At 9:01am the elevator left the bottom landing area with the door not completly closed. The elevator was removed from service until repairs could be made. At 10:10am the door switch was replaced and the elevator put back in service.

At approximately 3:00 pm an ignition occurred on the J- main section in the #3 entry. The roof bolter operator was re-tighting a rib bolt when an ignition occurred around the rib bolt. The flames were extinguished in approximately 10 seconds with rock dust.

On the tailgate end of the G-9 Longwall, the rib above the last shield on the tailgate end rolled out. Although the rib shows some deterioration, the mine roof was never compromised and the tailgate gate entry was never blocked from passage. The operator implemented it block tailgate plan as a precautionary measure.

At approx. 7:44 a.m. the C.O. room operator recieved a phone call from underground stating that the I-O section belt line was on fire. At approx. 10:10 a.m. the belt line was reported out.

At 8:51am at the G-8 Pump Power center (SN- 48207-7/05)located at crosscut #7 on G-8, an employee saw arcing and smoke coming from around the transformer inside the power center. At 9:11 am the small fire was out.

At 7:55 am the service elevator went down as it was approching the landing at the top of the elevator shat. As the elevator went into slow down speed it blew a fuse. The fuse was replaced and the elevator was put back in service at 8:30am.

The mine roof fell above the anchorage of the roof bolts in the intersection on old # G-7 Section at cross cut #7 in the #2 entry.

At 8:25pm the bearing on the governor rope to the service shaft elevator failed. An EE was on the elevator at the time this occurred. The elevator was brought back to the surface landing. The bearing was replaced and the service elevator was put back in service at 11:15pm on 06/28/2010.

At approximately 8:55pm the CO room received a call that a hummer was on fire at the motor pit area. The fire was possibly caused by a short in the electrical system. The fire was put out at approximately 9:17pm.

At approximately 1:35pm the elevator stopped approximately 30 feet from the top landing with two employees on the elevator. The elevator was examined and the cause was found to be the top landing enter lock door switch. The switch was replaced and the elevator was put back in service at approximately 2:07pm.

Approximately 7:am Owl shift reported vibration on the service elevator as they was comming out. The elevator was examined during the morning hours and nothing was found. At approximately 12:10 pm the power was removed from the elevator to change out some control boards and make adjustment to the pods. Power was restored at approximately 2:30pm.

Elevator outage, no injuries, no emergency, no entrapment. Discovered CPI transformer primary side fuse was corroded. Clean it and the elevator worked. Out of service greater than 30 minutes.

At approximately 8:15pm the material hoist went down and showed motor field/lost fault. Trouble shooting was conducted and the hoist was put back in operation.

The material hoist went down at approximately 6:07pm and showed motor field/lost fault. Trouble shooting procedures was conducted and electrical parts was changed out. The material hoist was put back in service at approximately 8:30pm.

Elevator went down at 11:12pm the initial cause appear to be electrical. After a investigation was conducted it was determined that a fuse was bad in the control transformer. Fuse was changed and the elevator was back in service at 12:37am.

Elevator was shut down at approximately 2:48pm due to vibration. After an investigation it was determined that the vibration was due to slack in the hoist rope. The hoist ropes were tighten and the elevator was put back in service at 10:30pm.

During removal of rock on the east side of the Camp Creek Portal site are. The back-hoe struck and detonated an unfired detonator. At the time of the accident it was unsure what had happen. After an investigation was conducted it was determined that an unfired detonator had detonated. A call was made to Shoal Creek portal around 1:10pm, then a call was made to MSHA at 1:18pm.

At approximately 9:30am the elevator was shut down for testing. At approximately 9:50am it was discovered that the bolt to the tension spring bracket to the emergency brake system was damaged. New part was order from outside the United States and arrived at the mine on 01/28/2010. The elevator is back in service on 01/28/2010.

Ice had accumulated in the elevator shaft, the ice broke loose and fell down the shaft and damaged some beams and hoisting equipment in the elevator pit area at the bottom of the shaft.

Employee was walking across the roadway when he stepped on a rock and rolled his right ankle. Employee did not start missing time from this injury until 12/28/2010.

Employee was getting out of the service pick up truck and twisted his right knee.

On 11/23/2010 employee strained his back, shoulder and neck when he fell to the ground. Employee did not start missing time until 12/29/2010.

Employees was installing forks on a Lo-Trac and fork fell hitting top of left foot. Employee did not start missing time until 11/29/2010.

Employee was riding a rubber tire man trip out of the mine when the man trip hit a rib hurting the employee's lower left back. Employee did not start missing time until 11/09/2010.

Employee was riding a rubber tire man trip out of the mine when the rubber tire man trip hit a rib causing the employee to hurt his left knee. Employee did not start missing time until 11/10/2010.

