Accident type, without injuries

An unintentional roof fall occurred in the #2 roadway entry in the G6 section at x-cut 20. The fall had approx. measurements of 35'long X 25'wide and 20'above the original mining height.

An unintentional roof fall occurred in the G-6 section #2 entry just inby x-cut 17. The fall was approx. 45' in length beginning at the intersection edge and continuing inby and tapered from 0 to 16' wide. Height at the peak is estimated to be 8-9 ft. A fault ran diagonally across the entry with the edge of the fall being at the slickenside of the fault.

An ignition occurred on the N-1 longwall when the top fell behind the headgate shields.

A methane ignition occurred as the shearer was cutting toward the headgate. The flame lasted 2-3 seconds. Was extinguished by the bit sprays.

A pocket of CH4 ignited as the shearer cut toward headgate. The flame lasted 1-2 seconds and self-extinguished.

Employee was climbing into cab of pick-up truck at the prep. plant when his foot slipped out from under him & he fell backward landing on his back & hitting his head.

Employee stepped on pipe which gave way and caused him to lose balance. He caught himself against rib with right arm. When he did he felt pain in shoulder.

Employee was lifting a hydraulic ram and strained back. He was working on dozer at prep plant.

Employee was climbing into a mantrip and hit his head on a roof bar support.

Employee was driving a Kubota tractor when he ran over a rock in roadway and strained his neck. Employee began missing work 3/30/07.

EE was performing pre-op on a Lo-Trac. He was standing between equipment and rib, when a piece of middle man rock fell and struck EE on back of head and neck.

Employee was assisting with belt move. Tailpiece shifted and fell off crib blocks causing employee to react quickly and fall as he was getting out of the way.
