Accident type, without injuries

At approximately 2:50 pm the service elevator was removed from service due to arcing in one of the motor brushes. All motor brushes was replaced and the elevator was put back in service.

At approximately 1:00am the service hoist went down on fault because of the drum breaks. The drum breaks were adjusted and the service hoist was put back in service at approximately 6:00am on 12/21/2011.

While testing the escape hoist at crosscut 66 the hoist would go to the bottom landing but when trying to bring hoist back to the top landing the control fuses would blow. After investigating the problem it was found that the mini processor was bad.

At approximately 11:15pm the elevator stopped at mid shaft. After checking the control room of the elevator the elevator showed fault. The fault was due to the magnet on the north gate of the elevator. The elevator was put back in service at approximately 1:40am.

When testing the over travel switches above the surface landing of the Camp Creek portal elevator/hoist. The elevator/hoist would not re-set to allow the elevator/hoist to travel back to the surface landing.

At approximately 7:30am the hoist elevator malfunctioned that caused the elevator to travel approximately 12 to 15 feet above the surface landing with 6 miners on the elevator.

On the evening of August 10th the Tracking system stopped communicating to the surface from underground. At Approximately the same time the mine had failure the #2 fan. The repair work to the tracking system started on the evening of August 11th after the #2 fan was restored.

At approximately 10:35pm the elevator/hoist malfunction with the DC drive fault causing the elevator to travel approximately 10 feet above the surface landing with 12 miners on the elevator.

The service hoist at Camp Creek Portal went down at approximately 10:55pm because the EC-126 DC circuit breaker open when the elevator was at the bottom of the shaft.

The service hoist at Camp Creek Portal went down because of the sequence switch on the gate at the bottom of the elevator shaft.

The service elevator went down because the relay contactor would not allow the breaks to release on the elevator. Changed out a input card and main contactor and the elevator was put back in service at 8:20 am.

At 10:07 am the service elevator went down due to the elevator door switch. The elevator was put back in service at approximately 11:00 am.

A roof fall occurred on I-0 Section at approximately 3:15 am in the return between cross cut 32 and 33. The roof fall measured approximately 50'Long by 20'Wide by 6 to 8 feet high.

A methane Ignition occured in the number 1 entry on the west main left section as the roof bolters was installating roof bolts. As the roof bolter went to tighten the roof bolt he saw a flame from around the roof bolt plate.

Heard a noise coming from the service elevator. After investigation it was found that a bearing to the governor rope was bad. Elevator was taken out of service at 12:55pm and put back in service at 3:27pm.

At 2:18pm the service elevator at the bottom landing went down as it was being attempted to be called to the top landing. The service elevator breaks would not release to allow the elevator to come to the top landing. Replaced two relays and the elevator was put back in service at 6:45pm.

Employee twisted his left ankle when he was unloading brattice blocks and his left foot got tangled up in a rope.

Employee was picking up a top belt frame when he felt a pop in right shoulder.

Employee was walking beside the rib line checking the miner cable and as he raised up he jammed his head into the ventilation tubing that was hanging above him. Did not start missing time from this injury until 01/25/2012.

Employee was helping turn a top belt roller when he felt a pop in his lower abdomen area. Employee did not start missing time until 11/07/2011.

Employee was operating a 3.5 loader when a small rock fell from the roof and hit his left hand.

Employee was riding in back of a Ram car (fuel car) when the ram car bumped the rib causing a piece of middle man to fall striking employee on right foot.

Employee was working out of the bucket of a 3.5 loader when he slipped and hit his right knee on bucket. Employee did not start missing time from this injury until 10/21/2011.

Fall from machine

Employee was standing on the forks of a loader hanging belt chain in the mine roof for a up coming belt move when he fell to the mine floor.

Absorption of radiations, caustics, toxic and noxious substances

Pouring concrete. Concrete burns to both legs.

Struck by falling object

The employee was traveling down a hill in a shuttle car on West Main Right section when he hit the rib with the shuttle. A rock measuring approximately 20"" by 16"" by 51"" long came in the cab and hit the employee.

Employee was recovering ventilation tubing and the chain that was used to hold the tubing up broke causing the tubing to fall hitting employee on the left shoulder. Employee did not start missing time from this injury until 06/14/2011.

Employee was preparing to disconnect wiring in box, moved ground wire, arc flash burned hands and knocked backwards.

Fall to the walkway or working surface

Employee was using a pry bar to line up a differential joint on a piece of equipment, when the pry bar slipped, causing the employee to fall to the ground landing on left wrist. Employee did not start missing time until 3/22/11.

Employee was exiting a dozer when his right foot slipped causing him to twist his right knee. Employee did not start missing time from work until 03/24/2011.

Employee was running a miner when a rock fell from the roof hitting the employee on the head, shoulder and arm areas. Employee did not start missing time from this accident until 03/23/2011.

Reportable only due to fracture to index finger. Employee was helping with a belt splice when his right index finger was hit with a 4 pound sledge hammer.

Employee picked up a tow bar for the longwall shear and mashed his little right finger between the bar and the a cover. Reportable only due to fracture to tip of little finger.

Reportable only due to sutures. Employee was helping guide a tank car in a cross cut when a rock from the rib fell causing him to hit his head on a 25-X loader.

Reportable only due to sutures. As the employee was using a grinder to cut a bolt out on the drive unit his hand slipped and the employee cut the back of his right hand on a sharp piece of metal.

EE was installing the balance ropes underground, there was a lot of airflow coming in which was blowing debris all around, EE received debris in eye.

Employee was roof bolting in the face area when a small piece of rock fell from the roof hitting employee in the left forearm. Reportable only due to stitches to left forearm.

Employee was chipping concrete off bolts with pick hammer, arm hit post, hammer glanced off toe plate and stuck employee's mouth, beaking front tooth.

Reportable only due to sutures. Employee was using a pry bar to work on the cat track on the roof bolter. The pry bar slipped, pinched his right hand between the bar and hose manifold.