Employee was riding to the section in a Lee-Tram when it hit a hole in the roadway hurting employees back. Employee did not start missing time due to this accident until 12/03/2010.

Reportable Only due to sutures. Employee was helping move an 8"" steel water pipe, when Little right finger got caught between the pipe and a light bracket. **Employee did not start missing time until 10/26/2010.**

Employee was burning ludges off conveyor on a shuttle car and when ludges became dis-lodge a flame came out and burned the left side of employee face.

Employee was lifting a belt frame, when the frame slipped and as the employee tryed to catch the frame, he hurt his back and shoulder.

Employee stepped up on a ledge and slipped and fell hurting his lower back.

Employee was pushing a brattice block inplace and felt a pop on his left elbow.

When the employee stepped off a 10"" I-beam that was laying on the ground he stepped on a rock and twisted his left ankle.

Employee was attempting to unstop a dust hose on the roof bolter, when employee struck the top part of left hand with the bolt wrench.

Employee tripped over a extension cord and twisted left knee. Employee did not start missing time from work until 09/20/2010.

Employee was helping pull on a #2 cable when he felt a pain in his lower back.

EE was working in waterhole where mix had been spilled. He got his feet and ankles and lower legs wet causing burns.

Employee was walking by a shuttle car an tripped over the shuttle car cable and twisted his left knee.

Reportable Only Due to Stitches. While setting wedge the employee missed the wedge and struck his right index finger between the wedge and a tee board. (No Time Lost)

Employee was walking and fell down on a hill and hit his right elbow, knee and shoulder.Employee did not start missing time until 09/13/2010

EE was working at a seal site in B- bleeders and he had gunite/concrete in his eyes.

Employee was operating the long wall shear, when a rock fell between the shields, hitting the employee on the neck area.

Laying on top of belt ratcheting chain hoist and felt a pop in his left rib area.

Employee was climbing up on rear of equipment when his left knee popped. Employee did not start missing time from this injury until 10/21/2010.

The miner was giving work assignments, stepped off of belt structure and his ankle turned over on even bottom.

Employee was replacing a light fixture in the bath house. As the employee was rehanging the fixture, it slipped and as the employee grabbed the fixture, he felt a pull in his left shoulder. Employee did not start missing time until 07/14/2010.

EE was observing EE's couple conveyor chain together when a piece of the hammer broke off and hit EE in the lower right jaw.

Employee was operating the headgate drum on the longwall shear when a rock (6""x6"") fell between the shields stracking the employee right hand.

Employee was working out of the bucket of the loader when apperently the ventilation elbow tubing shifted hitting the employee in the lower left leg.

Employee stepped in a hole in the bucket of a loader and twisted right foot.

Employee stated as he was getting a T-Board he slipped and twisted his left ankle.

Employee was carrying a timber when the timber twisted and fell landing on his right knee. Employee did not start missing time until 06/28/2010.

Climbing down off 3 1/2 loader and stepped on a rock and twisted left foot. Employee did not start missing time from this injury until 06/29/2010.

Employee was walking toward the belt discharge from the G-8 belt power center when a rock fell from the rib and landed on his left foot and ankle.

Employee was walking through a water hole and twisted his right knee. Employee did not start missing time due to this injury until 7/19/2010.

Employee was moving a foam machaine on the decanter floor when he slipped and fell on right knee. Employee did not start missing time from the 6/2/10 injury until 6/22/2010 due to surgery to right knee.

Employee was carrying a bag of rock dust on the north main crosscut 30 when he stepped over a wall and his right foot went into a hole and twisted right knee.

Employee was roof bolting and as he reached back with his left hand to the roof bolt tray he put his right hand on the drill pod and a rock fell from the top hitting his right hand.

Employee was carrying a tray of block to West Mains when the employee went to set down the tray of block. The employee stated the hydraulics stuck and the tray hit the ground and throw the employee into the top of the cab. (The loader was checked and everthing worked properly).

Employee was lifting a drill guide assembly from the back of a piece of equipment when he felt a pain in the right lower abdoman.

Employee was using a ladder to install missing bolts in the top belt frame on north main #2 belt, when the ladder broke and the employee fell, landing on his right side.

Employee was carrying a timber when he slipped and fell hurting his right knee.

Employee was using a hammer to put in a foot shaft on the #2903 miner when the employee struck a board the hammer glanced off hitting the left index finger. Requiring 6 stitches

Employee was lifting top belt roller frame inplace, when he felt a pain in lower back. Employee did not go to the doctor from his injury until 04/14/2010, at that time the employee started missing time due to his injury that he had on 04/09/2010.

Employee was getting out of a 3.5 loader when his foot got caught on the loader an causing his body to twist injuring his lower back.

Employee was putting cover on a 3920 flinger duster, When his right hand got caught between the cover and a radiator cap. 3 sutures to right ring finger.

