovered while some repair work on the long wall stage loader was being conducted. The fire was located at the top of the Mary Lee coal seam between the coal seam and the main top.

At 10:15 am a roof fall was discovered in the intersection at cross cut 53 on West main. The roof fall is approximately 10 feet high and above the anchorage point. This is a non-traveled intake and no miner, equipment or ventilation controls was affected.

A methane gas ignition occurred on the H-2 section in the #3 entry approximately 20 feet back from the face.

A roof fall occurred on the West main far left roadway located at cross cut #62. The roof fall had fell in the intersection of cross cut #62 approximately 10 feet high.

Employee was cleaning muck and coal fines at the west main belt discharge area when ee bumped head on a pipe causing ee to lose balance hitting right forearm on belt structure.

Was going down steps when employee felt pop in right knee. Employee did not start missing time from this injury until January 15, 2018.

Employee was walking down the slope belt and pulled employees groin area when employee slipped and fell.

Struck by rolling or sliding object

Employee was working on slope belt when some wet material come off the belt knocking employee down. Employee did not start missing time from this injury until 12/4/2017.

Employee was getting out of a man trip when employee twisted employee's left ankle. Employee did not start missing time from this injury until 11/29/17.

Employee was lifting a water (trash) pump when EE felt a strain in lower back.

Caught in, under or between a moving and a stationary object

While moving a piece of ultra track on the long wall hand rail employee's right middle finger got mashed between the ultra track and hand rail. Did not start missing time from this injury until 10/13/2017.

Over-exertion in lifting objects

Employee was lifting a box to place on the bed of the supply truck and felt something pull in EE's lower back.

Employee was walking the north main belt entry when employee's feet slipped causing the employee to fall landing on left shoulder. Employee did not start missing time from the injury until 11/06/2017.

Over-exertion in lifting objects

Employee was helping set a pump beside the long wall crusher when ee felt a strain in ee middle and lower back. Employee did not start missing time from this injury until 9-29-2017.

Over-exertion in pulling or pushing objects

Employee was helping make a belt slice and as the employee was pulling on the belt to place the template felt pain in right shoulder. Employee did not start missing time from this injury until 09/19/2017.

Caught in, under or between running or meshing objects

Employee was working on the slope belt approximately 700 feet from the surface when EE got left arm caught between the top belt and the top side and middle rollers.

Employee was walking down the long wall face when employee slipped and strained employee's lower back.

Employee was trying to put bolt in relay bar when EE mashed right index finger.

Employee was checking under the hood of the EDV company number 2783 with the engine turned off when the radiator hose busted spraying the employee on right side of body.

Employee was climbing on set ladder when ee rolled left ankle.

Employee was using a wash down hose at cross cut 37 on West Main belt when employee tripped on hose and fell on left shoulder, neck and back area.

Employee was walking down the slope belt and slipped and felt pain in left knee.

Employee was walking down the slope belt with a 48""x32"" piece of guarding for a cross under when employee felt a pop in employee's right knee.

Employee was removing a 3"" dust hose off the back of the rock duster when the hose came around and hit the employee in the right upper arm area.

Employee was walking down the slope belt when a small rock came off the belt hitting the employee in the left ankle.

Fall onto or against objects

Employee had done work on the 216 Cat skid steer and had started letting the hinged canopy down when the employee feet slipped causing the front of the hinged canopy to hit the employee in the top of the head. The employee fell backward landing on employee's back between the bucket area and the frame of the skid steer.

Employee was in the cab of the scraper when employee reached around with the left hand to shut the cab door with the right hand resting on the door frame post, and closed the cab door on the right thumb.

Over-exertion in lifting objects

Employee was lifting a bottle of oxygen and felt something pulled in right shoulder. Employee did not start missing time from this injury until May 4, 2017.

Employee was standing at approximately shield 160 on the longwall face when the roof in the gob area behind the shields fell (approximately 150'). The force of the wind blew the employee down causing ee hit the back of ee head.

Employee was carrying a bottom belt roller when felt pain in lower back.

Employee was walking to the long wall face when stepped on a rock and twisted right ankle. Did not start missing time from the injury until 04/10/2017.

Employee was getting off of a 657B Cat scraper when EE slipped and fell landing on the left knee.

Employee was making an examination of the west main 4 belt when a coal rib rolled out causing the employee to jump back and slip and fall landing on right hand and wrist.

Employee was de-trashing at the H-2 belt discharge area when they strained their lower back.

Employee was working on the long wall when the roof behind the shields fell in the gob area causing the employee to jump back and hurting right foot when employee landed.

Employee was on the 2nd step of a ladder to disconnect a rope harness from the hoist that was used to move a motor. The employee foot slipped on the ladder and the employee fell landing on the left elbow.

Struck by powered moving object

As an employee was getting out of a parked piece of equipment it started rolling backwards down a small incline hitting an employee on left side knocking employee down before the other employee could stop the piece of equipment.

Reportable only due to sutures. Employee was on the long wall face between shields 138 and 139 when a rock fell between the shields hitting left hand causing laceration to hand.

Reportable only due to sutures. Employee was cutting some guarding when the knife slipped cutting employee on the left forearm.

Employee was closing a water gate valve when their nose came in contact with the wheel on the gate valve.
