At 9:01am the elevator left the bottom landing area with the door not completly closed. The elevator was removed from service until repairs could be made. At 10:10am the door switch was replaced and the elevator put back in service.

At approximately 3:00 pm an ignition occurred on the J- main section in the #3 entry. The roof bolter operator was re-tighting a rib bolt when an ignition occurred around the rib bolt. The flames were extinguished in approximately 10 seconds with rock dust.
On the tailgate end of the G-9 Longwall, the rib above the last shield on the tailgate end rolled out. Although the rib shows some deterioration, the mine roof was never compromised and the tailgate gate entry was never blocked from passage. The operator implemented it block tailgate plan as a precautionary measure.
At approx. 7:44 a.m. the C.O. room operator recieved a phone call from underground stating that the I-O section belt line was on fire. At approx. 10:10 a.m. the belt line was reported out.
At 8:51am at the G-8 Pump Power center (SN- 48207-7/05)located at crosscut #7 on G-8, an employee saw arcing and smoke coming from around the transformer inside the power center. At 9:11 am the small fire was out.
At 7:55 am the service elevator went down as it was approching the landing at the top of the elevator shat. As the elevator went into slow down speed it blew a fuse. The fuse was replaced and the elevator was put back in service at 8:30am.
The mine roof fell above the anchorage of the roof bolts in the intersection on old # G-7 Section at cross cut #7 in the #2 entry.
At 8:25pm the bearing on the governor rope to the service shaft elevator failed. An EE was on the elevator at the time this occurred. The elevator was brought back to the surface landing. The bearing was replaced and the service elevator was put back in service at 11:15pm on 06/28/2010.
At approximately 8:55pm the CO room received a call that a hummer was on fire at the motor pit area. The fire was possibly caused by a short in the electrical system. The fire was put out at approximately 9:17pm.
At approximately 1:35pm the elevator stopped approximately 30 feet from the top landing with two employees on the elevator. The elevator was examined and the cause was found to be the top landing enter lock door switch. The switch was replaced and the elevator was put back in service at approximately 2:07pm.
Approximately 7:am Owl shift reported vibration on the service elevator as they was comming out. The elevator was examined during the morning hours and nothing was found. At approximately 12:10 pm the power was removed from the elevator to change out some control boards and make adjustment to the pods. Power was restored at approximately 2:30pm.
Elevator outage, no injuries, no emergency, no entrapment. Discovered CPI transformer primary side fuse was corroded. Clean it and the elevator worked. Out of service greater than 30 minutes.
At approximately 8:15pm the material hoist went down and showed motor field/lost fault. Trouble shooting was conducted and the hoist was put back in operation.
The material hoist went down at approximately 6:07pm and showed motor field/lost fault. Trouble shooting procedures was conducted and electrical parts was changed out. The material hoist was put back in service at approximately 8:30pm.
Elevator went down at 11:12pm the initial cause appear to be electrical. After a investigation was conducted it was determined that a fuse was bad in the control transformer. Fuse was changed and the elevator was back in service at 12:37am.
Elevator was shut down at approximately 2:48pm due to vibration. After an investigation it was determined that the vibration was due to slack in the hoist rope. The hoist ropes were tighten and the elevator was put back in service at 10:30pm.
During removal of rock on the east side of the Camp Creek Portal site are. The back-hoe struck and detonated an unfired detonator. At the time of the accident it was unsure what had happen. After an investigation was conducted it was determined that an unfired detonator had detonated. A call was made to Shoal Creek portal around 1:10pm, then a call was made to MSHA at 1:18pm.
At approximately 9:30am the elevator was shut down for testing. At approximately 9:50am it was discovered that the bolt to the tension spring bracket to the emergency brake system was damaged. New part was order from outside the United States and arrived at the mine on 01/28/2010. The elevator is back in service on 01/28/2010.
Ice had accumulated in the elevator shaft, the ice broke loose and fell down the shaft and damaged some beams and hoisting equipment in the elevator pit area at the bottom of the shaft.
Employee was walking across the roadway when he stepped on a rock and rolled his right ankle. Employee did not start missing time from this injury until 12/28/2010.
Employee was getting out of the service pick up truck and twisted his right knee.

Employee was riding a rubber tire man trip out of the mine when the man trip hit a rib hurting the employee's lower left back. Employee did not start missing time until 11/09/2010.

Employee was burning ludges off conveyor on a shuttle car and when ludges became dis-lodge a flame came out and burned the left side of employee face.

Employee was operating the headgate drum on the longwall shear when a rock (6""x6"") fell between the shields stracking the employee right hand.

Employee was operating a 3.5 loader and had his arm laying on top of the door. When the 3.5 loader turned his arm got caught between the door and a SCSR storage box. Employee did not miss any work.
Employee states that we has working on top of derrick and a wire line jumped off spool. His boot slipped and he fell down the ladder. His left forearm got wedged between the ladder and the derrick, bending the forearm backward.

A dozer was drawn into a void over one of the feed points on the raw coal pile, trapping the operator for approximately 55 minutes.