Employee was picking up belt structure when he felt pain in back.

Employee started to get into loader bucket while the loader was still moving. Employee right foot got caught between bucket and a rock on the foot wall.

Employee strained his middle part of his back as he was greasing a SR-1 mantrip.

Employee mashed his finger as he was laying a brattice block into position and got his finger caught between two brattice blocks.

Employee was attempting to drive in a conveyor hinge pin with a 20lb sledge hammer. The hammer glanced off the pin an hit the employee in the left ankle area.

Employee was transporting parts to the rais bore project after arriving the employee was attempting to release the binders with a bar. The binders released and the bar hit the employee in the nose.

Employee slipped and fell while walking in mud and water.

Employee was using a set of come-a-long to set a motor in place, when the hook slipped and caught left hand.

Employee was drilling a roof bolt hole when a small piece of cap rock slid down the drill steel and broke in half. A piece of rock hit employee in the upper lip causing laceration to the upper lip that required 13 stitches.

Employee was attempting to rehang a roller hanger from the top. The employee dislodged the roller hanger and it fell hitting employee on the right hand.

Employee felt a pain in his left abdomen area as he was pluging in a high voltage cable.

Employee injured his right knee when the employee jumped onto the bucket of the loader to help another miner and hit his right knee. Employee did not start missing time on this injury until 06/15/2010.

Employee was stepping up on a block when it gave way. As the employee grabbed a rib bolt to catch himself he pulled a muscle behind elbow. Employee did not start missing time until 02/24/2010.

Employee was stepping off of ladder when he slipped off ladder an fell down approximately 3 feet and landed on right leg and heel.

Pulling emulsion hose from bore-hole when employee experienced pain in low back.

Employee was walking to the elevator when employee stepped on small rock and twisted right ankle.

Employee was riding Lee-Tram when equipment hit hole in roadway and employee jambed his neck and shoulder.

Employee was operating a water car (Ram Car 3923) up a hill. The car would not pull up the hill so the employee turned around and headed back down the hill. When the employee hit the brakes she lost control and hit the right and left ribs.

Employee was getting off break car at top of slope and twisted left ankle. Fracture to metatarsal left 5th.

Employee was helping pick up belt rails to adjust chain. The rails shifted and pinched right hand between rails.

Employee was going through a man door, when the air sucked him through the door, causing him to fall and twist his right knee.

Employee was pulling down on the outside elevator door located on the 4th floor of the pre-plant, when employee felt pain in lower back.

Employee stated he was walking and stepped off a small ledge on the floor and twisted his lower back.

Reportable only due to fracture. Employee was holding in the conveyor lever on the feeder when she felt a burning sensation between her shoulder blades (fracture to left 4th rib).

Employee stated he was reinstalling a rerailers on the stage loader when the rope broke and the rerailer fell landing on left wrist. Employee was taken to the doctor and the doctor placed the employee on restricted duty on 04/24/2010.

Employee was operating a 3.5 loader and had his arm laying on top of the door. When the 3.5 loader turned his arm got caught between the door and a SCSR storage box. Employee did not miss any work.

Employee states that we has working on top of derrick and a wire line jumped off spool. His boot slipped and he fell down the ladder. His left forearm got wedged between the ladder and the derrick, bending the forearm backward.

Reportable only due to sutures. Employee stated he was putting wheel unit in part tray when he tripped causing his left leg to catch the corner of the tray causing Laceration requiring 3 sutures.

Reportable only due to 3 sutures. Employee was standing talking to another employee when a small rock fell hitting employee on his right forearm.

Reportable Only due to stitches. The employee was helping build a brattice when a rock fell from the roof striking the employee in the left forearm.

A dozer was drawn into a void over one of the feed points on the raw coal pile, trapping the operator for approximately 55 minutes.

Employee struck his left middle finger as he was attempting to remove a stuck bolt wrench from a drill steel by stricking it with another drill steel. Reportable only due to stitches.

Employee was standing talking on the mine phone when a rock fell stricking emloyee on the left hand. Reportable only due to 4 stitches.

Reportable only due to sutures. Employee was assisting in using the headgate winch to change out the conveyor chain when the hook came loose and struck the employee in the left cheek area. 6 sutures to left cheek.

Reportable Only Due to Stitches. The employee was helping transfer a pump from one piece of equipment to another piece of equipment when the pump fell approximetly 2 inches and landing on his right ring finger.

Reportable Only due to stitches. The employee was helping install a AFC chain on the longwall setup when his finger was hit with a hammer by a co-worker.

Employee was hanging pipe, when the pipe shifted catching left middle finger between pipes.

Re-positioning belt rollers on a tractor when left hand was caught between two rollers. Laceration to top of left hand requiring 5 sutures.
