A roof fall occurred in the #3 entry at crosscut 30 on J-0 section. The fall measured approximately 30 feet long by 30 feet wide and approximately 5 to 7 feet in height.
The service hoist cage was located on the surface landing.The cage was sent down in freight mode empty and the counter weight struck the wood in the bottom of the shaft.
The service hoist went down due to the south gate switch. The south gate switch was replaced and the service hoist was put back in service at approximately 5:45pm.
A roof fall was reported outby the approved seal area at the old G-4 section in the number 3 entry at the approved #3 seal site area. The roof fall was approximately 90 feet long by 20 feet wide and fell above roof bolt support.
A roof fall occurred in the bleeder entry of the I-O longwall panel. The fall occurred in a 3 way intersection that is located in the #1 entry at crosscut 35 and the fall measured approximately 20' long by 30' wide by 4' in height.
A lightning strike knocked out the power on the Camp Creek portal side. The service hoist would operate and the #7 and #8 fans went to diesel backup. Power was restored at 6:40am with no damage to equipment.
During the daily examination of the hoist at Camp Creek a bolt was found broke off on the brake caliper on the drum. A new bolt was installed and the hoist was put back in service at 1:09pm.
At approximately 9:50pm the service hoist went down because of drum break fault. The break caliper was adjusted and the hoist was put back in service at 11:22pm.
At 11:00 pm on January 05, 2012 the service hoist was removed from service on a planed maintenance repair work. At 11:00 pm work started on the replacement of the service hoist motor.
At approximately 10:15am an employee received fatal crushing injuries as he was helping move a Longwall power center. The fatal injuries occurred when the employee was crushed between the power center and rib as the power center was being moved into a cross cut.
Electrician was performing work on a shuttle car cable reel on the West Main Left section when his received a fatal electrical shock. The electrician came in contact with the exposed 995 volt energized cable leads in the cable reel compartment.
Employee was walking on the slope belt when he slipped and fell landing on his tool bag.
