A methane ignition occurred during mining on the H-2 section in the #1 entry at approximately 12:30 pm.

At 5:10 pm on December 15th, 2016 a gas ignition occurred on the H-2 section in the #2 entry while the continuous miner was mining.

On November 29, 2016 a roof fall occurred on I-5 longwall panel at cross cut 24 tailgate entry adjacent the tailgate drive motor. The roof fall did not affect the longwall ventilation but did impede passage in this area. The longwall went under its egress procedures as required in the roof control plan.

The small two man elevator went down at 5:00pm due to the bottom gate losing connection with the gate switch.

The small two hoist went down at approximately 10:05pm on 09/04/2016 due to a bad brake pressure transducer. The brake pressure transducer was changed out and the elevator was put back in service at 8:15am on 09/05/2016.

The service hoist at Camp Creek Portal was taken out of service at 12:00pm in order to replace out a brake caliper. The service hoist was put back in service at 2:45pm on 05/16/2016.

The service hoist at Camp Creek Portal was taken out of service at 2:30 pm due to an over-travel fault. The elevator program was checked and re-programmed and tested several times. The elevator was put back in service at 5:03pm.

The elevator at the Shoal Creek portal went down at 7:15 pm due to a faulty contactor. The contactor was replaced and the elevator was put back in service at 8:25 pm.

Over-exertion in lifting objects

Employee was helping pull 72"" belt when employee felt pain in lower right abdomen area. Employee did not start missing time from this injury until 12/28/2016.

Over-exertion in lifting objects

Employee was loading 12""x12"" roof bolt plates for roof bolter on a 3.5 loader and felt pain in lower back.

Over-exertion in wielding or throwing objects

Employee was using a hammer to remove a pin from the rear end of 2783 man trip when ee felt a pain in right shoulder. Employee did not start missing time from the injury until 3/10/2017.

Employee was pulling shield #5 from shield #4 on the I-5 longwall when a fitting on a hose broke hitting employee in the left knee.

Over-exertion in lifting objects

Employee was picking up brattice block and heard a pop in right bicep.

Employee was bolting down a pinch clamp on H-1 belt when the hammer drill slipped pinching left thumb between the drill and clamp.

Fall onto or against objects

Employee was helping rock dust on west main 4 belt and slipped on a piece of Teflon and fell on SCSR left side.

Employee was setting up to change out a base lift jack on I-5 longwall and as the shield was being pushed out the bolts on the lift jack broke off hitting the employee in the groin area.

Employee was helping lift a base lift jack to shield when felt a pain in lower back. Employee did not start missing time from this injury until 09/21/2016.

Employee was working on a tractor at the electric shop when knee popped. Employee did not start missing time from the injury until 10/04/2016.

Struck against a moving object

Employee was backing a ram car into a cross cut when EE ran over a rock causing employee to hit head on canopy. Employee did not start missing time from this injury until 01/26/217.

Employee was checking SRLD around the bottom area of the service shaft when ee stepped in a hole and twisted right knee. Employee did not start missing time from this injury until 08/19/2016.

Employee was riding on a ram car (tank duster) when it ran over some material causing right hand to get caught between the swivel part of the ram car.

Contact with electrical current

Employee was checking the 120 volt UV input on the 4912 feeder when the meter lead slipped out of hand and as the employee grabbed for the meter lead little finger inadvertently touched an energized lead causing the employee to be pulled back landing on left shoulder.

Struck by concussion

Employee was working on I-5 longwall when the first roof fall in the gob area occurred behind the longwall shields. The force of air from the roof fall knocked the employee down landing on right side.

Struck against stationary object

Employee was walking the slope belt when ee stepped on a rock and twisted right knee. Employee did not start missing time from this injury until 04/04/2016.

Employee was stepping out of a tractor and twisted left ankle.

Struck by falling object

Employee was changing a differential in an EDV ride when the differential slipped and landed on the employee's little finger on right hand.

Over-exertion in lifting objects

Employee was picking up a 1/4"" chain when ee felt a sharp pain at middle back and right side area.

Struck against stationary object

Reportable due to sutures. Employee was attempting to break a bolt loose at the west 2 belt discharge when EE smashed left ring finger on the belt structure.

Reportable due to sutures. Employee was using a drill press to drill a hole in a metal plate when the drill bit caught the plate and mashed left ring finger.
