Accident type, without injuries

At approximately 11:46 am the service hoist went down when the magnet for the gate at the bottom of the shaft malfunctioned.

The small two man elevator was traveling from the bottom to the top when it stopped approximately 200 feet from the top due to the power cable getting damaged.

The small 2 man elevator was traveling from the bottom to the surface with a man onboard when it got stuck approximately 400 feet from the surface. The power cable at the elevator was pulled loose when it got hung on a bracket to the cross beam.

At approximately 11:10 pm the service hoist went down due to communication fault. It was found that one of the communication cards in the PLC was not making good connection.

A roof fall occurred on the J-O section in the No2 entry intersection in the last open cross cut. The fall is approximately 50 feet long by 20 feet wide and approximately 10 to 12 feet high.

Worker was using his little finger on left hand to line up the bolt hole. Anther worker jerked the Auger cutting off the tip of his little finger.

While starting to remove tubing from a loader bucket the employee stated he did not realize his right foot was under the bucket as it was being lowered to the ground.

Employee was picking up a belt frame when he felt a pain in his lower back. Employee did not start missing time from this injury until 02/19/2014.

Employee was building a brattice wall when he stepped on the edge of a pallet and twist right ankle.

Employee was binding down a MineArc on a flat car when he felt a pain in lower back.

Employee was removing a door on the head gate of the longwall in order to get to a sprocket. A rock fell from the roof hitting employee on the back of the head and shoulder area.

Employee was coming out of mandoor and stepped in a hole and twisted left knee. Employee did not start missing time from this injury until 10/04/2013.

Employee was moving a 13hp pump when he felt a pain in lower back.

Pulling a solenoid cover off 74 shield when a small rock fell between shields and struck right hand.

Employee received a small cut under his left arm as he was helping put up a longwall cable bretby that had fallen. Employee did not start missing time until 8-6-13 due to infection.

Employee bumped his right knee when he was unloading curtain from the bucket of a 3.5 loader. Employee did not start missing time from this accident until 8/12/20013.

Employee was working on belt line when he stepped in a hole and twisted left ankle.

Employee was bringing a diesel part to the underground diesel shop when he tripped over a cable causing him to hit his left arm on a table. Employee did not start missing time from this accident until 06/13/2013.

Employee was climbing down from above the de-slime screen after removing a pipe. The employee lost his balance and his hand got caught on the screen sheave.

Employee was walking down the pan line on the long wall when he tripped over a rock and fell on his left shoulder.

Employee was helping take apart a yellow discharge pipe when a rib rolled out hitting employee in the legs.

Employee slipped and fell off a material car at the bottom of the hoist an landed on his back.

Employee was unstopping a pipe and when the pipe broke loose the employee fell foward hitting his left knee on the ground.

Employee was riding on a man trip on North main roadway at crosscut 43 when the ride hit the rib hurting the employee`s right knee.

Employee stated he was aligning belt and cleaning material off the outer part of the top belt frame when his left hand got caught between the belt and roller.

Employee was walking and felt a pop in his left knee. Employee did not start missing time until 01/23/2013.

Employee was working in a 3.5 loader bucket when he was pinched between the stinger and the back frame of the bucket.

Employee was helping remove the pan line approximately 8 to 10 feet from the west maine raise bore shaft when some red clay mud fell down the shaft striking the pan line and employee left shoulder. As the employee went to get out of the way he fell down hurting his left ribs and shoulder.

Reportable due to sutures. Employee was removing a drill steel from a bundle when the band that was used to bundle the drill steel together hit him in the left wrist.

Employee was helping put side plate on the # 122 Raw Coal Screen when the huck bolt gun twisted and mashed his right ring finger between the huck bolt gun and the side plate.
