Accident type, without injuries

During welding, cutting & burning on the shear bit lugs, methane was ignited and immediate action put out the ignition. There were no injuries reported. Proper practices being followed.

Per contractor heat expansion caused the elevator's counterweight to come out of it's guide causing damage to approximately fifty feet of counterweight guide rail, support stubs and counterweight side brackets.

Preliminary information indicates that an ignition occurred in the rasie bore shaft causing injuries and damage to ventilation controls. Note-This accident was phoned to MSHA as required by Part 50 notification.

Anunintentional roof fall was found in the return entry of the G6 section between x-cut 15 & 16. the fall approx 12' wide x 60' long was just above the 4' roof bolt anchorage.

While lowering a roof bolter in the supply slope, it became fouled along roof and rib. Hoist operator tried to pull trip back out but chain broke and allowed bolter to slide off of material car. Supply hoist was out of service for more than 30 minutes.

Employee was working on the miner under a cover about mid way of the top of the miner. He needed access under an adjacent cover & it would not open. He stood on top of the miner to pry open the adjacent lid when he slid off the miner. In an attempt to catch himself he grabbed the cover that was open & inadvertently pulled the lid closed amputating the end of his right thumb.

miner was pulling a piece of roof rock with a pry bar and his shoulder popped. Note: miner did not begin missing work until 1/3/2008.

Fall from ladders

miner was at bottom of over cast ladder and missed step and fell off ladder.

miner was walking from elevator to electric shop, tripped over rock in roadway and fell. Note: Did not begin missing work until 12/31/07.

Miner was operating loader and while tramming he ran over crib tie, he came out of seat and struck canopy.

Miner was taking a tie-rod end off of tractor. Heating the tie-rod end caused the ball to come out of the socket under pressure striking the left index finger and right lower leg.

A rock came off the slope belt and struck the miner in the side.

Throwing a ventilation tubing regulator into a loader and the left ring finger was caught between the end of the tubing regulator and the side of the loader bucket.

While pulling belly pan from under 3 1/2 loader injured's foot slipped straining / spraining right arm. Note: Did not begin missing work until 7/20/07.

Employee was crossing the front bumper of the Wagner ram car duster and his left middle finger came in contact with the fan blade lacerating the end of his finger.

Picked up crib tie and felt pain in abdomen. Note: This employee began missing work on 6/5/07 under state law this was medically investigated and determined to on 6/12/07 to be a lost time injury.

Pulling on hoses while working on longwall setup, he strained his lower back.

Struck by flying object

Alleged injury occurred when he was striking a pick hammer in order to drive a staple lock that was frozen out of a hydraulic hose. A fleck of metal came off either the pick hammer or hammer being used to strike the pick hammer and entered the left hand. Note: Taken to Dr. same day, fleck removed 4/16/07, no work missed.

While unloading high voltage cable from a skid on a 25x the alledged injury occurred when the 25x slide into a roadway pot hole jerking the high voltage cable.

EE was lifting solenoid blocks to load on cart and deliver to work area on longwall face. While doing so, he felt pain in back between shoulder blades.

Employee was closing sliding door on #3 fan house when hand slipped and he jammed his left little finger. Began missing work 3/5/07.

EE slipped on wet ground and fell. He landed on his lunch bucket. Began missing work on 2/13/07.

EE was stepping through mandoor. He stepped on material on bottom and twisted his right knee. Began missing work on 2/13/07.

Employee was using come-a-longs when chain broke free releasing tension. Re coil of come-a-long struck employee on left wrist. He reacted to get out of way. He tripped, following forward on hands and knees.

Employee was stepping across cables. Foot slipped and he fell striking lower back on rock lying on bottom.

Employee strained himself lifting lid on toolbox. A hernia developed & required medical care. EE began missing work 2/5/07.

Employee was getting out of mantrip, caught foot and fell. Caught himself with right hand.

Employee was carrying 2 buckets of brattice sealant, he slipped and twisted his hip, knee and ankle. NOTE: This employee did not begin missing work until 4/24/07.

Miner was removing a cover from a love-joy coupling when the cover came loose. The cover struck and lacerated his right elbow.

Miner was moving a rock the adjacent rock slipped pinching the right middle finger.

Miner was cutting tin panel; struck it with axe. Corner flew up lacerating right hand required sutures.

While installing monorail was hanging a spacer, the spacer fell striking the employee in the face. Resulted in sutures.

A piece of rock fell off the miner while place changing. The rock broke apart upon impact on the mine floor. One of the pieces that broke off from impact on the mine floor struck the employee on the right side of the face. He received sutures.

Employee was driving the fuel car when he struck a hole in the roadway. His hand was resting on the fire extinguisher and it was thrown up towards the canopy lacerating a finger on his left hand. This resulted in sutures.
