An unintentional roof fall occurred in the #2 roadway entry in the G6 section at x-cut 20. The fall had approx. measurements of 35'long X 25'wide and 20'above the original mining height.

An unintentional roof fall occurred in the G-6 section #2 entry just inby x-cut 17. The fall was approx. 45' in length beginning at the intersection edge and continuing inby and tapered from 0 to 16' wide. Height at the peak is estimated to be 8-9 ft. A fault ran diagonally across the entry with the edge of the fall being at the slickenside of the fault.

An ignition occurred on the N-1 longwall when the top fell behind the headgate shields.

A methane ignition occurred as the shearer was cutting toward the headgate. The flame lasted 2-3 seconds. Was extinguished by the bit sprays.

A pocket of CH4 ignited as the shearer cut toward headgate. The flame lasted 1-2 seconds and self-extinguished.

EE was walking along side the 374 belt to reset the alignment switch, when he slipped and fell.

EE was walking across coal yard at surface prep plant, when he caught his foot and tripped and fell.

Employee was going through mandoor on g5 calline, slipped and twisted ankle. began missing work 12/15/06.

Employee was rebuilding ventilation brattice in b-1 area when a piece of rib rolled off and struck him on the foot and ankle.

Employee was cleaning pump, x-cut 7 of SME. His foot got stuck in the mud, he twisted his knee when trying to get free.

Employee was walking down belt slope and slipped on ice. He fell landing on back.

Employee was climbing into cab of pick-up truck at the prep. plant when his foot slipped out from under him & he fell backward landing on his back & hitting his head.

Employee stepped on pipe which gave way and caused him to lose balance. He caught himself against rib with right arm. When he did he felt pain in shoulder.

Employee was lifting a hydraulic ram and strained back. He was working on dozer at prep plant.

Employee was climbing into a mantrip and hit his head on a roof bar support.

Employee slipped on wet bottom in g7/nm area and strained lower back. Began missing work 1/12/07.

Employee slipped in mud, caught himself with right arm and felt pain in shoulder. Began missing work 3/6/07 for surgery.

employee was walking up stairs and twisted knee (note: employee did not begin missing work until 5/8/2007)

Employee was recovering hoses off of the longwall mono-rail & loading them into a lo-trac bucket. He felt his shoulder pop.

Employee slipped when entering mantrip and felt knee pop. He began missing work 11/27/06.

Employee was positioning crib blocks. As he bent over to lay block down, he felt pain in his lower back.

EE was using (ratcheting) com-a-long and felt pain in shoulder.

Employee was working next to rib. Piece of middle man rock rolled out and struck him on back of head and shoulder.

Employee was removing radiator from skip steer loader. Radiator fell on left hand.

Employee was hanging pump cable, slipped and fell on scoop bucket.

Employee was pulling drill steel apart and felt pain in shoulder. Began missing work 11-6-06.

Employee was loading material in loader bucket and caught hand between material and bucket.

Employee was driving a Kubota tractor when he ran over a rock in roadway and strained his neck. Employee began missing work 3/30/07.

EE bent over to cross under conveyor belt. Had difficulty standing back up when he reached the other side.

Employee tripped stepping across cable. Caught and twisted front of foot and toes.

EE was assisting in moving portable compressor and strained back. Began missing work 11/01/06.

Employee was operating ram car hauling sand. He stated he thought car jumped out of gear. Ram car steared and ran into rib before stopping.

Employee was operating 4 loader when wheel ran over ledge causing operator to bounce and strike head on canopy.

Employee was washing equipment when he lost control of the water hose. Hose struck the side of his head and cut hand.

Employee was cutting rope when knife slipped and lacerated opposite hand.

Employee was installing top belt rollers when roller slipped and smashed left ring finger.

Employee was operating roof drill installing roof bolts. while hauling roof bolt, piece of cap rock fell. As worker was moving out of way He bumped control lever which caused head to move and pinch finger between bit wrench and roof bolter.

Slipped and fell striking knee on mine floor.

While roof bolting, Employee was installing bolt, Bolt missed hole, bent and caught finger on steel guide.

EE was performing pre-op on a Lo-Trac. He was standing between equipment and rib, when a piece of middle man rock fell and struck EE on back of head and neck.

Employee was assisting with belt move. Tailpiece shifted and fell off crib blocks causing employee to react quickly and fall as he was getting out of the way.

EE was moving (breaking apart) CM conveyor, when chain came apart. It sprung back, hitting EE on the knee.

EE was stepping through mandoor, slipped and fell, injuring lower back and hip. Began missing work on 9/14/06.

Employee was operating a diesel ram car, backed over material laying in roadway and was thrown into canopy of operator's deck.

EE was attempting to hang bolter cable, when he felt discomfort in arms and shoulders. EE began missing work on 9/14/06.

EE was working in a loader bucket when he slipped & strained knee.

Employee stepped off bolter and strained knee.

Employee stepped off ladder onto a block and twisted knee. Started missing work 8/28/06.

Employee slipped getting off equipment and strained back and knee.

Employee slipped while walking down entry and fell on left leg.

Employee was pulling stuck drill steel out of a hole & felt pain in his shoulder. *EE began missing work 11/21/06*

Employee slipped and fell landing on his left hand and shoulder.

EE slipped and fell, landing on knees. EE started missing work-8/15/06.

Smashed end of right ring finger while loading 6x6 timbers.

EE was going through mandoor, slipped and strained his knee. Began missing work 7/14/06.

EE was carrying a bag of rock dust, when he hung his foot on a rock and strained his lower back and leg.

Injured was hanging miner cable out of an equipment bucket. Cable caused him to fall, landing on his SCSR (belt worn).

Employee was loading material and spreading rock dust and strained lower back.

Employee was moving pump and stepped in hole. Strained knee. Began missing work 5/8/06.

EE was working under a diesel mantrip and strained his back when crawling out from under it.

WALKING THROUGH MUD & PULLED SOMETHING IN HIS LOWER BACK.

A steel choker slipped while a load was being prepared to lift; the choker slid striking the employee's right hand.

EMPLOYEE WAS SERVICING A DIESEL UNIT WHEN HE SLIPPED & STRUCK RIGHT SIDE OF HIS BODY AGAINST THE MACHINE.

Employee was working on scaffolding when while moving material, he fell off end and struck right side.

Employee was laying concrete block and strained right shoulder. Began missing work 5/16/06

EE tripped over material. Fell and injured right ribs and right knee.

Rock fell from overhead belt, bouncing off the floor of backhoe the employee was operating (canopy is in place and in good working order). Rock bruised right ankle.

EE reported he jumped off miner and strained neck. EE started missing work on 3/4/06.

Employee stated he tripped & fell hitting his right shoulder. EE started missing work on 11/27/06.

EE was climbing from ground to equipment cab and pulled upper right stomach area. EE began missing work 6/6/06.

Employee stepped on uneven bottom and twisted right knee.

EE slipped and fell - spraining right ankle. EE began missing work 2/7/06.

EE tripped and fell against coal rib. He caught himself with one hand. A piece of coal/rock fell from rib and struck him in the side of torso. EE began missing work 01/24/2006.

MECHANIC WAS INSTALLING FEEDER DRIVE BELT WHEN WRENCH SLIPPED AND TWISTED THUMB. EE BEGAN MISSING WORK 3/13/06.

Employee was assisting crew in dragging submersible water pump. While doing so, he tripped and fell. He began missing work 1/16/06.

A 6 X 6 TIMBER WAS JARRED LOOSE, FELL & STRUCK EMPLOYEE ON HIS SHOULDER.

EMPLOYEE WAS REPAIRING A WATER LEAK WHEN A ADJACENT PRESSURIZED HOSE BLEW OFF PIPE TEE AND STRUCK HIM IN THE BACK.

Employee was collecting material to ship underground. While doing so he tripped and caught himself as he fell to the ground, jamming his right shoulder.

EMPLOYEE SLIPPED WHILE WALKING AND SPRAINED LEFT ANKLE.

Rock from longwall face rolled off & bounced into walkway striking employee on his foot. Medical, restricted duty, no lost time.

Employee was lifting engine cover on 3 1/2 loader. Cover slipped out of his hand and fell closed, falling on opposite hand.

EE was cutting a timber. Saw jumped out of cut and ran across back of his hand.

Employee slipped and fell striking right elbow.

Employee struck thumb on edge of disconnect box when wrench he was using slipped.

Employee was preparing to go through mandoor when small piece of cap rock fell and struck him on the nose.

Employee opened rock dust door in ventilation stopping to look through when door slipped out of hands and struck him in mouth. Laceration requiring stitches.

The employee stepped on a lump of coal causing him to trip over a pipe and fall to the ground. He tried to catch himself with his left hand, causing injury to the left wrist.

Employee was using sledge hammer installing tire on tractor. Handle caught on frame and finger wedged between frame and handle.

Employee was cutting flapper off conveyor belt when knife slipped and contacted finger on other hand.

EMPLOYEE WAS CLIMBING DOWN OFF BELTLINE WHEN HIS FOOT SLIPPED & HIS KNEE STRUCK THE SHARP EDGE OF FLOOR PLATE.

A ROCK FELL FROM BETWEEN SHIELDS AND STRUCK EMPLOYEE ON RIGHT WRIST.
